FIBROMA ON THE PALATE : A CASE REPORT (구개의 섬유종 : 증례보고)

  • The fibroma is the most common benign soft-tissue tumor found in the oral cavity and most commonly occurrs in sites predisposed to irritation or trauma. Treatment is conservative surgical excision. Seldom does the lesion recur. A 14-year old girl was referred to the Department of Pediatric Dentistry for evaluation and management of a mass on the palate. The patient's parents reported the slow growth of this soft tissue lesion over a 6-month period. A history of trauma or irritable habits was denied. Clinical examination revealed an asymptomatic, soft, pedunculated elevation of mucosa, measuring 3cm in diameter. Excisional biopsy of the lesion supported a diagnosis of fibroma. There have been no signs of recurrence during follow up of 4 years after surgical intervention. The clinical picture, pathology and management of the case are described, and the differential diagnosis and treatment are discussed.

  • 레이저가 의학영역에서 사용된 후 구강내 연조직과 경조직 병소에 대한 임상적 적용에 관한 관심이 커져왔다. 레이저가 조사되는 부위에서는 대부분 연기가 발생한다. 이 경우 병원체가 존재하는 병소에 대한 레이저치료시 발생되는 연기속에 병원체가 포함되어 이Tekas 환자와 술자는 이의 흡입가능성이 크므로 균혈증 및 상기도나 호흡계의 의원성 감염이 유발될 수 있다. 저자는 레이저치료시 발생되는 연기속에 병원체가 포함되어 있는지의 여부를 규명하기위해 각각 10개의 실험군에 통기성 미생물인 Escherichia coli(E.coli)가 배양된 brain geart infusion (BHI) 배지와 E.coli 가 존재하는 치근단 병소를 실험적으로 만든 후 각각 pulsed Nd:YAG 레이저를 조사해 연기를 채취, 배양, 분석하여 다음과 같은 결과를 얻었다. 1. Pulsed Nd:YAG 레이저를 E. colir 배양된 agar substrate 와 실험적으로 만들어진 치근단부위의 E.coli 현탁액에 조사시 발생된 연기에서 처음 접종된 것과 같은 E.coli가 검출되었다. 2. 감염병소에 pulsed Nd:YAG 레이저를 조사시 발생되는 연기의 유해효과로부터 환자와 술자 및 보조자의 보호를 위해 효과적인 흡입기를 사용이 권장된다.

  • Ameloblastic fibro-odontoma is a rare benign odontogenic tumor that mainly occurs at mandibular molar area. Radiography usually shows a well-defined radiolucent area containing various amounts of radiopaque material of irregular size and form. In histiologic aspect, Ameloblastic fibro-odontoma composed of connective tissue characteristic of an ameloblastic fibroma and calcined tissue identifying the tumor as a complex odontoma. Thirteen years two months aged boy of this case visited with chief complaint of delayed eruption of mandibular left canine. After enucleation of lesion, biopsy was done and diagnosed by ameloblastic fibro-odontoma. In continous follow-up check, canine erupted normally after operation.

  • Peripheral giant cell granulomas (PGCGs) are reactive, exophytic gingival growths, caused by regional irritation and chronic trauma. PGCGs are diagnosed through histopathologic evaluations and appear analogous to other soft tissue lesions. This report presents the case of a PGCG associated with the ectopic eruption of a maxillary central incisor. Following an excisional biopsy, the patient healed fully without recurrence for at least 1 year.

Benign Tumor-Like Developmental Salivary Gland Defect (양성 종양과 유사한 형태를 가진 발육성 타액선 골결손)

  • 50-year-old female and 50-year-old male were referred to the department of the oral and maxillofacial surgery of Kyungpook national university dental hospital with asymptomatic lesions on their posterior mandibular body areas. They were discovered incidentally on panoramic radiographs during routine dental examination. Physical examination revealed no remarkable findings. Each panoramic radiograph showed well defined radiolucent lesions without hyperostotic border on their posterior mandibular body area. At first they were diagnosed as benign tumors because they looked like multilocular pattern and one of the patient showed discontinuity of mandibular canal within the lesion. CT scans demonstrated well demarcated and irregular lingual depression filled with fat tissue and they were diagnosed as developmental salivary gland defects. One of the lesion showed no change on follow-up panoramic radiograph after 4 months. Developmental salivary gland defects resembling benign tumor are atypical cases and it is suggested that confirmatory imaging using CT or MRI should be taken.

Failure Pattern of Pineal and Ectopic Pineal Germ Cell Tumor after Gamma Knife Radiosurgery (송과선 및 이소성 송과선 생식세포종의 감마 나이프 수술 후 재발 양상)

  • Purpose :This study was performed to determine the optimal treatment velum of Patients treating with radiation therapy for intracranial germ cell tumor. Materials and Methods : From 1993 to 1998, 19 patients with intracranial germ cell tumors treated by gamma knife radiosurgery were analyzed. The location of tumor was as follows; 9 cases on pineal region, 1 case on suprasellar region, and 9 cases of multiple lesion. 7 patients were pathologically verified; 5 cases of germ cell tumor and 2 cases of non germinomatous germ cell tumor. Tumor volume was ranged from 2.4 cm$^{3}$ to 74 cm$^{3}$. Irradiation dose was 10 Gy to 20 Gy with 50% isodose curve. Follow up period was 10 months to 54 months. Results : Recurrences were observed in 14 cases among 19 (74%) patients. Complete remission and partial remission were achieved in 2 (11%) and 10 (53%) respectively. No response was observed in 7 (36%). 2 cases were recurred within original tumor bed. 6 cases were recurred beyond but contiguous with tumor bed. Ventricular relapses separated from pretreatment tumor bed were 3. Spinal recurrences were 4. Among 8 recurred cases of which tumor volume is smaller than 20 cm$^{3}$, 2 were recurred within original tumor bed, 4 were recurred beyond but contiguous with tumor bed, and 1 spinal recurrence. Meanwhile, 6 cases of which tumor volume larger than 20 cm3, 1 case was recurred beyond but contiguous with tumorbed, 2 ventricular recurrences separated with original tumor bed, and 3 spinal recurrences. 5 cases which did not show any recurrence sign showed characteristics of single lesion, tumor volume smaller than 20 cm$^{3}$ and normal tumor marker. All of 4 cases of spinal recurrences happened in the case having ventricular invasion or lesion. Among 9 cases having multiple lesion, only 3 cases recurred within original tumor bed or around tumor bed, the other 6 cases recurred separated from pretreatment tumor bed. Conclusion : Gamma knife radiosurgery is not recommended for the treatment of intracranial germ cell tumor. It is because of small treatment volume and inadequate radiation dose that are characteristics of gamma knife radiosurgery. Tumor volume, ventricular invasion or ventricular lesion in multiple lesion are important factors to be considered for the wide field radiation therapy Tumor volume smaller than 20 cm$^{3}$, single lesion, no ventricular lesion or invasion, and normal tumor marker are ideal indications for small involved field radiation therapy. Prophylactic spinal irradiation seems to be necessary when there is ventricular lesion, ventricular invasion, and multiple lesions. When the tumor volume is larger than 20 cm$^{3}$, multiple lesions, abnormal tumor marker, and whole ventricular irradiation or partial brain irradiation would be possible and neoadjuvant chemotherapy would be most beneficial in these group.

  • Carbondioxide($CO_2$) laser is one of the most widely employed lasers in oral soft tissue surgery because of its excellent affinity for water based soft tissues. It has some inherent advantages such as hemostasis, less postoperative swelling, reduction of bacterial population at surgical site, less need for suturing, less scarring, and less postoperative pain compared to conventional surgical therapies including the use of scalpel, diathermy, cryotherapy and electrosurgery. A 30-years-old male was presented with gingival swelling. Clinical examination revealed a well?defined pedunculated fibrotic mass on the buccal gingiva near right maxillary 1st molar. In radiographic examination, no remarkable abnormality was seen. Excisional biopsy was performed with $CO_2$ laser (continuous wave mode, 4.0W). Histological diagnosis was "Irritation fibroma". $CO_2$ laser has advantages those are suitable for surgical treatment of intraoral lesion. If appropriate training and experience are provided, the dentist would be able to manage intraoral lesions more efficiently and successfully with the use of the $CO_2$ laser.

Air Cavity Effects on the Absorbed Dose for 4-, 6- and 10-MV X-ray Beams : Larynx Model (4-, 6-, 10-MV X-선원에서 공기동이 흡수선량에 미치는 효과 : 후두모형)

  • Purpose : When an x-ray beam of small field size is irradiated to target area containing an air cavity, such as larynx, the underdosing effect is observed in the region near the interfaces of air and soft tissue. With a larynx model, air cavity embedded in tissue-equivalent material, this study is intonded for examining Parameters, such as beam quality, field size, and cavity size, to affect the dose distribution near the air cavity. Materials and Methods : Three x-rar beams, 4-, 6- and 10-MV, were employed to Perform a measurement using a 2cm $(width){\times}L$ (length in cm, one side of x-ray field used 2cm (height) air cavity in the simulated larynx. A thin window parallel-plate chamber connected to an electrometer was used for a dosimetry system. A ratio of the dose at various distances from the cavity-tissue interface to the dose at the same points in a homogeneous Phantom (ebservedlexpected ratio, O/E) normalized buildup curves, and ratio of distal surface dose to dose at the maximum buildup depth were examined for various field sizes. Measurement for cavity size effect was performed by varying the height (Z) of the air cavity with the width kept constant for several field sizes. Results : No underdosing effect for 4-MV beam for fields larger than $5cm\times5cm$ was found For both 6- and 10-MV beams, the underdosing portion of the larynx at the distal surface was seen to occur for small fields, $4cm\times4cm\;and\;5cm\times5cm$. The underdosed tissue was increased in its volume with beam energy even for similar surface doses. The relative distal surface dose to maximum dose was changed to 0.99 from 0.95, 0.92, and 0.91 for 4-, 6-, and 10-MV, respectively, with increasing field size, $4cm\times4cm\;to\;8cm\times8cm$, For 6- and 10-MV beams, the dose at the surface of the cavity is measured less than the predicted by about two and three percent. respectively. but decrease was found for 4-MV beam for $5cm\times5cm$ field. For the $4cm\timesL\timesZ$ (height in cm). varying depth from 0.0 to 4.8cm, cavity, O/E> 1.0 was observed regardless of the cavity size for any field larger than about $8cm\times8cm$. Conclusion : The magnitude of underdosing depends on beam energy, field size. and cavity size for the larynx model. Based on the result of the study. caution must be used when a small field of a high quality x-ray beam is irradiated to regions including air cavities. and especially the region where the tumor extends to the surface. Low quality beam. such as. 4-MV x-ray, and larger fields can be used preferably to reduce the risk of underdosing, local failure. In the case of high quality beams such as 6- and 10-MV x-rays, however. an additional boost field is recommended to add for the compensation of the underdosing region when a typically used treatment field. $8cm\times8cm$, is employed.

DESMOPLASTIC FIBROMA OF THE MANDIBLE IN A CHILDREN : A CASE REPORT (어린이의 하악골에 발생한 결합조직형성 섬유종(Desmoplastic fibroma) 1예)

  • This case describes a case of desmoplastic fibroma of the mandible. A 9-year-old boy was seen with a history of progressive swelling and expansion of the left mandible for one month period. Desmoplastic fibroma was diagnosed on histopathologic examination This report reviews the diagnostic criteria, differential diagnosis and surgical treatment of choice in brief of this uncommon primary bone tumor of the oral and maxillofacial region.

CASE REPORT OF PILOMAIRICOMA (Pilomatricoma 환아에 대한 증례 보고)

  • Pilomatricoma is a benign soft tissue tumor of hair follicle origin. They occur most commonly in the head and neck region and are usually found in girls during the first two decades of life. Although malignant transformation has been described, it is exceedingly rare. The clinical presentation is typically that of an asymptomatic, superfical, solitary, firm mass that is often accompanied by a reddish-blue discoloration of the overlying skin. Histopathologically pilomatricoma are seen as epithelial islands embedded in a cellular stroma. The epithelial component consist of two main cell types : basaloid cells and ghost cells. Intracellular and stromal calcification is reported in many cases. Diagnosis is usually suspected based on palpation of a superficial, rock-hard mass and confirmed by histopathologic examination. Surgical excision is both curative and the treatment of choice. Recurrence is rare. In this case, the radiographs showed a dense calcification, measuring about 1cm diameter. So simple excision was peformed intraorally, and resultantly the lesion was proved to be a pilomatricoma by histological examination.
