Esophageal foreign body with esophageal perforation (식도천공을 초래한 식도이물)

  • The authors experienced an unusual case of esophageal foreign body with esophageal perforation. A 8-month-old boy swallowed a long (about 20 cm) thick (about 3mm in diameter) wire, which had a hooked end and the another straight. The hooked end was located at near the first esophageal narrowing with swelling on the left lateral neck and the another end was out of his mouth. As failure of complete removal of the foreign body with esophagoscopy, the hooked end was cut and removed with the lateral incision of the neck and the remainder was removed was removed with esophagoscopy. After removal of the foreign body, nothing was given by mouth and the patient feed through nasogastric tube for about 4 weeks and the perforated esophagus was healed completely without any other complication.

Clinical Observation of Complications of Foreign Bodies in the Esophagus (식도이물의 합병증에 관한 임상적 관찰)

  • Though complications of foreign bodies in the esophagus are rare, they are sometimes fatal due to injuries to the esophageal wall by sharp pieces of bone, metals, long-term lodgement of foreign bodies in the esophagus or a little inadvertence during diagnostic or therapeutic esophagoscopy on patients with esophageal diseases. Therefore, failure to early diagnose and adequately treat the complications will lead to high morbidity and mortality. The authors have experienced 21 cases of complications of foreign bodies in the esophagus caused by foreign bodies themselves or inadvertence during esophagoscopy. So we report them with literature review.

  • An 18-year old woman had dysphagia and frequent vomiting after meals for 6 years. She lost 15 kg in 6 months recently. After esophageal manometry, she was diagnosed with achalasia. We decided to use laparoscopic surgery because there was no symptomatic improvement after medication. We made small 5 incisions on her abdomen. We performed Heller myotomy and Dor fundoplication. We performed esophagogram one day after the operation. There was no leakage of the contrast media, and it passed well. She started to eat at the 2nd day after the operation and was discharged on the 9th day without other specific problem.

  • Recently, video-assisted surgical approaches for achalasia have been adopted by many surgeons. Many reports showed that the minimal invasive video-assisted operations for Ihe achalasia revealed such good results as the conventional operations via thoracotomy. In some studies, among the minimal invasive video assisted surgeries for achalasia, the laparascopic assisted operations have some advantages mainly in respect to patient satisfaction over the thoracoscopic assisted surgeries. In this case, the patient had not responded to repeated balloon dilatation, and we made 5 small incisions over the abdominal wall and performed an esophageal myotomy and partial anterior fundoplication by laparascopic guide. The patient's symptoms were almost relieved, and the postoperative radiologic findings were satisfactory.

  • Endoscopic ultrasound in the diagnosis of esophageal carcinoma is an indispensable procedure, not only to discuss the preoperative staging of the lesion, but also to evaluate the therapeutic effect of chemo-radiation therapy. The recent increase in the incidence of superficial esophageal cancer and promising developments in potentially curative endoscopic therapies have placed EUS to a central position in decision making. Recent data have called into question the staging accuracy of EUS to distinguish mucosal from submucosal lesions, particularly in patients with early disease. In those cases, diagnostic endoscopic resection may be useful for staging and curative in superficial lesions. Nonetheless, EUS has been regarded as the most accurate staging tool and should be performed to identify potential candidates for endoscopic resection.

Transhiatal Esophagectomy in Esophageal Cancer (식도암에서 경열공 식도절제술의 적용에 대한 고찰)

  • Surgery remains the main stay in the treatment of carcinoma of the esophagus and the results of surgery for esophageal cancer have improved over the past 10 years. The ideal operation for cancer of the esophagus should have good palliation, low morbidity and mortality, and optimize both long-term function and survival. The two main approaches currently used for surgical treatment of esophageal cancer are: transthoracic esophagectomy (TTE) and transhiatal esorhagectomy(THE). The advantages of THE are low morbidity and mortality, short operating time, a short hospital stay and low interference with respiratory physiology The selection criteria for this procedure may differ but there are two situations which could clearly benefit from THE; these are epithelial and superficial submucosal lesions, particularly in cases of multiple lesions, and any resectable tumor at any stage with poor clinical status. I reviewed the selection criteria, surgical procedures, and results of THE in esophageal cancer with the literatures.

  • With the advent of stage-adapted multimodal regiments for esophageal malignancy, accurate staging has become of utmost importance. Endoscopic ultrasono graphy(EUS) i.: a diagnostic method for the local staging of esophageal cancer, in particular for T and N evaluation. The aim of this study was to examine the diagnostic efficacy of the EUS and to evaluate the relation between prognosis and EUS findings. Material and Method : We reexamined the cases of 88 consecuitive patients who underwent transthroacic esophagectomy and cervical esophagogastrostomy for esophageal cancer from January 1991 to July 2001 We compared EUS findings, histological results and outcomes. Result : There were 83 male(94.3%) and 5 female. Median age was 61.3 years old. Overall staging accuracy were 59.0% for T staging and 76.0% for N staging, In early T staging group, the accuracy was 74.9% except 74 stage. However significant differences were not found in the ability of EUS-determined T-stage and N-Stage to predict survival. Multivariate analysis showed EUS-stage to predict survival. Conclusion : EUS provides a high degree of accuracy in assessing both T and N parameters in staging esophagal cancer. EUS should be performed in all patients with esophageal cancer, not only for staging before therapy, but also as a valuable method of determining prognosis.

Endoscopic Removal of Esophageal Foreign Body in a Moluccan Cockatoo (Cacatua moluccensis) (몰루칸 앵무새에서의 내시경을 이용한 식도 이물 제거 일례)

  • A thirty-month-old male Moluccan Cockatoo (Cacatua moluccansis) with mild anorexia was referred. Through physical examination, fireign body was palpated at the crop region. Radiopaque, lineal foreign body was visualized on the lateral radiographs of the thoracic esophageal region. The patient was definitively diagnosed esophageal foreign body which is ingested feeding tube. The foreign body removal was undertaken using a flexible endoscope and a grasping forcep without any other complications. This case report demonstrated that successful esophageal foreign body removal with endoscopy in birds.

  • Corrosive injuries of the esophagus by accident or suicidal attempt, though decreasing in number, still represent an important problem of our national pathology. One of the most difficult problems facing any physician is the diagnosis and management of caustic ingestion. In order to determine the extent of esophageal bums, to prevent the potential complication, to increase the therapeutic effect, the use of esophagoscopy is an essential step, and it has lessened an unnecessary admission and treatment. The authors have found the value of early esophagoscopy in selected 41 corrosive esophagitis patient who were admitted to Dept. of ENT, Wonju medical college, Yonsei university between 1980. 1-1982. 12
