Irrigation-suction System for Management of Deep Sternal Wound Infection after Open Heart Surgery -Four cases- (개심술 후 발생한 심부 흉골 창상감염의 세척-흡입법을 이용한 치험 -4예 보고-)

  • Deep sternal wound infection (with or without acute mediastinitis) is a serious complication of open heart surgery, with high rates of associated morbidity and mortality, and prolonged hospitallization. The result of treatment largely depends on timely diagnosis and appropriate surgical management. Postoperative deep sternal wound infections in 4 cases were successfully treated with extensive debridement, pressurized jet irrigation with antibiotic solution, sternal refixation by Robicsek method, and dilute antibiotic irrigation via irrigation-suction system. We report 4 cases with review of articles.

  • Purpose: The purpose of this study is to evaluate characteristics of a deep infection after arthroscopic knee surgery. Materials and Methods: We selected 894 patients who underwent arthroscopic knee surgery between February 1994 and August 2006. We analyzed the results of the patients with definite infection. Results: Seven cases out of the 894 knee arthroscopic surgery which was performed by one surgeon during 12 years were diagnosed as postoperative deep infection (0.9%). Infection developed in one repair case among the meniscal surgeries (1419=0.2%). There were six infection cases in intraarticular ligament reconstruction (6/343=2%); 3 in ACL surgeries (3/152), 2 in PCL surgeries (2/70) and 1 in combined cruciate ligament surgery and extra-articular reconstruction (l/26). Conclusion: Postoperative infection rate of arthroscopic knee surgery was relatively low. However an attention for the prevention of postoperative deep infection should be paid in intraarticular ligament reconstruction because of its relatively high risk of infection.

  • Varicella zoster virus (VZV) causes two diseases: Varicella, a generalized, primary infection, and herpes zoster (zoster), a secondary infection caused by latent VZV reactivation. Zoster can also be caused by latent VZV reactivation after a varicella vaccination. The complications associated with varicella include cutaneous infections, which are the most common, as well as pulmonary and neurological involvement. However, a deep venous thrombosis (DVT) has been rarely described as a varicella-associated complication. Here, we describe the case of a child with varicella zoster who developed a DVT that completely resolved after intravenous acyclovir and subcutaneous low-molecular-weight heparin treatment.

  • '당뇨발'은 '당뇨환자에서 신경학적 이상이나 말초혈관 이상 때문에 발생하는 발의 궤양으로 감염이나 심부 조직의 파괴를 동반하기도 하는 발의 질병'이라고 정의할 수 있다. 하지만 실제로는 발에 궤양이 발생한 상태가 아니더라도 매우 포괄적인 용어(umbrella term)로서 당뇨병환자에게서 발생하는 발의 만성 합병증을 총칭하는 것이 일반적이다. 왜냐하면 발의 궤양을 동반하지 않는 괴저(괴사), 심부 감염, 신경병증성 관절증(Charcot arthropathy) 등도 당뇨발의 범주에 포함시켜야 하기 때문이다. 이렇게 보면 '당뇨발'이란 의학적 진단명으로는 적절하지 않은 용어라 할 수도 있다. 다만 이미 일반화된 용어로서 환자의 교육과 설명에는 더 유용하다고 할 수 있다.

  • The incidence of deep neck infection has been reduced by modern antibiotic therapy. Life-threatening deep neck infection of odontogenic or upper airway origin may extend to the thorax. Early diagnosis, administration of the potent antibiotics, and complete debridement and drainage are essential to improve the chances for survival in these very ill patients. We report two cases of deep neck infection which were complicated by thoracic infection such as mediastinitis, pericarditis, and empyema as a result of descending infection.

  • Necrotizing fasciitis is a rare, but life-threatening infection. Prompt diagnosis and early aggressive intervention is required for survival. However, there has been frequently occurred in delays of diagnosis and treatment due to its non-specific nature. Therefore, a high index of suspicion is needed to ensure timely intervention. We report a case of necrotizing fasciitis in a 7-day-old term healthy neonate.

  • Deep neck infections, which affect soft tissue and facial compartment of head and neck, may cause life-threatening complications despite the advent of antibiotics. The authors recently experienced 19 cases of these infections at Daegu Catholic Hospital and the following results were obtained. 1.The age of onset was from two to sixty one with the mean age of thirty nine, and 13 cases(68%) were male and 6 cases(32%) were female. 2.The sites of infections were submandibular space 11 cases(58%), parapharyngeal space 7 cases(37%), and retropharyngeal space 1 case(5%). 3.The isolated organisms of 12 cases were 6 cases(50%) of hemolytic streptococcus, 3 cases(25 %) of staphylococcus aureus, 3 cases(25 %) of no growth. 4.Without any remarkable complications, all cases were treated with a) surgical drainage with antibiotic therapy 13 cases(68%) b) conservative management 6 cases(32%)

  • Transesophageal echocardiography (TEE) is widely used to evaluate the heart function and the result of surgery during a cardiac operation. The incidence of complications associated with TEE is low, yet critical complications such as lower pharyngeal injury and esophageal perforation may happen. We report hereon a case of 77-year old male patient who suffered from injury to the pyriform sinus and concurrent deep neck infection after off pump coronary artery bypass surgery and intraoperative TEE.

  • The graft infection within the abdomen is a notorious condition, which usually develops serious complications of anastomotic rupture or distal embolism that ended in death. There has been many controversies in the treatment of an aortobiiliac graft infection and varying results have been reported. The authors treated a case of aortobiiliac graft infection after abdominal aortic aneurysm surgery. The operation was performed with re-aortobiiliac bypass using bilateral superficial femoral veins. The result was successful.
