The effect of residential environment satisfaction on elderly depression: The mediating role of physical activity (주거환경만족도가 노년기 우울에 미치는 영향: 신체활동의 매개효과를 중심으로)

  • The purpose of this paper is to investigate the association between residential environment satisfaction and elderly depression with a focus on the mediating effect of physical activity. The survey of Living Conditions and Welfare Needs of Korean Older Persons performed in 2017 was used for analysis. Among total 10,299 who aged 65 years or above and participated in survey, only 10,059 people who answered the survey questions by themselves without missing main questions were included in this analysis. Mediation effects were tested with Baron & Kenny(1986) method and Sobel test(Sobel, 1982). As results, first, residential environment satisfaction was negatively associated with elderly depression. Second, residential environment satisfaction was positively related to physical activity. Lastly, it was found that physical activity mediated the relationship between residential environment satisfaction and elderly depression. The results showed that older adults who satisfied in residential environment were more likely to participate in physical activity, and older people who participated in physical activity were less likely to experience depressive symptoms. Based on these results, the paper suggests the importance of subjective satisfaction of residential environment and participation of physical activity for mental health in later life, and some intervention strategies for Aging In Place.

  • Single-person households are steadily increasing every year, and unlike multi-person households, the incidence of depression is higher because they are mentally isolated. There are many causes of depression, including physical activity, family size, and stress. Less physical activity and more stress are associated with higher rates of depression. As the number of single-person households increased, and the COVID-19 outbreak in 2019 caused restrictions on physical activity due to the overlapping pandemic, the stress index increased, resulting in many depressed patients. Since many patients do not receive treatment even if they develop depression, we propose a depression management system to increase access to treatment for depression.

  • 현대인에게 있어서 피할 수 없는 스트레스. 이 스트레스로 인해 현대인들은 신체적인 건강뿐만 아니라 우울증 등의 정신건강까지 위협을 받고 있으나 정신과라는 좋지 않은 인식으로 인해 정신건강은 제대로 치료되지 못하고 있는 실정이다. 하지만 양극성 장애(조울증)를 앓고 있다면 이야기가 좀 달라져야 한다.

Comparison of Stress Perception and Depression between Gastric Cancer and Gastritis Patients (위암 환자들과 위염 환자들 간의 스트레스지각 및 우울의 비교)

  • A comparison was made between gastric cancer and gastritis patients regarding stress Perception and depression, using Global Assessment Recent Stress(GARS) scale and Beck Depression Inventory(BDI). 50% of gastric cancer patients and 38% of gastritis patients were found to be depressed on scores of BDI scale, respectively. There was no significant difference in scores of stress perception between both the groups. However, gastric cancer patients tended to be more depressed than gastritis patients, although the difference is statistically not significant. In the gastric cancer patients, severity of psychic distress showed significantly positive correlation with depression, whereas in the gastritis patients, severity of physical symptoms showed significantly positive correlation with depression. It suggested that depression of gastric cancer patients was more likely to be related to the extent of psychic distress than that of physical symptoms. In each of both the groups, female patients showed significantly higher stress perception than male patients, and age was found to have significantly negative correlation with stress perception. In conclusion, severity of pathology of the same organ was not related to extent of stress perception and of depression in which denial of gastric cancer patients might play a role. Thus, it is emphasized that psychosocial approach is more needed for gastric cancer patients than for gastritis patients.

The Influence of Family Support Degree on the Depression of the Elderly in a Rural Area (일부(一部) 농촌노인(農村老人)이 지각한 가족지지(家族支持)가 우울에 미치는 영향(影響))

  • To find the influence of family support degree on the depression of the elderly in a rural area, a questionnaire survey was made toward 163 persons who were more than 65 years old and living in a township at Chollanamdo province from January 1st to February 20th, 1998. The major findings are as follows : 1. By simple analysis significantly associated variables with depression score were residence, adequacy of pocket money, health consciousness, drinking alcohol, smoking, score of activity of daily living(ADL), score of instrumental activities of daily living(IADL), and degree of family support among female respondents(p<0.05). 2. To find the influence of family support on the depression score excluding other associated variables, multiple regression analysis was made and it was found that family support was associated significantly with affective, somatic, psychomotor and psychological domain of depression score among male, and somatic, psychomotor and psychological domain among female(p<0.05). The above finding suggest that family support degree is an important factor to be associated with the depression of the elderly in the rural area. So to deal with the depression of rural elderly, how to make close relation among family members must be considered.

Effects of the Upright Body Type Excercise on Lung Capacity and Depression of People with Mental Illness (바른체형운동 지도가 정신장애인의 우울감 및 폐활량에 미치는 효과)

  • The purpose of this study was to investigate the effects of the regular Upright Body Type Excercise on posture, muscle strength of leg, lung capacity and depression of people with mental illness. The subjects of this study were recruited from B mental health center (5 years and more mental illness patients, n=19) located in P city. For the exercise group, They were carried out the Upright Body Type Excercise Program during 8 weeks (60 min/time, 3 times/week). They were randomly divided into two groups. Exercise group (n=10) and Control group (n=9). And also, they were agreed with consent forms before the experiments. Research results were as follows. Through the upper body type exercise, there was significantly difference in PEF and FEV1/FVC in the trained group. And also, there was much improved in depression level in the trained group. The change of melancholy feeling before and after the program was not statistically significant. However, because of limitations of sampling size due to the peculiarities of the program participants, in consideration of the limit of statistical validation exists clearly, intended to better understand the subjective experience of attendees, qualitative analyzing(qualitative research) was carried out in parallel. It was conducted a deep interview only person accepted among program participants, thematic analysis, subject analysis tasks to be subdivided by classifying by considering the semantic units of what participants expressed, was thus carried out. It found that the degree of melancholy feeling of mental disorders who participated in the Upright Body Typed Exercise Program was reduced. The course of the experience of change in depression appered the three subjects and six sub-themes such as "the start of the change", "interest of the program", "recognition of the need of the body’s health", "physical health promotion", "recovery of physical function", "negative change of habits (attitude)", "reduction of sense of depression", "confidence that it is possible to", "hope for the future". Therefore, upper body type exercise is much helpful in lung capacity and mental health of people with mental illness. So, this type of exercise mostly needed in the people with mental illness group than the other group for the quality of life.

Factors Influencing Health-related Quality of Life in Patients with Coronary Artery Bypass Graft (관상동맥우회술 대상자의 건강관련 삶의 질 영향요인)

  • The purpose of this study was to determine the relationships among the social support, depression, and health-related quality of life and the factors affecting the health-related quality of life in patients with coronary artery bypass graft (CABG). The subjects consisted of 134 patients who had received CABG from S general hospitals in Seoul, Korea, and the results were analyzed using the SPSS Win 21.0 program. The physical component summary (PCS) was significantly different depending on the age (F=4.616, p=.004), gender (t=3.657, p<.001), education (F=7.688, p=.001), current job (t=-4.363, p<.001), spouse (t=-3.065, p=.003), and underlying disease (F=3.078, p=.030) of the subjects. There were differences in the scores for the mental component summary (MCS) according to their gender (t=2.243, p=.027) and monthly income (F=2.821, p=.041). A significant negative correlation was found between social support and depression (r=-.415, p<.001). Social support had a statistically significant positive correlation with the MCS (r=.261, p=.002). Significant negative correlations were found between depression and the PCS (r=-.459, p<.001) and MCS (r=-.554, p<.001). The depression and underlying disease accounted for 36.7% of the variance in the PCS. The depression accounted for 34.1% of the variance in the MCS. Based on these results, nurses should develop nursing intervention programs for patients who have undergone CABG which take into consideration their depression.

The Effect of Physical Health Status and Social Support on Depression and Quality of Life among the Elderly in G City (거제시 노인의 신체적 건강상태와 사회적 지지가 우울과 삶의 질에 미치는 영향)

  • The purpose of this study was to investigate the effects of physical health status and social support on depression and quality of life among the elderly in G City. This is a descriptive research study of 497 elderly residents in 45 senior citizen centers in G city; the data were collected from March 5 to 30, 2018. Data were analyzed using the IBM SPSS/win 24.0 program by t-test, ANOVA and multiple regression analysis. In physical health status, the chronic disease score was $1.35{\pm}0.91$, the functional status score was $1.80{\pm}4.45$, and the subjective health score was $3.14{\pm}1.13$. The average score for social support in the emotional network was $5.71{\pm}1.13$. In the sub-region of the social network, the score for frequency of contact with relatives was $2.92{\pm}1.31$, that for contact with friends was $3.18{\pm}0.98$, and that for social participation was $0.68{\pm}0.82$. In the multiple regression analysis of factors affecting depression and quality of life, the explanatory power of physical health status and quality of life was 45.5% and 21.1%, respectively. The explanatory power of depression based on social support and quality of life was 46.7% and 27.5%, respectively. This study indicates that physical health status and social support affect depression and quality of life. Therefore, programs should be developed to increase the physical health status and social support and thus improve the quality of life of the elderly in the community.

Study of Quality of Life and Related Factors in Veterans with Physical Dysfunction (신체적 기능장애를 가진 국가유공자의 삶의 질 수준 및 관련 요인 연구)

  • The aim of this study was to examine the quality of life (QOL) and its related factors in veterans with physical dysfunction. The survey data was collected from 412 patients with physical dysfunction managed at five veterans' hospitals in Seoul, Pusan, Daegu, Gwangju, and Daejeon city. The baseline for the quality of life was measured using the SF-36 (Short-Form 36), activities of daily living was measured using a Modified Barthel Index (MBI), pain level was determined using Quadruple Visual Analogue Scale (QVAS), and the depression level was assessed using Korean version of the Geriatric Depression Scale: Short-Form (SGDS-K). The SF-36 scores showed a significant correlation with age (r=-0.132), self-rated current health status (r=-0.545), active daily living (r=0.514), pain level (r=-0.243), and depression level (r=-0.565) (p<0.05). The most influential variables on the QOL were the depression level (B=-.969, p<0.001), QVAS (B=-0.163, p<0.001), and MBI (B=0.140, p<0.001). The results of this study showed that that depression, pain and activities of daily living are significant predictors of the QOL in veterans with a physical dysfunction. Therefore, healthcare providers need to consider these parameters for interventions to improve the QOL in veterans with a physical dysfunction.

Comparison of Alexithymia among Patients with Psychosomatic Disorders, Anxiety Disorders and Depressive Disorders (정신신체장애, 불안장애 및 우울장애 환자들 간의 Alexithymia의 비교)

  • A comparison was made regarding the degree of alexithymia among patients with psychosomatic disorders, anxiety disorders and depressive disorders. The author examined the degree of alexithymia in three groups : 100 psychosomatic patients(including 47 patients with tension headache), 52 outpatients with anxiety disorden, and 50 outpatients with depressive disorders. Alexithymia was assessed by Alexithymia provoked Response Questionnaires(APRQ) developed as a semi-structured interview form. No significant difference was found in the degree of alexithymia among Patients with Psychosomatic disorders, anxiety disorders, and depressive disorders. On the other hand, patients with tension headache were significantly more alexithymic than patients with anxiety disorders and depressive disorders, respectively. However, there was no significant difference in degree of alexithymia between patients with anxiety disorders and those with depressive disorders. Multiple regression analysis revealed that demographic variables such as set age, education level, and marital status did not make a significant influence on alexithymia scores. These results suggest a greater degree of alexithymia in patients with a specific group of psychosomiatic disorders such as tension headache than in patients with emotional disorders, unlike the previous report that in general, psychosomatic patients are alexithymic. Thus, it is necessary to develop special forms of interview which can induce and encourage expression of emotion as a therapeutic strategy for patients with tension headache.
