선천성 심질환에 대한 개심술후 발생한 폐동맥 고혈압 위기증이 혈관확장제 등의 내과적 치료에 반응하지 않는 경우에 체외막 산소화 장치를 사용함으로써, 안정적인 산소공급의 확보를 통한 폐혈관 저항의 상승 방지, 심박출량의 상당 부분의 바이패스를 통한 폐관류압의 감소, 우심실의 용적 부하 감소 및 심실보조 기능을 통한 우심부전 및 저심박출의 예방 등의 치료 효과를 기대할 수 있다. 본 증례의 경우, 동맥관 개존증과 이차성 폐동맥 고혈압이 합병되어 있었던 37세 남자에서 수술후 발생한 폐동맥 고혈압 위기증에 대한 치료시 체외막 산소화 장치를 함께 사용하여 좋은 결과를 얻었기에 보고하는 바이다.
환자는 8세 여자 환자로 확장성 심근병증을 진단 받은 환자이다. 환자는 심부전으로 인하여 양심실 순환보조를 15일간 시행하였다. 15일 중 후반기 7일은 급성 신부전증이 발생하여 지속적 정맥-정맥간 혈액투석을 시행하였다. 이후 뇌사상태의 A형 혈액형을 가진 심장 공여자에게서 심장을 이식받아 심장이식술을 시행하였다. 면역 억제제는 급성 신부전증을 고려하여 사용하였다. 신부전 상해는 2주간 지속되었고, 심장 이식술 후 약 14일 후 배뇨가 시작되었다. 환자는 수술 후 12일째에 시행한 우심실 조직검사에서 특별한 거부반응의 증거가 없었으며, 면역요법 후 수술 후 52일째에 특별한 문제 없이 퇴원하였다. 환자는 술 후 약 14개월간 조직검사에서 면역 거부반응 없이 외래 경과관찰 중이다.
소아에서 체외막 산소화 장치는 중증호흡부전, 약물에 반응하지 않는 심부전, 패혈증, 폐동맥고혈압에서 뿐만 아니라 심장이식의 가교로서도 기계적인 심폐보조를 제공한다. 그렇지만 소아에서 부정맥에 의해 생긴 저심박출증에 대해 체외막 산소화 장치를 이용하여 심장보조를 시행한 증례는 많지 않다. 본 저자들은 부분 양심실 교정을 받은 엡스타인 기형에서 심실성 부정맥 때문에 순환 부전에 빠진 15세 여자 환자를 체외막 산소화 장치를 이용하여 치료하였기에 문헌 고찰과 함께 보고하는 바이다.
배경: 전격성 심근염은 드물지만 사망을 초래할 수도 있는 치명적인 질병으로서, 급성기 치료가 예후에 매우 중요한 것으로 알려져 있고 이러한 급성기 치료에 순환 보조 장치가 도움이 될 수 있다. 이에 전격성 심근염 환자에서 급성기에 순환 보조 장치를 사용하여 치료한 결과를 보고하고자 한다. 대상 및 방법: 2006년 9월부터 2008년 10월까지 전격성 심근염으로 순환 보조 장치를 삽입한 9명의 환자를 대상으로 후향적 연구를 시행하였다. Capiox emergency bypass system (Terumo Inc, Tokyo, Japan)을 사용하였고 경피적으로 삽입하여 순환 보조 장치를 운용하였다. 환자들은 모두 심실 빈맥 또 는 강심제에 반응하지 않는 심인성 쇼크 상태에서 순환 보조를 받았고 심초음파상 평균 심구출률은 $20{\pm}6%$였다. 결과: 3명이 순환 보조 장치를 유지하던 도중 사망하였고 한 명의 환자가 순환 보조 장치를 유지하던 중 심장 이식 수술을 받았다. 나머지 환자들은 평균 $107{\pm}70$시간 순환 보조 장치 유지 후 성공적으로 발관하여 퇴원 후 심초음파상 심구출률 $56{\pm}7%$로 경과 관찰 중이다. 결론: 전격성 심근염은 급성기에 매우 치명적일 수 있으나 치료 후 예후는 양호한 질환으로 이러한 급성기 치료에 순환 보조 장치가 매우 유용하게 사용될 수 있다. 또한 순환 보조 장치의 도입 시점, 운용 방법 등에 대한 연구를 통하여 합병증을 줄일 수 있도록 노력해야 할 것이다.
It is important to begin left ventricular assist device (LVAD) treatment at appropriate time for heart failure patients who expect cardiac recovery after the therapy. In order to predict the optimal timing of LVAD implantation, we predicted pumping efficacy of LVAD according to the severity of heart failure theoretically. We used LVAD-implanted cardiovascular system model which consist of 8 Windkessel compartments for the simulation study. The time-varying compliance theory was used to simulate ventricular pumping function in the model. The ventricular systolic dysfunction was implemented by increasing the end-systolic ventricular compliance. Using the mathematical model, we predicted cardiac responses such as left ventricular peak pressure, cardiac output, ejection fraction, and stroke work according to the severity of ventricular systolic dysfunction under the treatments of continuous and pulsatile LVAD. Left ventricular peak pressure, which indicates the ventricular loading condition, decreased maximally at the 1st level heart-failure under pulsatile LVAD therapy and 2nd level heart-failure under continuous LVAD therapy. We conclude that optimal timing for pulsatile LVAD treatment is 1st level heart-failure and for continuous LVAD treatment is 2nd level heart-failure when considering LVAD treatment as "bridge to recovery".
In this paper, we described 23 cases of animal experiment with our pneumatic ventricular assist device and new durability-improvement method. The blood pump consists of blood housing, and back plate made by the injection molding of isoplast, and the diaphragm fabricated by dipping of polyurethane solution onto the aluminum mold. Its volume was 75 $m\ell$ and in-vitro test showed that maximum output was 4.5 $\ell$/min at the 100 mmHg. The adult female sheep with weight of 50 + 10 kg were employed for tile in-vivo experiments and the mean blood flow was sustained at 3.0 1/min. 4 animals survived more than 15 days and the longest survival time was 28 days. In the prior 10 cases, the major causes of death were the tearing of diaphragm at the diaphragm to blood housing junction. By the new mesh and alumina ball milling methods, the durability was enhanced, and its qualitative and quantitative improvement was proved via the in-vivo and in-vitro methods. Animal experiments demonstrated that all the physiologic parameters a ere maintained within the permissible ranges and no thrombus formation was observed through the visual and blood test. The in-vivo experiments demonstrated our pneumatic ventricular assist device to he one month's bridge to transplantation device.
The purpose of this paper is to develop diaphragm-type pneumatic ventricular assist device(VAD) for clinical application and to evaluate its performance through the mock circulation system. The blood housing and diaphragm are made by coating pellethane on the metallic mold and the back plate is made by machining process. The relations of cardiac output(CO) vs. beat rate and CO vs. systolic-to-diastolic rate was estimated through the mock test and hemodynamic waves are recorded for the evaluation of VAD. As a result, the volume of blood pump is 70 ml, maximum CO is 5 L/min and CO has a close relation to the input resistance of blood pump. The hemodynamic data and waves showed this system can be applicable to the animal experiment.
We developed a ventricular assist device(VAD) to be used as a bridge to heart transplantation or a circulatory assist device for patients with end-stage heart disease or poor myocardial function after cardiac surgery. It is a pneumatic and diaphragm-type VAD and its stroke volume is 90 cc. The upper housing and diaphragm is made of a segmented polyurethane and the lower back plate is made of a polycarbonate. Two Carbomedics$^R$ mechanical valves are used as inlet and outlet valves. It was tested in vitro and the cardiac output of the VAD was 7 L/min at a heart rate of 70 bpm. The line pressure from a pressure transducer inserted in the pneumatic line was recorded and compared with the ideal pressure tracing.

A Ventricular assist device (VAD) is one of the most efficient treatments to raise the survivability of the end stage heart failure patient. However, some of LVAD patients have died for the failures and improper control of LVAD. To detect critical dangers in LVAD, the monitoring methods of LVAD outflow have been requested, because it can be affected by patient's hemodynamic states and abnormal conditions of LVAD. In the case of an external pulsatile LVAD, the air movement through the air line can be used to estimate LVAD outflow. In this study, the air movement in the air-line of the extracorporeal pulsatile LVAD was measured with a differential pressure sensor between different points. The precise estimation of air movement could be achieved by additional measurement of air pressure. In a series of in-vitro experiments, the LVAD outflow were changed according to the afterload of LVAD and the differential pressure of LVAD didn't have close correlation with the LVAD outflow that were measured with an ultrasonic flowmeter at the same time. However, new precise estimation with the data from differential pressure and one point pressure in the air-line showed higher correlations with LVAD outflow.
