An Analysis of Infrastructure and Provision of Forest Welfare Service in Nursing Homes for the Elderly (노인요양시설의 산림복지서비스 인프라 및 제공 실태)

  • This is a cross-sectional study that suggests ways to activate forest welfare services (FWS) by investigating the infrastructure, service status, and perception on FWS in Korea. In August 2016, a structured email survey was conducted in nation widely. The respondents were mostly directors and general secretary (75.0%). The considerable number (16.3%) of nursing homes (NH) use some floors of the complex buildings that would be difficult to have FWS infrastructure and about 30% of those without forests near the facilities. The directors of NH recognize that FWS has positive effects on the elderly. However, FWS is not an requisite of the longterm care insurance benefit, and so costly and effort-intensive that FWS has not been activated so far. In order to activate FWS in NHs, it is necessary to develop and disseminate the guidelines on FWS that anyone can easily followed. In addition, when the National Health Insurance Corporation evaluates NHs, they should evaluate not only whether there is a wandering or walking space, but also whether it has forest healing factors such as forests. It is also necessary to create a barrier-free environment both inside and outside of NHs, increasing accessibility to the toilet in gardens, paving a passage for wheelchairs and lifts in forests near NHs. Through these efforts, it is expected that FWS will be activated to provide physical, mental rest and comfort, appropriate cognitive stimulation to the NH residents at the end of life.

Effects of 4% Hypertonic Saline Solution Mouthwash on Oral Health of Elders in Long Term Care Facilities (4% 고농도 식염수를 이용한 구강함수가 요양시설 입소노인의 구강건강에 미치는 효과)

  • Purpose: This study was done to examine the effects of 4% hypertonic saline solution mouthwash and tooth brushing education on the oral health of elders living in long term care facilities. Methods: In this quasi-experimental study, the participants were assigned to a 2% experimental group (n=20), a 4% experimental group (n=20), and a control group (n=20). Data were analyzed using ANOVA, repeated measures ANOVA, Fisher exact test, Chi-square test, Kruskal-Wallis test and multiple response analysis with the SAS program. Results: Regular tooth brushing and use of 4% hypertonic saline solution mouthwash by elders provided better oral health by decreasing xerostomia, oral tongue plaque, halitosis, and the number of oral bacteria. Conclusion: The results indicate that regular tooth brushing with continuous 4% hypertonic saline solution mouth washing education promotes oral health for elders in long term care facilities, thus the dental care described in this study is recommended for elders in long term facilities.

The Related Factors with Improvement of Long-term Care Need of Residents and Quality of Service in Long-term Care Facility (노인요양시설 입소자의 장기요양등급 개선과 서비스 질 관련요인)

  • The purpose of this study was to investigate the relationship among staffing, occupancy rate, upward level change of long-term care need, and evaluation grade of facility. Data were obtained from National Health Insurance Corporation Database. Occupancy rate and evaluation grade were highest in National/public operating facilities, while they were worst in individual operating facilities. The percents of A or B grade in evaluation grade (by newly enforced law) is highest in National/public operating facilities. Multiple regression analysis showed that upward level change of care needs was very weakly associated with the number of doctors. Evaluation grade showed a weak and significant association with occupancy ratey(by old-version law)(r=.20, p<.01), upward level change of care need in group home(r=.23, p<.01) Staffing in facility did not show significantly consistent association with upward level change of care needs, evaluation grade, and occupancy rate.

Analysis of the Influence of Physical and Mental Function, and Oral Health Impact Profiles on Quality of Life in the Elderly People in Long-term Care Centers using the Structural Equation Model (구조방정식모형을 이용한 장기요양시설 노인의 신체적·정신적 기능 및 구강건강영향지수가 삶의 질에 미치는 영향 분석)

  • This study examined the effects of the physical functions (ADL, IADL), mental functions (CES-D, MMSE-K) and oral health impact profile (OHIP-14) on the quality of life (WHOQOL-BREF) of the elderly in long-term care facilities. The study subjects were 602 elderly people, who were surveyed at the entrance to the long-term care facilities. The interviews were conducted using a questionnaire during the period from May 1, to June 30, 2014. As a result, the quality of life was affected more by the CES-D and MMSE-K than by the ADL and IADL or by OHIP-14; the quality of life improved with a higher ADL, IADL, and OHIP-14 and a higher CES-D and MMSE-K. A higher OHIP-14, ADL and IADL resulted in a higher CES-D and MMSE-K. These results suggested that the quality of life is correlated significantly with the physical functions (ADL, IADL), mental functions (CES-D, MMSE-K), and the oral health impact profile (OHIP-14).

Development of Daycare Center for Senior Users as Community-Cohesive Facility (노인데이케어센터의 지역융합형 발전방향)

  • Facing aged society, long-term care insurance for senior citizen has been introduced in Korea since 2008 in order to take away care burden from family. Daycare Center is one of the institutions developed by the idea of social care. Noticing many problems around adult day services (ADS) such as low quality of services and lack of running sources, this paper would suggest the norm of day care center in terms of community cohesive facilities. In addition, to find out the future vision of ADS, interviews with 10 experts are also analyzed. The result is that new health care program would be community-friendly, tailor-made, communication-based and intergenerational one.

Effects of Multisensory Stimulation Using Familiarity: Persons with Dementia in Long-term Care Facility in Korea (익숙함을 이용한 다감각적 자극 프로그램의 효과: 시설 입소 치매 노인)

  • Purpose: The purpose of the study was to examine the effects of multisensory stimulation (MSS) using familiarity on persons with dementia (PWDs) residing in nursing homes in Korea. Methods: A nonequivalent control group with pre and posttests was used. Fifty one PWDs were included if they: 1) were over 65 yr old, 2) were diagnosed with dementia, 3) had no visual or speech impairments, 4) were able to communicate, and 5) had spent more than one month in a nursing home. The experimental group (n=25) received a 55 min MSS program twice a week for 10 weeks. The outcome variables included were cognition, activities of daily living, grip strength, depression, wandering, and aggressive behaviors. Repeated ANOVA was used for data analysis. Results: There were no significant differences in demographics or the main variables at pretest. Cognition, depression, wandering, and aggressive behaviors were significant over time between the two groups. Grip strength was only significant when accounting for interaction between group and time. Conclusion: An intervention of MSS using familiarity was marginally effective in improving cognition, depression, wandering, and aggression. Future study is suggested with a larger sample and longer treatment to retest the effects of MSS.

Nursing Staffs' Experiences of Managing Medical Needs of the Elderly in Korean Long-term Care Facilities (노인요양시설 입소자들의 의료적 요구에 대한 간호직 종사자들의 관리 경험)

  • Purpose: This qualitative study was designed to explore and describe the nursing staff reported experiences of managing the medical needs of nursing home residents in South Korea. Methods: Using a focus group interview method, qualitative data were collected from March to May 2012. Twenty five registered nurses and nurse aids working in nursing homes were recruited through a convenient sampling method. Participants participated in one of four focus group interviews lasted up to two hours in which their day-to-day experiences of taking care of the medical needs of the elderly residents. All interviews were recorded and transcribed in verbatim, and analyzed according to the qualitative thematic analysis method using MAXQDA software. Results: The overriding theme was 'experiencing differences between the requirements of the regulatory system and the actual reality of the work place.' The findings of differences between what were true and what is required was based in four subcategories. The nursing home residents had far more medical needs than what were reported. Another finding was that the family members were seen as non-supportive and negligent towards the residents. Conclusion: The findings of the study support the need for changes in the regulatory system. The regulatory limitations of current system with a shortage of resources pushed the participants to experience discrepancies between the required regulation and the reality. Additional research could contribute more exemplars to support changes.

A Study on the Residents' Behaviors in Living Area of Special Nursing Homes for the Elderly (노인전문요양시설 입소노인의 거실이용실태에 관한 조사연구)

  • Nursing Homes are different from other medical facilities, because they have a living space. And most of residents in a nursing home have dementia, therefore nursing homes are in control of outdoor activities for resident's safety and most of residents live in the living room at the daytime. The purpose of this study is analysis of spacial behaviors of residents in the living room, and reflection of the analysis into the architectural plan of nursing homes.There are two activities, individual activity and social interchange activity, in a living room and the role of a living room is semi-private space that includes both of them. Most numerous activities are watching TV and take a eat. Activities are different in accordance with the position at the living room. Individual activity and social interchange activity is same ratio in the middle and the ratio of social interchange activities are more than individuals in outside of living room. but alcove use of private.The types of living rooms are distinguished by combination of functions of livingroom, activity-room, and diningroom. The activity of use of residents have been shown different phases following the types of livingroom. The type of combination has been seen high rate of use and private activities and social communications are expressed evenly. The type of mixing has been seen low rae of use, but two activities, the above, are equal. The type of separation has been seen not only low rate of use, but also there has been only private activities.

Oral health care of the elderly in long-term care facilities in Korea (요양보호사가 입소노인에게 제공하는 구강관리실태)

  • Objectives : The aim of this study was to assess the oral health care of the elderly in long-term care facility on caregivers' behaviors. Methods : Cross-sectional study in a cluster sample of 171 caregivers recruited from 17 facilities located in the Province of Gangwon. The Questionnaire was consisted of 16 items contained oral hygiene care, denture care, oral health education and general characteristics(Chronba's ${\alpha}$=0.87). Using SPSS WIN 12.0, descriptive statistics and chi-square test were conducted to examine the subjects general characteristics, the status of oral health education and denture care. Results : 1. The mean age of care givers was 42 years and 88.9% education experience rate was found. 2. Above 70% of them was found in daily oral hygiene care after every meal. They used toothbrush and toothpaste, they keep the toothbrush properly. 3. The denture care was conducted by most of care givers, containing proper storage. But three of ten care givers was cleaned denture by toothpaste or used water only. It was severe at the care givers didn't received oral health education(p<0.05). Conclusions : The contents of oral health education for care givers should contain the denture care(time and the reason shouldn't use toothpaste). Further large-scale longitudinal studies are needed to determine professional oral health care and to develop evidence of the dental hygiene practice for the elderly in long-term care facilities.

The effect of a Cognitive Occupational Therapy program on cognition and hand functions in patients with dementia living in a community (지역 요양시설 치매환자에서 인지작업치료프로그램의 인지와 손기능에 대한 효과)

  • The aim of this study is evaluate the effect of a cognitive occupational therapy program on cognitive function, depression and hands function for patients with dementia living in a local community. A cognitive rehabilitation program of 10 weeks focusing on the occupational therapy is conducted from September to December 2012 on 21 patents (experimental group: 12, control group: 9) with dementia who are admitted to nursing homes in a metropolitan region. In the experimental group, the cognitive function, depression level, hand strength, and hand coordination ability is significantly improved after the application of the cognitive program (p<.05). In conclusion, the cognitive occupational therapy program may be a useful intervention for dementia. Because the therapeutic goal for dementia treatment is mainly concentrated on the amelioration of dementia symptoms, thus it is necessary to develop a various cognition training program that can be maintained the patient's residual functional capacity and returned to the social community through the early detection and the early intervention.
