Neurobiology of Alzheimer's Disease (알쯔하이머 질환의 신경생물학)

  • Alzheimer's disease(AD) is associated with a characteristic neuropathology. The major hallmarks of AD are senile plaques (SPs) and neurofibrillary tangles(NFTs). ${\beta}$-amyloid protein($A{\beta}$) is derived from the proteolysis of amyloid precursor protein(APP) and then converted to SPs. Mature SPs produce cytotoxicity through direct toxic effects and activation of microglia and complement. NFTs are composed of paired helical filaments(PHFs) including abnormally phosphorylated form of the microtubule-associated protein(MAP) tau and increased tau level in cerebrospinal fluid may be observed in most AD. The aggregation of $A{\beta}$ and tau formation are thought to be a final common pathway of AD. Acetylcholine, dopamine, serotonin, GABA and their receptors are associated with AD. Especially, decreased nicotinic acetylcholine receptors(nAChRs) in AD are reported. Genetic lesions associated with AD are mutations in the structural genes for the APP located on chromosome 21, presenilin(PSN)1 located on chromosome 14 and PSN2 located on chromosome 1. Also, trisomy 21, Apo-E gene located on chromosome 19, PMF locus, low density lipoprotein receptor-related protein and ${\alpha}$-macroglobulin increase risk of AD. In this article, we will review about the neurobiology of AD and some newly developed research areas.

Brain Atrophy and White Matter Lesions on Magnetic Resonance Imaging in Alzheimer's Disease (알쯔하이머 치매 환자의 뇌자기공명영상(腦磁氣共鳴影像)에 나타난 뇌위축(腦萎縮)과 뇌백질병변(腦白質病變)에 대한 연구)

  • Objects : To investigate the relationship between the age of onset with the atrophy and the white matter hyperintensities observed in the brain MRI of Alzheimer patients. Methods : The authors measured volumetrically cortical and ventricular brain atrophy and rated semiquantitatively white matter signal hyperintensities in nine presenile and 18 senile Alzheimer patients, who were matched for dementia severity, according to NINCDS-ADRDA criteria and in age-matched 10 presenile and 11 senile control subjects. Results : Presenile Alzheimer patients showed significantly greater cortical and ventricular atrophy indices(p<0.05) but no difference in white matter hyperintensity scores compared to the age-matched control group. On the contrary, senile Alzheimer patients showed significantly greater white matter hyperintensity scores(p<0.05) but no difference in cortical and ventricular atrophy indices compared to the age-matched control group. Conclusion : An earlier onset was related to marked brain atrophy with less white matter lesions and a later onset is related to marked white matter lesions with less brain atrophy in Alzheimer's disease. Our results suggested the passible difference in the pathophysiology between the presenile and the senile-onset Alzheimer's disease.

Effects of a Computer-based Cognitive Rehabilitation Therapy on Mild Dementia Patients in a Community (지역사회 경증치매환자를 대상으로 한 전산화 인지재활 치료(COMCOG) 효과)

  • This study aims to understand the effects of a Computer - based Cognitive Rehabilitation Therapy(CBCRT) evidence based on mild dementia patients' ability to activities daily living(ADL), cognitive function and measure of occupational performance and to suggest basic data for a cognitive rehabilitation therapy for dementia patients. Method : A CBCRT was applied two times a week for 5 weeks to 14 mild dementia patients who visited Yongin Center for Managing Dementia in Gyeongi-do between February and August 2009. Based on frame of reference for Visual-Perception a CBCRT was applied at home. Moreover, a one group pretest-post test design was, which is a quasi-experiment and research, also applied in order to verify the effects of the rehabilitation therapy on the subjects' ability to ADL, cognitive function and occupational performance skills. Results: A significant effect was confirmed (p<.05) from the CBCRT which Assessment of Motor and Process Skills(AMPS) processing skills and cognitive function and occupational performance skills. Neither was found any significant effect in improving motor skills from AMPS. Conclusion: It seems that a CBCRT based on evidence and has an effect on the improvement of the ability to ADL and cognitive function of mild dementia patients living in a community. The present author hopes that, in the future, more cognitive rehabilitation programs will be developed to improve the functions of mild dementia patients living in a community.

Animal Models of Alzheimer's Dementia (알쯔하이머 치매의 동물모형)

  • Transgenic mice models of Alzheimer's disease were produced by overexpressing APP(amyloid precursor protein) mutant and presenilin mutant genes using the promotors that induced neuronal expression. The neuropathologies, electrophysiological changes and behavioral changes that were demonstrated in these transgenic mice models were amyloid changes, gliotic changes, A-beta increases, deficit in LTP(long-term potentiation) and behavioral changes. Some or all of the above changes were found in each transgenic mice model. These models generally showed amyloid neuropathology but they usually lacked the neurofibrillary tangles. So, they can be regarded as partial models of Alzheimer's disease. The development of them is undoubtedly the great progress toward future research.

  • 본 논문은 뇌의 수직단면에 대하여 촬영된 자기공명영상에서 뇌 영역을 분리한 후 백질과 회백질 및 뇌척수액을 분리하고 각각의 체적을 산출하기 위한 것이다. 본 연구는 먼저 뇌의 자기공명영상에서 영상의 배경 및 뇌 내부를 둘러싸고 있는 외피 및 지방층으로부터 뇌 영역 전체를 분리하였으며, 부분체적의 문제(partial volume artifact)에 의해 명암값의 번짐 현상을 보이는 뇌의내부 영역에서 각 성분의 부분체적을 산출하여 각 조직을 분리하기 위한 명암 값을 결정한 후 백질과 회백질 및 뇌척수액의 영역을 분리하였다. 본 연구는 뇌의 위축을 보이지 않은 정상인의 자기공명영상을 대상으로 하였으며, 향후, 이러한 연구 결과는 알쯔하이머 병이나 뇌성마비 등과 같은 퇴행성 뇌질환 환자의 뇌 위축정도를 객관적으로 진단하는 방법으로 사용될 수 있도록 하는데 있다.

Neuroprotective Effect of Yukmijihwamg-tang(Liuweidihuangtang) Gamibang on the Deficits of Learning and Memory in Trimethyltin-Intoxicated Rats (트리메틸틴에 의해 유도된 흰쥐의 학습과 기억력 손상에 대한 육미지황탕가미방의 신경보호 효과)

  • 목적 : 이 연구는 알쯔하이머 병에 대한 육미지황가미방의 효과를 조사하였다. 육미지황가미방은 산약, 산수유, 복령, 목단피, 구기자, 택사, 숙지황을 포함한 여러 가지 한약재의 전탕액으로 치매의 한약 치료제로 널리 사용되어 왔다. 방법 : 이 약물의 신경보호 작용을 조사하기 위해, 수중미로를 사용하여 학습과 기억에 대한 육미지황가미방의 효과를 평가 했고, 트리메틸틴은 강력한 유독물질로 선택적으로 중추신경계와 면역계의 세포를 파괴시킨다. 트리메틸틴(6.0 mg/kg, i.p.)주입 후, 쥐에게 육미지황가미방(400mg/kg, p.o.)를 2주 동안 날마다 복용시켰으며, 수중미로를 수행시켰다. 결과 : 육미지황가미방을 트리메틸틴에 노출시킨 쥐에 투약했으며, 그들은 수중미로에서 학습과 기억의 향상을 보였고, 이는 육미지황가미방이 어떠한 환경에서는 트리메틸틴으로 유발된 신경퇴화 후 중추신경계의 결손을 감소시킬 수 있음을 보여준다. 결론 : 이러한 결론은 육미지황가미방이 인지능력을 증가시키고, 트리메틸틴으로 유발된 신경퇴화에서 콜린 아세틸전환효소의 정도를 변화시킬 수 있음을 보여준다.

A Systematic Review on the Effects of Intervention for Caregivers of People with Dementia to Reduce Their Burden (치매환자 돌봄제공자의 부양부담감(Burden)을 감소시키기 위한 중재프로그램: 체계적 문헌 고찰)

  • Objective : The purpose of this study was to look at a systematic review on the effects of intervention for caregivers of people with dementia to reducing burden. Through this study, we have to analysis the studies. Methods : We systematically examined papers published in journal from 2005 to 2014, using RISS, Pubmed, 9 studies were included in the analyses. Results : Selected 9 studies were Pedro score from 3.5 to 7. The most using intervention is educational intervention and the Zarit Burden Interview(ZBI) was used in all studies for measured the degree of burden of caregivers. Conclusion : The studeis about interventions for caregivers to reduce their burden are limited in Korea. In the future, the research and development of studies for intervention for caregivers of people with dementia must be activate.

Region Segmentation and Volumetry of Brain MR Image represented as Blurred Gray Value by the Partial Volume Artifact (부분체적에 의해 번진 명암 값으로 표현된 뇌의 자기공명영상에 대한 영역분할 및 체적계산)

  • This study is to segment white matter, gray matter, and cerebrospinal fluid(CSF) on a brain MR image and to calculate the volume of each. First, after removing the background on a brain MR image, we segmented the whole region of a brain from a skull and a fat layer. Then, we calculated the partial volume of each component, which was present in scanning finite thickness, with the arithmetical analysis of gray value from the internal region of a brain showing the blurring effects on the basis of the MR image forming principle. Calculated partial volumes of white matter, gray matter and CSF were used to determine the threshold for the segmentation of each component on a brain MR image showing the blurring effects. Finally, the volumes of segmented white matter, gray matter, and CSF were calculated. The result of this study can be used as the objective diagnostic method to determine the degree of brain atrophy of patients who have neurodegenerative diseases such as Alzheimer's disease and cerebral palsy.
