Background: The most distal sensory fibers of the feet are often affected first in polyneuropathy. However, they are not evaluated in routine nerve conduction studies. Thus we evaluated the dorsal sural sensory nerve in patients with sensorimotor polyneuropathy with normal sural response, in order to assess the usefulness in electrodiagnostic practice. Methods: In this study, 53 healthy subjects and 27 patients with clinical evidence of sensorimotor polyneuropathy were included. In all subjects, peripheral motor and sensory nerve studies were performed on the upper and lower limbs including dorsal sural nerve conduction studies. On electrodiagnostic testing, all patients had normal sural responses. Results: The dorsal sural sensory nerve action potentials (SNAPs) mean amplitude was $13.12{\pm}5.68{\mu}V$, mean latency was $3.12{\pm}0.43msec$, and mean sensory conduction velocity (SCV) was $36.50{\pm}3.40m/s$ in healthy subjects. In 7 of 27 patients, the dorsal sural nerve SNAPs were absent bilaterally, and in 20 patients, the mean dorsal sural nerve distal latency was longer($3.40{\pm}0.48ms$, P=0.006), and mean SCV was slower than in healthy subjects($35.08{\pm}4.59$, P=0.043). However, dorsal sural nerve amplitude was not different between the groups (P=0.072). Conclusions: Our findings suggest that dorsal sural nerve conduction studies should be included in the routine electrodiagnostic evaluation of patients with suspected polyneuropathy and normal sural nerve responses.
The Lateral dorsal cutaneous branch of sural nerve (LDCB) is a terminal sensory branch of lower extremities. It can be injured frequently in peripheral nerves. However, the normal data of each component of nerve conduction study (NCS) of were not studied at this time. The Nerve Conduction Study of LDCB adults were assessed for amplitude, area, duration and nerve conduction velocity (NCV) in normal fifty. We also evaluated how age, sex and dexterity affect the various components of NCS. The Mean amplitude of LDCB was $9.45{\pm}1.93{\mu}V$, area was $4.05{\pm}0.55{\mu}V/s$, duration was $1.50{\pm}0.13s$, and NCV was $37.9{\pm}3.09m/s$, respectively. The amplitude of right was $10.1{\mu}V$ in men, $8.65{\mu}V$ in women. The area of right was $3.83{\mu}V/s$ in less than 40 years and $4.24{\mu}V/s$ in older than 40 years. The areas of left was $3.86{\mu}V/s$ in less than 40 years and $4.30{\mu}V/s$ in older than 40 years. The NCV was 39.0 m/s in less than 40 years and 36.7 m/s in older than 40 years. All of above differences were statistically significant. There were no statistically significant differences between right and left NCS. Normal data of LDCB could be applicable in peripheral neuropathy or nerve injury.
Objectives : We have evaluated the effects of conservative treatment on two patients who were diagnosed to lumbosacral radiculopathy. Methods : Two patients were diagnosed as lumbosacral radiculopathy through needle electromyoram. We used acupuncture therapy, Cox technique to the patients. We measured visual analog scale(VAS) and needle electromyogram before and after treatment. Results : After treating acupuncture therapy, Cox technique in the cases, We find out that the patients were improved, and changed of needle electromyogram result. Conclusions : These results suggest that Cox thechnique and acupuncture therapy were effective to lumbosacral radiculopathy and reduced the low back pain and the leg myasthenic.
Objectives : This study was performed to confirm the usefulness of EMG and early-performed ENoG as predicting factor for facial palsy by clinical outcome. Methods : We gathered patients who visited the Facial Palsy Center in Kyung Hee University Hospital at Gangdong between January 2010 and September 2012. We used inclusion/exclusion criteria and reviewed 231 patients' medical records including gender, age, HB grade and EMG & ENoG axonal loss. We compared the relation between EMG axonal loss and improvement degree at 4 & 6 weeks after treatment, and between ENoG axonal loss at 3-5 & 6-8 days after onset and EMG axnoal loss using regression analysis. Results : Each analysis had statistical significance, but EMG & improvement degree at 6 weeks after treatment showed statistically higher correlation than that at 4 weeks after treatment, and ENoG at 6~8 days after onset & EMG showed statistically higher correlation than that at 3~5 day after onset. Conclusions : We could get the evidence of the validity of EMG and early-performed ENoG as predicting factor for facial palsy based on collaborative treatment between Korean and western medicine.
Background: Carpal tunnel syndrome (CTS) is a common condition characterized by entrapment neuropathy of the median nerves. Clinical manifestations are the most important findings for diagnosis and assessment of therapeutic effects. But, objective indicators, such as electrophysiological findings, are also valuable supplementary tools. This study investigated the relationship between clinical grading and sensory nerve conduction velocity (SNCV) of median proper palmar digital nerve (MPPDN) in CTS patients. Method: This study was done on 90 upper limbs of 53 patients with CTS (men: 6, women: 47, age: 26~69 years, mean age; 52 years). Each SNCV of MPPDN was recorded with bar electrode using antidromic method. Each SNCV was compared with clinical grading of CTS. The clinical grades of CTS were designated as follows; group 1 is mild symptoms, 2 is moderate symptoms, and 3 is severe and longstanding symptoms. Result: In thumb, the SNCV of MPPDN was not different significantly between 3 groups (p=0.817). In the index finger, the SNCV was the fastest in the group 1, but faster in group 3 than in group 2 (p=0.001). In the middle and ring fingers, SNCV was decreased in higher clinical grading groups (middle finger: p=0.015, ring finger: p=0.044). Conclusion: SNCV of MPPDN of middle and ring finger correlated with the clinical grading of CTS. SNCV of index finger was the fastest in group 1. But SNCV of thumb did not correlate with the clinical grading of CTS.
Background: Meralgia paresthesia (MP) is characterized by sensory impairment in the anterolateral aspect of the thigh and usually caused by a lateral femoral cutaneous nerve (LFCN) lesion. It is well known that several physiologic factors including age, obesity, and sex can affect nerve conduction. This study aimed to determine whether body mass index (BMI) and age can influence on the conduction velocity and action potential amplitude of the LFCN. Methods: Fifty six individuals without any previous neuromuscular disease participated in this study. LFCN was studied orthodromically, distally from the anterior superior iliac spine. The values, such as sensory nerve action potential (SNAP) amplitude and sensory nerve conduction velocity (NCV) were obtained. SNAP of the LFCN were formed on both sides in forty three individuals. Results: No difference of demographic factors was observed between two groups divided according to the presence of SNAP formation. BMI had a significant relationship with SNAP amplitude and NCV of the LFCN. Moreover, Multiple regression analyses of nerve conduction values showed the significant correlation of body mass index and age with nerve conduction velocity. Conclusions: We may suggest that nerve conduction of the LFCN can be affected by age and BMI. Further study to obtain normal nerve conduction data and compare these data with those of meralgia paresthetica patients should be continued.

라임병은 진드기를 매개로 하여 보렐리아속균 감염에 의해 발생하는 세균 질환이다. 가장 흔한 증상으로 초기에 유주성 홍반이 대부분의 환자에게서 관찰되며, 신경, 심혈관계 및 관절을 침범하는 증상이 발현할 수 있다. 저자들은 미국 코네티컷주로 여행력이 있는 13세 남자에게서 발현한 라임병을 진단하여 보고하는 바이다. 환자는 2016년 여름에 미국에서 숲 체험에 참여하던 중 발열, 두통, 양쪽 무릎 관절통을 호소하여 귀국하였으며, 내원 시 양 하지에 다수의 유주성 홍반이 관찰되었다. 심전도 검사에서 1도 방실 차단이 확인되었다. 간접면역형광항체법과 웨스턴블럿법 검사에서 라임병 특이 IgM 항체가 양성으로 확인되었다. 경구 doxycycline으로 4주간 치료하였으며, 유주성 홍반이 소실되고 심전도가 정상 소견으로 회복되었다. 라임병의 진단에 있어 호발 지역으로의 여행력을 확인하는 것이 중요하다. 라임병에 의한 심장염은 무증상 방실 전도차단으로 나타날 수 있고 고도 방실 차단으로 진행할 수 있어 주의를 요하며 적절한 항생제 치료를 통해 회복될 수 있다.
건강한 성인 83명을 무작위로 선정하여 실시한 신경전도속도 검사에서 다음과 같은 결과를 얻었다. 1) 상지의 정중신경에서는 운동신경의 TL이 3.0-4.2msec 이고, MNCV는 각각 52.1-70.3m/sec(W-E), 44.6-71.0m/sec(E-Ax), 56.6-70.8m/sec(W-E), 50.5-75.1m/sec(E-Ax)이며, CNAP의 진폭은 $6.5-46.1{\mu}V$였다. 2) 척골신경에서는 운동신경의 TL이 2.4-3.4msec이고, MNCV는 각각 54.6-72.8m/sec(W-E), 41.1-64.9m/sec(E-Ax)1987년 3월부터 동년 12월까지 영남대학교 의과대학 부속병원에서 시행된 개심술마취 40례를 대상으로 저체온 체외순환시 저체온상태의 동맥혈과 정맥혈을 동시에 각각 채혈하여 $37^{\circ}C$ 혈액가스분석기로 측정한 비보정치와 환자의 실제 저체온으로 보정한 보정치를 각각 얻어 이를 비교하여 보았던 바 보정치가 비보정치에 비하여 pH는 높았고 이산화탄소분압과 산소분압은 낮았으며 $HCO_{3^-}$, 염기과잉, 이산화탄소분압 및 산소포화도는 온도보정에 영향을 받지 않았다. 이상의 결과로 미루어 보아 저체온상태의 혈액가스분석시 pH와 이산화탄소분압에 대한 온도보정의 영향은 크며, 저온상태에서의 pH 중심점이동과 산염기가 환자상태에 미치는 영향을 고려한다면 환자실제 저체온으로의 온도보정은 필요치 않을 것 같으며 오히려 $37^{\circ}C$ 비보정치로 판독하여 환자상태를 판정하는 것이 임상적으로는 더 타당한 방법이라 사료된다.
목 적: 바이오피드백은 정신생리연구에서 중요한 위치에 있으며 정신생리장애 치료에도 유용하다. 만성통증은 매우 흔히 관찰되는 대표적 정신생리장애이다. 이 연구에서는 바이오피드백 기기를 이용한 스트레스 반응 검사로 만성통증 환자와 정상인 간에 생리신호의 특성을 비교하였다. 방 법: 만성통증 환자 42명(남자 17명, 여자 25명, 44.67${\pm}11.10$세)에게 치료 첫째 시간에 바이오피드백 기기를 이용하여 스트레스 반응검사를 하였다. 그 결과를 정상인 42명(남자 17명, 여자 25명, 45.17${\pm}10.46$세)과 비교하였다. 스트레스 반응검사에서는 기저, 스트레스, 그리고 회복 상태에서 각각 전두근 근전도(frontalis electromyography), 피부전도(skin conductance), 피부체온(skin temperature)을 각각 2분씩 측정하여 평균값을 산출하였다. 측정값들의 양 군간 비교에는 독립 t-검정(independent t-test), 각 상태에 따른 측정값의 변화는 대응표본 t-검정(paired-test)로 분석하였다(양측검정, p<0.05). 결 과: 만성 통증 환자는 정상인에 비해 기저, 스트레스, 그리고 회복 상태 모두에서 전두근이 더 긴장해 있었다 (기저: $8.10{\pm}10.465.97{\mu}V$ vs $4.72{\pm}1.52{\mu}V$, t=-3.56, p<0.01; 스트레스: $11.25{\pm}6.89{\mu}V$ vs $8.49{\pm}4.78{\mu}V$, t=-2.13, p<0.05; 회복: $7.12{\pm}3.77{\mu}V$ vs $4.78{\pm}1.59{\mu}V$, t= -3.70, p<0.01). 또한 만성 통증 환자는 세 상태 모두에서 피부전도가 더 잘 되었다(기저: $1.06{\pm}1.0{\mu}S$ vs $0.42{\pm}0.29{\mu}S$, t=-4.0. p<0.01; 스트레스: $1.87{\pm}2.05{\mu}S$ vs $1.03{\pm}0.86{\mu}S$, t=-2.47, p<0.05; 회복: $1.74{\pm}1.77{\mu}S$ vs $0.64{\pm}0.59{\mu}S$, t=-3.8, p<0.01). 그러나 피부체온은 기저, 스트레스, 그리고 회복 상태 모두에서 두 군 사이에 유의한 차이가 없었다(p<0.05). 결 론: 만성 통증 환자는 정상인에 비해 전두근 근전도와 피부전도 측정값이 전반적으로 증가된 소견을 보였다. 이는 통증으로 인하여 교감신경계가 정상인에 비해 자극에 더 예민한 상태에 있음을 시사한다.
