Full mouth rehabilitation of a patient with severe tooth erosion with a digital crown lengthening guide (심한 부식 환자의 디지털 치관연장술 가이드를 이용한 전악 수복 증례)

  • In the oral cavity, the teeth undergo wear and corrosion throughout their lives. Progressive and constant tooth wear is a natural phenomenon of aging, but wear and corrosion due to specific factors are pathological factors. It can cause pathological damage of the occlusal surface, aesthetic problems, dimensional loss and jaw joint disorders. This case is a 26-year-old female patient with general tooth abrasion and erosion on the entire dentition. Diagnostic wax-up was fabricated based on the information including digital facial analysis, physiological stabilization, and evaluation of anterior crown length. Through the digital analysis, the necessary guides for crown lengthening were prepared and the mastication function and esthetics were evaluated by using temporary crowns. Definitive prosthesis was fabricated with the zirconia restorations. The results were satisfactory when they were observed 3 months of follow-up.

  • Aesthetics means differ from cultures and times and also differ from each person's own thinking. But as a restorative dentist who works for esthetic areas, we need to have certain principles and rationale. Some functions in CAD/CAM looks very useful to match harmonious shape to adjacent teeth and also very effective and efficient when compare to the traditional laboratory technics. Also we will discuss about link abutment (Titanium-Zirconia abutment) which we need for dental implant placed anterior area where the soft tissue is thin.

  • In maxilary anteriors, aesthetic aspect are of critical importance. but it is difficult to achieve esthetic results because of the narrow buccal-lingual alveolar bone width compared to the posterior teeth and alveolar bone resorption during tooth extraction. This case report describes how to minimize alveolar bone resorption and soft tissue collapse when immediate implant placement is done after extraction of the maxillary anterior teeth due to trauma.

  • Recently, many patients undergo anterior prosthetic treatment for esthetic reasons. In some patients complain about functional reasons such as inaccurate pronunciation and occlusal discomfort after the treatment. Anterior teeth are important esthetically but in the occlusal point of view, anterior guidance is the second most important factor in occlusion, next to centric relation. Failure to determine an appropriate anterior guidance might lead to posterior occlusal interference, which can highly affect the stability of the posterior teeth. Also, discomfort might occur if the customized interior guidance is not in harmony with the patient's envelope of function. The patient in this case complained of overall discomfort in the maxillary anterior area after prosthetic treatment. The expressed difficulty in pronunciation, unstable occlusion due to lack of stable holding contact and discomfort of the facial muscles. Maxillary anterior prostheses were refabricated through systematic diagnosis and treatment and thus, this case presents esthetical and functional satisfaction to both the patient and the dentist.

  • Immediate implant placement and restoration have the advantage of shortening the treatment period and maintaining aesthetics until final restoration. However, single implant restoration in the maxillary anterior region may be aesthetically difficult, and surgical trauma delivered to soft and hard tissues during implant placement may affect future aesthetic results. In order to obtain predictable results during anterior implant placement and restoration, surgery and restoration plans must consider the height of the interdental bone, characteristics of the gingiva, and morphological characteristics of the patient's teeth. In this case, we attempted to reproduce the emergence profile and stabilized soft tissue through gingiva modification and digital impression of a labially placed maxillary anterior implant in a patient who desired porcelain fused-metal (PFM) restoration due to economic issues.

  • Nowadays, the various developments of the CAD/CAM have been coming to realization at an intimidating speed. However, in the field of the aesthetic dentistry, the extremely natural aesthetics must be realized eventually by human hands, and for this purpose, it is considered that the endless challenges and practitioners' own beliefs are necessary. While practitioners are normally exposed to surgical methods of aesthetic restorations via domestic and foreign books and lectures, it appears that there are not many of such necessary information that is summarized for easy understanding by dental technicians. Accordingly, as a result of pondering over this matter for years, the presenter has been devising the 'Esthetic Check List' that is a guideline for the aesthetic restoration practice, and the balanced and harmonious prostheses have been produced based on this guideline. In this presentation, this protocol is introduced.

  • In fixed prosthodontic treatment of the maxillary anterior region, where high esthetics are required, it is crucial not only to achieve a successful prosthesis but also to form symmetrical and natural gingiva. To achieve esthetic gingiva, it is necessary to adjust the subgingival contour of the temporary prosthesis or to modify the base of pontic. However, during the transition from temporary to definitive prosthesis, the conventional method of removing the temporary prosthesis and taking an impression after gingival retraction often fails to accurately reproduce the gingival profile. In such cases, the gingiva is arbitrarily trimmed on the model to fabricate the definitive prosthesis, which reduces the predictability of esthetic treatment outcomes. This case report introduces methods to transfer the subgingival contour and the pontic base formed with the temporary prosthesis to the final model and definitive prosthesis through two cases.

  • In order to solve the problem of repositioning the old hader bar attachment and to restore the non-esthetic facial profile and improper occlusion of the edentulous patient due to inadequate jaw relation, determination of vertical dimension of occlusion and tooth arrangement were performed similar to the natural teeth before the teeth loss. In addition to improving the esthetics by restoring the inadequate facial appearance, the composite resin was used to maximize the patient's secondary esthetic satisfaction with the denture.
