Treatment Response and Symptomatic Changes after Eye Movement Desensitization and Reprocessing in Psychiatric Disorders Other than Posttraumatic Stress Disorder (외상후 스트레스 장애 이외의 정신 장애에 대한 EMDR 전후의 치료 반응 및 증상 변화)

  • Objective : Eye movement desensitization and reprocessing (EMDR) has been established as an effective treatment for patients with posttraumatic stress disorder (PTSD). However, the literature is unclear as to whether EMDR is effective in the treatment of other psychiatric disorders. The purpose of this study was to evaluate the potential use of EMDR in the treatment of psychiatric disorders other than PTSD by using a clinician's impression of patient response and a subjective symptom evaluation. Methods : Seventeen diagnostically heterogenous patients without PTSD underwent an average of 4.3 sessions of EMDR. Symptom severity was assessed by the Clinical Global Impression-Change Scale (CGIC), Beck Depression Inventory (BDI), State-Trait Anxiety Inventory (STAI), and Symptom Checklist-90- Revised (SCL-90-R) before and after EMDR. Those whose CGI-C scores were 'very much improved' and 'much improved' after EMDR were classified as 'responders.' The patients' before and after treatment scores of symptom severity and group differences were compared. Results : Twelve of the 17 participants (12/17, 71%) were classified as 'responders.' The patients' scores on all of the scales, with the exception of the trait anxiety scale and obsession-compulsion scale of the SCL- 90-R, significantly decreased after treatment. There was no difference in sociodemographic and clinical variables between the responders and non-responders. Conclusion : The results of our study suggest that EMDR can be a promising candidate for the treatment of patients with psychiatric disorders other than PTSD, and thus further controlled studies are needed to determine whether EMDR can be applied to various psychiatric populations.

A Cohort Study of Children and Adolescents Victims with Sexual Abuse in Korea and Their Initial Assessment Results (아동청소년 성폭력 피해자들을 위한 코호트 연구 : 코호트 구축과 초기 평가 결과)

  • Objectives : The goals of the study are how to establish the cohort systems for the children and adolescents victims with sexual abuse in Korea and to identify the risk and protective factors that influence mental health in child sexual abuse (CSA). This is initial assessment data based on the analysis of cohort variables for baseline evaluation of subjects. Methods : We constructed the cohort systems for CSA victims recruited by Seoul Sunflower Children Center, CSA victims protection center. The initial assessment data which consisted of demographic and psychological inventories of CSA victims and their parents/families, psychiatric diagnoses were the results of statistical analysis of 65 subjects under 19 years old for 3 years 7 months. Results : The initial data were followings : female participants, N=56; mean age, 11.6 (SD=4.5); the most sexual assault, molestation 71.8%; victims, family and acquaintance 87.1%; 61.5% of the subjects diagnosed with psychiatric disorder; 29.2% diagnosed with PTSD and 23.1% diagnosed with depression. Mean duration for abuse to report is 1.5 years. Mean score of IES-R-K, TSCYC-avoidant and CBCL-problematic behavior were increased above clinical cut-off. Conclusions : CSA victims tend to have high risks in mental health problem. The cohort study could provide the risk and protective factors of CSA in mental health, and construct the predictive model for mental illness in Korea.

The Effect of Mental Health Improvement Programs for Firefighters (소방공무원에서 정신 건강 증진 프로그램의 효과)

  • Objective : The purpose of this study was to investigate the effectiveness of the mental health improvement program in Korean firefighters. Methods : A total of 502 firefighters working at Daegu, Ulsan and Gumi participated in the mental health improvement program for an 8-week period. The program included evaluation and counseling as well as psychoeducation for mental health awareness. Subjects were divided to two groups including the PTSD high risk group and PTSD non-high risk group. Results : The program resulted in a significant decrease in mean scores of depression and insomnia for all firefighters. PTSD high risk group showed a significantly higher level of depression, anxiety, and insomnia. Multiple logistic regression analysis with PTSD high risk group as a dependent variable indicated that moderate depression was a high-risk factor for PTSD in firefighters, as compared to no depression ; in addition, subthreshold and moderate insomnia was another risk factor. Conclusion : The evaluation and management of psychopathologies including depression and insomnia associated with traumatic events can be helpful to improve the mental health of firefighters.

  • A lot of research have been done so far in terms of traffic accident sequela, but most of them have stressed simply on physical treatments, resulting in less attention to the psychological ones. This is a case report of a patient with post-traumatic stress disorder. Although that is simply following in the wake of past therapy, we were experienced an improved case of post-traumatic stress disorder patient treated by herbal medicine, acupuncture, mental therapeutics. This case effectively describes the characteristics of the post-traumatic stress disorder, and it also suggested one approach for the treatment of the disease. This report is for the better understanding of the disease, and also the search for the advanced treatments to it hereafter.

  • The purpose of this study is to investigate the differences in irrational faith and the school adaptation of elementary school students by experimental group and control group. To achieve these goals, the followings are setting the programs. The subjects are selected from grade 4, 5, 6 students from the elementary schools. The tools used for this study were Irrational faith Test and School adaptation Test. The followings are the results of the analysis of the collected data. First, in the analysis of irrational faith, there is no significant difference between experimental group and control group. Second, in the analysis of school adaptation, there is significant difference between experimental group and control group(t=.05, p<.05).

  • Objectives: The purpose of the case study is to report the effects of post traumatic stress disorder from sexual violence treated by an oriental medical treatment. Methods: The patient was treated by acupuncture at Baekhoi, Naegwan, Hapkok, Taeyang, Changmun and moxa at Baekhoi, Sin-Guel, Chungwan, Kwanwon. Results: The oriental medical treatment resulted considerable improvements for the patient. Conclusions: This study shows that the oriental medical treatment can be an effective treatment for sexual violence victims. However, the symptoms may recur, so we need to observe and manage the patients continuously.

A meta-analysis of the effects of post-traumatic stress disorder intervention programs on firefighters (소방공무원의 외상 후 스트레스 장애 중재 프로그램 효과에 관한 메타분석)

  • Purpose: This study aimed to identify the effects of post-traumatic stress disorder intervention programs on firefighters through a meta-analysis. Methods: Seven studies were selected for meta-analysis. The overall average effect size, effect sizes for each intervention variables, and publication bias were analyzed using Comprehensive Meta-Analysis(CMA) version 2.0 (Biostat, Englewood, NJ, USA). Results: Post-traumatic stress disorder intervention programs were found to have a very low, not statistically significant effect (effect size=-.171, confidence interval=-0.361~0.019, Z=-1.768, p>.05). However, these programs was less than 50 (effect size=-.622). Conclusion: The results of this study suggest that further research on the effects of post-traumatic stress disorder intervention among firefighters is needed in order to develop more effective programs.

  • Korea has the highest suicide rate in the world, and firefighters have the highest suicide rate among the professional population. As a result of the study, post-traumatic stress disorder education is essential in all occupations. Post-traumatic stress disorder is the most common disease and the biggest obstacle for firefighters in Korea. Firefighters who have suffered physical and mental damage in many types of disaster sites due to their extreme occupations are rapidly increasing the suicide rate in recent years. In order to maximize the effect of job training, education on post-traumatic stress disorder from new employees to leadership training is urgently needed.

Life Stress, Social Support and Suicidal Ideation of North Korean Refugee Women in South Korea (탈북여성의 일상생활 스트레스가 자살생각에 미치는 영향과 사회적 지지의 조절효과)

  • The purpose of this study is to identify the moderating effect of social support between daily life stress and suicidal ideation of North Korean refugee women in South Korea. We survey with self-report questionnaires for 156 refugee women, conduct descriptive and regression analysis. As results, (1) almost half of the surveyed experience suicidal ideation in a year, (2) daily life stress affects suicidal ideation controlling with depression, PTSD, alcohol use, income, and age, (3) social support which North Korea refugee women perceive buffers the relationship between life stress and suicidal ideation. From these results, implications are suggested. Gender cognitive policies are needed in North Korea refugees in South Korea. Also, community mental health services and sustainable community program for North Korea refugee women are important to prevent their suicidal attempts.

The Relationships among Experiences of Traumatic Events, Post-traumatic Stress and the Needs for Health Promotion Programs of 119 Paramedics (119구급대원의 외상사건 경험, 외상후 스트레스와 건강증진 프로그램 요구도와의 관계)

  • Purpose: This study aims to examine the relationships among experiences of traumatic events, post-traumatic stress (PTS), and the needs for health promotion programs of 119 paramedics. Methods: Experiences of traumatic events, PTS, and needs for health promotion programs were measured using a structured questionnaire. The subjects were 193 paramedics in Jeju Island. Data were analyzed using descriptive statistics, t-test and $x^2$ test. Results: The most experienced event out of the 16 different traumatic events was 'retrieve a suicide's body.' A high-risk classification of PTS was observed in 36.2% of the subjects. The rates of experiences were higher than those of the actual needs for all 26 health promotion programs. The programs reported as highly needed by respondents were stress management (75.5%) followed by exercise (74.6%) and PTS management (72.5%). Subjects with longer career periods, a fire sergeant, perceived obesity, numerous experiences of traumatic events, and in the PTS risk group had a higher need for a stress management program. Conclusion: These findings indicate that a significant number of 119 paramedics experienced PTS. They also show that onsite strategic management is strongly required. In addition, implementation of health promotion programs based on the needs of 119 paramedics is highly needed.
