Effects of Euphorbia Pekinensis RUPR. on Rats with Acute Renal Failure induced by Gentamicin Sulfate (대극전탕액(大戟煎湯液)이 Gentamicin sulfate로 유발(誘發)된 백서(白鼠)의 급성(急性) 신부전(腎不全)에 미치는 영향(影響))

  • This article is to investigate the effects of Euphorbia Pekinensis RUPR. on rats with acute renal failure induced by gentamicin sulfate. So this experimental research was focused on measuring (1)the levels of creatinine, blood urea nitrogen, glutamic oxaloacetic transaminase, glutamic pyruvic transaminase, total protein, sodium, potassium, chloride in the serum and (2)the levels of protein, blood, sodium, potassium, chloride, pH, specific gravity and volume in the urine and (3)intake water. The results were summarized as follows. 1. While the levels of creatinine and glutamic oxaloacetic transaminase, glutamic pyruvic transaminase in the serum were significantly increased, the level of BUN in the serum were partially increased and the levels of sodium and potassium in the serum were partially decreased on the Euphorbia Pekinensis $R_{UPR}$. group. 2. While the urine volume was significantly increased, the levels of protein, blood, sodium and chloride in the urine were partially increased and the specific gravity, pH in the urine were partially decreased. According to the above results, it is shown that Euphorbia Pekinensis $R_{UPR}$. without refinning had a diuretic effect on rats with acute renal failure induced by gentamicin sulfate, but aggravated the glomerular filtration rate and tubular reabsorption. So, it is supposed that Euphorbia Pekinensis $R_{UPR}$. without refinning needs to be used carefully on renal disease and futhermore research about the kinds of extracts form is necessary.

Serum Carcinoembryonic Antigen(CEA) in Chronic Renal Failure (만성신부전증(慢性腎不全症)에서와 혈청(血淸) CEA 치(値)에 관(關)한 연구(硏究))

  • The serum CEA levels were measured by radioimmunoassay technique in 15 patients with chronic renal failure, who were not treated with hemodialysis, in 39 patients under hemodialysis and in 23 patients who received renal transplantation. The results were compared with those in 65 normal adults and the following results were obtained. 1) Serum CEA concentrations.in 65 normal adults were in the range of 1.0 to 4.3ng/ml with a mean value of $1.6{\pm}0.66ng/ml$. 2) Serum CEA concentrations in 15 chronic renal failure patients who were not treated with hemodialysis, were in the range of 0.3 to 8.3ng/ml with a mean value of $3.6{\pm}2.10ng/ml$ which was significantly higher than those of normal controls(P<0.001). 3) Serum CEA concentrations in 39 chronic renal failure patients under hemodialysis were also much higher than normal controls(P<0.001), but not significantly different from those of the patients who were not under hemodialysis(P>0.05), 4) In 23 patients who received renal transplantation, serum CEA levels were significantly higher than normal controls(P<0.001), but not significantly different from those of chronic renal failure patients.

  • As the semiconductor processing technology has been developing, multiple cores or NoC(network on chip) can be contained in recent chips. GALS(globally asychronous locally synchronous) clocking scheme that has multi-clock domains with different frequencies or phase differences is widely used to solve power consumption and clock skew in a large chip with a single clock. A synchronizer is needed to avoid a synchronization problem between sender and receiver in GALS. In this paper, the setup and hold time of DFF required to design the synchronizer are measured using 180nm CMOS processing parameters depending on temperature, supply voltage, and the size of inverter in DFF. The simulation results based on the bisection method in HSPICE show that the setup and hold time are proportional to temperature, however they are inversely proportional to supply voltage, and negative values are measured for the hold time.

A Study on the Sodium and Potassium Intakes and Their Metabolism of the Pregnant Women in Korea (한국인임신부(韓國人姙娠婦)의 Sodium과 단백질섭취량(蛋白質攝取量) 및 대사(代謝)에 관(關)한 연구(硏究))

  • The purpose of this study was to investigate sodium intake and protein metabolism for pregnant women in Korea. The relationship between salt consumption and hypertension, electrolyte changes in pregnancy, and the tendency of weight gain of pregnant women were also examined. The dietary survey and twelve or twenty-four hour's urine collection was performed in this study. The subjects were pregnant women who came to gynecology clinic for prenatal care from July to August in 1984. The results were as follows: 1) Intakes of calorie and iron of Korean pregnant women were 2,036 Kcal and 16mg per day per person. The calorie composition consists of 65% carbohydrate, 16% protein, and 19% lipids. The average level of all nutrients was about 87% of Korean Recommended Dietary Allowances. 2) The sodium intakes in pregnant women estimated by dietary survey was 318.6 mEq Na (7,966 mg Na), which is equivalent to 20.2 g NaCl. 3) Daily sodium excretion was 112.5 mEq per liter, that is equal to 147.4 mEq. Potassium excretion was 24.95 mEq per liter and creatinine excretion was 594.0 mg per day. 4) There were statistically significant correlations between sodium intake and the excretions of sodium, potassium and creatinine. The blood pressure was positively correlated with sodium intakes. As sodium and potassium ratio was increased, systolic blood pressure and diastolic blood pressure were also significantly increased 5) Urinary nitrogen and creatinine ratio known to be reliable index for nutritional status of protein was 5.5 : 1. These values were comparable to the values in American pregnant women. 6) The weight gain during the pregnancy was examined. There was little weight gain during the first trimester, because of nausea, vomit and decrease of appetite. The weight gain during the second trimester was 4.9Kg, and 11.1Kg for third trimester.

  • Battler and Bartter-like syndromes, which include classic Bartter syndrome(type III), neonatat Bartter syndrome(type I, II or IV) and Gitelman syndrome, are autosomal - recessively inherited renal tubular disorders characterized b)r hypokalemic metabolic alkalosis, salt wasting and normal to low blood pressure. Neonatal Bartter syndrome is characterized by intrauterine polyhydramnios, premature delivery, life-threatening episodes of fever and dehydration, subsequent failure to thrive, and severe hypercalciuria with nephrocalcinosis and osteopenia. It is caused by mutations in NKCC2(type I), ROMK(type II) or BSND(type IV) genes. If diagnosed and treated early, the progression to renal failure can be prevented and catch-up growth and normal development are achieved. We report here a 6 month-old infant with neonatal Bartter syndrome who presented with hypokalemic metabolic alkalosis, polyhydramnios and premature delivery, persistent high fever and dehydration, failure to thrive, hypercalciuria, and nephrocalcinosis. He received indomethacin and potassium chloride per os and show ed catch-up growth and normal developmental profile at 19 months of age. (J Korean Soc Pediatr Nephrol 2005;9:255-262)

  • A 9-year-old boy of B blood group with end-stage renal disease due to IgA nephropathy received group O kidney transplantation from his father On day 9, he developed intravascular hemolysis, and anti-B autoantibody formation was confirmed. We diagnosed as immune hemolytic anemia due to passenger lymphocyte from donor, and cyclosporine withdrawl was done. Anemia resolved spontaneously, but on day 18, graft dysfunction developed, and graft biopsy revealed acute allograft rejection. Although hemolysis due to autoantibody is very rare and often mild, and the role of hemoglobinuria on acute rejection in this case is not certain, we recommend consideration of aggressive management on severe hemolysis after minor mismatched kidney transplantation.   • Diabetic nephropathy is a major cause of chronic renal failure in developing countries, and the prevalence rate has markedly increased during the past decade. Diabetic nephropathy shows various specific histological changes not only in the glomeruli but also in the tubulointerstitial region. In the early stage, the effacement of podocyte foot processes and thickened glomerular basement membrane (GBM) is noticed even at the stage of microalbuminuria. Nodular, diffuse, and exudative lesions, so-called diabetic glomerulosclerosis, are well known as glomerular lesions. Interstitial lesions also exhibit fibrosis, edema, and thickened tubular basement membrane. Diabetic nephropathy is considered to be multifactorial in origin with increasing evidence that one of the major pathways involved in the development and progression of diabetic nephropathy as a result of hyperglycemia. Hyperglycemia induces renal damage directly or through hemodynamic alterations, such as, glomerular hyperfiltration, shear stress, and microalbuminuria. Chronic hyperglycemia also induces nonhemodynamic dysregulations, such as, increased production of advanced glycosylation endproducts, oxidative stress, activation of signal pathway, and subsequent various cytokines. Those pathogenic mechanisms resulted in extracellular matrix deposition including mesangial expansion and GBM thickening, glomerular hypertrophy, inflammation, and proteinuria. In this review, recent opinions on the histopathologic changes and pathophysiologic mechanisms leading to initiation and progression of diabetic nephropathy will be introduced.

A Case of Diabetic Nephropathy Progressed to End-Stage Renal Disease in an Adolescent with Type 1 Diabetes (사춘기에 말기 신질환으로 조기 진행한 소아 제1형 당뇨병성 신병증 1례)

  • Type 1 diabetes mellitus (T1DM) commonly occurs in childhood and adolescence and diabetic nephropathy is a serious metabolic complication of T1DM that leads to serious morbidity. With poor glycemic control prepubertal diabetes duration contributes to the risk of long-term microvascular complications, however, the younger age at onset or longer prepubertal diabetes duration seems to prolong the time to development of microalbuminuria or later end-stage renal disease (ESRD). Therefore, there have been a few cases of diabetic nephropathy in prepubertal patients and therefore the ESRD cases developed during adolescence in T1DM children were very rare. Here we report an adolescent with T1DM who had poor glycemic control and was diagnosed as diabetic nephropathy in a prepubertal period and leading to end-stage renal disease during adolescence.

Nephroprotective Effects of Taeksa-san Aqueous Extracts on Cisplatin-induced Rat Acute Renal Failure (택사산(澤瀉散) 추출물(抽出物)이 Cisplatin으로 유발(誘發)된 흰쥐의 급성신부전(急性腎不全)에 미치는 영향(影響))

  • Objectives : This study was to observe the nephroprotective effects of the traditional prescription, Taeksa-san(TSS). TSS has generally been used for treating various renal diseases, including renal failure. Methods : Three different dosages of TSS extract were orally administered once a day for 28 days, At the 23rd day after TSS extract treatment, cisplatin was also treated. Then, 5 days after cisplatin treatment, all the rats (6 groups of 8 rats each) were sacrificed. Changes in the body weight, kidney weight, serum BUN and creatinine levels were observed with changes on the kidney MDA and GSH contents. The results were compared with captopril 100mg/kg, of which the effects on cisplatin-induced acute renal failures are already confirmed. Results : Dramatic decrease of body weight, increase of kidney weight, increase of serum BUN and creatinine levels were detected in the cisplatin control as compared with the intact control. In addition, marked increase of kidney MDA contents and decrease of kidney GSH contents were also detected in the cisplatin control as compared with the intact control. This means that cisplatin-induced ARF is induced by oxidative stress and related lipid peroxidation in the present study. However, these ARFs and inhibition of antioxidant effects induced by cisplatin were dose-dependently reduced by treatment with all three different dosages of TSS extracts. Conclusion : This study suggests that TSS extracts show favorable effects on cisplatin-induced rat ARF.
