A CLINICAL COMPARATIVE STUDY BETWEEN ANTERIOR ILIAC AND PROXIMAL TIBIAL METAPHYSIS PARTICULATED CANCELLOUS BONE GRAFTS (장골과 경골의 자가입자망상골 이식에 관한 임상적 비교연구)

  • This is a clinical and retrospective study of 36 patients received the autogenous particulated cancellous bone grafts from anterior iliac and proximal tibial metaphysis and we compared the clinical postoperative complications in operation sites and donor site morbidity. The results of this study indicate that, in all our patients, the proximal tibia provided an adquate volume of cancellous bone and there were no special contraindications, in choosing and using the proximal tibia as a donor site in most oral and maxillofacial cancellous bone graft surgeries. Furthermore, the proximal tibial metaphysis would appear a more easily obtainable cancellous bone source and offer a superior clinical results than anterior iliac crest in donor site morbidity.

  • There are many residual nasal deformities following midface injury. The treatment of primary nasal deformities is delayed frequently due to several factors. And then, we usually perform the secondary nasal reconstruction. Autogenous bone is the material of choice for major nasal augmentation. It can be contoured appropriately and securely immobilized. It is rapidly incorporated as living tissue by the recipient bed. We used with iliac bone, contochondral graft and ear cartilage for secondary nasal augmentation and obtained the relatively fair results.

  • Bone grafts are becoming increasingly common in oral and maxillofacial surgery to improve bone healing procedures. Bovine bone as a xenograft is a representative osteoconductor and space filler; however, sometimes complications, such as infection and wound dehiscence are encountered with its use. We report the result of an eight-year follow-up of a xenograft case and processing methods of inorganic bovine bone along with a review of the literature. Xenograft ($LUBBOC^{(R)}$) was used in a cyst enucleation site of the maxilla, as a bone substitute and space filler. Inflammation and infection were defined several times as lack of osseous contact between the graft and host bone, caused by remodeling failure over an eight-year period. Pathologic findings of the xenograft revealed dead bony trabeculae with inflamed fibrous tissue and actinomycosis.

  • Transverse mandibular deficiency has a many problem in growing patients. Therefore, Mandible symphysis widening is recommended. A new surgical technique has been developed to widen the mandible. The purpose of this study is to report the effect of mandibluar symphyseal distraction osteogenesis by use of bone-borne type distractor. The surgical procedure was accomplished under general anaesthesia with step-line osteotomy. Bone-borne type distractor was used to generally widen the mandible. The expansion achieved in the mandible was 8.31mm at the device, 6.32mm at the canines, 4.06mm at the first molars. The symphyseal distraction gaps were bridged by new bony regeneration. Mandibular symphseal distraction osteogenesis increased mandibular arch width, stabilized occlusion, and corrected dental crowding.

  • It would be desirable to regenerate bone vertically in a predictable way; such a technique would allow for more favorable implant - crown ratio and better esthetics for implant placement. Traditionally, several techniques has been proposed for this purpose including GBR with particulated bone and block bone graft using mandible or illium however, the efficacy of these techniques has not been firmly established because they have some week points or complications each other that it is difficult to draw firm conclusion for superiority. In recent years, We have treated 11-cases of vertical deficiency of alveolar bone using thin block and chip bone graft technique and the postoperative results showed good prognosis with few complications. So we report the results of its treatment and cases with review of literature.

  • The purpose of this study was to evaluate the stability and efficacy of biologic membrane made of freeze-dried cartilage as a barrier to facilitate guided bone regeneration in experimental non-healing bone defects in the rat mandible. Nine adult Sprague-Dawley rats (400-500g) were used in experiment. 5.0mm in diameter were created on the mandibular angle area by means of slow-speed trephine drill. In microscopic examination, dynamic immature bone forming at 2 weeks and its calcification at 4 weeks were observed. The membrane made of lyophilized cartilage taken from human costal cartilage seems to be very effective for guided bone regeneration as a biologic membrane and the scaffold for attachment of cells or local drug delivery system of growth factor, which may meet the ideal requirement of a barrier membrane and graft materials.

  • Neural crest cells have embriologically important role for the development and growth in oral and maxillofacial region. If the early hereditary defect occurs or environmental factors affect these cells diminutive mesenchymal cells are disabled to make neural plate and decreased proliferation of mesenchymal cells result in hypoplastic development of neural crest. As a result, this brings about severe facial malformations such as various located facial clefts and/or loss or duplication of facial structure. These are two cases of accessory maxilla and zygomatic deformity with and without facial cleft.

  • The author examined the clinical signs and symptoms , routine radiographs, laboratory test and bone scan in 73 subjects with TMJ disorders and studied the responses of clinical test on patients, the distribution of signs and symptoms of joints, the simple uptake rate(SUR) of affected joints , the SUR of subclassified groups of TMJ disorders, active joints of subclassified groups of TMJ disorders and the SUR of joints with noises. The obtained results were as follows : 1. The percentage of joint pain on palpation, joint noises joint pain on function, mandibular dysfunction and active finding of bone scan in aptients with TMJ disorders were higher 2. The SUR was higher in joints affected by joint pain on function, joint pain on palpation, mandibular dysfunction and creptius. 3. The SUR of osteoarthrosis was the lowest. 4. The percentage of active joints were the highest in joints with discdisplacement without reduction, and followed by osteoarthritis, and disc displacement with reduction. 5. The SUR of TMJ showing joint noises only was lower.

  • Although various methods had been reported for reanimination of facial nerve palsy, interposition nerve graft remains superior to other methods if there is a wide gap to be bridged. Dott described a excllent facial nerve reconstruction by sural nerve graft bypassing petrous bone. But his method needs two surgical fields and is performed in two stages. Authors desribe a traumatic facial nerve palsy treated by one stage facial nerve reconstruction that is performed in one surgical field by using a great auricular nerve interposition graft and bypass the petous bone.

  • Methodology for finite element analysis of vertebral column and pedicle screw system, which circumvents the tremendous difficulties in geometric, material, and structural modeling, is proposed. The simplification is focused on the modeling of the cancellous bone in vertebral body the intervertebral disc. and the instrumented internal fixation devices. Each proposed modeling technique is justified to result in reasonable accuracy. These methods are believed to be suitable for the development of pedicle screw systems, not only because modeling itself is much simpler. but also because reliable empirical data for disc stiffness may be incorporated with little additional effort, and presumably frequent design change may be easily reflected on the analysis.
