THE EFFECT OF DENIAL ORTHOPEDIC FORCE TO IMPLANTS ON BONE TISSUE BEFORE COMPLETE OSSEOINTEGRATION (골유착 이전에 악정형력이 임프란트 주위조직에 미치는 영향)

  • The dental implants for edentulous Patients have been used for more than 20 years. After the introduction of osseointegration by $Br{\aa}nemark$, the commercially pure titanium implants were accepted by most practitioners. Recently dental implants are used for orthodontic anchorages as well as prosthetic abutment. Many researchers have reported implants as a good orthodontic anchorage through basic research and clinical evaluation. But previous researches were done after the healing time for osseointegration of inserted implants. If dental implants are to be used for prosthetic abutment the healing time for osseointegration is necessary, but orthodontic forces to implants are different from bite force regarding its amount of force, duration and direction. The authors evaluated the effect of orthopedic force to implants on bone tissue before osseointegration. 48 implants were placed at 12 rabbits. 2 implants into left side and 2 implants into right side were inserted along the long axis of femur respectively 2 weeks (2 weeks group), 4 weeks (4 weeks group) and 6 weeks (6 weeks group) after implants placement, 300g force had been applied to the implants at left side femur by Ni-Ti close coil spring for 4 weeks (experimental group) and no force applied to implants at right side femur (control group). After the force application for 4weeks, rabbits were sacrificed and microscopic evaluation was done by hematoxylin-eosin stain and Masson trichrome stain. The result3 were followed. 1. All implants in experimental group remained rigid after the force application for 4 weeks. 2. More fibrous tissue between bone and implants were noticed at 2 weeks experimental group than 2 weeks control group 3. More bone remodeling was noticed at 4weeks group than 2 weeks group and it was difficult to find out fibrous tissue between bone and implants at both experimental and control group of 4 weeks group. 4. It was hard to distinguish experimental group from control group at 6 weeks group. Therefore if initial stability can be obtained on implant insertion, it can be possible to use implants as a orthodontic anchorage before the healing time for osseointegration.

The experimental study of early loading on the Miniplate in the beagle dog (성견의 하악골에 식립된 Miniplate에 가한 조기 부하의 영향에 관한 실험적 연구)

  • Conventional osseointegrated titanium implants have many limitations; large size, limited location for placement of the implant, severity of the surgery, discomfort of initial healing, difficulty of oral hygiene and uncontrollable force direction. Recently titanium miniscrew and miniplate have been used for an alternative to conventional dental implant. But in relation to miniplate, miniscrew has disadvantages in that more potential inflammation, light orthodontic force application and limited orthodontic application. This study was conducted to evaluate the effectiveness of miniplate by observing the reactions of peri-implant tissues to early orthodontic and orthopedic loading of titanium miniplate. In four adult beagle dogs 10 miniplates were inserted into the alveolar bone using 20 osseointegrated titanium screws. 4 miniplates were placed in two dogs(dogA, B) and 6 miniplates in two dogs(dogC, D). In dogA, B miniplates were loaded with 200gm of force immediately after placement for 15 weeks. In dogc, D, miniplates were loaded with 400gm of force immediately after placement for 8 weeks. Miniplates of dogA were removed, dogA was healed for 4 weeks, and the area which was removed of miniscrew was examined. Following an observation period, the miniplates including miniscrews and the surrounding bone of dogB and dogC, D were removed, respectively. Undecalcified section along the long axis of miniscrews were made and the degree of osseointegration was examined under the light microscope. The results were as follows. 1. In the histologic features there was direct contact between bone and miniscrew in all groups except one, dogC control group. The loaded miniscrew demonstrated only a slight increase of the osseous proximaty when compared with unloaded miniscrew 2. There was no significant difference of the osseointegration of Peri-miniscrew surface between dogB and dogC, D. But dogB showed slightly more increased bone apposition than dogC, D 3. The gingiva overlapping the miniplate and miniscrew showed no inflammatory sign in clinical and histological aspects. 4. The impaled hard and soft tissues at the area which was removed of miniscrews showed good healing without inflammatory reaction. 5. The mobility showed slight increase in un-loaded miniplate but that was insignificant. Based on the results of this study, miniplate(C-tube) can be used as a firm osseous orthodontic and orthopedic anchorage unit immediately after insertion.

EFFECTS OF MAXILLARY PROTRACTION ON THE DISPLACEMENT OF THE MAXILLA (상악골 전방 견인이 상악골체의 변위에 미치는 영향)

  • In the orthopedic therapy, the biomechanical analysis of the appliance is necessary to get a desirable orthopedic effect. The purpose of this study was to investigate the desirable direction and application position of the protraction force. The protraction force of 500g was applied to the first premolar or to the first molar. The direction of force application was paralell or $20^{\circ}$ downward to the occlusal plane respectively. The stress distribution and the displacement within the maxilla was analyzed by a 3-dimensional finite element method. The findings obtained were as follows 1. Protraction forces caused a counterclockwise rotation of the maxilla. 2. The degree of maxillary rotation was less when the force was applied $20^{\circ}$ downward direction to the occlusal plane than when applied to the parallel direction. 3. The degree of rotation of maxilla was greater when the parallel force was applied to the 1st premolar than when applied to the first molar, whereas it was greater when force is applied $20^{\circ}$ downward than at the first premolar. In conclusion, the $20^{\circ}$ downward protraction from the first premolar induced the least counterclockwise rotation of the maxilla and was thought as the desirable direction and application position of the protraction force.

  • 공작기계는 금속가공 공업에 있어서 가장 중요한 생산수단의 요소이며, 공작기계의 발달없이 인류의 생활수준을 향상시킨다는 것은 생각도 할 수 없는 일이다. 선진국에서는 제조된 기계의 약 10%가 공작기계이며, 기계공업 노동력의 악 10%가 공작기계 산업에 종사하고 있다. 공작기계는 금속재료를 절삭(Cutting) 또는 연삭(Grinding)하여 원하는 모양, 치수 표면정도의 제품을 얻는 것이고 기계본체와 공구의 두부분으로 되어있어 동력에 의해 기계를 운전하고 절삭공구로써 가공을 한다. 국제 규격의 ISO에서는 '한 운동원에 의해서 작동하고 물리적, 화학적 또는 기타의 방법으로 성형해서 공작물을 생산하는, 수작업은 하지 않는 기계'라고 정의 하고 있으며 미국에서는 금속가공기계를 두가지로 나누어 정형형을 Forming machine이라 하고 절삭형을 Cutting machine 이라 분류하며 이 둘을 합하여 공작기계(Machine Tool)라고 부르고 있다.

A Development of on Electronic Starter for Single Phase Induction Motor (단상 유도전동기의 전자식 기동장치 개발)

  • This paper presents a simple electronic starter for single phase induction motor (SPIM). It replaces the centrifugal switch in the auxiliary winding circuit of the capacitor start type SPIM. The electronic starter observes the auxiliary winding voltage, and disconnects the auxiliary winding when the motor gets close to its rated speed. Because of its possibly fluttering contacts of the centrifugal switch which mounted on the conventional SPIM, the reliability of SPIM can be reduced. Developed new electronic starter has no mechanical contacts. Therefore, the reliability and the performance of SPIM can be improved. The operational principle of the proposed electronic starter is explained, and illustrated with experimental results.

Treatment strategies on Class III malocclusion based on Long term follow up study (III급 부정교합의 치료전략)

  • The author obtained some useful information for the class III treatment from long term observation on the growing patients with class III malocclusion. 8 patients were selected for this study and presentation. From these observation so far my conclusions might be as follows: First in the early correction of the anterior crossbite, considerable forward growth changes were observed in the maxilla Second, as for the growth modification of jaws by orthopedic treatment only limited effects were recognized from the long-term observation Thrid, at early age of patients with anterior crossbite, any data couldn't make me predict the stability after treatment on the long-term basis. Fortunately, however, genial angle showed a marginal possibility of it prediction. Fourth, at an advanced age/ retraction orthopedic force on the mandible and the rapid change in the mandibular position may cause some trouble in the T.M.joint. Finally, the followings are recommendable. As for the anterior crossbite, correct it early as possible, and use orthopedic force under the age of ten. Do not enter the phase II treatment directly. Just wait and observe until the growth were almost completed, focusiong on some important factors such as airway problem, tongue position, and third molar development. Of course, these factors may have some effects on the mandibular growth. for the female, at the age of around 14 years old and the male, around 17 years old, make a final decision whether the patients will continue to be treated orthodontically or surgically Thereby, (I think) the relapse and retreatment problem after treatemnt we have observed so far might be minimized. Furthermore, the active treatment time may be also reduced.

INFLUENCE OF MINIPLATE SHAPES AS SKELETAL ANCHORAGE FOR APPLICATION OF ORTHOPEDIC FORCE: A THREE-DIMENSIONAL FINITE ELEMENT ANALYSIS (악정형력 적용을 위한 골내 고정원으로서 미니플레이트 형상의 영향: 3차원 유한요소법적 연구)

  • Purpose: This study was performed to evaluate the stress distribution in the bone and the displacement distribution of the miniscrew under orthopedic force with two different types of miniplate design as skeletal anchorage for orthopedic treatment. Materials and methods: Finite element models were made for 6-hole miniplate (0.8mm in thickness), which were designed in two different shapes-one is curvilinear shaped (C plate, Jeil Medical Co., Korea) and another, Y shaped (Y plate), fixed with 3 pieces of miniscrew 2mm-diameter and 6mm-long respectively. A traction force of 4 N was applied in $0^{\circ}$, $30^{\circ}$ and $60^{\circ}$ to imaginary axis connecting two unfixed distalmost holes of the miniplate. Results: The maximum von Mises stress in the bone was much greater in the cortical portion rather than in the cancellous portion. C plate showed greater maximum von Mises stress in the cortical bone than Y plate. The maximum displacement of the miniscrew was greater in C plate than Y plate. The more increased the angle of the applied orthopedic force, the greater maximum von Mises stress in the bone and maximum displacement of the miniscrew. It was observed that in C plate, the von Mises stress in the bone and displacement of the miniscrew were distributed around the distalmost screw-fixed area. Conclusions: The results suggest that Y plate should have the advantage over C plate and in the placement of the miniplate, its imaginary axis should be placed as parallel as possible to the direction of orthopedic force to obtain its primary stability.

SHORT-TERM EFFECTS OF THE FUNCTIONAL REGULATOR III APPLIED TO ANTERIOR CROSSBITE PATIENTS IN EARLY MIXED DENTITION (기능성 반대교합을 가진 초기 혼합치열기 어린이에서 FR III를 단기간 장착했을 때의 효과)

  • In case of treating the functional type of anterior cross bite, removing the cause in early stage prevents from turning to skeletal type, leads to perform normal function, and has improved facial appearance. Functional Regulator III (FR III), one kind of the Functional regulator(FR)s suggested by R$\ddot{o}$lf Fr$\ddot{a}$nkel in 1966, applied to patients with the functional and skeletal anterior crossbite in early mixed, and permanent dentition. This appliance improves unbalanced power condition by blocking abnormal muscle-power effect, so that normal growth can be expected. In this case report, favorable results were obtained by selecting clinical cases of children in their early mixed dentition with functional cross bite. 1. FR IIIs were applied to patients with anterior crossbite for 5~6 months. Anterior crossbite patients were corrected favorably, nevertheless they didn't show any horizontal skeletal-changes by buccal shields. 2. Normal occlusion and esthetic facial contour were achieved from dental movement of maxillary and mandibular anterior teeth while the mandible rotates to posterior and inferior direction.

Analysis of Treatment Period on the Intraoral Removable Appliance Utilizing Vertical Facial Growth on Class III Malocclusion (얼굴의 수직성장을 이용하여 III급 부정교합을 치료하는 구강내 가철식 장치의 치료기간분석)

  • Vertical facial growth triggers the rotation of mandible to move the chin point to the downward and backward direction, which showed remarkably effective result making the less prominent chin. Recently, the intraoral removable appliance utilizing class III elastic demonstrated the vertical growth trigger mechanism. The treatment change was very fast and wearing was quite easy, compared to extraoral appliances. The purpose of this study was to verify the duration of the treatment on class III malocclusion using intraoral removable appliances, which designed to accelerate vertical facial growth. 56 patients were selected with the complaint of the protruded mandible and class III malocclusion (overjet : -3 - 0 mm, overbite : 0 - 4 mm). Information like; age at start, duration of the treatment events, type of the treatment, overjet, overbite etc. was collected and calculated. The average age of the patients delivering the initial brace was $8.75{\pm}1.10year$. Most of the anterior crossbite was resolved within 6 months. The total treatment period was $21.79{\pm}10.73months$ with the additional procedures like the alignment of anterior teeth and torque control using additional removable and fixed orthodontic appliances. The correlation study showed that patient's cooperation (p = 0.000) and the use of fixed appliance (p = 0.032) were significantly influenced on treatment duration.
