Prevalence and Risk Factors of Depression in Patients with Chronic Obstructive Pulmonary Disease (폐쇄성폐질환에서 우울증상의 빈도 및 위험 인자)

  • Background: Due to the irreversible nature of chronic obstructive pulmonary disease (COPD), the treatment aim in patients with COPD is not to cure but to reduce the symptoms, increase lung function, and improve the quality of life. It has been suggested that depression is a common emotional disturbance in patients with COPD who are faced with a major physical impairment and embarrassing symptoms. This study evaluated the prevalence and risk factors of depression in patients with chronic obstructive pulmonary disease. Methods: A total of 59 patients with a registered diagnosis of chronic obstructive pulmonary disease were selected. Depression was assessed using the Centers for Epidemiologic Studies Depression (CES-D) scale. The quality of life was assessed using the Korean version of the St. George's Respiratory Questionnaire. Results: The prevalence of depression was 17.0%. In the correlation model, the interaction of the $FEV_1%$ over predicted value and SGRQ score(symptom, activity, impact, overall score) was statistically significant. The interaction of the $FEV_1%$ over predicted value and depression scale(CES-D) was also statistically significant. There was a positive correlation between the SGRQ scores(symptom, activity, impact, overall score) and the depression scale. Conclusion: The prevalence of depression in patients with chronic obstructive pulmonary disease is relatively high. The pulmonary function and the living standards were found to be significant risk factors for depression.

  • Bipolar disorder, also known as manic-depressive illness, is a brain disorder that causes unusual shifts in person's mood, energy, and ability to function. Compared with manic episode, the depression episode causes more serious results such as restless, loss of interest or pleasure, or thoughts of death or suicide and the cure rate of depression episode is lower than that of manic episode. Furthermore, a long term use of antidepressants in bipolar patients may result in manic episode. Our interest is to investigate the effect of antidepressant on switch of moods of bipolar patients and to estimate the transition probabilities of switch between moods, depression and (hypo) manic. In this study, three approaches are applied in terms of multi state model. Parametric model is applied using left censoring data and nonparametric model is implemented under illness-death model with counting process. In order to estimate the effect of covariates, a multiplicative model is used. These all methods have similar results.

  • The increasing prevalence of diabetes is a socioeconomic and health issue. Diabetes control is a lifelong management rather than a cure, and self-care should be undertaken. This study is to classify diabetes mellitus into non-regulatory group, and to prove the difference in risk factors for diabetes. Furthermore, we want to clarify the relationship between the emotional factors by diabetics and the prevalence of diabetes. As a result, the factor of prevalence for glycemic control which is a diabetic person was demonstrated. In addition, negative emotions such as depression and stress were found to be associated with diabetes mellitus and blood sugar control. Therefore, the diagnosis of diabetes is considered to be necessary in combination with negative emotions such as depression and stress.

  • This paper proposes a machine learning-based emotion analysis system that detects a user's depression through their SNS posts. We first made a list of keywords related to depression in Korean, then used these to create a training data by crawling Twitter data - 1,297 positive and 1,032 negative tweets in total. Lastly, to identify the best machine learning model for text-based depression detection purposes, we compared RNN, LSTM, and GRU in terms of performance. Our experiment results verified that the GRU model had the accuracy of 92.2%, which is 2~4% higher than other models. We expect that the finding of this paper can be used to prevent depression by analyzing the users' SNS posts.

  • Depression is common in patients with end-stage renal disease(ESRD) and has a negative effect on the quality of life, functional ability, and mortality of the patients, with a prevalence rate as high as 20-25%. Especially, the increasing tendency of mortality in ESRD patients is associated with recent or current depression, and the suicide rate is also increased by depression in patients with ESRD. Therefore, accurate detection and appropriate treatment of depression is very important in ESRD patients. Also, a deferential diagnosis is needed concerning uremic symptoms and depression in ESRD patients. However, there has been little data so far particularly in terms of randomized clinical trials. This review focused on the recent knowledge of depression in ESRD, and could encourage clinical study and trials in this field.

  • Despite the fact that pharmacotherapy depressive disorders have proven efficacy, a substantial number of patients are resistant to conventional management. As neuroscientific research about pathophysiology of depression have accumulated, repeated transcranial magnetic stimulation (rTMS) and transcranial direct current stimulation (tDCS) have emerged as an important mechanism-based treatment modality. This overview provides a review of therapeutic application of rTMS and tDCS in patients with depression. The clinical and basic studies of rTMS and tDCS in depression were reviewed and integrated using a literature review and interview with experts. rTMS is a noninvasive procedure of a localized pulsed magnetic field to the surface of the head to cause a depolarization of neurons in the brain cortex. tDCS has a mechanism of modulating cortical excitability in a polarity-specific manner without eliciting action potentials. rTMS and tDCS seem promising for treating depression. Although therapeutic parameters and further technical improvement remain to be systematically investigated, rTMS and tDCS would be a safe and effective intervention to treat depression.

  • 사람은 노화가 진행됨에 따라 기억력, 지능, 언어, 운동 기능 등의 인지능력이 저하되므로 대상 노인에 특성에 맞는 적절한 재활치료 방법을 적용해야 한다. 체감형 3D 인지시스템은 가상현실을 기반으로 하고 인지능력 향상을 위한 컨텐츠를 적용하여 환자의 특성에 맞는 의료 활동을 수행할 수 있다. 특히 원예 컨텐츠는 노인들의 참여도를 높일 수 있고 인지능력 향상과 우울증 감소에 도움이 된다. 이러한 가상현실 시스템은 위치인식, 동작인식, 디스플레이 등의 하드웨어 장치들을 이용하여 구현 가능하다. 본 논문에서는 노화에 따른 인지기능 저하와 종류와 특징을 분석하고 관련 재활치료 방법을 조사하였다. 또한 체감형 3D 인지시스템을 구성하는 모듈과 재활치료에 적합한 가상현실 컨텐츠를 제안하였다.

Differences of Illness Behavior in Depressive Patients According to the Presence of Somatization (우울증 환자에서 신체화 증상에 따른 질병행동의 차이)

  • Objectives : Illness behavior is defined as the persistence of an adaptive/maladaptive mode of perceiving, evaluating and responding to health status and symptoms according to the status. In a cognitive aspect, somatizing symptoms are regarded as being originated from distortions, including magnification and/or amplification, of perceiving, evaluating and responding to symptoms such as bodily sensations. That is somatization may be explained by maladaptive illness behavior. In this study, we tried to investigate differences of illness behavior in depressive patient according to the presence of somatization. Methods : We divided 45 patients who were diagnosed as depression with ICD-10 diagnostic criteria into two groups(somatizing and non-somatizing group) according to the somatization subscale of Korean Depression Scale and compared two groups in the differences of illness behavior using the Illness Behavior Questionnaire. Results : Somatizing group showed significantly higher scores than non-somatizing group on the disease affirmation subscale($6.79{\pm}2.08$ vs. 4.76, p=0.003) and the denial subscale($3.25{\pm}1.22$ vs. $2.10{\pm}1.41$, p=0.006). There were no significant differences between two groups on the general hypochondriasis subscale and the affective unstability subscale. In a logistic regression analysis, somatizing group also showed higher odds ratio (OR) scores on the disease affirmation subscale(OR=1.418, p=0.089) and the denial subscale(OR=1.880, p=0.083). Conclusion : The disease affirmation and denial may be a discriminative mechanism of somatization in depressive patients. These subscales of Illness Behavior Questionnare could be useful markers, and psychiatric illnesses with somatizing and depressive symptoms may be differentially diagnosed and be predicted through these subscales.

  • This case is about the patient who has experienced available effect as a result of executing the treatment of physical constitution after deteriorating by symptoms being R/O because of side effects by Western medicine during the treatment by Western medicine after having diagnosis of melancholia. In the beginning of being clouded consciousness, it has paralleled the treatment by Oriental medicine and Western medicine. After restoration of consciousness the treatment by Oriental medicine was only executed. As a result of that, the initial symptoms and the existing melancholia have gotten better, and then it presented that the normal daily life would be available. We could examine that the treatment of physical constitution had available effect against melancholia. And it needs to widen it to the disorder of psychoneurosis hereafter. Also it showed that it needs systematic clinical research.

The effect of pretreated Lithospermum erythrorhizon derived-naphthoquinone on anxiety, depression in mice (지치 유래 naphthoquinone을 전처치한 생쥐에서 우울 및 불안 조절 효과)

  • This study was undertaken to investigate the influence and related mechanisms that have yet to be clearly demonstrated of Lithospermum erythrorhizon derived-naphthoquinone (shikonin) on the anxiety, insomnia, depression in rats. We hypothesized that naphthoquinone, the primary ingredient of Lithospermum erythrorhizon, plays a role in the modulation of insomnia evoked by stress, depression evoked by forced swimming or anxiety evoked by elevated plus maze. Male ICR (Institute of Cancer Research) mice were used and the immobility or swimming time, the duration of sleep, the duration and entry frequency into open arms were measured and recorded. The administration of naphthoquinone (10, 30 and 100 mg/kg) potentiated barbiturate-induced sleep suggesting the activation of GABAA receptor. It also potentiated the time spent in open arms of the maze and decreased the immobility time in forced swimming. In conclusion, naphthoquinone has anxiolytic, hypnotic and anti-depressant properties and is a potential therapeutic for anxiety, insomnia and depression.
