Intercostal Neuralgia and Spinal Cord Compression Symptom due to Spinal Tumor -A Case Report- (척추 종양에 의한 늑간 신경통 및 척수 압박 증상 -증례 보고-)

  • A 49 years old male patient was admitted to our neuro-pain clinic with symptoms of left 11th intercostal neuralgic pain and low back pain that developed 2 months prior to admission. Upon initial physical examination, motor weakness or sensory deficit were absent. Intercostal neuralgic pain improved significantly after we performed thoracic root thermocoagulation. However on the afternoon of the procedure the patient started to experience voiding difficulty, saddle anesthesia and rapidly progressing motor weakness and hypoesthesia that involved the lower back area and the lower extremities for three days. Based on these symptoms spinal cord compression was suspected and subsequently plain T-L spine X-rays and T-L spine MRI were performed. A spinal tumor that appeared metastatic in origin was seen at the T11 and T12 level. Liver ultrasonography demonstrated the presence of a $4{\times}4cm$ sized ill defined mass in the posterior segment of the right lobe. The patient was diagnosed to have hepatocellular carcinoma after needle aspiration biopsy and cytologic studies. Further orthopedic surgery was recommended but as the patient rejected any further treatment and examination, it was not possible to confirm the primary focus of the tumor. However as metastasis of a primary liver tumor to the spine is a rare occurrence, some other primary focus of metastasis or even a malignant primary tumor of the spine is more likely to explain this patient's condition.

Spinal Syringomyelia Secondary to an Intracranial Microcystic Meningioma in a Dog (개에서 척수공동증을 동반한 소낭성 뇌수막종 1례)

  • A 5-year-old, 2.7 kg female maltese dog was presented a local veterinary clinic with one week history of hindlimb lameness associated with patellar luxation. Reduction of bilateral medial patellar luxation was operated using trochlear resection and lateral reinforcement technique. Three weeks after the surgery, the dog showed bending spine with pain in thoracic and lumbar region, continuous ataxia and intermittent convulsion. Magnetic resonance imaging scanning revealed a hyperintense mass in right frontal lobe of brain and abnormal cavitation from cervical cord to third lumbar cord. Histopathologically, neoplasm in brain composed of meningothelial cells showed loosely reticular or lace-like morphology with numerous extracellular cystic spaces of variable size and shape. Neoplastic cells were positive for vimentin and negative for neuron specific enolase and glial fibrillary acidic protein. Irregular shaped enlarge central canal-like cavity was existed in cervical and lumbar cords. In our best knowledge, this report described the clinical findings, imaging and histopathologic characteristics of unusual intracranial microcystic meningioma with secondary syringomyelia in a dog.

Interobserver and Interaobserver Variability in Interpretation of Lumbar Disc Abnormalities on Magnetic Resonance Images (자기공명 촬영상 요추 추간반 병변의 판독자내 및 판독자간 해석의 다양성)

  • Objective : The terminology of degenerative disc disease lacks official standardization. Lacks of such standardization may provoke some clinical and litigation problems. The authors investigated interobserver and intraobserver variability in interpretation of lumbar disc abnormality. Methods : Magnetic resonance imaging studies of the lumbar spine performed prospectively in 50 patients, were read blindly by three doctors dealing spinal disorders, using two nomenclature. Nomenclature I was normal, bulging, protrusion, extrusion. Nomenclature II was normal, bulging, herniation without neural compression, with neural compression. Intraobserver and interobserver variation were measured statistically. Results : Interobserver agreement was 70.4-80.8% for nomenclature I, 76.2-80.2% for nomenclature II. Intraobserver agreement was 84.0-88.0% for nomenclature I, 79.2-86.8% for nomenclature II. Interobserver Kappa statistic was 0.53-0.56 for nomenclature I, 0.54-0.57 for nomenclature II. Intraobserver Kappa statistic was 0.60-0.85 for nomenclature I, 0.53-0.72 for nomenclature II. Conclusion : Experienced doctors showed only moderate interobserver agreement when interpreting disc status on lumbar magnetic resonance imaging. Intraobserver agreement was superior to interbserver. The standardization of nomenclatures for lumbar disc extension beyond interspace are needed.

Use of Digital Infrared Thermography on Experimental Spinal Cord Compression in Dogs (개에서 실험적으로 유발한 척수압박에 대한 적외선 체열촬영술의 적용)

  • This study was performed to examine the absolute skin temperature and temperature difference $({\Delta}T)$ between the left and the right, the upper and the lower parts on the back of dog and to investigate the clinical usefulness of infrared thermography as diagnostic or prognostic aid on experimental spinal cord compression by using Digital Infrared Imaging System. In normal dogs, symmetrical and contouring patterns were observed in thermogram. The skin temperature difference was not significant between the left and the right, the upper and the lower parts on the back. In spinal cord compressed dog, there was symmetrical temperature reduction pattern around lesion. Patterns of ${\Delta}T$ between thoracic and lumbar portions were remarkable and showed the tendency to return to normal after 4 weeks of operation. It is considered that infrared thermography is a useful diagnostic and prognostic aid for spinal cord injury in dogs.

Effect of Lumbar Epidural Nerve Block using the Transforamimnal Approach and the Interlaminar Approach on Magnetic Resonance Imaging Findings (추간공 접근법과 추궁간판 접근법을 사용한 요부 경막외 신경차단술이 자기공명영상 소견에 미치는 영향)

  • This study was conducted to investigate the differences in magnetic resonance imaging (MRI) findings after lumbar epidural nerve block using the transforaminal approach and the interlaminar approach in patients with low back pain. This study was an observational analysis study of abnormal findings of MRI after epidural nerve block. This study included 78 patients who underwent MRI at approximately 24 h after lumbar epidural nerve block at a pain clinic of a university hospital between January 2007 and December 2016. Among patients who received epidural nerve block, 36 used the interlaminar approach and 42 used the transforaminal approach. The incidence of patients with abnormal changes in MRI findings was higher among patients using the interlaminar approach (53%) than those using the transforaminal approach (7%). Abnormal MRI findings included epidural air or fluid, needle tracks, and soft tissue changes, with epidural air being the most frequent abnormal finding (72%). We recommend use of the transforaminal approach to reduce the possibility of misreading or difficulty in interpretation of images of patients who underwent MRI at approximately 24 h after lumbar epidural nerve block. Practitioners should consider the possibility of abnormal findings such as epidural air on MRI in cases of epidural nerve block using the interlaminar approach.

Cross-calibration of Bone Mineral Density between Two Different Dual X-ray Absorptiometry Systems: Hologic QDR 4500-A and Lunar EXPERT-XL (서로 다른 이중에너지 방사선흡수기계 기종(Hologic QDR 4500-A와 Lunar EXPERT-XL) 간의 골밀도 교차 보정)

  • Purpose: A cross-calibration equation is needed to compare bone mineral density measured by different dual X-ray absoptiometry systems. We performed this study to establish appropriate cross-calibration equations between two different dual X-ray absorptiometry systems. Materials and Methods: Bone mineral density of anterior-posterior lumbar spine (L2-4 level) and femoral neck were measured in 109 women ($55{\pm}11yr$) using two different dual X-ray absorptiometry systems (Lunar EXPERT-XL and Hologic QDR 4500-A). Bone mineral density values measured by two systems, including area, bone mass content, bone mineral density and percentile of young normals were compared and cross-calibration equations between two systems derived. Results: The bone mineral density values of 109 women measured by Lunar system were $0.958{\pm}0.17g/cm^2$ at L2-4 and $0.768{\pm}0.131g/cm^2$ at femur neck, which were significantly higher ($13{\pm}6%$ at L2-4 and $19{\pm}7%$ at femur neck, p<0.001) than those ($0.851{\pm}0.144 g/cm^2$ at L2-4 and $0.649{\pm}0.108 g/cm^2$ at femur neck) by Hologic system. Bone mineral content and percentile of young normals measured by Lunar system were also significantly higher than those by Hologic system (p<0.001), whereas there was no difference in area (p>0.05). There was a high correlation between bone mineral density values of L2-4 and femoral neck obtained with both dual X-ray absortiometry systems (r=0.96 and 0.95, respectively). Cross-calibration equations relating the bone mineral density were Lunar= 1.1287${\times}$Hologic -0.0027 for L2-4 and Lunar= 1.1556${\times}$Hologic+0.0182 for femoral neck. Conclusion: We obtained cross-calibration equations of bone mineral density between Lunar EXPERT-XL and Hologic QDR 4500-A. These equations can be useful in comparing bone mineral density obtained by different dual X-ray absorptiometry systems.

Correction of Single Thoracic Adolescent Idiopathic Scoliosis Using Pedicle Screw Instrumentation: Comparison of Stainless Steel to Titanium Alloy Instruments (척추경 나사못 고정술을 이용한 단일 흉추 청소년기 특발성 척추 측만증의 치료: 스테인리스강과 티타늄 합금 기기의 비교)

  • Purpose: To compare the results of two different instruments made of stainless steel and titanium alloy for correction of single thoracic adolescent idiopathic scoliosis (AIS) using pedicle screw instrumentation. Materials and Methods: A total of 141 patients with single thoracic AIS treated with pedicle screw instrumentation and selective thoracic fusion were retrospectively reviewed after a follow-up of 2 years. The patients had a main thoracic curve of 40° to 75° and were divided into two groups based on instrument materials; S group (stainless steel, n=90) and T group (titanium alloy, n=51). The diameter of the stainless steel rod used was 7.0 mm while that of the titanium alloy rod was 6.35 mm or 6.0 mm. Standing long-cassette radiographic measurements including various coronal and sagittal parameters for the preoperative, early postoperative and 2-year postoperative followup were analyzed. There were no significant differences in the preoperative curve characteristics between the two groups. Results: In the S group, the preoperative main thoracic curve of 51.3°±8.4° was improved to 19.0°±7.6° (63.1% correction) and the lumbar curve of 32.3°±8.4° spontaneously decreased to 12.7°±8.2° (62.9% correction) at 2 years postoperatively. In the T group, the preoperative main thoracic curve of 49.5°±8.4° and the lumbar curve of 30.3°±8.9° was improved to 18.8°±7.4° (62.2% correction) and 11.3°±5.4° (63.3% correction), respectively. The corrections of coronal curves were not statistically different between the two groups (p>0.05). The thoracic kyphosis was changed from 16.8°±8.5° to 24.3°±6.1° in the S group and from 19.6°±11.2° to 26.6°±8.5° in the T group. There were no significant differences in the changes of sagittal curves, coronal and sagittal balances at the 2-year follow-up and the number of fused segments and used screws between the two groups (p>0.05). Conclusion: When conducting surgery for single thoracic AIS using pedicles screw instrumentation, two different instruments made of stainless steel and titanium alloy showed similar corrections for coronal and sagittal curves.

Prevalence of Incidentally Detected Spondylolysis in Children (소아 환자에서 우연히 발견되는 척추분리증의 유병률)

  • Purpose To assess the prevalence of incidentally detected lumbar spondylolysis in children. Materials and Methods We retrospectively reviewed the data of 809 patients under the age of 11 years (mean age, 7.0 ± 2.7 years; boys:girls = 479:330) who underwent abdominal and pelvic CT between March 2014 and December 2018. We recorded the presence, level, and laterality (unilateral or bilateral) of spondylolysis. Patients were divided into two groups based on the presence of spondylolysis: the spondylolysis (SP) and non-SP groups. Results In total, 21 cases of spondylolysis were detected in 20 patients (20/809, 2.5%). The mean age of the SP group was higher than that of the non-SP group (7.8 ± 1.8 vs. 6.9 ± 2.7 years, p > 0.05). The prevalence of spondylolysis in boys was higher than that in girls (15/479 [3.1%] vs. 5/330 [1.5%], p > 0.05). The prevalence of spondylolysis in school-age children (6-10 year olds) was higher than that in preschool-age children (0-5 year olds) (17/538 [3.2%] vs. 3/271 [1.1%], p > 0.05). L5 was the most common level of spondylolysis (76.2%); one 8-year-old boy had two-level spondylolysis. One case of isthmic spondylolisthesis was detected in a 10-year-old boy (1/809, 0.1%). There were 11 unilateral spondylolysis cases (11/21, 52.4%). Conclusion In our study, the prevalence of spondylolysis in children under the age of 11 was 2.5%. The prevalence was higher in boys than in girls and in school-age than in preschool-age children, despite the lack of any statistically significant differences.

Evaluation of Radiation Exposure Dose for Examination Purposes other than the Critical Organ from Computed Tomography: A base on the Dose Reference Level (DRL) (전산화단층촬영에서 촬영 목적 부위와 주변 결정장기에 대한 피폭선량 평가: 선량 권고량 중심으로)

  • In this study measured patient exposure dose for purpose exposure area and peripheral critical organs by using optically stimulated luminescence dosimeters (OSLDs) from computed tomography (CT), based on the measurement results, we predicted the radiobiological effects, and would like to advised ways of reduction strategies. In order to experiment, OSLDs received calibration factor were attached at left and right lens, thyroid, field center, and sexual gland in human body standard phantom that is recommended in ICRP, and we simulated exposure dose of patients in same condition that equal exposure condition according to examination area. Average calibration factor of OSLDs were $1.0058{\pm}0.0074$. In case of left and right lens, equivalent dose was measure in 50.49 mGy in skull examination, 0.24 mGy in chest, under standard value in abdomen, lumbar spine and pelvis. In case of thyroid, equivalent dose was measured in 10.89 mGy in skull examination, 7.75 mGy in chest, 0.06 mGy in abdomen, under standard value in lumber spine and pelvis. In case of sexual gland, equivalent dose was measured in 21.98 mGy, 2.37 mGy in lumber spine, 6.29 mGy in abdomen, under standard value in skull examination. Reduction strategies about diagnosis reference level (DRL) in CT examination needed fair interpretation and institutional support recommending international organization. So, we met validity for minimize exposure of patients, systematize influence about exposure dose of patients and minimize unnecessary exposure of tissue.

Bone Marrow Scintigraphy with Antigranulocyte Antibody in Multiple Myeloma: Comparison with Simple Radiography and Bone Scintigraphy (항과립구 항체 골수스캔을 이용한 다발성 골수종 병변의 평가: 단순골X-선점사 및 골스캔과의 비교)

  • Purpose: Simple X-ray study and bone scan have limitations for early diagnosis of bone or bone marrow lesions in multiple myeloma. The purpose of this study was to evaluate the diagnostic usefulness of bone marrow immunoscintigraphy using anti-granulocyte monoclonal antibody for the evaluation of bone involvement in multiple myeloma. Materials and Methods: In 22 patients (Male: 15, Female: 7) with multiple myeloma, we performed whole-body immunoscintigraphy using $^{99m}Tc$-labelled antigranulocyte antibody (BW 250/183, Scintimum $Granulozyt^{(R)}$ CIS, France) and compared the findings with those of simple bone radiography and $^{99m}Tc$-MDP bone scan. Abnormal findings in bone marrow scintigraphy were, considered to be present in case of expansion of peripheral bone marrow or focal photon defect in axial bones. Results: Marrow expansion was noted in 15 of 22 patients (68%). Focal photon defects were found in 18 patients (82%). While one (33%) of 3 patients with Stage II disease showed focal defects in bone marrow scan, abnormal focal defects were observed in 17 of 19 (90%) patients with Stage III. Among 124 focal abnormal sites which were observed in bone marrow scan, bone scan or simple bone radiography, bone marrow scan detected 92 sites (74%), whereas 82 sites (66%) were observed in simple bone radiography(58 sites, 47%) or bone scan(40 sites, 32%). Fifty-one (41%) out of 124 bone lesions were detected by bone marrow scan only, and located mostly in thoracolumbar spine. Conclusion: Bone marrow scan using $^{99m}Tc$-labelled antigranulocyte antibody seems to be a more sensitive procedure for the detection of pathologic bone lesions than simple bone X-ray or bone scan in patients with multiple myeloma.
