PULSE RATE AND OXYGEN SATURATION IN CHILDREN DURING ROUTINE RESTORATIVE DENTISTRY (소아 치과치료시 심박동과 동맥혈 산소포화도의 변화)

  • Pulse oximeter to monitor oxygen saturation during pediatric dental sedations enables early detection of hypoxemia. The purpose of this study was to monitor the hemoglobin oxygen saturation level and pulse rate of nonmedicated pediatric patients during routine restorative procedures to study the effect of procedure and treated jaw. We obtained data from 53 children treated at the Department of Pediatric Dentistry, Chonbuk national university hospital. Pulse rate and oxygen saturation were measured and recorded using pulse oximetry at each step of treatment. The results are as follows: 1. In non-anesthesia group, steep increase of pulse rate was observed during rubber dam application in the maxilla and during cavity preparation in the mandible. 2. In anesthesia group, pulse rate started to decrease after the rubber dam application in the maxilla, while its decrease observed since cavity preparation step in the mandible. 3. In non-anesthesia group, oxygen saturation level was relatively constant during all steps in the maxilla, but in mandible, it was higher during operation compared to its pre and post operation baseline. 4. In anesthesia group, oxygen saturation level was observed at 99% level through all steps in both jaw groups, and there was no statistical significance between the maxilla and the mandible groups(p>0.05). The purpose of this study was to evaluate the effect of routine dental treatment on the pulse rate and oxygen saturation level in nonmedicated pediatric patients during routine restorative procedures in the maxilla and mandible.

  • 본 논문은 음악 리듬의 비트간의 간격의 변화가 사람의 심박동 및 쾌적감에 미치는 영향을 평가하기 위하여 음악 리듬의 박자를 규칙적인 다섯 단계의 변화를 적용하여 평가했다. 평가 항목으로는 (1) LF성분에 많은 영향을 보이는 심박동의 전력스펙트럼 분석과 (2) S.D.법으로 심리적 평가항목을 수행하였다. 실험 결과는, 박자의 변동이 일정한 정박자 리듬에서는 생체에 특이한 변화가 나타나지 않았지만 박자의 변동이 변화하는 리듬에서는 LF성분이 증가함으로 긴장과 불안이 유발됨을 알 수 있었다. 추가적으로 심박변동의 스펙트럼 분석를 통하여 얻을 수 있는 LF와 HF 성분사이의 비를 조사함으로 심리학적 불안을 평가하는 가능성을 제안한다.

  • 최근 유비쿼터스 헬스케어가 부각됨에 따라 심장의 활동상태를 보다 편리하게 측정하기 위하여 이동성 및 휴대성을 강조한 심전도 계측 시스템 및 스트레스 상태를 분석하기 위한 연구들이 수행되고 있다. 하지만 기존 심전도 계측은 전극의 부착 및 계측시스템과의 연결을 위한 리드선의 사용으로 인해 활동의 불편함을 유발한다. 본 연구에서는 기 연구 수행된 가정 또는 사무실에서 무구속적인(unconstrained) 방법으로 지속적인 심장의 활동상태의 모니터링이 가능한 무구속 의자형 심탄도 계측 시스템을 구현하였다. 무구속적인 방법으로 심탄도 신호를 계측하고 심탄도신호로부터 심박동변이율을 추출함으로써 일상생활 중 스트레스를 모니터링 하고자 하였다. 구현된 시스템을 통한 스트레스 모니터링의 가능성을 평가하기위하여 안정상태의 심박동변이율과 인위적인 신체적 스트레스인 Valsalva 조작를 유도한 후 심박동 변이율을 비교평가 하였다. 건강한 대학생 10명을 대상으로 비교분석을 수행한 결과 안정 상태와 육체적 스트레스 인가 후 심박동 변이율의 변화 양상을 관찰할 수 있었으며, 무구속적인 방법에 의해 스트레스의 모니터링이 가능함을 확인 할 수 있었다.

  • 본 연구에서는 일상생활에서 보다 편리하게 건강모니터링을 수행하기 위해 신체에 착용 가능한 심전도 및 맥파 계측 시스템을 구현하고자 하였다. 이를 위하여 배터리로 구동 가능한 초소형의 심전도 및 맥파 측정 시스템을 구현하였으며, 계측된 생체신호의 무선전송을 위해 초저전력 무선 센서네트워크 기술을 적용한 무선 생체신호 전송시스템을 구현하였다. 무선으로 전송된 심전도 및 맥파 신호는 잡음 제거 및 심박동을 검출하기 위하여 전처리과정과 적응 가변형 문턱치를 적용하였으며, 검출된 심박동으로부터 동맥순환계의 긴장도 및 유순도의 변화를 반영하는 맥파전달시간(pulse transit time, PTT)을 계산하였다. 구현된 무선 맥파전달시간 계측시스템과 기존 상용시스템의 비교 평가를 수행함으로써 구현된 시스템의 유용성을 평가하고자 하였으며, 혈압 및 맥파전달시간의 동시계측을 통해 자세 변화에 따른 혈압의 변화 및 맥파전달시간의 변화양상을 관찰함으로써 혈압과 맥파전달시간의 관계를 추정하고자 하였다.

  • In mobile healthcare device, when to measure the heart rate by using the PPG signal, its performance is reduced according to the motion artifacts that is the movement of user. This is because the frequency range of motion (0.01-10 Hz) and that of PPG signals overlap. Also, the motion artifacts cannot be rectified by general filters. To solve the problem, this paper proposes a method using filter banks and independent component analysis (ICA). To evaluate the performance of the proposed method, we were artificially applied various movements and compared heart rate errors of the moving average filter and ICA. In the experimental results, heart rate error of the proposed method showed very low than moving average filter and ICA. In this way, it is possible to measure stable heart rate if the proposed method is applied to the healthcare terminal design.

Effects of Nasal Continuous Positive Airway Pressure Application on Sympathetic Activation : Power Spectrum Analysis of Electrocardiogram in Obstructive Sleep Apnea Syndrome (폐쇄성 수면무호흡증에서 지속적 상기도 양압술 시행이 교감신경계 활성도에 끼치는 영향 : 심전도 스펙트럼 분석)

  • Objectives: Obstructive sleep apnea syndrome(OSAS) is known to be associated with the changes of autonomic nervous system (ANS). Nasal continuous positive airway pressure(nCPAP) treatment was found to correct abnormal ANS changes in OSAS but it remains to be further clarified. We aimed to assess the effects of nCPAP on ANS manifested on electrocardiogram, using spectrum analysis in the subjects with OSAS. Methods: Digital polysomnography was performed in 18 patients with OSAS(mean age $43.7{\pm}16.6$ years ; 17 males, 1 female ; mean respiratory disturbance index (RDI) $48.6{\pm}20.9$) for one baseline and another CPAP nights. From each night, 300 continuous beats of ECGs without artifact were chosen from both stage 2 sleep and REM sleep and they were used for power spectrum analysis. We compared between baseline and CPAP nights the heart rate variability including VLF(very low frequency power), LF (low frequency power), HF(high frequency power), R-R means, R-R variance, and LF/HF ratio, using Wilcoxon signed ranks test. Results: In all patients, nCPAP proved to be effective in relieving apneas and snoring. During nCPAP night compared with baseline night, decreases in VLF(p<0.05), LF(p<0.01), and R-R variance(p<0.05) were found in stage 2 sleep, and decreased LF(p<0.05) was found in REM sleep. No significant differences in each sleep stage were found in other variables between the two nights. Conclusion: Our findings suggest that OSAS increases the activity of sympathetic nervous system and nCPAP application effectively decreases the activity. And nCPAP does not appear to influence the parasympathetic nervous activity in OSAS.

  • 심장관련 질환은 현대사회에서 업무 과중과 스트레스에 의해 발병 가능성이 높아지고 있으며, 일상생활 중 건강상태를 지속적으로 모니터링하여 심장질환 관련 응급상황에 대처하기위한 많은 연구들이 수행되고 있다. 본 연구에서는 가정 또는 사무실에서 무구속적인 방법으로 지속적인 심장 활동상태의 모니터링이 가능한 무구속 의자형 심탄도 계측 시스템을 구현하였다. 구현된 시스템에서 계측된 심탄도 신호로부터 건강모니터링을 위한 특징성분을 검출하기위해 웨이브렛 변환과 템플릿 매칭을 혼합한 신호처리방법을 제안하였다. 또한 적응 문턱치를 통해 심탄도 신호에서 심박동을 검출하였으며 심박동의 간격으로부터 HRV(heart rate variabillity)를 계산하였다. 구현된 시스템의 성능평가를 위하여 심전도와 동시에 심탄도를 측정하였으며, 두 신호로부터 심박동 검출 성능을 비교하여 구현된 무구속 의자형 심탄도 계측 시스템의 유용성 및 무자각 건강모니터링의 가능성을 확인하였다. 또한 스트레스에 따른 HRV의 변화를 관찰하기 위하여 피실험자로부터 인위적으로 숨을 참고 강제호기를 통해 흉강내압을 증가시켜 인위적인 육체적 스트레스를 가하는 발살바를 유도하였으며, HRV의 시간 및 주파수 영역에서 도출되는 파라미터들을 평가하여 심탄도 모니터링을 통해 안정 상태와 스트레스 상태의 판별 및 무구속 건강모니터링의 가능성을 평가하였다.

Effects of Heart Rate Variability after Visiting the Gotjawal Forest in Jeju (제주 곶자왈숲 삼림욕 후에 심박동변이에 미친 영향)

  • The purpose of this study is to statistically investigate the changes in mean differences of the vascular age types and index, heart beating, vessel elasticity, diastolic reserves contents measured by the fingertip plethysmogram before and after the subjects (49 people) visit the Gotjawal forest for two hours. While there is no significant change in the mean differences of vascular age types and index, heart beating, diastolic reserves contents in the control group before and after the visit, the differences in the test groups gets higher than the control group and shows a significant difference. Moreover, the variance analyses of the mean values of the contents shows a significant change after the visit. There is no significant change in the values of vessel elasticity changes between the groups after the visit, but the change of values is significant in the within group, and it is not significant between group by variance analysis. The differences of the heart beating was insignificant in the within group, but significant between group after the visit. In conclusion this study proves the effects of forest bathing, because the HRV index significantly improved after the bathing. Therefore the b-c-d-e/a ratio may be useful for evaluation of HRV.

  • Recently, economic growth in the world has increased interest in healthy life, and the smart health care industry is growing. In the field of smart healthcare, wearable-type biometric information measurement technology has been highlighted due to the importance of IoT technology. The purpose of this study is to develop a wearable heart - rate monitoring system that can be applied to wearable health care and glove - type monitoring that enables convenient monitoring of heart rate during activity. For this purpose, a glove - type wearable health care system was developed and its performance was evaluated. Experimental results showed that the heartbeat monitoring was possible even in the presence of actual daily activities.

Factor Analysis of Decreased Score on Coronary Artery Calcium Score (관상동맥 석회화점수 감소 요인 분석)

  • The purpose of our study was to retrospectively evaluate the cause of a decreased calcium score of follow-up studies on coronary artery calcium scores (CACs) computed tomography (CT). The subjects were healthy 100 people(85 males $60.6{\pm}6.9$ years, 15 females $67.2{\pm}7.3$ years). The subjects decreased CACs were divided into 4 subgroups depending on Agatston classification, minimal (1-10), mild (11-100), moderate (101-400), severe (400<). As a result of decreased CACs were scan location disagreement 51%, motion artifact 26%, equipment changes 14%, operator mistakes 5%, input miss 2%, image loss 1%, arrhythmia 1%. In the mild group, the most common decreased CACs were 49 people. In the minimal group, the most significant variation reduction has occurred to 6 people. Scan location disagreement was considered a partial volume effects due to the scan starting position. It showed less than 100 CACs a high variation (19.7%) in more than 100 CACs, a lower variation (2.2%), these could be seen that the variation range is different that can be tolerated according to the calcification score. Motion artifact factor was found in 26%, which is so closely related to the preceding tests that affect the higher heart rate like this pulmonary function test, exercise stress test.
