Safety and efficacy of the ultra-rush immunotherapy with extracts of Dermatophagoides pteronyssinus in children (집먼지 진드기 항원을 이용한 초급속면역치료의 안전성과 효과)

  • Purpose : Immunotherapy is accepted as the only treatment of allergic disease that can modify the natural course of the disease and ameliorate symptoms. This study aimed to evaluate the safety and efficacy of ultra-rush therapy using Dermatophagoides extracts in children. Methods : Of children older than four years who had visited Bundang CHA Pediatric Allergy Clinic, those showing positive reactions only to Dermatophagoides in the skin prick test and to the nasal provocation test were included. In all, 11 and 12 patients respectively preferred conventional and ultra-rush immunotherapy. We elevated allergen concentrations diluted to 1,000:1 of the end strength by 2-3 times with 30-minute intervals and checked oxygen saturation, pulse rate, blood pressure, and systemic reactions every 15 minutes. Immunotherapy effectiveness was valued by changes in nasal provocation test scores before and after immunotherapy. Results : The average ages of patients in the conventional and ultra-rush immunotherapy groups were $8.3{\pm}2.3$ and $9.2{\pm}2.8years$, respectively. Systemic reactions were observed in six in the ultra-rush group (50%) without anaphylaxis and one (9%) in the conventional group. The average scores in the nasal provocation test before and after treatment in the conventional group were $8.2{\pm}1.5$ and $4.6{\pm}2.1$, respectively (P=0.043). In the ultra-rush immunotherapy group, the scores changed from $6.2{\pm}2.2$ to $3.7{\pm}2.5$ (P=0.017). Conclusion : Ultra-rush immunotherapy using Dermatophagoides in children is effective for treating allergic disease but can induce systemic effects rather than conventional immunotherapy.

Evaluation of Genetic Safety in Genome-editing Rice Through Comparative Analysis of Genetic and Agronomic Traits (유전적 특성과 농업형질의 비교분석을 통한 유전자 교정 벼의 안전성 평가)

  • New breeding techniques (NBT) recognize specific DNA sequences and remove, modify, or insert DNA at a desired location, and can be used to treat genetic diseases in humans or to improve the traits of livestock or crops. In this study, we conducted a comparative analysis of various agricultural traits and assessed the safety of gene transferability in third-generation genome-editing rice (OsCKq1-G3) with T and G nucleotide insertions developed using the CRISPR/Cas9 SDN-1 method, in comparison to its parental line (Oryza sativa L., cv Ilmi). The analyzed traits included heading date, culm length, panicle length, tiller number, yield, germination rate, viviparous germination rate, shattering, after wintering seed viability, the presence of toxins and allergens. The target trait, heading date, exhibited a high significant difference of approximately 5 days. Culm length, panicle length, tiller number, yield showed no significant differences compared to the parental line. No T-DNA bands indicating gene transfer were detected. In the third generation of genome-edited rice, the T-DNA was confirmed to be eliminated as successive generations advanced through self-pollination. Through the analysis of germination rate, viviparous germination rate, shattering, and after wintering viability, we confirmed that the genome-editing rice has no potential for weediness. The ORF and amino acid sequences of the genome-editing rice did not reveal any toxins and allergens. The results of this study can be utilized as important data for the environmental risk assessment of genome-editing rice.

A PCR Method for Rapid Detection of Buckwheat Ingredients in Food (식품에서 메밀 성분의 검출을 위한 PCR 방법)

  • Buckwheat often causes severe allergic reactions in sensitive people. One of the major allergenic proteins in common buckwheat (Fagopyrum esculentum) has been found to be a BW10KD protein. In this study, we developed a PCR method to detect buckwheat ingredients in food using primers corresponding to the allergenic BW10KD gene. Five pairs of oligonucleotide primers successfully enabled PCR amplification of the specific regions of the genomic BW10KD DNA from buckwheat, but no amplification from seven other cereals and beans (barley, wheat, German millet, African millet, soybean, red bean, and black bean). The proposed PCR method was applied to analyze 12 processed foods (buckwheat flour, buckwheat noodle, buckwheat jelly, wheat noodle, instant noodle, black sesame gruels, sunsik, cookie, misutkaru, and three kinds of cereal); among them, only three samples including buckwheat flour, buckwheat noodle and buckwheat jelly showed a positive reaction to the detection. This PCR method was able to detect as little as 1 ng of common buckwheat DNA. This rapid and specific PCR method would be applicable to detect allergenic buckwheat ingredients in food.

Effect of Korean Red Ginseng on the Stability of the Tight Junction of Intestinal Epithelial Cells (홍삼에 의한 Caco-2 단세포층 간극의 안정화)

  • Bioactive components involved in the tight junction stabilization of intestinal epithelial cells from Korean red ginseng were studied by analyzing transepithelial electrical resistance (TEER) values of the Caco-2 cell monolayer between the apical and basolateral sides for 96 hr. The treatment with less than $20\;{\mu}g/mL$ of the Korean red ginseng extract to the apical side of Caco-2 cell monolayer gave higher TEER values than the control. However, the treatment with more than $130\;{\mu}g/mL$ of the Korean red ginseng extract drastically decreased the TEER values, and these effects were not due to its cytotoxicity. When fractions of low molecular weight compounds, polysaccharides, proteins, saponins, and polyphenols derived from Korean ginseng were applied to the apical side of the Caco-2 cell monolayer, polyphenols showed high tight junction stabilizing activity and saponins showed low activity, but the others showed no significant activity. These results suggest that Korean red ginseng might be useful for the prevention of food allergy by stabilizing the tight junction of intestinal epithelial cells leading to hindering absorption of food allergens.

Der p 1 Inhibits Spontaneous Neutrophil Apoptosis by Cytokine Secretion of Normal and Allergic Lymphocytes (Der p 1에 의한 정상인과 알레르기 환자의 림프구에서 사이토카인 분비를 통한 자발적인 호중구 세포고사 억제)

  • Dermatophagoides pteronissinus (DP) is one of the house dust mites (HDM) associated with allergic diseases. The cysteine protease Der p 1 from DP is a powerful allergen. The pathogenic mechanism of allergy is involved in cytokine secretion of lymphocytes and spontaneous apoptosis of neutrophils. In this study, we investigated whether Der p 1 induces cytokine secretion of lymphocytes and if the release of cytokines is associated with regulation of neutrophil apoptosis. In normal and allergic subjects, Der p 1 increased IL-6, IL-8, MCP-1, and GM-CSF release in a time-dependent course. Supernatants collected from normal and allergic neutrophils after Der p 1 stimulation suppressed the apoptosis of normal and allergic neutrophils, although Der p 1 alone has no effect on neutrophils. Der p 1 suppressed neutrophil apoptosis in coculture of normal neutrophils with normal lymphocytes. Der p 1 more strongly suppressed apoptosis of allergic neutrophils cocultured with allergic lymphocytes than normal neutrophils cocultured with normal lymphocytes. In summary, Der p 1 increases the secretion of cytokines, which has anti-apoptotic effects on neutrophils of normal and allergic subjects. These results will contribute to elucidate the pathogenic mechanism of allergic diseases.

Production of A Monoclonal Antibody (MAb) Against a Thermal Stable-Soluble Protein in Mackerel and Confirmation of the Properties for the MAb (고등어 어육 중 열안정성 단백질에 특이한 단클론성 항체 개발과 특성 확인)

  • For people who have a food allergy the only way to manage the allergy is to avoid the food allergen. The mackerel is one of the major food allergens, but no immunoassay for the rapid and simple detection of mackerel has been reported. The objectives of this study are to develop and characterize monoclonal antibodies (MAbs) specific to mackerel using thermal stable-soluble proteins (TSSP) as an immunogen and to characterize the MAbs by indirect enzyme-linked immunosorbent assay (iELISA). The mice immunized with mackerel TSSP and showing high titer were used for cell fusion and cloning. The characterization of MAbs produced from hybridoma cells obtained was confirmed by indirect ELISA and western blot. Four MAbs were confirmed to be specific to mackerel without cross-reaction to other marine products and livestock products in the both methods. The iELISA and western blot based on the MAbs can sensitively detect 1% mackerel protein in other marine products. These results support that immunochemical methods based on the MAb produced could be used as rapid means to detect low levels of mackerel and to identify mackerel adulterated in food.

The Preventive and Therapeutic Effects of Probiotics in Allergic Diseases Via Immune Modulation (프로바이오틱스의 면역조절을 통한 알레르기 예방 및 치료효과)

  • The demand to develop more safe and efficient methods for treating allergic patient is now continuously growing due to the increasing prevalence of allergic diseases. Probiotics are endogenous microbial flora that gives health benefits within hosts. Probiotics are now considered as one of solutions to treat allergic patients since recent evidence shows that some of probiotics have immunomodulatory function. Also, the treatment of probiotics to patients is relatively safer than other anti-inflammatory agents. In this review, we summarized on immunomodulatory function of some probiotics which show preventive or therapeutic effects on major allergic diseases such as atopic dermatitis, allergic rhinitis, asthma, or food allergy. Based on previous literature, the treatment of probiotics can alleviate the symptoms of allergic diseases via balancing $Th_1/Th_2$ response or increasing the number of regulatory T ($T_{reg}$) cells.

Frequency of Food Allergy in Korea and the Causative Food Allergens (한국인의 식품 알레르기 빈도 및 알레르기 원인식품에 대한 연구)

  • To evaluate the prevalence of food allergies and the causative food allergens, 1,469 subjects in various age groups were studied by using a food Allergy questionnaire. Also, specific food antibodies were tested with the MAST method, a chemiluminescent assay, in 84 people who have a positive history of food allergies. The results were as follows : Among the total number of 1,469 subjects, about one- half of the total number(46.6%) have a family history of allergies. According to the questionnaire, the prevalence of food allergies was 11.4%. The group with a family history of allergies showed a higher Prevalence than the group without a family history. Skin allergies were the most common type of food allergies in both groups of male and female subjects. The questionnaire revealed mackerel as the most common food allergen, followed by the allergens of peach, pork, chicken, milk and eggs in decreasing order. According to the MAST allergy test, these diagnosed with an actual food allergy totalled 4.1% of the subjects. Peach and pork were the most common food allergens listed on the MAST results.

The Allergenicity of Ovomucoid in Treated Egg Whites to Human IgE Antibody from Egg-Allergic Patients (계란 알레르기 환자의 IgE 항체를 이용한 처리 난백 중 ovomucoid의 알레르기성 조사)

  • The ovomucoid (OM) of egg whites is recognized as a major allergen. Here, the allergenicity of OM in egg whites (EW) treated by chemical, enzymatic, and physiological methods were investigated by competitive inhibitory ELISA using human IgE antibody acquired from egg-allergic patients. Enzymatic hydrolysis, irradiation, and succinic anhydride treatments did not reduce the allergenicity of the OM effectively. Allergenicity was reduced to only 1/20 by deglycosylation with trifluoromethanesulfonic acid (TFMS). Heat treatment of the OM at $121^{\circ}C$ for 10 min reduced allergenicity to 1/100. Furthermore, NaOH (over 3%) treatment reduced allergenicity to 1/10,000, and the combinatory treatment of NaOH (over 0.3%) and heat ($70^{\circ}C$, 15 min) reduced it to less than 1/10,000, which was the most effective method. In this study, which analyzed treated EW using ELISA and patient-derived IgE, the OM allergenicity was nearly the same as its antigenicity according to ELISA using rabbit IgG. However, in the case of the TFMS-treated EW, the antigenicity was much lower than the allergenicity. These results suggest that the allergenicity of OM is slightly different from its antigenicity.

Allergenicity of Treated Chicken Egg Whites as Determined by a Passive Cutaneous Anaphylaxis Test, Immunoblot Analysis, and a Mouse Model of Food Allergy (수동피부아나필락시스 시험, immunoblot, 식품알레르기 생쥐모델에 의한 난백 처리물의 알레르기성 평가)

  • The allergenicity of treated chicken egg whites (EW) was evaluated by a passive cutaneous anaphylaxis (PCA) test, immunoblot analysis, and a mouse model of food anaphylaxis. The results of the PCA test revealed that treatment with 0.3% NaOH (w/v) decreased the antigenicity of native EW to 1/4. In addition, treatment with heat ($121^{\circ}C$, 30 min) or 1% NaOH (w/v) decreased the antigenicity to 1/8 and combined treatment with 1% NaOH (w/v) and heat ($70^{\circ}C$, 15 min) decreased the antigenicity to 1/32 of that of the native EW. Immunoblot analysis revealed that the density of EW protein bands decreased in response to heat treatment, and were almost not detectable following the combined treatment. Finally, the murine model of EW anaphylaxis revealed that the mean score of systemic anaphylactic symptoms in EW challenged mice was 1.85, while the mean score in mice challenged with EW that that had been subjected to the combined treatment was only 0.20. The results of this study indicate that the most effective method of reducing EW allergenicity is combined treatment with 1% NaOH (w/v) and heat ($70^{\circ}C$, 15 min).
