즉발감마선을 이용한 70MeV 양성자선량 급락지점 위치 측정에 관한 연구

  • 양성자 빔을 이용한 치료는 종양부위에 높은 선량을 균일하게 전달하고 정상세포에는 적은 선량을 전달할 수 있어 암치료 효과가 높으나 정확한 치료와 환자의 안전을 위해서는 양성자선량의 급락지점을 정확히 아는 것이 중요하다. 본 연구에서는 양성자와 물질과의 핵반응으로 직각방향으로 방출되는 즉발감마선을 측정하여 양성자선량 급락지점을 측정할 수 있는 검출시스템을 몬테칼로 전산코드로 전산모사하였으며, 70MeV 단일에너지 빔과 최대에너지가 70MeV인 SOBP 빔을 모의피폭체인 물팬텀에 조사하고 검출시스템을 통해 직각방향으로 방출되는 즉발감마선의 분포를 계산하였다. 모의피폭체 안에서의 양성자선량의 분포와 측정된 즉발감마선의 분포를 서로 비교하여 두 분포 사이의 상관관계를 찾고 이 상관관계를 이용하여 양성자선량 급락지점을 결정할 수 있음을 확인할 수 있었다.

Fabrication and Optimization of a Fiber-optic Dosimeter for Proton Beam Therapy Dosimetry (의료용 양성자선 계측을 위한 광섬유 방사선량계의 제작 및 최적화)

  • In this study, we have fabricated a fiber-optic dosimeter for a proton beam therapy dosimetry. We have measured scintillating lights with the various kinds of organic scintillators and selected the BCF-12 as a sensor-tip material due to its highest light output and peak/plateau ratio. To determine the optimum diameter of BCF-12, we have measured scintillating lights according to the energy losses of proton beams in a water phantom. Also, we determined the adequate length of organic scintillator by measuring scintillating lights according to the incident angles of proton beam. Using an optimized fiber-optic dosimeter, we have measured scintillating lights according to the dose rates and monitor units of proton accelerator.

A Research on Superparamagnetic Iron Oxide Nanoparticles' Toxicity to U373MG Cell and its Effect on the Radiation Survival Curve (산화철 나노입자의 U373MG 세포 독성평가 및 방사선 세포생존 곡선에 미치는 영향에 대한 연구)

  • This research was performed to evaluate the superparamagnetic iron oxide nanoparticles'(SPIONs) cell toxicity and to measure the radiation cell survival curve changes of SPIONs-uptake glioblastoma multiforme cells. The results could be practically used as the fundamental data to ameliorate proton beam cancer therapy, for example, providing necessary GBM treatment dose in the proton beam therapy when the therapy takes advantage of SPIONs. The assessment of the toxicological evaluation of synthesized SPIONs was accomplished by MTT assay as an in vitro experiment. The results showed no meaningful differences in the cell survival rate at the $1-100{\mu}g/ml$ SPIONs concentrations, but the cell toxicity was shown as the cell survival rate decreased up to 74.2% at the $200{\mu}g/ml$ SPIONs concentration. Then, we measured each radiation cell survival curve for U373MG cells and SPIONs-uptake U373MG cells with 0~5 Gy of proton beam irradiations. It is learned from the analysis of the experimental results that the SPION-uptake cells' radiation survival rate was more rapidly decreased as the irradiation dose increased. In conclusion we confirmed that SPIONs-uptake in U373MG cells induces cell death at the much less dose than the lethal dose of SPION-non-uptake cell. This research shows that the therapeutic efficacy of glioblastoma multiforme treatment in proton beam therapy can be improved by SPIONs targeting to the GBM cells.

Absorbed Dose Analysis in Water for Proton Beam using PTRAN Code System (PTRAN 코드를 이용한 양성자선에 대한 물 흡수선량의 해석)

  • The absorbed dose for proton beam in water was calculated using the PTRAN code system. The proton interactions with water and the description on absorbed dose calculations are discussed, and the file structure and an execution example of the PTRAN codes are described. For 60, 100, 150, 200, and 250 MeV proton beams it is demonstrated that the absorbed dose is determined from the sum of Coulomb interactions and nuclear reactions, and that the Bragg peak feature depends On the energy straggling and multiple scattering. The PTRAN code was useful for studying the fundamental mechanism of the absorbed dose to water for clinical proton beams.

The Effect of PET Scan Time on the Off-Line PET Image Quality in Proton Therapy (양성자 치료에서 영상 획득 시간에 따른 Off Line PET의 효율성 검증)

  • Purpose Proton therapy can deliver an optimal dose to tumor while reducing unnecessary dose to normal tissue as compared the conventional photon therapy. As proton beams are irradiated into tissue, various positron emitters are produced via nuclear fragmentation reactions. These positron emitters could be used for the dose verification by using PET. However, the short half-life of the radioisotopes makes it hard to obtain the enough amounts of events. The aim of this study is to investigate the effect of off-line PET imaging scan time on the PET image quality. Materials and Methods The various diameters of spheres (D=37, 28, 22 mm) filled with distilled water were inserted in a 2001 IEC body phantom. Then proton beams (100 MU) were irradiated into the center of the each sphere using the wobbling technique with the gantry angle of $0^{\circ}$. The modulation widths of the spread out bragg peak were 16.4, 14.7 and 9.3 cm for the spheres of 37, 28 and 22 mm in diameters respectively. After 5 min of the proton irradiation, the PET images of the IEC body phantom were obtained for 50 min. The PET images with different time courses (0-10 min, 11-20 min, 21-30 min, 31-40 min and 41-50 min) were obtained by dividing the frame with a duration of 10 min. In order to evaluate the off-line PET image quality with the different time courses, the contrast-to-noise ratio (CNR) of the PET image calculated for each sphere. Results The CNRs of the sphere (D=37 mm) were 0.43, 0.42, 0.40, 0.31 and 0.21 for the time courses of 0-10 min, 11-20 min, 21-30 min, 31-40 min and 41-50 min respectively. The CNRs of the sphere (D=28 mm) were 0.36, 0.32, 0.27, 0.19 and 0.09 for the time courses of 0-10 min, 11-20 min, 21-30 min, 31-40 min and 41-50 min respectively. The CNR of 37 mm sphere was decreased rapidly after 30 min of the proton irradiation. In case of the spheres of 28 mm and 22 mm, the CNR was decreased drastically after 20 min of the irradiation. Conclusion The off-line PET imaging time is an important factor for the monitoring of the proton therapy. In case of the lesion diameter of 22 mm, the off-line PET image should be obtained within 25 min after the proton irradiation. When it comes to small size of tumor, the long PET imaging time will be beneficial for the proton therapy treatment monitoring.

  • Simulations were performed using a Monte Carlo technique in order to show physical phenomena occurring when a heavy charged particle such as proton or alpha particle traverses the medium. It was confirmed that the sharp Bragg peak occurred deeper in the water with the increasing proton energy. It is found that the use of such a sharp Bragg peak due to heavy charged particles would be far superior to the case of the photon or electron, since the absorbed dose in the target tissues would be better localized, thereby minimizing the damage to the surrounding tissues.

Evaluation of Biological Characteristics of Neutron Beam Generated from MC50 Cyclotron (MC50 싸이클로트론에서 생성되는 중성자선의 생물학적 특성의 평가)

  • $\underline{Purpose}$: To evaluate biological characteristics of neutron beam generated by MC50 cyclotron located in the Korea Institute of Radiological and Medical Sciences (KIRAMS). $\underline{Materials\;and\;Methods}$: The neutron beams generated with 15 mm Beryllium target hit by 35 MeV proton beam was used and dosimetry data was measured before in-vitro study. We irradiated 0, 1, 2, 3, 4 and 5 Gy of neutron beam to EMT-6 cell line and surviving fraction (SF) was measured. The SF curve was also examined at the same dose when applying lead shielding to avoid gamma ray component. In the X-ray experiment, SF curve was obtained after irradiation of 0, 2, 5, 10, and 15 Gy. $\underline{Results}$: The neutron beams have 84% of neutron and 16% of gamma component at the depth of 2 cm with the field size of $26{\times}26\;cm^2$, beam current $20\;{\mu}A$, and dose rate of 9.25 cGy/min. The SF curve from X-ray, when fitted to linear-quadratic (LQ) model, had 0.611 as ${\alpha}/{\beta}$ ratio (${\alpha}=0.0204,\;{\beta}=0.0334,\;R^2=0.999$, respectively). The SF curve from neutron beam had shoulders at low dose area and fitted well to LQ model with the value of $R^2$ exceeding 0.99 in all experiments. The mean value of alpha and beta were -0.315 (range, $-0.254{\sim}-0.360$) and 0.247 ($0.220{\sim}0.262$), respectively. The addition of lead shielding resulted in no straightening of SF curve and shoulders in low dose area still existed. The RBE of neutron beam was in range of $2.07{\sim}2.19$ with SF=0.1 and $2.21{\sim}2.35$ with SF=0.01, respectively. $\underline{Conclusion}$: The neutron beam from MC50 cyclotron has significant amount of gamma component and this may have contributed to form the shoulder of survival curve. The RBE of neutron beam generated by MC50 was about 2.2.

  • Research about the proton nuclear reaction is actively achieving on the proton therapy including material development of fusion reactor. The proton induced gamma ray energy(2754, 1386 keV) spectrum of 27Al(p,3p+n)24Na reaction was measured with 100 MeV high energy proton beam. The proton beam in the experiment was derived from 100 MeV proton linear accelerator in the KOMAC. We measured the gamma ray intensity ratio of the decay level from the energy spectrum. The previous results have been compared with the current result. Strength of measured gamma rays will provide very important information though decide high energy gamma radiation detection efficiency.

Evaluation of Usefulness of Iterative Metal Artifact Reduction(IMAR) Algorithm In Proton Therapy Planning (양성자 치료계획에서 Iterative Metal Artifact Reduction(IMAR) Algorithm 적용의 유용성 평가)

  • Purpose: To evaluate the accuracy of the Iterative Metal Artifact Reduction (IMAR) algorithm in correcting CT (computed tomography) images distorted due to a metal artifact and to evaluate the usefulness when proton therapy plan was plan using the images on which IMAR algorithm was applied. Materials and Methods: We used a CT simulator to capture the images when metal was not inserted in the CIRS model 062 Phantom and when metal was inserted in it and Artifact occurred. We compared the differences in the CT numbers from the images without metal, with a metal artifact, and with IMAR algorithm by setting ROI 1 and ROI 2 at the same position in the phantom. In addition, CT numbers of the tissue equivalents located near the metal were compared. For the evaluation of Rando Phantom, CT was taken by inserting a titanium rod into the spinal region of the Rando phantom modelling a patient who underwent spinal implant surgery. In addition, the same proton therapy plan was established for each image, and the differences in Range at three sites were compared. Results: In the evaluation of CIRS Phantom, the CT numbers were -6.5 HU at ROI 1 and -10.5 HU at ROI 2 in the absence of metal. In the presence of metal, Fe, Ti, and W were -148.1, -45.1 and -151.7 HU at ROI 1, respectively, and when the IMAR algorithm was applied, it increased to -0.9, -2.0, -1.9 HU. In the presence of metal, they were 171.8, 63.9 and 177.0 HU at ROI 2 and after the application of IMAR algorithm they decreased to 10.0 6,7 and 8.1 HU. The CT numbers of the tissue equivalents were corrected close to the original CT numbers except those in the lung located farthest. In the evaluation of the Rando Phantom, the mean CT numbers were 9.9, -202.8, and 35.1 HU at ROI 1, and 9.0, 107.1, and 29 HU at ROI 2 in the absence, presence of metal, and in the application of IMAR algorithm. The difference between the absence of metal and the range of proton beam in the therapy was reduced on the average by 0.26 cm at point 1, 0.20 cm at point 2, and 0.12 cm at point 3 when the IMAR algorithm was applied. Conclusion: By applying the IMAR algorithm, the CT numbers were corrected close to the original ones obtained in the absence of metal. In the beam profile of the proton therapy, the difference in Range after applying the IMAR algorithm was reduced by 0.01 to 3.6 mm. There were slight differences as compared to the images absence of metal but it was thought that the application of the IMAR algorithm could result in less error compared with the conventional therapy.

A Comparative Study of Branching Ratio of 167Yb Radioactive Isotope from Gamma-ray Spectrum Produced by 169Tm(p,3n)167Yb Reaction with 100-MeV Proton Beam (100-MeV 양성자 빔을 이용하여 169Tm(p,3n)167Yb 반응에 의해 생성된 167Yb 방사성동위원소에서 방출되는 감마선 스펙트럼 비교 연구)

  • The measurement of branching ratio of 167Yb radioactive isotopes from gamma-ray spectrum of 169Tm(p,3n)167Yb reaction were performed by using a 100-MeV proton linear accelerator of the Korea Multi-purpose Accelerator Complex (KOMAC). The 167Yb isotope has a half-life of 17.5 minutes and decays to 169Tm. The gamma rays generated from the 167Yb isotope were measured using an HPGe detector gamma ray spectroscopy system. The energy calibration of the detector and the efficiency measurement of the detector were determined using a standard source. The gamma rays of known main energy (62.9, 106.2, 113.3, 143.5 and 176.3 keV) were measured. On the other hand, information about the intensity of the generated gamma rays is very inaccurate. Therefore, in this study, the decay strength of the main gamma rays was accurately measured. Overall, it was different from the previously known results, and in particular, it was found that the intensity of the main decay gamma ray, such as the 113.3 and 106.2 keV gamma ray, was overestimated, and it was found that the gamma ray, such as 62.9, 116.7 and 143.5 keV was underestimated. The present results are considered to be important information in the fields of nuclear fusion, astrophysics and nuclear physics in the future.
