Impact of COVID-19-Related Stress and Depression in Public Sector Workers (코로나 19 관련 공공근로자에서의 우울증상 및 스트레스 반응)

  • Objectives : The coronavirus disease 2019 (COVID-19) outbreak is a global medical crisis imposing particular burden on public sector employees. The aim of this study was to investigate the psychiatric distress among public sector workers amid the COVID-19 pandemic. Methods : We conducted a cross-sectional study with 531 public sector workers in Gwangmyung city who completed Korean versions of the Perceived Stress Scale (PSS), Patient Health Questionnaire-9 (PHQ-9), and Impact of Event Scale-Revised-Korean (IES-R-K). Results : The results revealed more than moderate levels of stress (85.2%), depressive symptoms (22.2%), and posttraumatic stress symptoms (38.8%). PSS total score was significantly correlated with PHQ-9, IES-R-K total scores as well as IES-R-K subscale scores. Total scores on the PSS, PHQ-9, and IES-R-K were all inversely correlated with age. Conclusions : COVID-19-related workers experience considerable stress and depressive symptoms, with self-rated stress correlating significantly with depression scores. Age may serve as a protective factor against occupational stress and burnout. These findings highlight the need for adequate psychiatric screening and intervention for public sector workers.

  • 현대인은 대부분 시간을 보내는 노동 환경에 의해 영향을 받는다. 이에 따른 정신적, 신체적 질환은 개인의 건강 문제를 넘어 삶의 질과 연결되며, 결국 사회문제로 귀결된다. 대한민국은 여전히 '산재 공화국'이라는 오명에서 벗어나지 못하고 있는 듯 보이지만, 분명한 것은 끊임없는 성토와 연구를 통해 꾸준히 개선되고 있다는 점이다. 그 중심에서 근로자들과 함께 더 나은 사회를 만들기 위해 노력하는 곳, 한양대학교 의과대학 직업환경의학교실이다.

Pharmacotherapy for Patients Complaining With Somatic Symptoms (신체증상을 호소하는 환자의 약물치료)

  • Patients complaining with somatic symptoms are very common in clinical practice, and are often consulted to medical department. But it is difficult to treat well. The treatment of somatic symptom disorder is multi-modal as none of the methods on their own provide a satisfactory outcome. The treatment of somatic symptoms disorders is complicated by lack of boundary, conceptual clarity, and overemphasis on psychosocial causation and effectiveness of psychological treatments. In clinical practice all classes of psychotropics are used to treat somatic symptoms disorder. Drugs such as tricyclic antidepressants, serotonin reuptake inibitors(SSRI), serotonin and noradrenalin reuptake inhibitors (SNRI), atypical antipsychotics are studied. The evidence indicates that these drugs are effective in somatic symptom disorders. All classes of antidepressants seem to be effective against somatic symptom disorders. SSRIs are more effective against hypochondriasis and body dysmorphic disorder, and SNRIs appear to be more effective than other antidepressants when pain is predominant. The author suggest that psychiatrists should know how to treat patients complaining with somatic symptoms by using not only psychotherapeutic approach but also pharmacological treatment. It will be helpful to reduce suffering and increase quality of life of these patients.

  • 노년기에 나타나는 신체적인 특징은 일반적인'노화 현상'이라고 볼 수 있다. 체력의 저하로 운동 기능이 둔화되고, 심폐 기능과 면역 능력이 저하되어 쉽게 병에 걸리고 주위 환경에 대한 적응력이 저하되는 현상이나타난다. 정신적인 노화도 나타나는데 일반적으로 감각, 지각, 지능 등과 같은 정신 및 신경기능의 저하와불안 또는 우울 등의 정서 및 성격의 변화를 나타내기도 한다.그러나 이러한 변화는 노화 과정에서 초래되는 것이기도 하지만 젊더라도 운동을 거의 하지 않는 사람들에게도 똑같이 일어나고 있다. 따라서 노화 현상이라기 보다는 근육의 불용성 위축으로 초래되는 것이기에 적절한 운동으로 예방 또한 가능하다.

  • "건강이란 질병이나 허약함이 없는 상태를 말함이 아니라, 신체적 정신적 사회적인 복지(Wellbeing)를 뜻하는 것"이라고 WHO(1948)는 정의하였다. 건강은 환경과 생활풍습(Life Style), 유전 그리고 의학적 치료 등에 의하여 결정되며 최근 WHO는 건강제도가 국민건강에 지배적인 역할을 한다고 주장한다. 이런 관점에서 건강제도가 국민 건강에 미친 총괄적인 성취도를 비교해 보면 한국이 전 세계에서 35위, 일본이 1위, 미국은 15위를 차지하고 있다는 통계가 나왔다. 개인의 건강은 지역사회의 건강과 밀접한 관계를 가지고 있으며, 각 지역사회의 건강상태는 국가의 건강상태를 결정짓는다. 그러므로 건강증진 교육과 사업, 연구는 건강상태에 영향력을 미치는 생활풍습과 행동의 변화와 환경조성에 초점을 두고 활성화되어야 한다. 아울러 국민 건강증진 사업을 성공시키기 위해서는 건강정책을 수립하는데 모든 국민의 적극적인 참여가 절대적으로 필요하다.대적으로 필요하다.

  • Korean government has taken various population policy so far According to demographic results, The aging population is rapidly aging and it is expected to deepen in the future. However, many senior citizens are also exposed to poor economic conditions and inconsistent health care risks Most seniors suffer from mental illness and chronic diseases. seniors in traditional cities feel very alienated as they change from large family to small family. As the number of elderly people living alone grows, social problems increase. At present, the health care for the elderly who live alone is not being taken properly, and they are under a very poor management system, both physically and mentally. Every human being has to be old, and we aim to spend our old age processes physically and mentally healthy. As the basic age of the elderly increases, it is necessary to consider various aspects of the welfare policies and medical policies that are followed. In this study, 100 elderly people over the age of 65 were surveyed for economic situations and physical and mental health conditions, and specially performed study on utility of 'urban senior multi care zone' to prove the necessity of introducing the latest digital devices to resolve these problems more effectively by analyzing serious feelings of alienation, loneliness, and emotional situations. Lastly, We conducted this research to find ways to help the elderly through customized health care.

A Study on the Determinants among Types of the Korean Seniors' Physical Activity and Related Health (국내 노인들의 신체적 활동 유형별 참여와 건강 관련 요인 연구)

  • Objective : This study aim to identify the relationships among the types of physical activities included related included related work, sports, leisure, transport and sedentary behaviors and mental health in elderly person. Methods : We analyzed using the data of the 7th Korea National Health and Nutrition Examination Survey(KNHANES VII-1) of 978 subjects who underwent the types of physical activities, quality of life, depression, level of stress, and sleeping hours survey. For statistical analysis, the Complex samples linear regression was used for relationships among the variables. Results : High level of physical activity and house-keeping activity associated negatively on health related quality of life. Mid-High level of leisure activities associated positively on health related quality of life. Contrasting from other studies, which show sedentary activities associated negatively on the health related quality of life, and sedentary activities associated positively on the health related quality of life and mental health. Conclusion : This study shows that physical and leisure activity of an elderly person whilst sedentary activities related positively on health related quality of life, and physical activities related to labor associated negatively on health related quality of life. Future studies should be referred when planning an occupation-focused and meaningful activity program for the elderly people.

Factors Affecting the Mental Health of Nurse's Aides (간호조무사의 정신건강에 영향을 미치는 요인)

  • This study is a descriptive research to identify the factors affecting the mental health of nursing aides. The subjects were 227 nursing aides working in the metropolitan area of South Korea, and the survey was conducted in October 2019 using the structured questionnaires. The results showed that the factors affecting mental health of nursing aides were emotion stability, emotional intelligence, occupational stress, and subjective perception of physical health, and their explanatory power for mental health was 64.6%. As a result, it is necessary to develop an intervention program to improve emotion stability, emotional intelligence, occupational stress, and subjective perception of physical health in order to improve mental health of nursing aides.

Health-Related Quality of Life by Socioeconomic Factors and Health-related Behaviors of the Elderly in Rural Area (농촌지역 노인들의 사회경제적인 요인 및 건강습관에 따른 건강관련 삶의 질)

  • Objectives: This study was to identify socioeconomic factors and health-related behaviors influencing on HRQOL(health-related quality of life) for the older adults in rural area. Methods: 483 subjects aged over 65 years responded a direct interview, which covered HRQOL, BMI, socioeconomic characteristics, and health-related behaviors including smoking, drinking, and exercise. Results: Overall, the mean number of healthy days were 15.1 days and not significantly different by sex. Men didn't show a significant difference in HRQOL by age group. But women reported lower levels of healthy days and higher levels of activity limitation and physical unhealthy days with increasing age. Results from ANCOVA showed HRQOL to be significantly associated with education, job, and family type. Men presented no significant difference in HRQOL by health-related behaviors, but women who have been drinking, or have less number of chronic diseases reported higher mean healthy days and lower activity limitation days, physical unhealthy days, and mental unhealthy days. Older adults who reported good to excellent self-rated health were higher healthy days and lower activity limitation, physical unhealthy days, and mental unhealthy days than those who reported fair to poor health status. Conclusions: The HRQOL for the older adults in rural area was related to socioeconomic characteristics, health-related behaviors and self-rated health status. A better understanding of factors related to HRQOL would help to improve the older adults' quality of life.

Correlation between oral frailty and health-related quality of life (HINT-8) among older adults in Korea (한국 노인의 구강노쇠와 건강관련 삶의 질(HINT-8)의 관련성)

  • Objectives: This study aimed to confirm the correlation between oral frailty and health-related quality of life (HINT-8) among older adults in Korea. Methods: The data of 1,318 individuals aged ≥65 years who participated in the eighth Korean National Health and Nutrition Examination Survey (2019) were analyzed using complex sample statistical analysis. Results: Chewing discomfort was found to decrease the HINT-8 scores by 1.246, 1.324, and 1.089 times in the physical, social, and mental domains, respectively. Speech discomfort was found to decrease the HINT-8 scores by 1.275, 1.449, and 1.175 times in the physical, social, and mental domains, respectively. The HINT-8 scores of participants with ≤19 natural teeth were lower in the physical and social domains. Similarly, the HINT-8 scores of participants with brushing frequency of ≤2 were lower in the positive health domain. Non-use of oral hygiene products led to a reduction in the HINT-8 score in the social health domain. Conclusions: Oral frailty in older adults reduces the health-related quality of life. Thus, it is necessary to formulate policies to manage oral frailty in this population and develop specialized programs for the management of oral frailty.
