The study on outpatient-clinic practice by shift system at a hospital in Taegu (대구지역 한 중소병원의 교대제 근무에 의한 외래진료에 관한 연구)

  • Medical consumers(patients) want evening-clinic because of busy work. For patient's convenience and improving service, hospital should accept it. Considering payment system and patient's demand, personnel expenses, hospital can not accept. The practice of shift system to accept patient's demand and hospital's economic aspect was made. To analysis the effect of the system and probability to alternative to evening clinic this study was done. This study was composed of basal study, intervention, evaluation of effect. The basal study were composed of studying demand on evening clinic, the number of beds, doctors employee, the time table of practice and work, and the number of patients at arrival time. The intervention composed of changing of practice time, changing of working time by the number of patients at arrival time, increasing of employee. The evaluation of effect were composed of evaluating the number of patient at time, the effect of shift system, the comparison of the number of in and out patients and questionnairing the practice of shift system. In the practice time at 2 shift system First team works 7-15 hours and Second team 12-20 hours. there are no lunch and supper time. At 18-20 hours the number of patients were 25-30. The number of patient a depart were 6-7. The number of out-patient increase in 13% and inpatient increase in 10% before the system. Doctors(100%), employee(94.6%), and patients(86.4%) approved this system. The advantage of this system were utilization of surplus time, lengthen the practice time, even distribution of patients and shortening of waiting time, rapid treatment of emergent patients. The disadvantage of this system were shortage of manpower, not all depart practice, continuity of practice, no lunch and supper time, irregular rounding. At present because of small Demanding on evening clinic, this shift system was economical. To succeed this study more effectively all depart in hospital participate. But because of economical reason it is impossible for hospital to do it. If the government assist the economic loss that all depart participate in this system it is very helpful for hospital to succeed in implementing this system more early.

The Effects of the Revised Elderly Fixed Outpatient Copayment on the Health Utilization of the Elderly (노인외래정액제 개선이 고령층의 의료이용에 미친 영향)

  • Background: In January 2018, revised elderly fixed outpatient copayment for the elderly were implemented. When people ages 65 years and older receive outpatient treatment at clinic-level medical institutions (clinic, dental clinic, Korean medicine clinic), with medical expenses exceeding 15,000 won but not exceeding 25,000 won, their copayment rates have decreased differentially from 30%. This study aimed to examine the changes of health utilization of elderly after revised elderly fixed outpatient copayment. Methods: We used Korea health panel data from 2016 to 2018. The time period is divided into before and after the revised elderly fixed outpatient copayment. We conducted Poisson segmented regression to estimate the changes in outpatient utilization and inpatient utilization and conducted segmented regression to estimate the changes in medical expenses. Results: Immediately after the revised policy, the number of clinic and Korean medicine outpatient visits of medical expenses under 15,000 won decreased. But the number of clinic outpatient visits in the range of 15,000 to 20,000 won and Korean medicine clinic in the range of 20,000 to 25,000 won increased. Copayment in outpatient temporarily decreased. The inpatient admission rates and total medical expenses temporarily decreased but increased again. Conclusion: We confirmed the temporary increase in outpatient utilization in the medical expense segment with reduced copayment rates. And a temporary decrease in medical expenses followed by an increase again. To reduce the burden of medical expense among elderly in the long run, efforts to establish chronic disease management policies aimed at preventing disease occurrence and deterioration in advance need to continue.

  • The medicine development and upwardly raised standard of living brings out the increased dental needs at dental hospital OPD. The dental college hospital, follows up the raised dental needs, was subdivided by specialized dental office. And now a specialized dental office needs co-work with the other specialized dental office for special dental illness and treatment. The purpose of this study is to provide a proper area distribution and location of each specialized dental office and relationship between dental office, clinic and other department by analyzing the space, area and characteristics of dental hospital OPD. The results could be used for helping its users, of patients and dentists.

  • Introduction : 최근 의료기관 내 진료비 정산으로 인한 전산화사업이 적극 추진되면서 각 의료기관은 생성된 자료의 목적과는 무관하게 대량의 자료를 지니고 있으면서도 병원경영과 관련하게 자료를 활용하고 있지 못하다.Objectives : 병원 입원환자의 데이터를 분석하여 입원환자의 이탈요인을 파악하여 이탈을 방지하고 지속적인 의료서비스를 제공할 수 있도록 하고자 한다. Method : 본 연구는 일개병원을 퇴원환자를 대상으로 2006년 1월 1일~2007년 12월 31일 동안의 퇴원환자 데이터를 기반으로 연구되었다. 퇴원환자 38,359명의 병원데이터 베이스를 기반으로 동일 연구기간 외래 누적데이터 375,659건의 외래환자 데이터와 비교하여 퇴원환자 중 재입원 예약 및 외래진료 예약 부도여부를 비교하는 후향적 연구를 수행하였다. 2008년 1월 1일 기준 재입원 예약 및 외래진료 예약부도 발생여부를 기준으로 계속진료환자 그릅과 이탈환자그룹을 생성하였다. 중복을 제거한 총 대상은 3,503명이였다. Discussion :본 연구에서 얻어진 결과를 기초로 퇴원환자의 이탈률이 높은 환자그룹을 집중 관리하여 이탈을 방지하고 관심을 갖고 향후 잠재적 이탈 고객을 다시 유입할 수 있는 많은 방안들이 마련되어야 할 것이다.

  • 문제: 본원의 외래 상황은 진료의가 짧은 시간 내에 많은 환자를 진료해야 하고, 진료 환자 모두에게 충분히 만족하도록 설명해주는 것은 매우 어려운 실정이었다. 2007년의 의료기관 본 평가에 앞서 병원 자체 평가 결과 외래환자의 설명 만족도는 57점으로 저조하였다. 목적: "이해하기 쉽게 설명하는 서울대학교 병원" 이라는 슬로건 하에 설명간호사 제도를 운영하기 시작하여, 설명간호사실 운영 활성화를 통한 외래 환자 만족도를 향상시키고자 한다. 의료기관: 서울시 종로구에 소재한 대학병원 질 향상 활동: 설명간호사실 방문율 증가 및 one-stop service care을 실현할 수 있는 방안을 모색하였고, 검사 시행 이유, 귀가 후 주의사항 검사 결과, 검사일정 외래 진료 안내 브로셔 및 표준화된 교육 자료를 개발하였다. 또한 설명간호사의 역할 및 업무의 확립을 위한 업무기술서를 개발 및 설명간호사실 환경을 개선하였다. 개선효과: 설명간호사실의 방문율이 내과는 2배, 신경과는 3배 정도 증가하였다. 표준화된 교육 자료 개발로 설명 요구도를 충족시켰고, '환자의 알 권리'와 '설명을 들을 권리'를 보장하였다. 업무기술서 개발로 설명간호사의 업무 및 역할을 확립하였으며 설명간호사실의 환경 개선으로 개인 신상 비밀을 보호받을 권리를 보장하였다. 외래 설명만족도가 2007년 57점에서 2008년 74점으로 향상되었다. 설명간호사실 자체 설문조사에서도 97%가 만족한다고 답하였고, 100% 모두 재방문 의사를 보였다. one-stop service care를 제공함으로써 외래가 복잡하다는 부정적인 이미지를 긍정적인 이미지로 변화시켰다.

  • This study aims to decide optimized size of a dental care unit. The efficient treatment room makes its user so comfortable that treatment can be easier and faster. Therefore, an operating room is the most important part in the dental hospital. This study focused on the outpatient care unit of dental hospital. The hospital for the case study is selected recently rebuilt dental university hospital. This study is to survey the behavior of all user in the sector and treatment unit. As the result, optimized plan type and size for each cases are proposed.

Analysis of the Continuity of Outpatient among Adult Patients with hypertension and its Influential Factors in Korea (우리나라 성인 고혈압환자의 외래진료 지속성과 이에 영향을 미치는 요인 분석)

  • The administration data of the national health insurance and health insurance bills were utilized in this study. The data of 485,953 patients who were at the age of 30 and up and used the out-patient departments of every medical institution located in some regions involving two southern and northern provinces once or more during a 184-day period from July to December, 2008. As a result of analyzing their Continuity of Ambulatory Care and factors affecting it, the following findings were given: The continuity of ambulatory care among the adult patients with hypertension in our country turned out to be on a high level(MMCI $0.96{\pm}0.13$, MFPC $0.96{\pm}0.12$). Given examining the outpatient medical-cure continuity level according to index, the averagely medical-cure continuity level was calculated to be high level with MMCI, $0.96{\pm}0.13$, and MFPC $0.96{\pm}0.12$. Thus, the tendency of visiting only one medical provider was high. The findings of the study illustrated that the average continuity of ambulatory care among the adult patients with hypertension in our country was on a high level, and it seemed that special care should be provided to patients with a low-level continuity of ambulatory care, such as women and elderly people aged 64 and over. The findings of the study are expected to serve as one of the barometers for the health care of patients with hypertension and for the performance of national hypertension management plans.

  • 현재 대구가톨릭대학교 의무부총장인 채영희 요셉 신부가 가톨릭피부과의원 제3대 원장으로 취임하였을 때, 한센인을 위해 33년을 봉사 헌신하신 전임 엠마 프라이싱거 원장의 업적과 정신을 유지, 발전시키기 위하여, 나균으로 인해 말초신경의 감각손실과 손, 발, 얼굴 등의 근육위축으로 인해 일반병원에서는 편히 진료 볼 수 없는 한센환우들의 불편한 점을 개선하기 위해 칠곡가톨릭병원을 개원하게 되었다. 한센환우들을 위하여 칠곡가톨릭병원 모든 진료과목 의료진은 매주 금요일 외래진료를 제공하고 있으며, 입원환우는 필요시 수시로 진료를 해 드리고 있다. 이와 같이 칠곡가톨릭병원과 가톨릭피부과의원은 병원 설립목적 달성을 위해 관심과 배려를 기울이고 있다. 칠곡가톨릭병원과 가톨릭피부과의원은 지역민의 질병을 단순히 치료하는 것 뿐만 아니라 지역민의 건강유지 및 향상 그리고 지속적인 관리를 통하여 진정으로 지역민의 건강 증진에 도용이 되기 위하여 노력하고 있으며, 시랑과 봉사의 정신으로 지역민을 위한 병 의원으로 거듭나고 있다.
