Effect of Ephedra Sinica and Evodia Rutaecarpa on Resting Metabolic Rate in Obese Premenopausal Women during Low-calorie Diet: A Randomized Controlled Clinical Trial (저열량 식이요법을 한 폐경 전 비만 여성의 휴식 대사량에 대한 마황과 오수유의 효과)

  • Objective: To evaluate the effect of Ephedra Sinica and Evodia Rutaecarpa on resting metabolic rate(RMR), weight and body composition in obese women during low-calorie diet. Methods: We recruited 90 healthy premenopausal women$(body\;mass\;index{\geq}25kg/m^2)$ and randomly assigned to three groups(ephedra group, evodia group and placebo group). Subjects were administered Ephedra Sinica(6g twice a day) or Evodia Rutaecarpa(6g twice a day) or placebo in a double-blind fashion and participated in low-calorie diet for 8 weeks. Resting metabolic rate and body composition were measured at baseline, 4 and 8 weeks. Results: RMR change for 4 weeks was significantly higher in the ephedra group compared with the placebo group(p<.05). But the change for 8 weeks was not significant and the Evodia group showed no significant RMR change. Weight and percent body fat changes for 4 weeks and 8 weeks were significantly higher than the placebo group in the ephedra group(p<.05), but the Evodia group didn't show significant level. Conclusions: This study supports and emphasizes the benefits of herbal medicine in maintaining or increasing RMR during low-calorie diet. Especially Ephedra treatment was effective on significant maintainment of RMR, loss of weight and percent body fat(P<.05).

Influence of Non-compliance of Treatment and Family Support on Depression in Diabetic Patients with Hypoglycemia (저혈당을 경험한 당뇨병 환자의 치료지시 불이행, 가족지지가 우울에 미치는 영향)

  • This study was to identify the effective factors on the Depression in Diabetic patients with hypoglycemia. This study used 119 questionnaires from Diabetic patients with hypoglycemia in B city to collect data from October 1, 2015 to March 15, 2016. Data were analyzed by SPSS 20.0 program using descriptive statistics, t-test, ANOVA, Scheffe's test. Pearson's correlation coefficient and Stepwise Multiple Regression. The Depression in Diabetic patients with hypoglycemia showed a positive correlation with Non-Compliance of Treatment(r=42, p<.001), a negative correlation with Family support(r=-.38, p<.001). According to the result of multiple regression analysis, the significant factors influencing depression were Family support(${\beta}=-.29$, p<.001), Drug therapy(${\beta}=.21$, p=.020), Dietetic therapy(${\beta}=.23$, p=.014), Monthly average income over 3,000,000won (${\beta}=-.20$, p=.014), Monthly average income 2,000-3,000 thousand(${\beta}=-.17$, p=.033), it was explained by 30.0%. Based on these findings, it is suggested to develop and apply a family support program to reduce the depression of diabetic patients with hypoglycemia.

Treatment of Obese Women with Low Calorie Diet, Aerobic Exercise and Behavior Modificaiton (저열량균형식, 운동 및 행동수정에 의한 비만여성의 치료에 관한 연구)

  • A multidisciplinary weight control program was conducted for obese women. The major components of the program included low calorie diet therapy, exercise, behavior modification and nutritional education and counseling. Sixteen healthy volunteers in excess of body fat, above 30%, were enrolled in the group support program. But 5 person were dropped out in the 2nd week of treatment. During the 1st week of group orientation, individual cause of obesity was assessed through a computer program including survey of dietary intake, activity, eating habits and life styles. During the 5 weeks of treatment, 4.8kg of average weight loss was accomplished using a following program ; low calorie diet(1200kcal/day with all essential nutrients), low impact aerobic exercise(50~60% of $VO_{2max}$, 1 hour/day in a group, 3~5 days/week), behavior modification of individual life styles and eating habits causing obesity and nutritional education concerning nutrition, role of exercise such as brisk walking, importance of slow eating in regular meal pattern and internal motivation for weight reduction, health risk of obesity and rapid weight loss, weight recycling and yo-yo syndrome, etc. Nutritional conseling was conducted 3 times per week with checking self-records of foods, activity, emotional state and tiredness. Before and immediately after 5 weeks of treatment, blood pressure, fasting blood glucose, cholesterol and triglyceride were measured and comparied with paired t-test. After 5 weeks of treatment, body weight, body mass index, body fat and circumferences of waist, upper arm and hip were significantly decreased. Also LDL-cholesterol was significantly decreased after obesity treatment.

고추(번초(番椒), 고초(苦椒))의 어원(語源)연구

  • 1. 연구 목적 한국의학사(韓國醫學史)의 한 분야인 식품사(食品史) 및 약물사(藥物史)는 미개척분야라 해도 과언은 아닐 정도로 연구 업적이 부족하다. 특히 식물명칭에 대한 정리는 기초학문이지만 방치되어진 것도 부인할 수 없다. 따라서 본고는 식품문화사(食品文化史)를 정리하는 과정중의 일환으로 고추의 어원(語源)을 중심으로 살펴보고자 한다. 이는 식품사(食品史) 연구에 도움이 되는 동시에 체질식이요법을 강조하는 사상의학(四象醫學)의 식품 분류에 대한 학술적 근거를 제시하는 계기로 삼고자 한다. 1493년 콜럼버스에 의해 알려진 고추는 우리 나라에 들어와 천초(川椒)대신 사용됨으로서 현재 중요한 양념이 되었다. 이에 고추의 도입과정과 호칭변화를 살펴봄으로서 현재 사전류에서 잘못 서술되고 있는 것을 시정하고, 고추가 도입된 이래 우리가 어떻게 이해하고 응용하였는 지에 대하여 문헌고찰을 통하여 의학사적(醫學史的) 입장으로 정리하는 것이 본 연구의 목적이다. 2. 연구 방법 "산림경제(山林經濟)"(1715), "임원경제지(林園經濟志)"(1827)같은 농서(農書), "물명고(物名考)"(1830), "오주연문장전산고(五洲衍文長箋散稿)"(1850) 같은 유서(類書), "훈몽자회(訓蒙字會)"(1527), "신증유합(新增類合)"(1574)같은 사전류, "지봉유설(芝峰類說)"(1614), "성호사설(星湖僿說)"(1763)같은 문집류, "흠정수시통고(欽定授時通考)"(1737), "본초강목습유(本草綱目拾遺)"(1765)같은 중국본초서(中國本草書) 등과 국내의서 등 을 통하여 고추에 대한 문헌정리를 시도하였다. 3. 연구 결과 고추가 도입된 이래 남만초(南蠻椒), 남초(南椒), 번초(番椒), 왜초(倭椒), 왜고초(倭苦椒), 왜개자(倭芥子), 고초(苦椒), 랄가(辣茄), 고쵸, 등으로 표기되어 왔다. 하지만 고추가 도입되기 전에 출간된 "구급간이방(救急簡易方)"(1489)에 '초(椒) 고쵸, 죠피'라는 기록이 나온다. 이는 천초(川椒)(초피나무)를 의미하는 '고쵸'로 보아야 한다. 후에 고추가 도입됨에 따라 의미가 변하여 고추를 의미하게 되는데 이를 정리하면 다음과 같다. 첫째, 현존하는 문헌 중 고추에 대한 최고기록인 "지봉유설(芝峰類說)"(1614)에 근거하여 고추(남만초(南蠻椒))는 임진왜란때 일본에서 도입되었다는 것이 정설이었다. 하지만 임진왜란 이전에 일본을 통하여 고추가 도입되었고 이의 개량종을 오히려 임진왜란때 일본으로 전해주었을 가능성이 높다. 둘째, 현재 대부분의 사전류에서 "훈몽자회(訓蒙字會)"(1527)의 '고쵸 쵸(초(椒))'를 고추에 대한 최초의 어원으로 보고 있으나 이는 잘못된 것이다. "훈몽자회(訓蒙字會)"의 '고쵸(초(椒))'는 고추가 아닌 매운 열매의 총칭으로 호초(胡椒), 천초(川椒), 진초(秦椒)를 의미한다. 이는 "구급간이방(救急簡易方)"(1489)의 천초(川椒)를 의미하는 '초(椒) 고쵸'에서도 확인할 수 있다. 셋째, 현재 대부분의 사전류에서 고추의 한문표기가 고초(苦草)로 되어 있으나 고초(苦椒)로 정정되어져야 한다. 초(草)(상성(上聲))와 초(椒)(평성(平聲))는 발음은 같지만 성조(聲調)가 다르며, 또한 "본사(本史)"(1787)에 고초(苦椒)와 고초(苦草)는 서로 다른 식물로 기록되어 있다. 넷째, 고초(苦椒)는 '고쵸'라는 우리말에 맞추어 만든 국자(國字)로 고추가 천초(川椒)와 비슷하지만 맵고 쓰다라는 뜻으로 사용된 것이다. 다섯째, 맵고 뜨거운 성질이 있는 고추는 냉증(冷症)을 유발시킬 수 있는 냉성(冷性)야채를 중화(中和)시키는 작용을 한다. 여섯째, 고추는 소음인 식품의 경향성을 띈 훈채류에 속한다.

고혈압 노인과 건강한 노인의 항산화 영양상태 비교

  • 최근 노인인구의 증가와 함께 식생활의 변화에 의해 사망원인의 변화가 일어나고 있다 한국인의 주요 사망원인 중 뇌혈관 및 심장질환 등 순환기계 질환에 의한 사망이 증가하고 있는 추세이며, 심장순환계 질환의 위험요인인 고혈압의 이환률도 65세 이상 노인에게서 증가하고 있다. 고혈압은 만성질환으로 산화적 스트레스를 증가시키는 질환이므로 본 연구에서는 건강한 노인과 고혈압 노인의 체내 항산화 영양상태를 비교하기 위해 울산지역 50세 이상 노인을 대상으로 하였다. 대조군은 질환이 없거나 골다공증, 위장질환과 같은 기타 질환을 가진 사람 96명이었으며, 고혈압환자군은 고혈압을 가진 모든 질환자 118명이었고, 항산화 영양소 섭취나 혈장내 항산화 영양상태에 영향을 미치는 성별, 연령, 흡연 및 음주상태를 보정하여 비교하였다. 고혈압환자의 평균 유병기간은 6.1년이었으며, 항산화 비타민 섭취량은 비타민 A, retinol, $\beta$-carotene과 비타민 C 섭취량 모두 고혈압 유무에 따른 유의적인 차이가 나타나지 않았으나 고혈압환자의 섭취량이 높은 경향을 보였다. 한국인영양권장량과 비교한 결과 대조군의 비타민 A 섭취는 권장량의 86.2%, 고혈압환자군은 권장량의 103.0%를 섭취하여 고혈압군에서 유의적으로 높았고, 권장량의 75% 이하 섭취자의 비율은 대조군에서 46.9%, 고혈압군에서 41.5%로 조사되었다. 비타민 C 섭취는 대조군에서 권장량의 176.6%, 고혈압군에서는 189.0%로 나타나 질환의 유무에 관계없이 두 군 모두 권장량의 170%이상을 섭취하고 있었으나 권장량의 75% 이하를 섭취하는 사람의 비율이 대조군에서 13.5%, 고혈압군에서 17.0%로 고혈압군에서 높게 나타났다. 혈장 비타민 C의 농도는 대조군 15.3 mg/L, 고혈압환자 15.9mg/L, 혈장 $\beta$-carotene 농도는 대조군 0.21mg/L, 고혈압환자 0.22 mg/L로 나타나 고혈압 유무에 따른 차이가 없었고, 혈장 비타민 A,E 농도 및 총 항산화능력은 대조군에 비해 고혈압환자에게서 유의적으로 높게 나타났다. 또한 혈장 지질과산화물의 농도도 유의적이지는 않지만 고혈압환자에게서 높은 경향이 나타났다. 따라서 고혈압에 따른 항산화 영양상태를 비교한 결과 전반적인 항산화 비타민의 섭취량, 혈중 비타민 농도 및 총 항산화능력이 예상과 달리 대조군 보다 고혈압환자에게서 유의적으로 높게 나타났다. 이런 결과는 질환자들이 자신이 가진 질병의 진전을 막거나 치료의 목적으로 치료제를 복용하고, 식이요법을 실시하고 있기 때문으로 사료되며, 고혈압과 같이 산화적 스트레스가 증가하는 질환의 경우 체내 항산화 체계 반응이 이러한 변화에 효율적으로 대처하기 위한 방향으로 증가되었을 가능성이 있을 것으로 사료된다. 특히, 본 연구의 대상자들의 유병기간이 6.1년으로 비교적 길기 때문에 질환의 진행정도에 따른 항산화 상태를 측정하거나 비교적 질환으로 판정된 직후의 사람들의 대상으로 한 연구 등 더 많은 연구가 필요하다.

Effects of Kimchi Supplementation and/or Exercise Training on Body Composition and Plasma Lipids in Obese Middle School Girls (운동 및 김치 보충제 섭취가 비만 여중생의 신체조성 및 혈중지질에 미치는 영향)

  • The effects of aerobic exercises and/or supplementation of kimchi on changes of the body composition and plasma lipids of obese middle school girls were studied. Thirty eight girls, 28 obese girls and 10 normal weighed girls, were paricipated. Among obese girls, 8 were assigned to exercise group (FG) 12 were grouped as kimchi group (KG) , and 8 were asked to practice excercise and to take kimchi simultaneously(excercise kimchi group, EKG). Ten girls whose weight is normal asked to remain o their own diet during 6 weeks of experiment (control group, CG)/ EG practiced jogging and rope-jumping for 60 minutes four times a week and KG took 3 g of freeze-dried kimchi packed in a 500 mg capsule daily which is equivalent to 30 g of fresh kimchi, EKC, EG and KG showed beneficial effects on changes of the body composition and plasma lipids compared to those of CG, EG showed greater effect than KG in reducing body fat resulted decrease in BMI, fat mas,. abdominal fat, and triglyceride concentration and increase in HDL-cholesterol. KG seemed to have greater effect on lowering plasma cholesterol and LDL-cholesterol than EG/ But the greatest effects in terms of reduction in weight, BMI fat mass, abdominal fat, total cholesterol, LDL-cholesterol, triglyceride, and increase in HDL-cholesterol were observed from EKG. These results indicate that kimchi supplemenation while practicing excercise might improve the obese state by reducing body fat content as well as reducing plasma lipids.

  • Coronary artery disease(CAD) is associated with a high risk of recurrence and an increased mortality rate upon recurrence. Consequently, there is a need for continuous health behavior recommendations from medical staff. Therefore, a study was conducted on 219 patients hospitalized for coronary angiography. The study aimed to investigate the influence of medical staff recommendations on exercise adherence among CAD patients. In conclusion, regular exercise(RE) recommendation(OR 3.52, p=.036) and dietary advice from medical staff(OR 6.48, p=.022), having high exercise self-efficacy(OR 1.36, p=.001) positively affected RE adherence. Therefore, medical staff should strengthen personalized recommendations for RE and healthy diets, as well as develop exercise self-efficacy enhancement programs to encourage patient participation. Future research is recommended to explore the impact of the degree and method of medical staff recommendations on self-management practices among CAD patients.

Literary Investigation of Food-Therapy(食治方) Using Foxtail Millet (Setaria italica L. Beaur) - Korean Medicine Literature in 1300's-1600's - (조(속미(粟米)·출미(秫米))를 이용한 식치방(食治方)의 문헌(文獻) 조사 -1300년대에서 1600년대 한국 의서(醫書)를 중심으로-)

  • Foxtail millet (Setaria italica L. Beaur) is a native Korean herbal medical food and a native millet, and Koreans have eaten it as a substitute for rice since ancient times. Foods using foxtail millet (Setaria italica L. Beaur) have been recorded not only in cookbooks but also in Korean traditional medical books several times. Therefore, the purposes of this study was to investigate Food-Therapy (食治) using foxtail millet (Setaria italica L. Beaur) recorded in the literature from 1300 to 1600 from early to mid-Joseon (朝鮮) and provide data required to develop menus for Yaksun (藥膳, herbal food). This study examined Food-Therapy using foxtail millet (Setaria italica L. Beaur) in 10 types of literatures from the 1300s to the 1600s. and is described in the literature a total of 63 times. According to classification by cooking method, porridge (粥) was most frequently mentioned in the literature at 27 times. The cooking method of Soup (湯) is described 11 times. Cooking methods such as porridge juice and soup are frequently used since those methods are digestive and absorptive. Other food ingredients described using foxtail millet (Setaria italica L. Beaur) are white leek (Allii Fistulosi Bulbus) ginger (Zingiber officinale), chicken egg, Allium chinense, sparrow (Passer montanus), rooster liver, Du-si, crucian carp (Carassius auratus L), and white broiler. Other medicinal herbs described with Setaria italica are Panax ginseng (人蔘), Poria cocos (茯笭), Angelica acutiloba (當歸), Ziziphus jujuba (大棗), Liriopeplatyphylla (麥門冬), and cinnamon (肉桂). Food-Therapy using Setaria italica L. Beauv was described as a prescription for stomach and spleen (脾胃), stomach reflux (反胃), defecation and urinary disorder (大小便難), cholera, deficiency syndrome (虛症), and tonification (補益). This focus on promoting health and preventing diseases by strengthening the stomach and spleen and improving defecation and urination using Food-Therapy when herbal medicine was rare.

  • Purpose: The purpose of this study was to examine the effect of black soybean (CJ-3) testa extracts on lipid profiles in streptozotocin (STZ)-induced diabetic rats. Methods: One control group and four STZ-induced diabetic groups with different doses of black soybean (CJ-3) testa extracts treatment [0 mg/kg (diabetic control, EX), 250 mg/kg (EX-250), 500 mg/kg (EX-500), 1,000 mg/kg (EX-1000)] were orally administered for 4 weeks. Results: All CJ-3 treatment groups had remarkably lower serum triglyceride (TG) levels than that of EX group (p < 0.05) whereas hepatic TG contents did not show any differences. Results from serum total cholesterol (TC) concentrations of EX-250 and EX-1000 groups were decreased compared to EX group (p < 0.05). Furthermore, protein levels of 3-hydroxy-3-methyl-glutaryl-Coenzyme A (HMG-CoA) reductase from the liver decreased in all treatment groups (p < 0.05). However, significant differences were not observed in serum glucose and insulin, and glucose transporter 4 (GLUT-4) protein expression in skeletal muscle tissue. Conclusion: These results suggest that black soybean testa extracts could be useful for improvement of hyperlipidemia and hypercholesteremia in diabetes.

  • Medium-chain fatty acids (MCFA) are composed of 8-12 carbon atoms, and are found in coconut, cuphea, and palm kernel oil. MCFA were introduced into clinical nutrition in the 1950s for dietary treatment of malabsorption syndromes because of their rapid absorption and solubility. Recently, MCFA have been applied to Gastrointestinal Permeation Enhancement Technology (GIPET), which is one of the most important parts in drug delivery system in therapeutics. Therefore, to accumulate the MCFA in seed oil of rapeseed, much effort has been conducted by classical or molecular breeding. Laurate can be successfully accumulated up to 60 mol% in the seed oil of rapeseed by the expression of bay thioesterase (Uc FatB1) alone or crossed with a line over-expressing the coconut lysophosphatidic acid acyltransferase (LPAAT) under the control of a napin seed-storage protein promoter. Also, caprylate and caprate were obtained 7 mol% and 29 mol%, respectively, from plants over-expressing of the medium-chain specific thioesterase (Ch FatB2) alone or together with the chain-length-specific condensing enzyme (Ch KASIV). Despite the success of some research in utilizing parallel classical and molecular breeding to produce MCFA, commercially available seed oils have for the most part, not been realized. Recent research in the field of developing MCFA-enriched transgenic plants has established that there is no single rate-limiting step in the production of the target fatty acids. The purpose of this article is to review some of the recent progress in understanding the mechanism and regulation of MCFA production in seed oil of rapeseed.
