  • Anaphylactic reaction to protamine sulfate, which is used widely to reverse the anticoagulative effect of heparin after cardiopulmonary bypass, is very rare. But the result of anaphylactic reaction can be very fatal and the mechanism of it is still not clear. We report. a. case of severe anaphylactic reaction to protamine sulfate following the replacement of the mitral valve and .Maze procedure using microwave in a non-diabetic 57-year-old female patient.

  • Quinolone계 항균제인 rufloxacin의 전임상시험의 일환으로 항원성 유발여부를 평가하기 여부를 평가하기 위하여 아나필락시스 쇼크 반응시험, 수동 피부 아나필락시스 반응시험 및 간접 적혈구 응집 반응시험을 실시하여 다음과 같은 결과를 얻었다. 1. 기니픽을 이용한 아나필락시스 쇼크 반응시험에서 rufloxacin의 저용량투여군(1mg/body, 추정임상용량), 고용량투여군(5mg/body), 고용량혼합투여군(5mg/body mixed with Freunds' complete or incomplete adjuvant) 및 단백결합혼합투여군(rufloxacin-BSA, 1mg/body)에 있어서 아나필락시스 쇼크 반응이 관찰되지 않았다. 2. 마우스-랫드를 이용한 수동 피부 아나필락시스 반응시험에서 저용량투여군, 고용량투여군, 오용량혼합투여군 및 bovine serum albumin(BSA)과 결합시킨 단백결합혼합투여군 모두에서 음성의 반응을 나타내었다. 3. 간접 적혈구 응집 반응시험 결과 저용량투여군, 고용량투여군, 고용량혼합투여군 및 단백결합혼합투여군에서는 각군당 1-4마리에서 16-32배에서 양성반응을 나타내었다. 그러나 음성대조군에서도 10마리 중 4마리에서 16-32배에서 양성반응을 나타낸 것으로 미루어 약물투여군에서의 미약란 양성반응이 약물특이적 반응은 아닌 것으로 사료된다. 이상의 실험결과로 미루어 rufloxacin은 항체 생성능은 가지지 않는 것으로 사료된다. 이상의 실험결과를 종합하여 볼 때 ndloxacin은 전반적인 항원성 potential은 없는 것으로 사료되며 또한 생체내에서 체내 단백과 결합하여 hapten으로 작용할 가능성은 없는 것으로 사료된다.

Inhibitory Effects of Several Korean Traditional Herbs on Anaphylactic Reaction and Mast Cell Activation (아나필락시스 반응과 비만세포 활성화에 대한 한국전통약제의 억제효과)

  • Korean traditional herbs, especially Anemarrhena asphodeloides (A. asphodeloides), Salvia miltiorrhiza (S. miltiorrhiza), and Terminalia chebula (T. chebula) have been known to have the immunomodulatory, anti-tumor, and anti-inflammatory effects. However, direct cellular mechanism underlying the mast cell-mediated anaphylaxis-like reaction has poorly been understood. In the present study, the effects of the methanol extracts of A. asphodeloides (MEAA), S. miltiorrhiza (MESM), and T. chebula (METC) on anaphylactic reaction were investigated. Using in vitro and in vivo experiments, the influences of MEAA, MESM, and METC on the compound 48/80-induced anaphylaxis-like reaction and mast cell activation, and IgEmediated PCA were examined. Results are below; 1) MEAA, MESM, and METC significantly inhibited systemic anaphylaxis-like reaction, ear swelling response, and IgE-mediated PCA. 2) the compound 48/80-induced mast cell degranulation, histamine release of rat peritoneal mast cells (RPMC) were significantly inhibited by the pretreatment with MEAA, MESM, and METC, and 3) the compound 48/80-induced calcium influx in RPMC was significantly inhibited by the pretreatment with MEAA, MESM, and METC. These results suggest that MEAA, MESM, and METC may be an activity to inhibit the compound 48/80-induced or anti-DNP IgE-mediated anaphylactic reactions by blocking histamine release and calcium uptake into RPMC. MEAA, MESM, and METC potentially may serve as an effective therapeutic tool for allergic diseases.

  • The purpose of this study is effect factors of Pre-Hospital Medication in the Patients with Bee-Sting. This study is to identify the necessity of using epinephrine in prehospital stage from the perspective of early management of anaphylaxis following a bee sting. Methods: Patients suffering from a bee sting who used the 119 rescue between 2013 from 119 center data were included. Age, sex, month of injury, time factors, vital signs, medication, signs, distance factors, presence of cardiac arrest, AVPU triage by EMT were extracted. The severity of bee sting injury was divided into mild, moderate, and severe according to the presenting symptoms and signs. Results: The severe patients treated by using oxygen, IV, ECG, Medication. Anaphylaxis is a serious allergic reaction of rapid onset that may lead to death. One of Anaphylaxis patients lived by using epinephrine. Most patients collapsed at the scene of mount. Bee sting injuries occurred primarily during summer to the farmer. Nine patients collapsed at the scene. Bee sting injuries occurred primarily from June to October. Conclusion: The primary treatment for anaphylaxis is epinephrine. Epinephrine in pre-hospital stage is factors of essential for patients. Also epinephrine will permit to be equipped in the EMT-P and use of epinephrine by law.

A Study on Antigencity (Immunotoxicity Study) to the Expressed Proteins of ${\beta}$-Carotene Biofortified Rice (베타카로틴강화미 발현단백질에 대한 항원성연구)

  • As part of safety evaluation of 2A (amono acid), PAT (phosphinotricin Acetyl-transferase), CtrI (Carotene desaturase) and PSY (phytoene synthase), the expressed proteins inserted to ${\beta}$-carotene Biofortified rice were tested for antigencity test. As a result, the group of administering high-concentration PAT, the expressed protein, showed a great content of total WBC; however, other expressed proteins did not show much difference. Against ASA (Active Systemic Anaphylaxis) test, the group of administering high-concentration PAT, the expressed protein, showed mild or medium degree of symptoms, but there was no dead entity. According to the result of the PCA (Passive Cutaneous Anaphylaxis test), the group of administering high-concentration PAT, 2A, PSY, and mixture of expressed proteins indicated positive response in low anti-serum concentration, and the group of administering the clinical concentration of mixture indicated mild positive response. However, because the group of administering the clinical concentration of expressed proteins, PAT, 2A, PSY, and CtrI, did not show positive response, it is thought that IgE is not generated. Further studies are needed to verify the safety of ${\beta}$-carotene Biofortified rice.

  • A 38-year old man visited a private clinic complaining of epigastric discomfort for 2 months A huge hepatic cyst was found by sonography and computerized tomography. An exploratory Laparotomy was performed under the impression of hydatid disease. The cyst was successfully removed. A lot of living protoscolices of Echinocucur Sranulosus were found from the cystic fluid under light microscopy. During the operation, however. the cyst was accidentally ruptured and the cystic fluid spilled out. The patient was medicated with albendazole, and had been well without any signs of anaphylaxis or recurrence for 1 year follow-up period. He had been in Saudi Arabia for 3 years. This is the 16th case of hydatid disease reported in Korea and a case without immediate complication in spite of rupture of the cyst.

  • Food allergy is not uncommon among small children. Cow milk and eggs are most frequently incriminated as the major cause of food allergy. A 4-month-old female infant who did not have a previous history of contact with the egg developed anaphylactic shock when an emulsion of raw egg white was rubbed on the buttock by her mother to relieve erosive diaper dermatitis. She had been fed on breast milk. She had no past medical history of any other allergy and no family history of atopy, asthma or allergic rhinitis. Her IgE PRIST was 29.46 IU/ml and multiple antigen simultaneous testing chemiluminescent assay for food specific IgE antibody showed a level 4 positive value only to egg white.

Epidemiology of anaphylaxis in Korean children (소아청소년에서 아나필락시스의 역학)

  • Anaphylaxis is an acute systemic reaction caused by IgE-mediated immunological release of mediators from mast cells and basophils to allergenic triggers, such as food, insect venoms, and medications. An alternative definition was recently proposed as follows: anaphylaxis is a "condition caused by an IgE mediated reaction" that is "often life threatening and almost always unanticipated." The reaction can be severe enough to lead to the rapid onset of symptoms, including dizziness, upper airway occlusion, bronchial constriction, hypotension, urticaria, cardiovascular arrhythmias and possible cardiac arrest. The incidence or prevalence of anaphylaxis in Korean pediatrics has not known. Thus, Epidemiology of Anaphylaxis in Pediatrics based on the data from Korean Health Insurance Review and Assessment Service (KHIRA) from 2001 to 2007 and questionnaire to the member of Korean Academy of Pediatric Allergy and Respiratory Disease (KAPARD) who are working at the training hospitals was studied. The incidence of anaphylaxis under age 19 is 0.7-1.0 per 100,000 year-person. The causes of anaphylaxis based on data from KHIRA were unknown (61.7%), food (24.9%), medications (12.4%), and serum (1.0%).

  • Anaphylaxis is a systemic allergic reaction which can bring fatal results. The common symptoms are erythema, angioedema, urticaria, hypertension and dyspnea. However, in very few cases, ST segment changes in the electrocardiogram can be seen. This is a case of a 51 year old female with normal heart function who showed reversible ST segment depression during anaphylaxis caused by a $H_2$-blocker agent. The cause of ST segment changes during anaphylaxis is thought to be the result of coronary vasospasm mediated by various factors.
