The Effect of Paroxetine on Symptom Improvement and Change of Heart Rate Variability of the Patients with Panic Disorder (Paroxetine이 공황장애 환자의 증상 개선과 HRV 양상 변화에 미치는 영향)

  • Object : Since autonomic nerve system dysfunction was known as the mechanism of panic disorder, many researchers used heart rate variability (HRV) as means of measuring autonomic nerve function of patients with panic disorder. We aimed to examine the effect of paroxetine medication for 3 months on symptom improvement and change of heart rate variability of the patients with panic disorder. Methods : The subjects were patients with panic disorder who visited the psychiatric outpatient clinic of Samsung Medical Center in Seoul. We included panic disorder patients who were aged from 20 to 50 and in normal BMI range (from 18 to 30) to minimize the effect of age and weight on HRV data. We excluded the patients with EKG abnormalities, hypertension or other major psychiatric disorders. They took 20-40 mg paroxetine medication a day for 3 months. Alprazolam was used only during the first month to control the acute panic symptoms and was tapered off after that. We measured the acute panic inventory (API), Hamilton rating scale for anxiety and depression (HAM-A & HAM-D), Spielberger state-trait anxiety inventory (STAIS, STAIT), and Beck depression inventory (BDI) in order to assess clinical improvement of the patients. And we measured time and frequency domain HRV in the resting, standing and cognitive stress states to assess the change of HRV. All measurements were done before and after paroxetine treatment. Result : After paroxetine medication, patients showed significant improvement in all psychiatric scales. In time domain of HRV, standard deviations of all R-R intervals (SDNN) were significantly increased in all states. In frequency domain of HRV, the ratio of high frequency to total power (HF/TP) in the standing state was significantly increased. Conclusion : After 3 months paroxetine medication, panic disorder patients showed significant clinical improvement and change in HRV data such as SDNN in all states and HF/TP ratio in the standing state. This result suggests that paroxetine medication is effective for the improvement of autonomic nerve system dysfunction in panic disorder patients.

Effects of Thoracic Flexibility Exercise Program on Pain, Heart Rate Variability, and Depression in Patients with Chronic Low Back Pain (흉추 가동성 운동프로그램이 만성 요통 환자의 통증과 심박 변이도 및 우울증에 미치는 영향)

  • Purpose : This study aimed to investigate the effects of a thoracic flexibility exercise program on pain, heart rate variability (HRV), and depression in patients with chronic low back pain. Methods : Fifty patients with chronic low back pain participated in this study. Each participant performed a pre-test and post-test. The outcome measures evaluated were a visual analog scale, HRV test results (automatic activity, standard deviation normal to normal [SDNN], low frequency [LF], high frequency [HF], and LF/HF ratio), and the Beck Depression Inventory. Patients underwent conventional physiotherapy for 35 minutes per session. In addition, a thoracic flexibility exercise program (trunk rotation exercise, McKenzie exercise, and supine thoracic extension exercise) were performed 3 times per week for 4 weeks. Results : After the training period, the visual analog scale results showed a significant decrease in the post-test compared to the pre-test. Automatic activity, SDNN, LF, HF, and LF/HF ratio significantly increased in the post-test compared to the pre-test. The Beck depression inventory results significantly decreased in the post-test compared to the pre-test. Conclusion : This study demonstrated that a thoracic flexibility exercise program improves a visual analog scale, HRV (automatic activity, SDNN, LF, HF, and LF/HF ratio), and the Beck Depression Inventory in patients with chronic low back pain.

Mild Cold Stimulus on Forehead of Healthy Men and Heart Rate Variability (이마의 냉각자극이 건강인의 심박 변이율에 미치는 영향에 대한 실험적 연구)

  • The aim of this study was to investigate the change of Heart Rate Variability(HRV) that mild cold stimulus on the forehead of healthy men induces. 34 healthy male subjects participated in the experiments. On the first test series, 15 subjects were applied to the mild cold stimulus by the devise for cold stimulation. In the second test series, 10 subjects With heat Syndrome and 5 subjects With cold Syndrome were applied to the mild cold Stimulus. Aa a additional test, 4 subjects with cold syndrome were applied to the warm stimulus in the last test series. We analyzed the HRV through measuring electrocardiogram.(ECG). The result of this study is comparatively clear. In the first test series, mild cold stimulus made parasympathetic nervous system be activated. In the second test series, mild cold stimulus made parasympathetic nervous system be activated both in subjects with heat syndrome and subjects with cold syndrome, and heat syndrome shows more active parasympathetic nervous system rather than cold syndrome subjects do. In the last test series, 2 subjects with cold syndrome respond the mild cold stimulus. That means warm stimulus of cold condition subjects made parasympathetic nervous system active in 2 of 4 subjects. We found out that mild cold stimulus on forehead makes parasympathetic nervous system be activated in healthy male, and it can be interpreted that mild cold stimulus make healthy human be relaxed. Also, subjects have different fondness of thermal stimulus according to their Cold or Heat condition Preferences. Last test series shows that we need to investigate effect of the warm stimulus heat syndrome subjects.

A Study on the Relation between Anemia and Heart Rate Variability in Postpartum Women (산모에서 빈혈과 심박변이도의 관계 연구)

  • Purpose : This research was designed to determine whether HRV can be used as an assessment index in weakly postpartum women. Methods : The subjects were 232 women who admitted for postpartum care in Oriental Hospital of Woosuk University from 19, December 2005 to 21, July 2006. They took the test of CBC and HRV. And then we selected 200 cases of postpartum women. The SPSS 10.0 for windows was used to analyse the data and the simple correlation coefficient and the independent samples t-test were used to verify the results. Results : The following results were obtained : 1. There were positive correlations between items of CBC(RBC, Hb, Hct) and SDSD, RMSSD, SDNN, and also between items of CBC and log-transformed index of spectral analysis. There were negative correlations between items of CBC and Mean-HRV, pNN50. 2. SDNN of anemic postpartum women group significantly decreased compared with that of normal postpartum women group. HRV-Index, RMSSD and SDSD of anemic postpartum women group significantly decreased compared with that of normal postpartum women group. pNN50 of anemic postpartum women group significantly increased compared with that of normal postpartum women group. 3. Ln(TP), Ln(VLF), Ln(LF) and Ln(HF) of anemic postpartum women group significantly decreased compared with that of normal postpartum women group. Conclusion : The results suggest that the function of heart of anemic postpartum women group significantly decreased compared with that of normal postpartum women group.

Effects of Hwangryunhaedoktang Herbal-acupuncture at G21(Kyonjong: 肩井) on the Heart Rate Variability (견정혈(肩井穴) 황련해독탕약침(黃連解毒湯藥鍼)이 심박변이율(HRV)에 미치는 영향)

  • Objective : In this study, we investigated the effects of Hwangryunhaedoktang Herbal-acupuncture(HHT) at a acupoint, $G_{21}$(Kyonjong) on the Heart Rate Variability(HRV) in adults, as well as we tried to observe how HHT affects on the balance of the autonomic nervous system. Methods : The subject were comprised of 20 healthy adults, who didn't take any drugs which affected on the autonomic nervous system and had normal sinus rhythm in electrocardiogram. The control group was comprised of 9 subjects, who were injected normal saline at $G_{21}$(Kyonjong). The experiment group consisted of 10 subjects, who were injected HHT at $G_{21}$(Kyonjong). We measured HRV by CANS 3000 on three times : before, immediately after, and 20 minutes after injection. The SPSS 10.0 for windows was used to analyze the date and the Wilcoxon signed rank test (in group) and Mann-Whitney U-test(between two groups) were used to verify the results. Results : The following results were obtained ; 1. In each group, Mean-HRV and Mean-RR showed a significant change during 20 minutes after injection. But there were no significant difference between two groups(p>0.05). 2. In experiment group, 20 minutes after injection, norm LF and LHR showed a significant decrease and norm HF showed a significant increase. There were significant difference between two groups(p<0.05). Conclusions : The findings suggest that HHT at $G_{21}$(Kyonjong) in adults relatively activate parasympathetic nervous system and inactivate sympathetic nervous system.

Study on the Change of the Autonomic Nervous System by HRV Test in Musculoskeletal Pain Patients Treated with Acupuncture (근골격계 통증 환자에서 침 치료 후 심박수 변이도(Heart rate variability) 변화에 관한 연구)

  • Objectives : This study was designed to evaluate the changes of autonomic nervous system by heart rate variability(HRV) test on the musculoskeletal pain patients treated with acupuncture on the Back-su points and the unfixed points, and untreated healthy adults. Methods : This clinical study have been carried out with 64 cases of musculoskeletal pain patients and 26 healthy adults. 35 patients were treated with acupunctureon on the Back-su points(Group I) and 26 patients were treated on the Unfixed points(GroupII), and two groups were tested by HRV at pre and post acupuncture treatment. 26 healthy adults(GroupIII) were tested twice by HRV without any stimulation. Results : In Group I, VAS was decresased $5.83{\pm}1.62$ to $5.17{\pm}1.89$ significantly(p<0.05) after acupuncture treatment, and LF/HF ratio was decreased significantly(p<0.05) as compared with healthy control Group. In GroupII, SDNN & RMSSD were increased singnificantly(p<0.05) as compared with other Groups. in Group I & II, MHRT was decreased significantly(p<0.05) as compared with healthy control Group. Conclusions : These results suggest that acupuncture treatment on the Back-su points was able to reduce the pain, suppress the sympathetic activities and enhance the parasympathetic activities in musculoskeletal patients.

Comparison of Psychological and Physiological Differences of Human due to the EEG Type Scent (뇌파유형별 향기에 따른 인체의 심리적 및 생리적 차이 비교)

  • In this study, the three scents effects on subjects due to EEG type, and it researched to find optimal scent to increase their amenity and productivity. EEG and HRV were compared and analyzed in the environmental test room by classifying subjects into two type: A and B. The condition of the environmental test room was in temperature 31[$^{\circ}C$], relative humidity 50[RH%], air current speed 0.02[m/s] and illuminance 1000[lux] with setting up three different scents which are Rosemary, Jasmine and Hazelnut. The result of this study, at Jasmine scent for A and B type, relative ${\alpha}$ wave, SEF50, $\frac{SMR}{\theta}$ and SDNN were revitalized, and ${\alpha}$ wave asymmetry index, HRT, stress index and fatigue degree were decreased. It was found that favorite scent of A and B type is same as Jasmine scent, but A type was more sensitive than B type about scent. Therefore Jasmine scent is very effective to increase amenity, productivity and concentration, and to decrease stress and fatigue degree.

Correlation between HRV and Bowel Sound Parameters in the Evaluation of Gastrointestinal Parasympathetic Nerve Activity from Patients with Functional Dyspepsia (기능성 소화불량증에서 위장관 미주신경 활성 평가를 위한 심박변이지표와 장음지표의 상관성 연구)

  • Objectives : The aim of this study was to investigate the evaluation of gastrointestinal parasympathetic nerve activity by comparing between HRV and bowel sound parameters in patients with functional dyspepsia. Methods : 62 patients (male 22, female 40) were enrolled. HRV was measured in all patients. and TP (total power), LF (low frequency density: 0.04$\sim$0.15Hz), HF (high frequency density: 0.15$\sim$0.4Hz). and LF/HF ratio were analyzed. HF band is a reflection of parasympathetic activity. and the LF-to-HF ratio is considered a marker of sympathovagal balance. Bowel sounds were recorded at the day after, and postprandial %BS was used to indicate the gastric vagal activity after eating. HRV and bowel sound parameters were compared. and correlation between them was analyzed. Results : There was no difference in LF/HF ratio between patient group and healthy group of other report. According to values of %BS, each patient was classified into a normal (>6 %BS) or hypoactivity (<6 %BS) of gastric vagus nerve group. HF component of the hypoactivity group was not significantly higher than that of the normal group. There was no correlation between HRV and bowel sound parameters. Conclusions : Because no correlation was found between HRV and bowel sound parameters in patients with functional dyspepsia, we concluded that it is not proper to evaluate gastric vagus nerve activity by HRV measurement.

  • The purpose of this review was to investigate the neurophysiological and neuroimaging characteristics of patients with depression and anxiety reported in previous studies. A literature search was conducted using Medline and psychiatric textbooks. "Electroencephalography (EEG)", "Event Related Potentials (ERP)", "functional neuroimaging", "heart rate variability (HRV)" and "depression or anxiety" were used as key words. A physiological finding indicated that there was a higher degree of relativity with regards to prefrontal dysfunction in patients with depression. Right prefrontal lobe hyperactivity and left prefrontal hypoactivity were consistently observed, and abnormalities were observed in other regions (ACC, hippocampus, amygdala, etc.). Therefore, dysfunctions in these areas are related to depressive symptoms. In patients with anxiety disorder, each emotional condition showed specific activation patterns in different brain regions, such as the prefrontal cortex, occipital lobe, temporal lobe, hippocampus, and limbic system, including the amygdala. However, in the majority of patients with anxiety disorder, the degree of activation was higher in the right hemisphere than in the left hemisphere. The current data supports that there is a difference in brain dysfunction characteristics between depression and anxiety and that the different activations of various brain regions would play a significant role in the pathophysiology of depression and anxiety disorder.

A Case Report on the Use of Heart Rate Variability and the Treatment Effect of Bunshimgi-eum on Autonomic Dysfunction After Cerebral Infarction (뇌경색 후 발생한 자율신경 장애에 대해 심박변이도로 살펴본 분심기음의 효과 증례보고 1례)

  • Background: The aim of this study is to investigate the use of heart rate variability on a stroke patient with autonomic dysfunction and the effectiveness of Bunshimgi-eum. Case report: The patient showed autonomic dysfunction, including chest discomfort, 煩燥 (sweating, anxiety, insomnia, nausea, lightheadedness), and abdomen discomfort (dyspepsia, heartburn) after ischemic stroke. She was treated with a herbal medicine (Bunshimgi-eum) through complex Korean medicine treatment. The effect was evaluated using heart rate variability (HRV), Mini-Mental State Examination-DS (MMSE-DS), mean pulse rate, and subjective percentage of symptoms. After treatment, the HRV score, MMSE-DS score, and subjective percentage of symptoms improved, and the pulse rate decreased from the upper boundary value. Discussion: The results suggest that HRV may be useful in evaluating stroke patients with autonomic dysfunction and that Bunshimgi-eum is suitable for treatment.
