An Electromyographic Study on Changes of Mandibular Position (하악위 변화에 따른 저작근의 근전도학적 연구)

  • 저자는 하악의 위치변화가 저작근에 미치는 영향을 근전도학적으로 평가하고자, 측두하악장애의 징후와 증상이 없는 정상성인 28명을 대상으로 중심위 교합장치(centric relation splint), 중심교합위 교합장치(centric occlusion splint), 전방 재위치 교합장치(anterior repositioning splint)를 장착시킨 상태에서 하악의 중심의, 중심교합위, 전방위률 유도한 후 최대 이악물기 상태의 좌우측 교근 및 전측두근의 근전도를 채득, 분석, 평가한 결과 다음과 같은 결론을 얻었다. 1. 전측두근에서 하악의 위치변화에 따른 최대 근활성도의 차이가 나타났으나(p<0.05), 교근에서는 나타나지 않았다. 2. 전측두근의 경우 중심교합위에서 최대 이악물기 상태의 최대 근활성도는 교합 장치물을 장착하지 않은 경우 및 중심위에서와 비교하여 감소하였다(p<0.05). 3. 전측두근의 경우 전방위에서 최대 이악물기 상태의 최대 근활성도는 교합 장치물을 장착하지 않은 경우 및 중심위에서와 비교하여 감소하였다(p<0.001). 4. 교근과 전측두근의 좌우 비대칭지수(asymmetry index)는 하악의 위치변화에 따른 차이가 나타나지 않았다.

THE CLINICAL RELIABILITY AND EFFECTIVENESS OF THE POSTERO-ANTERIOR CEPHALOMETRIC VIEW, SUBMENTOVERTEX VIEW & PANORAMIC VIEW IN THE DIAGNOSIS OF THE FACIAL ASYMMETRY (안면 비대칭 환자 진단에 있어서 후전방두부 방사선사진과 이하두정 방사선사진 및 파노라마 방사선사진의 임상적 효용성)

  • Clinical examination, photometric evaluation, & radiographic analysis have been used to evaluate the asymmetry of the face. Commonly used skull radiographs to assess skeletal asymmetry include poatero-anterior cephalometries, submentovertex view, & panoramic view. The purpose of this study is the comparison of the reliance of the postero-anterior cephalometric view, submentovertex view, & panoramic view in the asymmetry evaluation. All measurements were performed on the each radiographs of 31 control group & 30 asymmetric group. The measurements are MSL(midsagittal plane)-Co, MSL-Go, MSL-Me, MSL-Al, MSL-Bl, Mn. Ramus Height(Co-Go), Mn. Body Length(Go-Me), and Total Mn. Length(Co-Me). The results were as follows: 1. The lack of either a right-sided or left-sided asymmetric dominant was found. 2. The postero-anterior cephalometric view & submentovertex view relatively agreed with each other in the result. The postero-anterior cephalometric view & submentovertex view had the clinical reliability & effectiveness in the diagnosis of the skeletal asymmetry(p<0.05). 3. The panoramic view showed more magnification compared to the other radiographs. In the vertical measurements the panoramic view had clinical reliability relatively(p<0.05). But we cannot rely on the horizontal measurements in the panoramic view(p>0.05).

  • When we establish treatment planning of facial asymmetry, we must predict each asymmetrical element that will be changed upon coronal, axial, sagittal plane. At the visual point, prediction of the change of coronal plane is most important. It is important difference between Rt. and Lt. mandibular angle belonging to posterior coronal plane, as well as anterior coronal plane, such as upper and lower incisor, or midline of chin point. Several methods for control bulk of mandibular angle are additional angle shaving after osteotomy, grinding contact area between proximal and distal segment for decrease the volume, or bone graft for increase the volume. But, at the point of bimaxillary surgery, transverse position of posterior maxilla is an important factor for control it. So, we would report transverse movement of posterior maxilla for decrease asymmetry on the posterior coronal plane of face, that is, asymmetry of mandibular angular portion.

Three dimensional CT analysis of facial asymmetry (안면비대칭 3차원 CT 분석)

  • Purpose : This study aimed to identify the range of normal facial asymmetry using three-dimensional CT and to develop a simple method of diagnosis of facial asymmetry. Materials and Methods : Twenty eight adults with normal occlusion (16 males and 12 females; mean age 24 years and 1 month) were selected whose faces were assessed to be symmetric by an orthodontist. Three-dimensional reconstructions were obtained utilizing spiral CT scans and an oral and maxillofacial radiologist evaluated nineteen anatomic landmarks in three-dimensional coordinates. Facial asymmetry index of each landmark was caluculated. Results : The range of normal facial asymmetry of each landmark was identified using mean and standard deviation of facial asymmetry index. Conclusions : The range of normal facial asymmetry identified in this study may be used as a diagnostic standard for facial asymmetry analysis.

A POSTEROANTERIOR ROENTGENOCEPHALOMETRIC STUDY OF SKELETAL CRANIOFACIAL ASYMMETRIC PATIENTS (골격형 안면 비대칭 부정교합자에 대한 정모두부방사선 계측학적 연구)

  • This study was undertaken to investigate the degree of asymmetry in each part of the head in skeletal craniofacial asymmetric patients, and secondarily to determine the nature of difference existed between asymmetric patients and normal persons. The subjects consisted of 49 asymmetric adult patients and 52 normal adults, and the average ages were 21 years 5 months and 23 years 5 months, respectively. The computerized analyses of 33 linear measurements, 12 angular measurements, and 8 surface areas from posteroanterior cephalometric radiograph were carried out. The conclusions were as follows; 1. Asymmetry was a common finding in both normal and asymmetric group. 2. When the analyses were undertaken after the head was divided into 3 parts anteroposteriorly and superoinferiorly in the asymmetric patients, there were significant asymmetries in all parts excepts in the cranial base region. 3. When the analyses were undertaken after the head was divided into respective 3 parts anteroposteriorly in the asymmetric patients, the more posterior part showed relative stability than the more anterior part. 4. When the analyses were undertaken after the head was divided into 3 parts superoinferiorly in the asymmetric patients, the more superior part showed relative stability than the more inferior part. 5. Twelve variables indicating asymmetry were selected and the highest ranked variable was Me-Mid-sagittal reference line.

  • Although osteochondroma is not rare in the axial skeleton and long bones, it is very rare in the jaw. It is a benign chondroma within which partial endochondral ossification occurs. There are two types, the central one and the peripheral one. Peripheral type is more common than central one in the jaw, but it is not frequent. Especially it is rare at the mandibular condyle. When it occurred at the mandibular condyle, it is generally located at the lateral portion of the condyle. In that case, facial asymmetry with occlusal change is the characteristic clinical feature. But it is similar to condylar hyperplasia so that misdiagnosis can sometimes occur. The differential point is as follows: Hyperplasia generally appears as a generalized enlargement of the condylar process with a normal cortical thickness, but osteochondroma usually appears as a focal growth or mass. We report a very rare case of peripheral osteochondroma at the mandibular condyle in a 27-year- old male patient who visited DKUDH with a chief complaint of the facial asymmetry.

  • Fibrous dysplasia is a developmental tumor-like condition that is characterized by replacement of normal bone by an excessive proliferation of cellular fibrous connective tissue intermixed with irregular bony trabeculae. Craniofacial lesions may cause facial pain, headache, cranial asymmetry, facial deformity, tooth displacement and visual or auditory impairment. In this case, a 2-year-9-month old boy who was diagnosed as the fibrous dysplasia showed delayed eruption on affected area. Teeth of left lateral dentition group have erupted completely but teeth of right lateral dentition group are erupting slowly. Eruption and maturation of affected teeth are in progress, so continuous observation is required presently. If the eruption state stops, surgical opening or forced eruption of the impacted teeth will be considered.

  • In patients with unstable mandibular movements who are accompanied by involuntary facial muscle movements and asymmetrical changes in the facial features, many difficulties exist in determining the vertical dimension of occlusion, recording centric relation, and transferring information about the occlusal plane. In this case, for a patient with unilateral facial nerve palsy who was edentulous for a long period of time, a CAD-CAM provisional prosthesis was fabricated using a 3D printing customized gothic arch tracer and a prefabricated occlusal plane recorder, the POP bow system, to achieve a stable interarch relationship. Afterwards, a full mouth implant fixed prosthesis was fabricated by delivering an appropriate vertical dimension of occlusion, a reproducible centric relation, and a correct three-dimensional occlusal plane, and continuous maintenance was performed to restore functional and aesthetic oral health to the patient.

Effects of enucleation and chemotherapy in advanced intraocular and intraorbital retinoblastoma with or without radiotherapy (진행된 안구내 및 안와내 망막모세포종에서 안구적출술과 항암화학치료 및 방사선조사 유무에 따른 효과)

  • Purpose : Radiotherapy is effective in local treatment for retinoblastoma. However, asymmetric facial hypoplasia after radiation is a serious late effect. This study was performed to investigate the effects of enucleation and chemotherapy with or without radiotherapy in advanced intraocular and intraorbital retinoblastoma. Methods : Between 1985 October and 2006 December, the records of thirty five patients who were diagnosed as retinoblastoma at Yeungnam University Hospital were reviewed. Advanced intraocular and intraorbital retinoblastoma patients classified as Reese-Ellsworth group III, IV, and V and Grabowski- Abramson class II were selected for the study. Results : Eighteen patients were enrolled in this study. All patients were enucleated and had received chemotherapy. Nine patients received radiotherapy and nine patients didn't receive radiotherapy. Tumor cells were found on resection margin of optic nerve in five of nine patients who received radiotherapy, but none of nine who didn't receive radiotherapy. Chemotherapy included vincristine, adriamycin, cyclophosphamide, VM-26, cisplatin before 2001, and vincristine, etoposide, and carboplatin after 2001. There were no recurrences or metastases in nine patients who didn't receive radiotherapy. But two of nine patients who received radiotherapy had metastases to brain. However, all survivors who received radiotherapy had significant facial asymmetry. Conclusion : In advanced intraocular and intraorbital retinoblastoma without tumor cell on resection margin of optic nerve, enucleation and chemotherapy without local radiotherapy appears to be safe for long-term survival. However, in those with tumor cells on resection margin of optic nerve, enucleation and chemotherapy with local radiotherapy seems to be necessary to improve survival.

  • The clinical application of the three-dimensional radiographic technique had been limited to standard Broadbent-Bolton cephalometer with biplanar stereoradiography. We developed a new method for compensating the error of head position in ordinary non-biplanar cephalostat. It became to possible to use the three dimensional cephalogram commonly in clinical bases. 1. The method of methemetical compensation of head positioning error in non-biplanar condition was evaluated with dry skull. The error of the method of first and the second trial was $0.46{\pm}1.21$, $0.33{\pm}0.90mm$, which means the error of the head positioning correction in conventional cephalogram was within clinical acceptance. 2. The reproducibility of this system for clinical application was 0.54 mm ($-2.99{\sim}2.26mm$) which defines the absolute mean difference of the first and second trial. Compare to the The landmark identification error $1.2{\pm}1.6mm$, the error of the measurement was within the range of landmark identification error. The result indicates the adequate clinical accuracy of the computation of three-dimensional coordinates by compensation of the error of the head position in ordinary non-biplanar cephalostat.
