THE LONGITUDINAL GROWTH CHANGES OF CRANIOFACIAL STRUCTURE IN KOREAN ADULT (DURING THE AGE OF 24 TO 31) (성인 두개안면골의 성장변화(24세에서 31세까지))

  • Recently, according to the Increase of adult patient, it is neccessory to understand the growth changes of adult after cessation of active pubertal growth in clinical orthodontics. The purpose of this study was to investigate the growth changes of craniofacial structure after active growth period(adult) in order to use as reference in clinical orthodontics. Authors followed the 40 sample(male 25, female 15) from 24 to 31 years of age. By analysing the serial cephalograms, authors could get the following findings. $\cdot$ The mandible rotated clockwise in female, but not in male, and no incremental growth change in both genders. $\cdot$ The anterior facial height and lower anterior facial height were increased in both genders, the increase of lower anterior facial height exceed the posterior facial height increase in female. $\cdot$ The cranial base was stable throughout observation period. $\cdot$ The upper incisors uprighted slightly in female. $\cdot$ There were great the individual variation in the growth change of craniofacial structure in adult.

  • Along with traditional goal of 'straightening teeth', today's orthodontics put more and more emphasis on facial esthetics. Naturally, the importance of soft tissue, which dictates one's facial form, is noticed. There have been many cephalometric studies on growth of hard tissue ; however, only a few on that of soft tissue. In Korea, various methods of sampling, measuring and analysing were used to study soft tissue changes, and yet, soft tissue changes along with process of normal growth have not been studied. The author carried out this study as a part of semi-longitudinal study on craniofacial growth and development of Korean children from ages of six to seventeen. 409 boys and 436 girls who'd had no systemic disease for the last three years and shown normal developmental and occlusal status were chosen as subjects. Cephalometric X-rays were taken of each subject, and facial form, lip position & form, nose form, and lip thickness were measured. Mean values and standard deviations were calculated according to age and gender, and figures and tables were drawn accordingly. The following results were obtained: 1. In respect to facial form, boys showed growth for longer period compared to girls. Also, lower face showed higher growth rate than upper face. 2. There was not much change in thickness of upper and lower lips with age; however, they appeared more prominent compared to caucasian children. 3. Nose grew horizontally with age. 4. Horizontal thickness of lower face increased with age, and upper lip thickness at nose base[A-Sn(FH)] showed more growth than anywhere else.

  • Maxillary-malar deficiency is the most frequently occurring midface dentofacial deformity. Clinicaly patients with maxillary-malar deficiency exhibit malar and infraorbital rim deficiency and class III malocclusion. For treatment of these deformities, modified LeFort III osteotomy have been used. Modified LeFort III osteotomy advances maxilla with orbital rims and zygomatic bone anteroposteriorly. This is a case of patient who had severe mandibular prognthism with midface deformity. We performed modified LeFort III osteotomy for maxillary-malar advancement and simultaneous bilateral sagittal split ramus osteotomy for mandibular prognathism and autogenous iliac bone graft.

STUDY ON MAXILLOFACIAL GROWTH AND PHYSICAL DEVELOPMENT OF GROWING CHILDREN (성장기 아동의 악안면 성장과 신체발육에 관한 연구)

  • Orthodontic patients are individuals that grow and develop ;therefore selection of the proper time for orthodontic treatment Is considered to be one of the most difficult and yet difficult factor.Since the development of cephalometric X-ray, amount and pattern of craniofacial growth change with aging could be predicted and became useful in the process of orthodontic treatment. The relationship between the mean values of cephalometric measurements and body height and weight was studied among the groups(boys and girls) of Korean children from the ages of 3-years to 12-years. 126 boys and 90 girls with no abnormality in growth and development and no history of orthodontic treatment from the ages of 3 years to 12 years were chosen as subjects: Cephalometric X-ray were taken for 2 years and hard tissue analysis based on Burstone's COGS, which was divided into measurements of 6 parts (Cranial base, Maxillar and Mandible, Vertical measurements, Horizontal measurements, Basal bone relationship, Dental measurements). The relationship between craniofacial growth and body height & weight was studied. The following conclusions were obtained : 1. The maximum growth in the measurements of cranial base, N-Ar(FH), N-Ba(FH) corresponded with the age with the maximum increasein body height & weight in both boys and girls. 2. Genial angle gradually decreased with aging in both boys and girls. 3. N-ANS(L) showed greater amount of growth than ANS-Me(L), and this had greater influence on facial profile. 4. $N-A-Pog^{\circ}$ decreased with aging, and mandibular growth exceeded maxillary growth in amount and rate. 5. Length of Y-axis increased, but Y-axis to FH plane remained constant. This show that mandible grows at a constant angulation to cranial base. 6. As Permanent teeth erupt, interincisal angle deceased.

A LONGITUDINAL STUDY ON THREE FACIAL GROWTH PATTERNS IN KOREANS WITH NORMAL OCCLUSION (정상교합자의 3가지 안면골 성장양상에 관한 누년적 연구)

  • The purpose of this study was to compare the difference of the growth aspects in three facial growth patterns. The biennial serial cephalometric radiographs of 33 samples(19males, 14females) with normal occlusion from 8.5 years to 18.5 yews of age were used in this study. The facial growth patterrn was categorized in 3 types(Drop type, Neutral type, Forward type) by the total amounts of the Y-axis which changed from 8.5 years to 18.5 years of age. The growth change of the craniofacial area during 10 years in each growth type was analyzed and was compared among the 3 growth types. The results of this study might be summarized as follows. 1. The samples that were classified by total change of the Y-axis during this study period were distributed to 52% of the neutral type, 27% of the forward type, 21% of the drop type. 2. The anterior growth of the maxilla to the cranial base(N per A) showed larger in the forward type than in other 2 types(p<0.05). 3. The palatal plane to the FH plane showed more anterior-superior inclination in the forward type with age during this study period. 4. The anterior growth of the mandible to the cranial base(N per Pog) appeared large in rank order, of largest the forward type, second the neutral type, and third the drop type(p<0.05). 5. During this study period the mandibular plane(SN/MN,FMA) showed more counterclockwise rotation in the forward type than in the drop type(p<0.05), and this tendency was stronger in males than in females(p<0.05). 6. The growth of the mandibular corpus length(Go-Me) showed smaller in the drop type than in the other 2 types(p<0.05). 7. In the forward type and the neutral type, the anterior growth of the mandible was larger than that of the maxilla(p<0.05). 8. In the craniofacial growth distances and angulations turned out to be somewhat variable, but the vertical proportion had a strong tendency whose original relation was maintained consistently during this study period. 9. Through these analyzed data, the profilograms on each growth type were constructed to evaluate individual growth pattern in the orthodontic diagnosis.
