Clinical and Cytogenetic Analysis of Children with Maternal Chromosomal Balanced Translocation (모체의 염색체 균형전좌를 가진 환아들의 임상적 세포 유전학적 관찰)

  • Purpose : Parents' genetic information plays an important role in their children's genetic expression. Human chromosome has 23-paternal chromosomes and 23-maternal chromosomes. Parental chromosomal translocation can induce clinical problems in their children because of imbalance in genetic information. We intent to analyze the cytogenentic and clinical features about children with maternal balanced translocation between chromosome 15 and 18. Methods : We detected by one family's FISH study of chromosome 15. We have evaluated children born to clinically normal parents about peripheral bood analysis, endocrine, metabolic, radiologic study, electroencephalogram and social & intelligence scale. and We analysis their clinical manifestation by hospital records. Results : Patient's father and elder sister are normal clinically and genetically. Her mother's chromosome show balanced translocation, 46, XX, t(15;18)(p11.2;p11.3). One child has 46, XX, der(18) t(15;18)(p11.2;p11.3), mental retardation, growth retardation, speech & social developmental delay, recurrent infection and mild mitochondria dysfunction. Her young brother has 46, XY, der(15) t(15;18) (p11.2;p11.3), mental retardation, aggressive behavior, obesity and speech developmental delay. Conclusion : In this study we observed the children with developmental delay, dysmorphic facial features, mental retardation, growth retardation associated with growth hormone deficiency and aggressive behavior due to unbalanced translocation between chromosome 15 and 18.

Return to Sports Activity After Anterior Cruciate Ligament Reconstruction (전방십자인대 재건술 후 스포츠 활동 복귀)

  • Purpose: The purpose of this study was to determine how many of our patients who had had anterior cruciate ligament (ACL) reconstruction returned to their previous levels of sports and to identify why not doing so. Material and Methods: We evaluated 43 knees with a minimum of two years follow-up among whom undergoing 109 ACL reconstruction from September 2003. The mean age of patients was 26 years (17 to 45 years) Clinical results was evaluated according to the Lysholm Knee scores, Tegner activity score, subjective IKDC questionnaires, objective IKDC evaluation form and KT-1000 arthrometer. Results: The overall clinical results demonstrated significant improvement at the final follow-up comparing to preoperative data (P<0.05). Among 43 patients, 25 patients (58%) returned to their previous level of sports and 18 patients (42%) did not. Of whom, 11 patients (61%) were not able to return to sports due to fear to re-injury and 7 patients (39%) suffered from instability and pain. The differences in the Lysholm Scores, KT-1000 arthrometer and subjective IKDC in the 3 groups of patients by ANOVA test were found to be statistically significant (P<0.001). Although there is no difference in objective IKDC and KT-1000 arthrometer, the difference was observed in Tegner activities, subjective IKDC obtained by those who returned to sport and those who had fear to reinjury (P<0.001). Conclusion: This study showed that a quater of patients could not return to their previous level of sport after ACL reconstruction because of fear to reinjury. We thought that not only improving the technique of ACL reconstruction and systemic rehabilitation but also considering psychological factor of patients could allow more patients to return to the previous level of sports.

  • The number of the disabled person had been increased for the industrial accident and the environmental pollution. Especially, developmental disability has the high prevalence rate between 5% and 10% of the whole children. The children with a developmental disability can be treated by the physical therapy, the occupational therapy, the psychology therapy, speech therapy, and art therapy. Visual preception which is function to recognize the external environment through the optic organ could be related to most behaviors on the everyday life. But because the children with disability could not develop the visual-preception enough, they came to have difficulties in executing daily life project. For this reason, it is most important to understand the estimation and the cure on the visual-preception in the pediatric occupational therapy. To improve the visual-preception power, we have many kind of methods including sensory integration, training program for the visual-perception and art-craft program. Particularly, the art-craft which is the representative activity for making something by hands, can be applied to anyone. As the study on the brain has been activated, it was proved that handicraft actives could have an good effect on the brain function and using brain. When the fine motor exercise and more delicate and accurate motion were carried, these motions need the essential help of the visual-perception. So it could be expected that using the repetitive hand function by art-craft makes the brain function improve, when a activity that needs a fine motor exercise and more delicate, accurate motion was carried, It also indicates that the art-craft program has a clear treatment value. Though the intervention between visual-perception development and visual-perception disability have a majority in the field of occupational therapy, there is a few study yet. Therefore, this study tried to look back on the necessity of applying the art-craft program to the children with disability as the prestudy for preliminary validity of the master's thesis.

Research on the New and Senior Security Guard's State Anxiety before Task Performance (신입경호원과 선임경호원 간의 업무수행 전 상태불안에 관한 연구)

  • This research analyzed how the new and senior security guards employed privately are influenced on their task performance by the factors operated before task performance: cognitive state anxiety, physical state anxiety and state confidence which are the sub-factors of the Competitive State Anxiety(CSAI-II) according to the educational background, career, gender and athletic capacity. First, as for the new and senior security guards' state anxiety before task performance, the senior guards felt the cognitive anxiety more than the new guards, but there was not a distinct difference statistically. Therefore, it is estimated that there is little difference in the level of cognitive anxiety between the new security guards with insufficient career and the seniors. Second, the level of the physical state anxiety was shown higher from the senior guards than from the new ones and had a distinct difference. However, it seems to be derived from the extent of physical development as the seniors are at the time of vigorous physical development. Third, the level of state anxiety had no distinct difference between them statistically though the new security guards felt it higher than the seniors. Fourth, the state confidence was shown higher from the seniors than from the new and had a distinct difference statistically, also. Fifth, as for gender, the male and female guards did not have a difference so much in the cognitive and physical state anxiety, and state confidence. Sixth, as a whole, the state anxiety was highest as for the security guards with athletic capacity level 1 and 2. Those with the level 2, and 3 and the level above 7 felt it less than those with the level 5-6, which had a distinct difference in statistics. The manager of the new and senior security guards should make them dispel anxiety in piece of mind through physical and mental education encouraging them to have self-confidence, practical training, and psychological training based on the analysis of causes of various cases.

  • Objective : The Mini-Mental State Examination(MMSE) and Brief Cognitive Rating Scale(BCRS) are frequently using screening tests fur evaluating the cognitive function in clinical practice and research. The authors tried to evaluate the clinical usefulness of these tests for the patients with non-traumatic subcortical cerebrovascular disease. Method : We administered the MMSE and BCRS to 85 patients and 195 normal control group. In order to compare the test results according to the lesion site, we divided patients into left sided lesion group(21 patients), right sided lesion group(31 patients) and both sided lesion group(13 patients). Their cognitive function was evaluated by the BNA and daily living functional activity was examined by the IADLs(Instrumental Activities of Daily Living Scale)and GERRI(Geriatric Evaluation by Relative's Rating Instrument). Results : The results are as follows : 1) In the BNA, the patients scored significantly lower than control group at all items(except Right-Left Orientation and Motor Impersistence), but there were no difference in the MMSE(total score and all 5 items), and only 2 items(recent memory and self-care) were significantly different between two groups in the BCRS. 2) In the comparison by lateralization, there were significant differences among three groups at 3 items(Left Tactile Form Perception, Left Finger Localization and Right Finger Localization) in the BNA. But, there were no difference in the MMSE and BCRS. 3) In the correlation between daily living functioning and the MMSE/BCRS, control group showed no relation(except item of cognitive functioning), but patient group was significantly correlated with 3 items(social functioning, instrumental activities of daily living and cognitive functioning). Conclusions : These findings suggest that MMSE and BCRS are not useful as the test for cognitive function and discrimination of lateralization in patients with non-traumatic subcortical cerebrovascular disease. However, scores of these tests may be related with the functional level(such as daily living function) of patients.

Antiobesity Effect of Citrus Peel Extract Fermented with Aspergillus oryzae (Aspergillus oryzae로 생물전환한 감귤박의 항비만 효과)

  • There is a rising trend in obesity due to various factors, including changes in eating habits, lack of exercise, and genetic and psychological factors. Citrus peel has been reported to prevent obesity via antioxidative, antihypertensive, and LDL cholesterol-lowering effects. This study investigated the effects of citrus peel extract fermented with or without Aspergillus oryzae in a mouse model of diet-induced obesity. The animals were divided into four groups: a high-fat diet group (HFD), a normal fat diet (NFD) group, a citrus peel extract (CP) group, and a citrus peel extract fermented with A. oryzae (CPA) group. The citrus peel extract improved lipid metabolism and weight loss in the high-fat diet-induced obese mouse model. As expected, the body weight was higher in the HFD group compared with the NFD, CP, and CPA groups. However, the concentrations of total cholesterol (TG) and triglyceride (TC) in the serum and liver of the CP and CPA groups were lower than in the HFD group. There were no significant differences in the HDL cholesterol concentration among the groups. Taken together, our results suggest that extract of citrus peel biotransformed with A. oryzae had more antiobesity activity than citrus peel not transformed by A. oryzae through the fermentation of metabolites.

The Relationship of Anxiety Symptoms and Depressive Symptoms to Glycemic Control in Diabetic Patients (당뇨병 환자에서 우울증상 및 불안증상과 혈당조절과의 연관성)

  • Objectives : Diabetes patients suffer from severe stress in maintaining the diet therapy and exercise therapy as well as the disease itself, and this stress has bad effects on controlling the glucose level and causes high prevalence rate of depressive and anxiety disorders. These symptoms again have deleterious effects on blood glucose control. A lot of researches about the relationship between glycemic control and symptoms of depression and anxiety and about the positive effects of the treatments of depression and anxiety disorder on glycemic control in diabetic patients are being performed. In Korea, the research regarding the relationship between glycemic control and depression and anxiety symptoms are seldom performed. In this study, we tried to find out the correlation between the glycemic control and depressive symptom and anxiety symptom. Methods : The study included 65 patients(male 34, female 31) with Diabetes in outpatient clinic of the Department of Endocrinology in Dankook University Hospital. We used the HbA1c levels to check glycemic control through blood sample analysis and used Beck Depression Inventory(BDI) and Beck Anxiety Inventory(BAI). Results : Among the 65 Diabetes patients, 21(32.30%) had mild depressive symptoms, and 6(9.23%) had moderate or severe depressive symptoms. The relation of HbA1c and BDI was not statistically significant, but was significant between HBA1c and BAI, (R=0.567, P<0.001). In the linear regression analysis, BAI had an effect on HbA1c($\beta=0.533$, T=5.012, P=0.00), but BDI, diabetes complications, diabetic morbid period and BMI had no effect on HbA1C. The relationship between HbA1c and BDI was not statistically significant, but the relationship between HbA1c and BAI was statistically significant(R=0.254, P<0.001). Conclusions : In this study, the rates of diabetic patients with depressive symptoms were higher, but those with anxiety symptoms were not higher than the general population. We could not find out significant relationship between depressive symptom and glycemic control, but found the significant relationship between the anxiety symptom and glycemic control in diabetic patients.

Association of diet-related quality of life with dietary regimen practice, health-related quality of life, and gastrointestinal symptoms in end-stage renal disease patients with hemodialysis (혈액투석을 실행중인 말기 신부전 환자의 식사관련 삶의 질(Diet-Related Quality of Life)과 식사요법 실천도, 건강관련 삶의 질 및 위장관 증상과의 관련성 연구)

  • The purpose of this study was to evaluate diet-related Quality of Life (QOL) and to analyze the relationship among diet-related QOL, dietary regimen practice, health-related QOL, and gastrointestinal symptoms in hemodialysis patients. Subjects were recruited from an artificial kidney center in Seoul. The self-report questionnaire consisted of socio-demographic characteristics, diet-related QOL, compliance with dietary regimen practice, health-related QOL, and gastrointestinal symptoms was distributed. Diet-related QOL includes a 'Quality of Life and Related to Dietary Change Questionnaire', 'Satisfaction Survey', and 'Dietary Impact Survey'. According to their responses, participants had experienced the greatest difficulty with 'Dietary Impact'. In particular, the mean score for general health was low. Association of dietary regimen practice showed a positive association with 'Cost' and 'Self-care' score. In addition, diet-related QOL showed positive correlation with health-related QOL, particularly in 'taste'. Scores for 'Taste', 'Convenience', and 'Dietary Impact' were lower for subjects with constipation compared to those of subjects without constipation. Scores for 'Taste' and 'Dietary Impact' were lower in subjects with irritable bowel syndrome compared to those of subjects without the syndrome (p < 0.05). Diet-related QOL showed a positive association with health-related QOL (p < 0.05), whereas it showed a negative association with constipation (p < 0.01). In conclusion, hemodialysis patients in Korea suffer from burden of dietary regimen practice and most scores for diet-related QOL and health-related QOL from patients with gastrointestinal symptoms were low. Therefore, appropriate nutrition education considering gastrointestinal symptoms is necessary for improvement of patients' QOL during dietary regimen practice.

The Clinical Utility of Korean Bayley Scales of Infant and Toddler Development-III - Focusing on using of the US norm - (베일리영유아발달검사 제3판(Bayley-III)의 미국 규준 적용의 문제: 미숙아 집단을 대상으로)

  • The study aims to investigate the clinical utility of Bayley-III using US norm in Korea. A total of 98 preterm infants and 93 term infants were assessed with the K-Bayley-III. The performance pattern of preterm infants was analyzed with mixed design ANOVA which examined the differences of scaled scores and composite scores of Bayley-III between full term- and preterm- infant group and within preterm infants group. Then, We have investigated agreement between classifications of delay made using the BSID-II and Bayley-III. In addition, ROC plots were constructed to identify a Bayley-III cut-off score with optimum diagnostic utility in this sample. The results were as follows. (1) Preterm infants have significantly lower function levels in areas of 5 scaled scores and 3 developmental indexes compared with infants born at term. Significant differences among scores within preterm infant group were also found. (2) Bayley-III had the higher scores of the Mental Development Index and Psychomotor Developmental Index comparing to the scores of K-BSID-II, and had the lower rates of developmental delay. (3) All scales of Bayley-III, Cognitive, Language and Motor scale had the appropriate level of discrimination, but the cut-off composite scores of Bayley-III were adjusted 13~28 points higher than 69 for prediction of delay, as defined by the K-BSID-II. It explains the lower rates of developmental delay using the standard of two standard deviation. This study has provided empirical data to inform that we must careful when interpreting the score for clinical applications, identified the discriminating power, and proposed more appropriate cut-off scores. In addition, discussion about the sampling for making the Korean norm of Bayley-III was provided. It is preferable that infants in Korea should use our own validated norms. The standardization process to get Korean normative data must be performed carefully.

The Effect of the Self-Efficiency of Older People on the Quality of Life - Based on the mediated effect of self-respect - (노인의 자기효능감이 삶의 질에 미치는 영향 - 자아존중감의 매개효과를 중심으로 -)

  • The purpose of this study is to analyze the effects of the old people's self-efficacy on the quality of life. In other words, the impact of old people's self-efficacy on self-respect, old people's self-efficacy on quality of life, and old people's self-respect on quality of life were analyzed in depth. In particular, it was intended to use the recently emphasized variables of self-respect to verify the mediated effects of self-respect in the relationship between self-efficacy and flesh quality, suggesting ways to lead a happy life by improving the psychological satisfaction and quality of life through the participation of the elderly in sports-for-all. To achieve this purpose, 297 people aged 60 or older who participated in the sports-for-all program conducted by the city and district offices in Seoul for five months from May 1 to September 30, 2018 or who used public sports facilities such as parks and playgrounds were analyzed as subjects for research. The findings are as follows. First, the "self-efficacy" of the elderly participating in the sports-for-all has been shown to affect "self-respect." Second, the "self-efficacy" of the elderly participating in the sports-for-all has been shown to affect the "quality of life." Third, it has been shown that the "self-respect" of the elderly participating in sports for all affects the quality of life. Fourth, the "self-esteem" of the elderly participating in sports for all occurred in the relationship between self-efficacy and quality of life. The implications of this study are that the self-efficacy of the elderly participating in sports for daily life has been identified by using the parameters of self-respect. Through this, it was provided as empirical basic data to establish welfare policies for senior citizens to improve the quality of life of senior citizens by identifying their self-efficacy, self-respect, and quality of life.
