A Study on the Distribution of Staphylococcus aureus in Atopic Dermatitis (아토피 피부염에서 황색포도상구균 분포에 관한 연구)

  • Staphylococcus aureus is found on the skin of $78{\sim}100%$ of children and adults with atopic dermatitis (AD) but only on the skin of $2{\sim}25%$ of healthy subjects. It is known that S. aureus and their endotoxins as superantigen have important roles in the exacerbation and prolongation of AD. This study was carried out for the detection of S. aureus in the skin of AD, age, sex, outbreak age of AD, treatment duration, aggravation season, and the relation of ooze and S. aureus. Most patients (84%) with AD show colonization of the skin with S. aureus and there is a correlation between the degree of colonization and the serous exudate. It seems likely that the inhibition of S. aureus is associated with improvement in the skin of AD patients.

Association between Atopic Dermatitis and Attention Deficit Hyperactivity Disorder Symptoms in Korean Children (소아.청소년에서 아토피 피부염과 주의력결핍과잉행동장애와의 관계)

  • Objective : The aim of this study was to examine the relationship between IgE-mediated atopic dermatitis and attention deficit hyperactivity disorder(ADHD) in Korean children. Method : This case-control study included 69 patients with atopic dermatitis and 42 normal students. Diagnoses of atopic dermatitis and assessments of its severity were made by the dermatologist. The MAST tests were administered to atopic dermatitis patients. The parents of all of the subjects completed the parent versions of the ADHD Rating Scale-IV(ARS). Results: Compared to the normal control group, the atopic dermatitis group scored significantly higher in the total, inattention, and hyperactivity/impulsivity profiles of the ARS(t=3.35, p<0.01 : t=6.41, p<0.01 ; t=3.93, p<0.01, respectively), The atopic dermatitis patients with ADHD hyperactivity/impulsivity subtype showed more severe symptoms of atopic dermatitis than those with ADHD inattentive subtype. The MAST-positive atopic dermatitis patients showed significantly increased incidence of ADHD symptoms than the MAST-negative counterparts. Conclusion : The results of this study suggest the possibility of an association between IgE-mediated atopic responsiveness and ADHD symptomatology.

  • As the increase of an environmental disease has become a social problem after industrialization, academic interest in a spatial difference and characteristics of an environmental disease is on rise. The purpose of this study is to analyze the spatial distribution and characteristics of an environmental disease using the data provided by National Health Insurance Corporation in 2009. This research is focusing on atopic dermatitis among a variety of environmental diseases and shows the map that atopic dermatitis patients are distributed. Also, The Local Moran's I show how spatial autocorrelation of atopic dermatitis patients are distributed. First, the distribution of atopic dermatitis patients show the spatial difference. Second, 42 places including the western part of Incheon are hot spots of atopic dermatitis. Third, 39 places including Danyang are cold spot of atopic dermatitis. Forth, Jeju-si and Seogwipo-si are unusually hot spot of atopic dermatitis. These results have important implications that further research need to be done in public health geography.

  • 목적 : 본 연구를 통하여 아토피 피부염 환아의 한약치료 후 임상 효능, 안전성을 평가하고, 나아가 효능과 안전성을 종합적으로 평가함으로써 아토피 피부염에 대한 한약치료의 유용성을 평가하고자 한다. 방법 : 경희대학교 한의과대학 부속한방병원 한방소아과에 내원한 아토피 피부염 환아 39명의 한약치료 전후의 의무기록을 검토하였다. 아토피피부염 치료에 대한 한약의 효능을 평가하기 위하여 치료 후 아토피 피부염의 개선정도에 대한 부모의 평가를 VAS를 사용하여 분석하였다. 아토피 피부염에 패한 한약치료 후 안정성의 평가를 위하여 치료 전후 혈중 AST, ALT, BUN, creatinine 변화를 검토하여 한약치료의 간신독성 여부에 대해 조사하였고, 보호자에 대한 설문조사를 통하여 한약치료 후 발생한 부작용 및 불편한 증상의 종류 및 정도를 평가하였다. 나아가 한약의 효능 및 안정성을 종합적으로 평가하여 아토피 피부염에 대한 한약 치료의 유용성을 분석하였다. 결과 및 결론 : 환아 39명 중 27명 (69%)의 부모가 한약치료가 아토피 피부염의 개선에 효과가 있다고 보고하였다. 안정성의 평가에 있어서, 39명의 환자에게서 한약 치료 후 간장과 신장에 미치는 독성이 발견되지 않았다. 2명의 환아가 일시적이고 경미한 복부 불쾌감을 호소하였으나, 한약투여와의 관련성은 관찰되지 않았다. 1명의 환아에서 경미한 복통이 발견되었으나, 의학적 치료 없이 증상은 소실되었으며, 치료 과정의 중단 없이 계획되었던 모든 치료 과정을 종료하였다. 치료 후 상기 환아는 아토피 피부염의 증상 개선 뿐 아니라 변비증상의 완화, 만성적 어지러움의 개선 등 제반 신체 조건의 호전을 보고 하였다. 임상적 효능과 안전성을 종합적으로 고려할 때, 환아 39명 중 27명 (69%)의 사례에서 한약치료가 아토피 피부염의 유용한 치료수단으로 평가되었다. 이를 근거로 한약치료가 아토피 피부염에 대하여 안전성을 가진 유용한 치료방법이라 사료되며, 향후 임상적 효능과 안전성을 객관적으로 평가할 수 있는 대규모의 임상연구가 필요하리라 사료된다.

  • 본 임상연구는 임상연구 모집 공고를 통하여 모집된 대상자 중 Atopic Dermatitis Research Group (2005) 한국인 아토피 피부염 진단기준에 따라 아토피 피부염으로 판정되고 현재 약물(한약, 양약)치료나 외용제 치료를 하고 있지 않은 만 5$\sim$50세의 아토피 피부염 환자를 대상으로 하였다. 피험자들을 이중맹검, 블록무작위배정하여 시험군과 대조군으로 나누어 6주간의 시험기간 동안 육안적 평가, 혈액학적 검사, 피부측정 장비를 통한 기기적 평가를 시험 전후에 실시하여, 시험군에 사용된 제품이 대조군 사용된 제품에 비하여 얼마나 효과적으로 아토피 피부염으로 손상된 피부 장벽을 회복하는지 관찰하였다. 본 임상시험을 통해 'Atomento solution & cream'이 손상된 피부장벽을 회복시키고 피부 보습력을 증가시켜 아토피 피부염에 효과적이었으며, 인체에 이상반응이나 부작용 없이 안전하게 사용될 수 있음을 확인하였다.

A Clinical Study of Shi Ho Cheong Gan-San on Blood Heat Pattern Atopic Dermatitis: a randomized, double-blind clinical trial (혈열형 아토피피부염에 대한 시호청간산의 유효성과 안전성 연구)

  • 목적 : 본 임상시험을 통하여 혈열(血熱)형으로 변증된 아토피피부염의 한약 치료 후 임상 효능과 안전성을 관찰함으로써 아토피피부염에 대한 한약치료의 유용성을 평가하고자 한다. 방법 : 본 임상시험은 무작위배정, 이중맹검, 양성대조군, 평행 설계로 진행되었다. 자의에 의해 임상시험 동의서에 서명한 대상자 중 선정기준 및 제외기준에 부합된 36명의 대상자들에게 시험약과 양성대조약 과립제를 1일 3회(5.0g* 3회/ 1일) 4주간 복용하도록 하였다. 36명 중 31명의 환자가 4주간의 치료를 종료하였다(시호청간산 복용군: n = 16, 소풍산 복용군: n =15). 아토피피부염 증상을 평가하기 위해 Scoring atopic dermatitis (SCORAD) index와 Eczemaarea and severity index (EASI) 를 사용하여 시험 시작 전과 4주 후 시험 종료일에 환자들의 피부소견을 평가하였다. 아토피 피부염에 대한 한약치료의 안정성을 평가하기 위하여 치료 전후 혈중 AST, ALT, BUN, creatinine 변화를 검토하여 한약치료의 간/신독성 여부에 대해 조사하였다. 결과 및 결론: 두 군 모두에서 유의하게 SCORAD 점수와 EASI 점수가 감소하였으나 두 군 간의 차이는 유의하지 않았다. 부종/구진, 찰상 항목의 점수에는 두 군 간에 유의한 차이가 관찰되었다. 31명의 환자에게서 한약 치료 후 간장과 신장에 미치는 독성이 발견되지 않았으며 한약투여와 관련된 이상반응은 보고되지 않았다. 위의 결과로부터 시호청간산과 소풍산이 혈열형 아토피피부염에 대한 효과적이고 안전한 치료 방법이 될 수 있을 것으로 생각된다.

Life Event Stress and Coping Strategy in Patient with Atopic Dermatitis (아토피 피부염 환자의 생활사건 스트레스와 대처방식)

  • Background : Various type of psychological and stressful events in life have been reported to have much effect in the onset, progress and exacerbation of psychosomatic disorders such as hypertension, bronchial asthma, peptic ulcer, tension headache, alopecia areata, and atopic dermatitis. However, the nature of the association between stress and psychosomatic disorders remains unclear. Objective : The purpose of this study is to determine the relation of stress and the progress of dermatologic disorder. Method : We examined 30 patients with atopic dermatitis and 30 control subjects with tinea pedis and onychomycosis who visited to Dept. of Dermatology, Chung-Ang University Hospital. To evaluate the stress, we used 'Scale of Life Events' and 'Multidemensional Coping Scale'. Result : 1) The score of life events stress in atopic dermatitis group was significantly higher than that of control group. 2) In the result of coping strategies, the atopic dermatitis group was significantly higher than control group at the active forgetting, positive comparison, and emotional pacification, while in control group religious seeking and accomodation tended to be higher with no statistical significance. Conclusion : These findings suggest that psychosocial stress may play a role in life pattern of atopic dermatitis. But further studies are needed to clarity the exact relationship between stress and psychosomatic disorder.

A study of the frequency and characteristics of minor clinical manifestations in children with atopic dermatitis (소아 아토피피부염 환자의 부증상 빈도 및 특징에 대한 연구)

  • Purpose : We aimed to evaluate the frequency and characteristics of minor clinical manifestations of atopic dermatitis (AD) in Korean children to aid the diagnosis and treatment of AD. Methods : From April 2007 to December 2007, we enrolled 106 children (aged 1 month [infants] to 15 years) diagnosed with AD at the Pediatric Allergy Respiratory Center in Soonchunhyang University Hospital. Clinical manifestations were examined and laboratory findings (total and specific immunoglobulin E [IgE] levels and peripheral blood eosinophil count) were analyzed and compared. Results : Minor symptoms, in order of frequency, included xerosis (78.3%), aggravation due to environmental or emotional stress (43.4%), lichenification (35.8%), orbital darkening (34.0%), periauricular eczema (33.0%), and cutaneous infection (31.1%). Older children (${\geq}2$ years) showed more orbital darkening (P=0.01), horizontal crease (P=0.01), and lichenification (P=0.001) than infants. Patients with severe AD (scoring atopic dermatitis [SCORAD] score, ${\geq}40$) showed higher frequencies of xerosis (P= 0.04), cutaneous infection (P=0.03), ichthyosis (P=0.18), keratosis pilaris (P=0.02), pityriasis alba (P=0.07), recurrent conjunctivitis (P=0.02), orbital darkening (P=0.001), aggravation due to environmental or emotional stress (P=0.05), facial eczema (P=0.001), lichenification (P=0.001), and hand/foot eczema (P=0.04) than those with mild-to-moderate AD. Children with atopic eczema showed more facial eczema (P=0.01) and lichenification (P=0.04) than those with non-atopic eczema. Conclusion : The clinical manifestations of AD were similar to those established by Hanifin and Rajka. However, we need to develop our own diagnostic criteria for AD, because the frequencies shown by our subjects differed from those observed in other countries.

An overview of the regulatory effect of vitamin D supplementation on atopic dermatitis (비타민 D의 기능성: 아토피피부염의 조절에 미치는 영향 연구)

  • Atopic dermatitis is a chronic, recurrent, inflammatory skin disease that is a well-known allergic disease with severe itching, making daily life difficult. Since the immunomodulatory action of vitamin D has been reported, several researchers have attempted to determine the correlation between vitamin D and atopic dermatitis. In this review, 41 articles meeting the inclusion criteria were selected and reviewed from the articles published to date. Several studies have reported that low vitamin D levels are associated with the onset of atopic dermatitis and severe atopic dermatitis, but the opinions remain conflicting. Similarly, there are conflicting opinions on the improvement effect of oral vitamin D supplementation on atopic dermatitis, but some possibilities have been suggested. To apply vitamin D as a therapeutic agent for atopic dermatitis, a more systematically designed experiment should be conducted, and an appropriate intake dose for an immunomodulatory function should be obtained.

Profile of the Staphylococcal Exotoxin Gene and its Relation with Canine Atopic Dermatitis (포도알구균의 외독소 유전자 분석과 그 외독소가 개 아토피 피부염에 미치는 영향)

  • Staphylococcus spp. is one of the most common bacteria isolated from the lesions of atopic dermatitis (AD) in humans, and their colonization is known to be a possible trigger factor of clinical signs. The aim of this study was to determine the prevalence of Staphylococcus spp. in canine AD (CAD), the types of exotoxins present, and their relation with the clinical severity of CAD. From 79 dogs with AD, 72 samples of Staphylococcus spp. were isolated (91.1%), and 65 (90.3%) were confirmed as Staphylococcus pseudintermedius. Concerning the profile of the exotoxin gene, 50 isolates (69.4%) contained at least one exotoxin gene, and 28 isolates (56%) were found to contain more than 2 different exotoxins. There was a significant difference in clinical severity with the presence of staphylococcal exotoxins (P=0.028), whereas no correlation was found with the presence of Staphylococcus spp. (P=0.598). The clinical severity of CAD increased only in relation to staphylococcal enterotoxin D (SED) and exfoliative toxins (P<0.05). Some clinical evaluation criteria (erythema, papule/pustule) were correlated with the presence of the exotoxin gene (P<0.05). This study showed that the high prevalence of Staphylococcus spp. and staphylococcal exotoxins in lesions from dogs with AD may be regarded as an important trigger factor for exacerbation of the clinical signs of CAD.
