Use of Digital Smile Design in esthetic restoration in anterior teeth: A case report (전치부의 심미 수복을 위하여 Digital Smile Design을 적용한 증례)

  • When restoring their anterior dentition, patients become more demanding on esthetics compared to posterior region during treatment planning phase. Digital Smile Design (DSD) procedure is performed in presentation software and digital photographs. This can widen diagnostic visualization and aid in transferring information between clinician, patient, and technician. This case presented is that of patient with dissatisfaction of his anterior old restoration. Retreatment procedures were carried out in two different manners: (1) using DSD protocol for diagnosis, smile simulation, communication and fabricating interim and definitive prosthesis by totally digitized workflow. (2) Using diagnostic wax-up for smile design and fabricating restorations by conventional workflow. Comparing two methods, DSD was easier to communicate between the dental team than the diagnostic wax-up method. But the final result obtained failed to meet total esthetic factors. Therefore, to obtain predictable esthetic results, more advanced design tool would be needed, including consideration of various esthetic factors besides successful communications.

  • In a single implant restoration of maxillary anterior teeth, it is difficult to accomplish an aesthetic restoration of the implant prosthesis in the case of gingival recession and bone defect problems. To maintain aesthetic stability in the long term, it is important to place the implant in the ideal position as well as the recovery of the soft tissue and harmony with the prosthesis. Not only ideal implant position but also the harmony with surrounding soft tissues are important to fabricate aesthetic implant prosthesis for these cases. for these cases, a 47- years -old male with lowered level of osseous crest and gingival recession on maxillary anterior tooth was treated with guided bone regeneration and gingival recontouring.

  • In the treatment of esthetically important areas such as maxillary anterior teeth, they should be corresponded with surrounding tissues, and shape of the smile line, soft tissue, and hard tissue, also the anatomical shape and proportion of the teeth should be considered as well. Esthetic analysis includes facial analysis which evaluates the proper parallelism between the occlusal plane and the horizontal reference line, dentolabial analysis which assesses the position of the incisal edge and the coherence between the occlusal plane and the commissural line, tooth analysis which evaluates not only esthetics but also morphology and appearance for proper function, and gingival analysis which forms ideal outline of gingival margins. A maxillary anterior diastema can be esthetically restored through the systematic diagnostic approach and treatment planning, and orthodontic, prosthetic, and conservative treatment can be applied for the treatment.

  • Aesthetics can be affected by the interest and the trend of the time. There are the limits of the equipment and materials required for the times, aesthetic case is also considered necessary to overcome them and will introduce the following case in accordance with the patients demands and circumstances. I hope the further development of CAD/CAM by taking over the things have done in the analogue by utilizing the latest technology that is thought to challenge the current limit itself is important. Digital has the advantages in various perspectives. Especially, I am very interested in as related to "digital communication." In particular, when utilizing the data using intra oral scanner can easily direct converse about prosthesis between the dentist and the dental technician through a software program Now, as the digital communication affects the integrity of the prosthesis directly, I had a expectation of a new era for the prosthetic and aesthetics. In this case report, I would like to introduce direct and real digital communication through the following 2 parts.

  • The aim of this study was to investigate the level of radiopacity of esthetic dental restorative materials and determine the optimum level of radiopacity in pediatric dentistry. Disks of 8 dental restorative material groups as the experimental group, 7mm in diameter and 2mm thick, were radiographed with intact human deciduous teeth and aluminum stepwedge standard. Radiopacity was evaluated with an image analysis program following the digitization of the radiographs using a flatbed scanner with transparency unit. All materials and tooth structure also the significant difference except FP, VB, VM. For the radiopacity of esthetic restorative dental materials to exceed that of enamel, it should be greater than 1.7mm of equivalent thickness of aluminum.

  • With the development of treatment of hard and soft tissue around the implants, the implant restoration is increasingly used in the maxillary anterior region which is very important aesthetically. However, the aesthetic reconstruction of the maxillary anterior region is still challenged. Three following conditions should be fully satisfied for aesthetic prosthesis restoration; reconstruction of hard tissue, soft tissue and harmonic prosthesis. In this case report, hard and soft tissue augmentations were performed at atrophied maxillary incisor. Additionally, customized impression coping and provisional restoration were used to make the final restoration.
