A series of 25 patients with residual, recurrent, and unresectable gastric cancer received various combination of surgery, radiotherapy (RT), chemotherapy (CT), and hyperthermia (HT). They were placed into 7 categories; 1) CT and HT-14 patients; 2) RT and HT-15 patients; 3) surgery, RT and HT-2 patients; 4) surgery, RT, HT and CT-1 patient; 5) RT, HT and CT -1 patient; 6) RT and CT-1 patient; 7) RT alone-1 patient. Three patients had curative resection. 21 patients received irradiation with tightly contoured portals to spare as much small bowel, kidney and marrow as possible. Hyperthermia was applied regionally once or twice a week for 23 patients using 8 MHz radiofrequency capacitive heating device (Thermotron RF-8). HT was given approximately 30 min after RT 7 patients were treated with CT: 4 patients received HT and concomitant Mitomycin-C; 3 patients received HT and sequential 5-FU+Adriamycin+Mitomycin-C. There was not any treatment related deaths. There was also no evidence of treatment related problems with liver, kidney, stomach, or spinal cord except only one case of transient diabetic ketoacidosis. The tumor response was evaluable in 22 patients. None achieved complete remission.11 ($50\%$) achieved partial remission. The response rate was correlated with total radiation dose and achieved maximum temperature. 9 of 14 ($64\%$) received more than 4000 cGy showed partial remission; especially, all 3 patients received more than 5500 cGy achieved partial response.8 of the 12 patients ($67\%$) who achieved maximal temperature more than $41^{\circ}C$ showed partial response in comparing with $25\%$ (2 of 8 patients, below $41^{\circ}C$). The numbers of HT, however, was not correlated with the response. 3 of the 25 patients ($12\%$) remain alive. The one who was surgically unresectable and underwent irradiation alone is in progression of the disease with distant metastases. The remaining two patients with curative resection are alive with free of disease, 24 and 35 months, respectively. The median survival by response are 11.5 months in responders and 4.6 months in non-responders.
Nowadays, laserthermia is widely used to treat malignant tumors with generating heat as the one of minimal invasive surgeries. Generally, the laserthermia probe system consists of the fiber-optic laser and light guides, image guide and temperature sensor. It is very important to measure the temperature of treating tumor and make a stable temperature ($42{\sim}43^{\circ}C$) during the treating time. Therefore, laserthermia probe needs temperature sensor which can measure it exactly and fast. In this study, to develop a new type of temperature sensor with LC(liquid crystal) and optical fiber, the reflectivity of LC according to the temperature changes are measured. Also, the relationships are derived from the results.
Objectives : The purpose of this paper is to establish theoretical bases upon which to complement the clinical evidence in representative literature "Dongeuibokam(東醫寶鑑)" of Korean medicine. Methods: We searched applicable paragraphs about thermotherapy in "Dongeuibokam(東醫寶鑑)" and defined them as historical and theoretical bases of thermotherapy in Korean medicine. Results : Three paragraphs about thermotherapy are in "Yehyung(外形篇)" and "Jabbyung(雜病篇)" of "Dongeuibokam(東醫寶鑑)". These records to treat the disease of the internal organs and meridian are enough to explain historical and theoretical evidences. Conclusions : Although only three paragraphs are associated with thermotherapy, they mean that thermotherapy has been explained under the system of Korean medicine. It is necessary for more literatural study and clinical trials to be carried out to secure the evidence of physical therapies in Korean medicine.

To investigate the effects of self-management programs for chronic low back pain (LBP), 63 subjects were assigned to three groups; self-exercise group (SEG), hot pack and low-frequency electrical stimulation group (HEG), and thermo-massage group (TMG). Parameters were the pain numeric rating scale (PNRS), Oswestry Disability Index (ODI), Roland Morris Disability Questionnaire (RMDQ) and Relapse frequency (RF). PNRS, ODI and RMDQ of SEG and TMG sustained effectiveness, however, PNRS, which improved after treatment in HEG, worsened in 6 month. Between the groups, all parameters were better in SEG and TMG compare to HEG. Exercise and thermo-massage can be considered as useful self-management performed at home to prevent the relapse of chronic LBP.
The renewed interest in the use of hyperthermia in cancer therapy is based on radiobiological and clinical evidence indicating that there may be significant thereapeutic advantages with the use of hyperthermia alone or combined with irradiation plus heat. Authors performed the experiment using the chemically induced mammary carcinoma of rats to observe the difference in temperature changes between tumor and normal tissue during heat, and to compare the response of the tumors to radiation alone and to radiation plus hyperthermia. The results were as follows 1. Temperature of tumors was significantly higher than in the normal tissue during heating and the difference was about $1.5^{\circ}C$. 2. $TCD_{50}$ in radiation alone and hyperthermia immediately following radiation was 1,282 rad and 795 rad, respectively and TER value was 1.81.
Purpose: The purpose of this study was to identify how heat therapy influences pain and sleep in rural elders who have osteoarthritis (OA). Method: A non-equivalent control group pretest-posttest design was used. Participants were 61 elderly farmers and they were assigned to an experimental group (31) or control group (30). The participants were living within the district boundaries of a public health clinic in North Choong Chung Province. The research was conducted from April 1, to 28, 2006. Results: The results of this study demonstrate that heat therapy is effective in reducing pain and promoting sleep in rural elders who have osteoarthritis. Conclusion: Heat therapy reduced pain, and improved sleep in rural elders, with osteoarthritis.
An interstitial radiofrequency needle electrode system was constructed for interstitial heating of brain tissue. Radiofrequency electrodes with Thermotron RF 8 were tested in an agar phantom and in a normal canine brain to determine how variations in physical factors affected temperature distributions. Temperature distributions were checked after heating with 1 mm diameter needle electrode implants on the corners of 1 and 2 cm squares in a phantom and plot isotherms for various electrodes arrangement. We observed that the 1 cm square array would heat a volume with a 1.25 cm radius circular field cross section to therapeutic temperatures ($90\%$ relative SAR using Tm) and the 2 cm square array with a 1.75 cm radius rectangular field with central inhomogeneity. With 2 cm long electrode implants, we observed that the 1 cm square array would heat a 3 cm long sagittal section to therapeutic temperature ($90\%$ relative SAR using Tm). We found that radiofrequency electrodes could be selected to match the length of the heating area without affecting its performance. The histopathological changes associated with RF heating of normal canine brains have been correlated with thermal distributions. RF needle electrode heating was applied for 50min to generate tissue temperatures of $43^{\circ}C$. We obtained a quarter of the heated tissue material immediately after heating and sacrificed at intervals from $7\sim30$days. The acute stage (immediately after heating) was demonstrated by liquefactive necrosis, pyknosis of neuronal element in the gray matter and by some polymer-phonuclear leukocytes infiltration. The appearance of lipid-laden macrophages surrounding the area of liquefaction necrosis was demonstrated in all three sacrificed dogs. Mild gliosis occurring around the necrosis was demonstrated in the last sacrificed (Days 30) canine brain.
Muscle stiffness, pain and discomfort in dally activities are cardinal symtoms on arthritis. To reduce these symtoms, a nonequivalent one-group pre & posttest experiment was carried out at a rural community health post area in Chonbuk province during eight weeks (from 25th of January to 20th of March, 1999). The subjects were sixteen rural old women(65-89yrs) with osteoarthritis. And to reduce the level of pain, discomfort in daily living and to increase that of life satisfaction, 30 minutes of flexibility exercise followed by $15{\sim}20\;minutes$ of local heat therapy were done three times a week. Pain scale from Arthritis Impact Measurement Scales developed by Meenan(1980) for pain, Jette's Daily Activity Discomfort Scales revised by Lee(1988) for discomfort and Life Satisfaction Scales developed by Youn(1982) for life satisfaction were used as research instruments. The data were collected by face-to-face interviews with Investigator and the experiments were done under demonstration and supervision of investigator in community health post. The data were analyzed in percentage and Wilcoxon sign rank sum test by SPSS/PC+ program. The result could be summarized as follows ; 1. The scores of experiment group who were applied hot bag followed by flexibility exercise were tended to be significant lower in the level of pain statistically(Z=4.949, P=0.0001). 2. The scores of experiment group who was applied hot bag followed by flexibility exercise were tended to be significant lower in the level of discomfort in daily living statistically(Z=2.7226, P=0.0279). 3. The scores of experiment group who was applied hot bag followed by flexibility exercise were tended to be higher in the level of life satisfaction, but there was no significant statistical difference(Z=-1.2087, P=0.2171). In conclusion, flexibility exercise with local heat therapy could be identified as an effective therapy to reduce pain and discomfort in daily living of the rural elderly with osteoarthritis. And as a community health practitioner, the investigator concluded that this exercise with local heat therapy could be developed as a health promotion program for the rural aged people.
From January 1985 to September 1990, 7 patients with carcinoma of the extrahepatic biliary system received external radiaiton therapy combined with hyperthermia. Of the 3 patients with extrahepatic bile duct cancer, two were primary cholangiocarcinoma and one was metastatic peripancreatic carcinoma. Of the 4 patients with carcinoma of the gallbladder, two were locor-egionally advanced and unresectable carcinoma and the remaining two were local-regional recurrence after cholecystectomy. They were all pathologicallly proven adenocarcinoma. The radiation dose received ranged from 3000 cGy/2weeks to 5040 cGy/7 weeks. The hyperthermia was done once or twice a week and 4 to 12 sessions in total. The tumor response was confirmed by T-tube cholangiography, percutaneous transhepatic cholangiography and CT scan. 6 out of 7 ($86\%$) showed partial regression of the tumor. The median survival time was 7 months (range $4\~11$ months).6 out of 7 patients were dead: one died of septicemia, 4 of primary disease, one of distant metastases. Only one out of 7 patients is still alive but new metastatic lesion was found. There was not any treatment related deaths. There was also no evidence of treatment related problems with liver, stomach and duodenum, although the observation period was short.

We fabricated flexible thermoradiotherapy probes to alternated combination with Interstitial hyperthermia and Brachyradiotherapy thermoradiotherapy probe was coated by gold plate on polyethylene brachytherapy probe. When Agar phantom was heated 15 minute with 30 W radiofrequency power, temperature increased as 5oC for polyethylene probe and 20oC for gold coated polyethylene probe. We observed that the 1 cm square array would heat a volume with a 1.25 cm radius circular field cross section to therapeutic temperatures (90% relative SAR using Tm) and the 2 cm square array with a 1.75 cm radius rectangular field with central inhomogeneity.
