요즘 같은 삼복더위엔 고온다습한 기후 때문에 체력 소모가 커서 쉽게 피로해진다. 하지만 적당한 피로감은 밤에 숙면을 이루게 해주는 상쾌한 피로이며 다음날 아침 유쾌한 기상을 할 수 있게 해준다. 따라서 단순한 피로 자체가 걱정거리는 아니다. 문제는 몇 주나 몇 달이 가도 가시지 않는 만성적인 피로감이다. 만성피로는 흔히 두통, 뒷목의 뻣뻣함, 식욕부진, 권태증, 성욕감퇴, 신경쇠약, 의욕상실, 우울증 등을 동반한다. 쉬운 말로 각종 종합검사에서는 정상이지만 온몸이 나른하고 무기력하며 아무런 의욕도 없고 만사가 귀찮아지며 단지 피로할 뿐이다. 만성피로는 모든 질병의 근원이다. 으레 그러려니… 하면서 애써 참다가 낭패를 볼 수 있다. 그렇기 때문에 만성피로를 예방하는 것은 건강한 삶을 위한 첫걸음이다.
Anti-depressant and anti-anxiety effects of Saccharomyces cerevisiae extract and its hydrolyzed fraction. The purpose of the present study was to examine the effect of Saccharomyces cerevisiae extract (SCE) and its hydrolyzed fraction (SCE-40) on depression and anxiety-related behaviors in mice. Actions of SCE and SCE-40 on serotonin, norepinephrine and GABAergic systems in the rat cerebral cortex membranes were also examined. SCE and SCE-40 significantly reduced the immobility time in the forced swimming and tail suspension test in mice. Duration time of the open arms in the elevated plus maze test was significantly increased in the SCE and SCE-40-treated groups, compared with the saline-treated control group. SCE and its fraction SCE-40 significantly inhibited serotonin and norepinephrine transporter and GABA receptor binding, compared to the saline-treated group. In addition, serotonin and norepinephrine reuptake were significantly suppressed by SCE and SCE-40. These results demonstrate that SCE and SCE-40 produce anti-depressant and anti-anxiety effects through enhancing central serotonin, norepinephrine and GABAergic transmissions. These results suggest that SCE and SCE-40 as functional food might prove to be an effective antidepressant and anti-anxiety agent.
We experienced a case of 51-year-old female patient who showed symptoms of persecutory delusion, auditory hallucination and hallucinatory behavior, severe insomnia, psychomotor retardation and social withdrawal, along with some clinical signs of the deficiency of various hormones those gradually progressed after massive postpartum vaginal bleeding 13 years ago. She was admitted to a psychiatric ward under the impression of psychotic depression. However careful history taking and evaluation of clinical feature gave rise to the possibility of underlying medical condition. Laboratory work-up revealed panhypopituitarism, hypoglycemia and hyponatremia. After replacement of thyroid hormone and cortisol for 1 week, her clinical symptoms including psychiatric symptoms were improved. Taken together, these findings were compatible with the diagnosis of Sheehan's syndrome. On reporting this case, we would like to emphasize again the importance of differential diagnosis of medical problems causing psychiatric symptoms those are easily neglected in the clinical approach toward psychiatric patients.
In order to examine the relationship between cholesterol level on serum and depression in Korean adults, this study used data of the fourth Korea National Health and Nutrition Examination Survey (KNHANES). 834 persons with depressive symptoms were selected as an index group and 3,317 persons without depressive symptoms as a control group, and odds ratios were calculated by multivariate logistic regression. As a result, odds ratio of depressive symptoms was higher in low LDL group (OR=6.435, 95% CI; 2,358-17.557) than in high LDL group adjusted after monthly income, marital status, period of education, occupation, smoking status, and physical activity. Etiopathogenic mechanism must be found by convergent study in the future.
목적 : 갱년기 여성은 내장비만의 위험이 높으며 또한 심리적 변화를 겪는 시기이다. 갱년기 여성의 복부지방, 내장지방, 피하지방 및 이와 관련된 단순비만지표, 간울증, 스트레스, 우울, 자존감 등을 조사하여 갱년기 건강의 위협이 되는 신체 심리 인자를 알아보고자 이 연구를 시행하였다. 방법 : 병원공고를 통해 체질량지수 $23kg/m^2$ 이상 비흡연자인 만 45세 이상 55세 이하의 폐경 전후의 갱년기 여성 환자를 모집하여 2007년 8월 20일부터 24일까지 47명이 모집되었으며 이중 체질량지수 $25kg/m^2$ 이상이면서 복부 전산화 단층촬영 상 내장지방이 $100cm^2$ 이상인 20명의 단순비만지표, 체성분검사, 혈액검사, 복부 전산화 단층촬영 및 한방비만변증 (간울) 및 스트레스, 우울, 자존감 설문검사 결과의 상관성을 분석하였다. 본 연구는 경희대학교 동서신의학병원의 임상연구심사위원회의 승인을 받았다. 결과 : 1. 갱년기 비만여성에서 복부지방 면적, 피하지방 면적은 체질량지수, 체지방률, 허리둘레/신장비 등의 단순비만지표와 유의한 상관성을 보인 반면 (p < 0.01), 내장지방 면적은 단순비만지표와 상관성을 보이지 않았다. 2. 내장지방 면적은 사회 재적응 평가척도 (${\gamma}=0.577$, p < 0.01)와 유의한 상관성이 있었고, 선형회귀분석에서 유의한 결과를 나타냈다. $VAT(cm^2)=116.1+0.101{\times}(SRRS\;score)$ (${\gamma}^2=0.332$) 3. 피하지방은 식이태도 점수와, 총복부지방은 식이태도 점수 및 갱년기지수 중 혈관운동증상과 상관성이 있었다. 4. 간울증은 스트레스 반응척도, 우울지수, 갱년기지수 및 그 하부항목 중 혈관운동, 정신, 운동, 소화, 전신증상과 양의 상관성을, 자존감척도와는 음의 상관성을 나타냈으며, 내장지방과 직접적인 상관성을 보이지 않았다. 결론 : 간울증은 높은 스트레스, 우울, 갱년기 증상 및 낮은 자존감과 관련되는 것으로 나타났다. 폐경전후 비만여성에서 내장지방 면적은 간울증과 유의한 상관성을 보이지 않았으나, 생활 스트레스 사건이 많을수록 높아지는 것으로 나타나 갱년기 여성 내장비만에서 스트레스에 대한 대처가 중요할 것으로 사료된다. 추후 임상적 연계성 및 설문 보완에 대한 연구가 필요하리라 생각된다.
The purpose of this study is to develop Music Recall Technique for affect modification using programmed series of classical music, and further to apply the technique as therapeutic intervention for clients with anorexia nervosa. Music Recall Technique is composed of two sub-technique: first one is 'Music Recall(MR)' which is the way to reproduce of existing music on one's head and mind after listening to the music, and second intervention is 'Creative Music Recall(CMR)' which is the way to modify the exposed musical pieces or newly learned musical pieces. A client with anorexia nervosa with severe anxiety, anger and depression participated in this case study. The MRT is implemented in different stages pertaining to client's level of utilizing the music recall skill for the period of 10 sessions. Situations eliciting negative emotions were identified and music recalling was reinforced for the pertaining situation in order for the negative emotion to be coped and reduced by the end of session time. The client participated for 10 sessions, and was asked to fill out self-report on the affect change using MRT. Visual Analogue Scale was used for pre and post test for each session to measure the self perceived level of negative affect. State-trait anxiety inventory was also used to measure the anxiety level following the implementation of MRT. As results, negative emotions, such as anxiety, anger and depression were modified during stimulative-sedative music recall process. These negative emotions were decreased mostly during stimulative MR, especially, in the beginning process. Each negative affect on negative situation measured by VAS during each and entire session have been decreased. Negative emotion measured by state-trait anxiety inventory, state-trait anger expression inventory, and hopelessness-depression inventory were decreased as well. The results suggest that stimulative music using contemporary classic music was effective in reduction and modification of negative affect such as anxiety, anger and depression. It shows that Music Recall Technique can be a meaningful intervention for affect modification, and further it can be utilized as self-help in the outside of music therapy setting.

Objective : The purpose of this study is to investigate the effect of a group occupational therapy program on the depression and quality of life of the elderly with Mild Cognitive Impairment(MCI). Method : The elderly with Mild Cognitive Impairment were selected based on the Mini Mental State Examination-Dementia Screening(MMSE-DS). The group occupatinal therapy program consisted of various cognitive domains and was performed 12 times for 6 weeks. The Geriatric Depression Scale(SGDS-K) and Quality of Life(QOL) were used to assess depression and quality of life of the subjects before and after the intervention. Result : The elderly with MCI improved significantly on the SGDS-K and QOL after the intervention program(p<0.05). Particularly, in some items of the QOL, there was a statistically significant difference, but the results of the SGDS-K and QOL did not show any correlation(p>0.05). Conclusion : In the case of the elderly with MCI, the QOL scale and the depressive symptom test were statistically significant when the group occupational therapy program was applied for 6 weeks.
Objectives: Continuous positive airway pressure (CPAP) is effective in the treatment of obstructive sleep apnea syndrome (OSAS), but the major limitation of CPAP may be poor compliance. The aims of the study were to investigate the compliance and side effects of CPAP, and to evaluate the efficacy of CPAP in patients with OSAS. Methods: This study enrolled 106 patients with OSAS who took the CPAP treatment. The severity of daytime sleepiness was measured using Epworth Sleepiness Scale (ESS), and sleep quality and depressive symptoms were assessed by Pittsburgh Sleep Quality Index (PSQI) and Beck Depression Inventory (BDI), respectively. Results: During 29 months of the study period, 41.5% of patients were using CPAP and 38.7% of patients stopped using it. Compared to non-compliant patients, compliant patients had a higher PSQI score and obstructive apnea index. Among non-compliant patients, 51.2% of them stopped using CPAP within 1months. 85.7% of non-compliant patients were discomforted by the CPAP, but much more nasopharyngeal symptoms were reported in the compliant group. ESS (p<0.01), PSQI (p<0.01) and BMI (p<0.01) were reduced significantly after CPAP treatment but not BDI (p=0.86). Conclusions: We concluded that CPAP can reduce the daytime sleepiness, nocturnal sleep disturbance, and body mass index. To increase the compliance of CPAP, we suggest that some education and support are needed at the early stage of the CPAP treatment.

The purpose of this study is to investigate how the combined intervention of occupation-based intervention and computerized cognitive rehabilitation treatment affects the psychosocial function, cognitive function, and depression of stroke patients. 23 stroke patients were divided into experimental group(n=11) and a control group(n=12), and the experimental group and the control group received CoTras for 4 weeks and 30 minutes each, and additionally the experimental group received task-based intervention for 30 minutes before the start of CoTras. It was conducted 20 times for a total of 4 weeks. Self-efficacy scale, stress scale, depression scale, neurobehavioral cognitive status examination were used. As a result of the study, the experimental group showed significant improvement in improvement of self-efficacy, stress, and depression than the control group, and the experimental group showed a small effect difference in magnitude of effect from the control group in cognitive function. Based on this study, it is possible to utilize the simultaneous task-based intervention with CoTras to improve psychosocial function, depression and cognitive function in stroke patients.
Chromatin remodeling regulates gene expression through epigenetic mechanisms. Aberrations in histone modification have been associated with depression-like behaviors in animal models. Additionally, growing evidence also indicates that epigenetic modification is associated with depression. p11 (S100A10) has been implicated in the pathophysiology of depression both in human and rodent models. In the present study, we investigated alterations in histone acetylation and methylation at the promoter of the p11 gene in the hippocampus of mice subjected to chronic unpredictable stress (CUS). C57BL/6 mice were exposed to CUS daily for 3 weeks. Depression-like behaviors were measured with the forced swimming test (FST). The levels of hippocampal p11 expression were analyzed by quantitative real-time polymerase chain reaction (PCR) and Western blotting. The levels of acetylated and methylated histone H3 at the promoter of p11 were measured by chromatin immunoprecipitation followed by real-time PCR. CUS-exposed mice displayed depression-like behaviors with prolonged immobility in FST. CUS led to significant decreases in the expression of p11 at both protein and mRNA levels. Meanwhile, there was a decrease in histone H3 acetylation (Ac-H3) and H3-K4 trimethylation (H3K4met3) and an increase in H3-K27 trimethylation (H3K27met3) at the p11 promoter. These results indicate that chronic stress causes the epigenetic suppression of p11 expression in the hippocampus.
