TEMPERAMENTAL CHARACTERISTICS OF KOREAN CHILDREN WITH COMMUNICATION DISORDERS (한국 의사 소통 장애 아동의 기질 특성)

  • Objectives:One of the most common developmental problems is communication disorder in which a child appears normal in every way but who has failed to begin speaking or speaks very little. A few studies have examined the temperamental characteristics of children with communication disorders. This study was to investigate the temperamental characteristics of Korean children with communication disorders. Methods:The parents of 20 Korean children with communication disorders and the parents of 50 normal control children, the age of both groups ranges from 3 to 7, completed Korean version of Parental Temperamental Questionnaire developed by Thomas and Chess. Children with a pervasive developmental disorder, mental retardation, or speech-motor or sensory deficit were excluded. The scores of each temperamental scale of two groups and the diagnostic clusters of two groups were compared. Results:The children with communication disorders were characterized by lower mood scores and higher intensity of reaction scores than normal controls. The two groups showed no significant correlation in terms of the temperamental diagnostic clusters. Conclusion:This findings suggest the existence of a distinct temperamental profile of the children with communication disorders. Early detection of the profile may be of great value for parents in understanding the developmental characteristics of the children with communication disorders and in providing appropriate parenting approaches.

Relationships between Parental Support and Monitoring and School Adjustment in Adolescents from Multicultural Families: Mediating Role of Ego-Resilience (부모의 지지 및 감독이 다문화 청소년의 학교생활적응에 미치는 영향: 자아탄력성의 매개효과)

  • The present study examined the direct effects of parental support and monitoring on multicultural adolescents' school adjustment and the mediating role of ego-resilience in the relationships, under the condition of controlling foreign mothers' Korean language abilities. We used data from 1,325 1st grade middle school students from multicultural families, who participated in the Multicultural Adolescent Panel Survey(MAPS) administered by the National Youth Policy Institute(NYPI). Structural equation modeling analysis revealed the following: parental support and monitoring directly and positively related with multicultural adolescents' school adjustment, and the direct effect of parental support on the school adjustment of multicultural adolescents was greater than that of parental monitoring. In addition, the adolescents' ego-resilience partially mediated the relationships between parental support and monitoring, and their school adjustment. Finally, we recommended the roles of parent education and home economics education in supporting school adjustment and elevating ego-resilience in multicultural adolescents.

  • 한국여성은 전통적으로 대가족제도 속에서 육아 및 가사활동에만 종사해왔다. 그러나 산업화 및 사회구조의 변화로 여성들도 교육의 기회를 갖게 되었으며 전통적인 역할 수행에 대한 가치변화와 함께 여성의 사회적 역할을 필요로 하게 되었다. 반면에 가족 형태가 핵가족화 함으로서 가족 내에서의 자녀양육을 비롯한 가정적 역할이 더욱 중요시 되게 되었다. 오늘날 한국사회는 여성에게 현재의 사회구조와 핵가족 속에서는 시간적으로 동시에 수행할 수 없는 두 가지 상반된 역할을 강조하고 있는 것이다. 이러한 상반된 역할사이에서는 여성은 갈등과 좌절감을 느끼게 되며 이중적인 부담 속에서 생활하게 되었다. 본 원고에서는 전통적인 한국의 가족가치관을 살펴보고 건강관리 측면에서 여성의 역할을 살펴보고 건강관리 측면에서 여성의 역할을 살펴보고자 한다. 전통적 가족가치관과 여성-우리나라의 전통적 가족은 부계 중심의 혈연 계승을 중요시하는 가부장적 대가족제도라 할 수 있다. 따라서 부계 계승을 통한 가족의 영속성과 가 중심사상에 기반을 둔 철저한 가족주의적 가치관이 전통적 사회를 지배하여 왔다. 그러므로 자연히 개인보다 가족집단이 우의적인 지위에 있을 뿐만 아니라 모든 행동의 결정에도 중요한 준거 집단이 되었다. 이러한 가의 영속 및 번영을 가장 효과적으로 수행하기 위해서는 많은 자녀를 필요로 했으며 부계중심 가족에서 자연히 남아 선호사상이 강할 수밖에 없었으며 이것은 조상에 대한 의무요 책임이라 생각했다. 이러한 가부장권의 확대에 반비례해서 가정 내에서 여성의 지위와 역할은 축소되어갔다. 여성들의 절대적인 예속을 필요로 하여 삼종지도니 칠거지악이니 불경이부등의 도덕률을 만들었으며 여성들 스스로가 이러한 정절과 복종을 미덕으로 생각하도록 교육받음으로서 여성들 자신이 자기희생의 굴레 속에서 인내와 복종의 생활을 운명처럼 받아들이게 되었으며 남편과 자식을 위해서는 목숨까지도 희생하게끔 철저히 사회화되었던 것이다. 그러나 가족 제도 안에서의 남녀의 지위는 동위 항렬 내에서만 해당되고 항렬을 달리할 때는 삼종지도의 이론에 부합된다. 어머니로서의 존장련이 인정되어서 가정 내에서의 여성의 종속적인 지위에 비하여 모의 권한은 절대적이었다. 상례와 제례에서 어머니와 아버지에 대한 의식에는 차이가 없으며 내외 명부제도에 의해서 부인도 남편과 똑같은 대우를 받도록 되어있다. 이러한 존장련에 의한 모의 권리와 더불어 부부유별에 의해서 가사권의 독자적인 결정권도 인정되고 있었다. 건강관리 측면에서 여성의 역할- 전통적으로 건강관리에 관련된 한국여성의 역할은 1. 씨받이로서의 역할로 생명을 잉태하도록 돕고 건강한 아이의 수태를 위해 태교에 힘썼으며 2. 자녀의 의식주를 해결하는 가사 역할만을 담당하는 전통적인 여성의 역할만을 수행하였으며 출산한 생명을 건강하게 자라도록 건강관리를 철저히 하였으나 체계적인 건강관리는 되지 못하였으며 특히 식생활에 유의하였으나 정서, 사회면은 도외시 한 과잉보호현상이었다. 3. 결혼 후에는 남편의 건강관리를 위해 철저하였으며 특히 식생활에 유의하였고 정서적으로 부담을 주지 않도록 유념하였다. 4. 또한, 임종시 평안한 죽음을 맞도록 도왔다. 전통적으로 한국여성의 역할은 돕는 역할이었다.

PSYCHOPATHOLOGY IN ADOLESCENTS WITH PARENT ABUSE (부모학대 청소년의 정신병리)

  • Aim:We think that the most important etiology in parent abuse is the psychodynamic and psychopathology in the family. So, we investigated the adolescents being admitted in SNMH, whose chief complaints were parent abuse. We were trying to explore families psychodynamic and psychopathology, especially mother-child interaction and to differentiate them in according to developmental psychopathology. Method:Our objects were the adolescent patients admitted in SNMH from 1987 to 1997 because they attacked parents verbally and physically. We examined 21 adolescents except those with psychosis, organic mental disorder, autism and mental retardation by means of interview or chart review. Result and Conclusion:The number of male patients was 14 and the number of female patients was 7. The most common diagnosis was conduct disorder and borderline personality disorder. The mean age was in the mid-teens. We observed 4 subgroups that were divided developmentally in object relation. 1) Symbiotic group with mother:(1) They did not separate and remain in symbiotic relationships with their mothers based on insecure attachment. Fathers were abscent emotionally and physically, and their mothers were prominent in close relationships with the patients in their family , where as the patients were the only man in the family. Adolescents entered the second separation-individuation. They expressed anger and internal tension involved with the close attachment with their mothers and also attempted separation from their mothers through physically attacking them. (2) These patients had suffered from physical illness and developmental delay since birth. Therefore the parents overprotected their children. The children had persistent infantile omnipotence and fantasies of power, so they could not deal with unrealistic states, adapt to reality, and depended on their parents overtly. They easily acted out unless their demands were fulfilled. 2) Borderline personality disorder:We observed deficiencies in care taking. Their parents had personality problems and immaturity. They coulden’t help their children to be separated in the rapproachment phase. Their conflict about dependence-independence was revived in the second separation-individuation adolescent period. We understand parent abuse as an attempt to overcome the conflict. 3) Conduct disorder:They did not build up basic attachment with their parents. They think of their parents as only a means of fulfilling their needs. When patients’ need were not fulfilled and remained in a conflicted state, they attacked their parents, unable to control their aggressions and impulses.

Depression, Anxiety, Alexithymia, Stress Response in Caregivers of Attention Deficit Hyperactivity Disorder Patient (주의력결핍 과잉행동장애 환아 보호자의 우울, 불안, 감정표현불능, 스트레스반응)

  • Objectives : This study was designed to investigate depression, anxiety, alexithymia, stress res ponses in caregivers of patients with attention deficit hyperactivity disorder. Methods : The subjects were 38 attention deficit hyperactivity disorder patients caregivers(38 women, mean age $37.5{\pm}6.5$). Patients were diagnosed with DSM-IV ADHD criteria. Korean version of Beck Depression Inventory(BDI), State and Trait Anxiety Inventory(STAI), Toronto Alexithymia Scale(TAS) and Stress Response Inventory(SRI) were used for assessment. Results 1) The BDI scores of ADHD patients caregiver group were significantly higher than control group$(16.4{\pm}7.1\;vs.\;10.9{\pm}5.5)(p=0.011)$. 7 of the 38 caregivers(18.4%) and none of control group(0%) had BDI scores over 20 points(p=0.021). Calculated relative risk for ADHD in the presence of caregivers' depression was 1.516 overall(95% confidence interval, 1.234-1.862). 2) In ADHD patient's caregiver group, the scores of Stress Response Inventory were significantly higher than control group$(44.2{\pm}20.2\;vs.\;26.5{\pm}16.8)(p=0.006)$. 3) No significant differences were found in the score of STAI, SIAIS, SIAI-T, TAS between caregiver and control group. Conclusion : This study suggest that ADHD patients' caregivers are likely to have more depressive symptoms and higher stress response level than control group. We propose that physicians should consider integrated approaches for caregiver's psychopathology in the management of ADHD.

A Study on the Relationship with Maternal Beliefs on the Child Rearing and Children's Self-perceptions (양육행동에 대한 어머니 신념과 아동의 자기능력 지각과의 관계)

  • The purpose of this study was to investigate the relationships between mother's beliefs on child rearing and children's self-perceptions. The subject of this study were comprised of 167 children aged 8-10 in Iksan & Kunsan city from Dec. 2 to Dec. 8, 2000. The children's self-perceptions were assessed by the self perceptions profile for children. Mother's beliefs questionnaire, developed by Okagaki and Sternberg, was used to obtain the data on mother's beliefs of parenting. The methods of analyses included basic descriptive categorical analysis as well as t-test, one way ANOVA-test, and multiple regression analyses. The major findings from the analyses are as follows: First, a significant difference was found in the degree of mother's beliefs about child rearing by mother's religion(p<.01), mother's education(p<.05), income(p<.001), level of life (p<.0l), mother's job, mother and fathers' agreement of child rearing(p<.05), relationship between mother and child(p<.05) in independence but a significant difference was not found in the degree of mother's beliefs about child rearing in accommodation. Second, the levels of children's scholastic performance were related to higher levels of mother's education(p<.001), mother's income(p<.001), mother and fathers' agreement of child rearing(p<.01). The levels of children's social competence was related to higher, child's gender (p<.01); girls were higher than boys. The levels of children's athletic abilities were not significant. The levels of children's physical appearance was related to higher levels of mother's education(p<.01), mother's income(p<.01), level of life(p<.05), mother's marriage(not divorce)(p<.01). The levels of children's behavioral conduct were related to higher levels of child's gender (p<.05), mother's education(p<.01), mother's income (p<.01), relationship between father and child(p<.0l). The levels of children's global self worth were related to higher levels of mother's age(p<.05), mother's education(p<.001), level of life(p<.05). Third according to the multiple regression analyses for the relationships between mother's beliefs about child rearing and children's self perceptions, mother's beliefs on the creativity were related to higher levels of children's scholastic performance, social competence, and mother's beliefs on the problem solving skills to higher levels of children's behavioral conduct and mother's beliefs on the accommodation to lower levels of children's scholastic performance, social conduct.

A Study on the Relationship Between Popularity, Social Behaviors, and Maternal Child-Rearing Practices in Korean Preschool Children (취학전 아동의 인기도와 사회적 행동 및 어머니의 양육태도간의 관계 연구)

  • The purpose of this study is to relate social behaviors and child-rearing practices to the popularity of preschool children in Korea. Specifically, four major questions were raised and studied First, the relationship between a child's popularity and his/her social behaviors was examined. Second, this relationship was explored as a function of the child's sex. Third, the relationship between a child's popularity and maternal childrearing practices was investigated. Fourth, this relationship was explored as a function of the child's sex. The subjects of this study were 113 middle class preschool aged children and their mothers. Sociometric choices and peer-perceived social behaviors were obtained from these children using Moore's (1973) Sociometric Status Test. Mothers of these children described their childrearing practices in a Q-sort format of Child-Rearing Practices Report (CRPR) developed by Block (1965). The analyses of the data using Kendall's Tau Correlation Coefficient bore the following results: 1. Patterns of social behaviors shown by either popular or unpopular children were distinct; Popular children were friendly and conformative, while unpopular children were aggressive, non-conformative and independent. 2. Patterns of social behaviors in relation to the popularity and the unpopularity differed as a function of the child's sex; Popular boys tended to be active while popular girls were likely to be unaggressive, independent and not-teasing. Unpopular boys showed a great amount of discipline their daughters by means of non-punitive punishment and to express negative reaction to the sibling rivalry among their children. These mothers tended not to enjoy their 3. Certain patterns of maternal child-rearing practices tended to be related to the popularity status of the child; The mothers of popular children as compared to those of unpopular children enjoyed their roles as mothers and didn't use non-punitive punishment. 4. Patterns of maternal child-rearing practices in relation to the child's popularity and unpopularity differed as a function of the child's sex; The mothers of popular boys unlike those of popular girls tended to emphasize on their sons' achievement and perceived that their husbands were greatly involved with their sons. The mothers of unpopular boys appeared to show negative affection toward their sons. The mothers of unpopular girls were likely to displine their daughters by means of non-punitive punishment and to express negative reaction to the sibling rivalry among their children. These mothers tended not to enjoy their roles as mothers and did not believe that their daughters would behave as they should.

CLINICAL CHARACTERISTICS AND TREATMENT COURSES OF THE CHILDREN WITH SELECTIVE MUTISM (선택적 함구증 아동의 임상특성 및 치료경과)

  • Selective mutism is a childhood condition defined by persisten failure to speak in specific social situation when speaking is expected, dispite preserved ability to comprehend spoken language and speak. Present study is to investigate clinical characteristics, treatment method and outcome of 23 children who were diagnosed as selective mutism by DSM-IV criteria at the child psychiatry ouptatient department of SNUH. The results were as follows : 1) The Sex ratio was 1: 4.8, female dominant Mear age of onset was 33 years old and mean age of first referral was 7.7 years old. 2) 22% of subjects had perinatal problem such as low birth weight, preterm birth, 26% of the subjects have history of delayed language development. There are subjects who had been separated with mam caretaker before 3 years old(26%) and who experienced physical or psychological trauma before 3 years old(26%). A few subjects had enurests(30%) and encoprests(4%). 3) Many subjects(65%) had symbiotic relationship with their mother. These families consist of dominant, verbally aggressive mother and passive father. Parents of 39% of all subjects were judged to have definite psychopathology(social phobic, depression, hysterical trait or alcohol problem) 26% of all subject, were reported physically abused. 4) The personality trait of the subjects were frequently described as follows(in order of frequency) ; Shy(100%), anxious(83%), stubborn(83%)m rigid and tense posture(78%), immature(65%) overdependent(65%), irritable(52%), manipulative(39%), depressive(39%). 5) The mean performance IQ of 16 subjects by KEDI-WISC was 88.3 Among them, the subjects with IQ below 69 were seven and those with IQ above 70 were nine. When comparing these two group(Mental retardation group vs Normal IQ group), we could find some difference in language development, personality trait, family dynamics and treatment outcome. 6) Among several treatment methods for selective mutism, play therapy was the most frequently used method(65%). Other commonly used treatment methods were pharmacotherapy(21%), behavioral therapy(8%), combined therapy(play therapy+pharmacotherapy+family therapy+behavioral therapy)(12%), 7) Regarding the outcome of treatment 8.6% was evaluated as Excellent, 30.4% as Good, 52% as Fair, 8.7% as Poor at the tinic of treatment. At follow up interview 21.7% was evaluated Excellent, 13% as Good, 21.7% as Fair, 34.8% as Poor. 8) We classified all subjects by Havden's 4 subtype. Symbiotic mutism was most common(65%) and other subtypes are Speech phobic mutism(8.6), Reactive mutism(13%) and Passive-aggressive mutism(30%).
