Prosthetic rehabilitation of partially edentulous patient after maxillectomy: A case report (상악골 부분절제술 시행한 부분 무치악 환자에서 폐쇄 장치를 이용한 보철 수복 증례)

  • Patients undergone partial maxillectomy experience post-operative masticatory, phonetic, and swallowing difficulties. They also encounter social and psychological challenges due to changes in their facial appearances. Thus, functional and esthetic recovery through maxillofacial prosthesis becomes significant for these patients. The objective of an appropriate obturator is to restore palate and improve phonetic and swallowing ability by separating the oral cavity, nasal cavity, maxillary sinus, and nasopharynx. In this case report, an obturator was fabricated for a patient who had partial resection from the maxillary posterior region to the pharynx due to squamous cell carcinoma. The purpose of this case study is to describe the results because the patient was successfully improved both functionally and esthetically.

  • Excessive teeth wear may result in the complications such as esthetic problems, hypersensitivity, and loss of vertical dimension. This clinical report focuses on the causes of severely worn dentition and the full-mouth rehabilitation of a patient with rheumatoid arthritis for 20 years. An interview, clinical and radiological examinations were performed to analyze the causes and decide the treatment plan. After delivery of the final prostheses, a night guard was used to protect the restorations and temporomandibular joints. The patient was satisfied esthetically and functionally.

  • Ectodermal dysplasia is a hereditary disease characterized by a congenital dysplasia of one or mote ectodermal structures. Characteristic manifestations Include scanty hair and eyebrows, pigmented and hyperkeratinized skin around the eyes and mouth, frontal bossing with prominent supraorbital ridges, nasal bridge depression and dental anomalies. Hyperthermia or unexplained high fever as a result of the deficiency of sweat glands is common medical history. Findings of intraoral structures are anodontia or oligodontia with conical crowns. Consequently, generalized spacing and loss of vertical dimension of occlusion. Interdisciplinary approach has been performed to treat a 10-year old boy with ectodermal dysplasia. Orthodontists and a prosthodontist worked together on this case, and the result was satisfactory.

  • The open bite malocclusion is a common clinical entity and has multifactorial causes. Development of effective treatment plan and management is dependent on proper diagnosis. The skeletal open bite patient requires a coordinated orthodontic and orthognathic surgical approach to achieve stable occlusion, acceptable esthetics, and improved function. But in case of open bite with severely decayed dentition, restoration in the entire dentition is necessary. Using the facial analysis and diagnostic wax-up, the most effective treatment was prosthetic rehabilitation. The provisional restorations were fabricated to satisfy esthetic and functional requirements, which result in the uniformly distributed occlusal force, anterior and canine guidance. The inter-arch relationship, labio-dental harmony, and the soft tissue aspect, which is important to estimate the longevity were evaluated. Definitive restorations of monolithic zirconia were made by replicating provisional restorations by using the latest CAD/CAM technology. They were delivered to the patient and clinical follow-up observation was satisfactory.

A study on exposure length of Maxillary central incisor in rest position in Korean (한국인의 안정위시 상악중절치 노출량에 관한 연구)

  • This study is intended to provide a referable information of exposed amount of maxillary central incisor of Korean by ages and gender under rest position. The result of this study will give guidelines for making prothesis. The subjects of this study are patients of Charmgoun Dental Hospital in Busan, Korea. A statistical analysis was conducted after taking digital photos of patients' teeth with a ruler and measuring the length of teeth on the computer program. The results of this study were as follows : 1. The length of maxillary central incisor that exposed under upper lip is decreased by increasing age in rest position.; the average length is 3.455mm in 20s, 2.525mm in 30s, and 1.543mm in 40s. 2. The exposal length in females is more than males, average length is 2.796mm in female and 2.342mm in male. However, there is not significant difference between the genders. 3. The exposed average length of maxiallry central incisor under upper lip is 2.618mm at rest. 4. The clinical crown average length of maxillary central incisor is 10.195mm, but incresing age, there is no significant defference. 5. There is significant difference between the genders in the clinical crown length of maxillary central incisor.; the length is 10.637mm in men, 9.90mm in women.

A STUDY ON THE BLEACHING EFFECTIVENESS OF BLEACHING AGENTS AND THE APICAL LEAKAGE ACCORDING TO THE LOCATION OF THE INTRACORONAL BASE (무수치 표백시 표백제의 성분에 따른 표백효과 및 이장재의 위치에 따른 치근부 누출에 대한 실험적 연구)

  • The purposes of this study were to compare the bleaching efficacy of sodium perborate when mixed either with Superoxol or distilled water and to evaluate the efficacy of different location of intracoronal base on the presence of apical leakage of tested bleaching agents. Forty eight extracted human permanent incisors were stained via whole blood and canal fillings with conventional gutta percha were performed after routine biomechanical perparations. The experimental intracoronal base was placed either at the cementoenamel junction(group 3, 4) or 2mm below cementoenamel junction(group 1, 2). Walking bleaching was performed by two different combinations of bleaching agents : sodium perborate with distilled water in group 1, 3 and sodium perbrate with Superoxol in group 2, 4. The roots of the teeth were evaluated for the presence of color change to assess the leakage of bleaching agents and the cervical one-thirds of the crown were evaluated for bleaching effect from the whiteness Indea calculated by spectrophotometer. The results were as follows : 1. At the end of 12 days, all the sample teeth demonstrated the increase of Whiteness Index at cervical 1/3 of crown although there were some minor differences among groups. 2. Regardless of location of the base, sodium perborate with superoxol(group 2, 4) showed better results in bleaching than the sodium perborate with distilled water(group 1, 3). 3. Bleaching agent leaked into the root area when the base was placed 2mm below cementoenamel junction but no leakage was found when the base was placed at the cementoenamel junction.

  • Tooth impaction is defined as a cessation of the eruption of a tooth at the level of the oral mucosa or alveolar bone. Maxillary canines are the most frequently impacted teeth next to the third molar. Maxillary canine impaction is associated with congenital missing of lateral incisors, peg lateralis and genetic factors such as ectopic positioning of a tooth germ. The clinicians have an important role in early detection of tooth impaction for prevention of esthetic and functional problems. There are specific methods to treat impacted tooth for different conditions. In this case, an 11-year-old girl with a horizontally impacted maxillary right canine in a palatal position was treated through orthodontic traction along with surgical button attachment procedure. On regaining of eruption space, canine traction was performed. At the completion of treatment, the canine was positioned fairly within the arch with proper keratinized gingiva and complications such as root resorption were not observed.

  • Fusion is defined as union of two separate tooth buds at some stage in their development with confluence of dentin and characterized by separate root canal and large single crown, while gemination is defined as an attempt of the single tooth bud to incompletely divide and usually result in a single root with one root canal and two completely or incompletely separated crowns. It is sometimes difficult to decide whether an abnormally large tooth is the result of fusion of a normal and a supernumerary tooth, or of gemination; use of the term 'Double tooth' may make the clinicians avoid this difficulty(Brook & Winter). Commonly there are no symptoms, but the problems associated with these anomalies include esthetics, possible loss of arch length and delayed or ectopic eruption of the permanent teeth, caries along the line of demarcation, and periodontal disease. Commonly, it dose not need to be treated in primary dentition but in case of permanent dentition, it may be requested to be treated due to esthetics and other problems. In our case, a 8 years old girl showed a Double tooth, we attained the favorable results by performing hemisection with apexification.

Reconstruction of implant prostheses under infraocclusion: a case report (저위교합된 임플란트 보철물 재수복 증례)

  • Implant has been an effective treatment option for the patients with oligodontia. However, it still remains unclear when the implant should be placed. Skeletal growth that can appear even after the growth period can lead to infraocclusion of the implant which can cause functional or esthetic complications. In order to minimize these problems, definitive restorations should be placed after the functional and esthetic rehabilitation is achieved through the use of provisional restorations. Definitive restorations made with monolithic zirconia were created by replicating provisional restorations by using the latest CAD/CAM technology. These definitive restorations were delivered to the patient and clinical observation after the treatment showed satisfactory result.

  • Functional and esthetic recovery through reconstruction of the oral and maxillofacial system is important for patients who underwent mandibulectomy due to lesions such as oral cancer. The fibula free flap, which is frequently used in the reconstruction of the mandible, is easy to prosthetic restoration with implant placement. However, patients with growing periods have difficulty in treatment with implants when the size of the fibula is small and residual growth remains. This case is an 11-year-old female patient who underwent mandibulectomy due to osteosarcoma and reconstruction of the mandible using a fibular free flap. Functional and esthetic recovery were obtained through prosthetic restoration using implant-supported fixed prostheses. Implantation site, number of implants, and type of prostheses were considered. Implant-supported fixed prostheses were delivered at 13 years old, and after an additional 1 year observation, stable results were obtained. However, observing changes in the maxillofacial system through further examinations is necessary.
