Esophagoaortic Fistula Caused by Esophageal Tuberculosis-A Case Report- (식도 결핵에 의한 식도 대동맥류-1례보고-)

  • 식도 결핵은 아주 드문 질환으로 연하곤란과 흉통이 가장 흔한 증상이면 다량의 토형은 드문 것으로 되어 있다. 본원에서는 다량의 토형을 동반한 식도 결핵에 의한 식도 대동맥루를 가진 환자를 지험했다. 4세 남자 환자는 다량의 토혈로 응습실을 통해 입원했다. 내원 당시 응급으로 시행한 내시경 검사상 incisor로부터 25cm 하방에 0.7 cm의 풍부한 혈관성의 육아종성 병변을 발견하고, 응급개흉술로 식도의 종양성 병변에 대해 쐐기 절제술을 시행하였다. 식도의 종양성 병변부위는 대동맥과 심게 유착되어 있었고 식도에서 대동맥쪽으로의 식도루를 이중 결찰했다. 환자는 술후 8일째 갑작스런 흉관을 통한 다량의 출혈과 구토 후 토형이 있어 응급 재 개흉술을 시행하여 대동맥파열과 식도 문합부 파열을 확인하였으나 더 이상의 교정이 불가능하여 사망하였다. 이에 문헌고찰과 함께 보고하는 바이다.

  • Esophagobronchial fistula is an uncommon complication of tuberculous lymphadenitis and usually requires surgical treatment in addition to antituberculosis medication. A case of tuberculous esophagobronchial fistula was healed effectively with antituberculous therapy alone.

  • The esophagus is a rate site for rarely involved site of tuberculosis. The most common cause of esophageal tuberculosis is secondary involvement from adjacent tuberculous lymphadenitis. Esophago-nodal or esophagobronchial fistulas may be formed when tuberculous lymph nodes erode the adjacent esophageal or bronchial wall. We report a patient diagnosed with esophageal tuberculosis, which was complicated by an esophago-mediastinal fistula, by endoscopy, sputum acid fast bacilli (AFB) stain, chest computed tomography (CT), and an esophagogram. The patient was treated with antituberculous agents and chest CT and endoscopy showed that the fistula had closed completely.

  • 기도이물의 진단 방법중 자세한 병력이 가장 중요하다는 것은 주지의 사실이다. 그러나 유소아에서는 부정확한 병력 때문에 기도이물의 진단에 어려움이 많아 보존적 치료를 계속하는 경우가 흔하다. 최근 저자들은 2명의 소아에서 장기간의 소아과적 치료에도 호전되지 않고 무기폐를 초래한 결핵성 기관지이물을 환기성 기관지경을 사용하여 제거함으로써 치유하였기에 문헌고찰과 함께 보고한다.

  • Despite the incidence of tuberculosis has been greatly reduced with chemotherpy, it is still a common disease in Korea. The pharyngeal and laryngeal tuberculosis usually result from direct contamination of the laryngeal or pharyngeal mucosa by sputum heavily laden with Mycobacterium tuberculosis, secondary infection from the lungs via lymphatic or hematogenous routes, or a primary affection from inhaled tubercle bacilli. Recently the authors experienced a pharyngolaryngeal tuberculosis and report this case with brief literatures review.

2 Cases of Nasopharyngeal Tuberculosis (비인강결핵 2례)

  • Nasopharyngeal tuberculosis is a rare pathological condition. It is most often associated with lymph node and pulmonary lesions, but it may be an isolated lesion. The clinical manifestation may resemble a malignant tumor of the nasopharynx and the nasopharyngeal tuberculosis is occurred occasionally primary infection but more frequently secondary infection to pulmonary tuberculosis. The nasal endoscopic evaluation of nasopharynx is necessary in patient with possible pulmonary or extrapulmonary tuberculosis. The author reports two cases of nasopharyngeal tuberculosis in a 45-years old and 34-years old woman with a review of the literature.

  • Tuberculous cervical lymphadenitis is one of the most common causes of neck mass in Korea. But the diagnostic confirmation is difficult and it's effective treatment modalities are still in controversy. Through a retrospective study of 32 cases of tuberculous cervical lymphadenitis treated with surgical treatment during recent 4 years, we have analyzed 5 subtypes of CT finding and surgical treatment modalities and received following results 1. The locations of lymphadenitis were posterior triangle(12 cases), submandibular area(9 cases), supraclavicular area(8 cases), submental area(2 cases), and parotid area(1 case). 2. Five subtypes of CT findings were observed : Homogeneous enhancement type(3 cases), peripheral rim enhancement type(6 cases), peripheral rim enhancement with perinodal fat obliteration type(12 cases), coalescent cold abscess type(8 cases), and mixed type(3 cases). 3. Surgical treatment modalities : Excision (13 cases), incision and drainage(9 cases), selective neck dissection(8 cases), and curettage(2 cases).

  • A diabetic 73-year-old man had admitted and examined gastrofiberoscopy due to dyspagia and weight loss of 6 kg during two months. He was confirmed diagnosis of esophageal squamous cell carcinoma. During the esophgography, we found the dye spillage into pericardia! space from the esophagus with air collection A following chest roentgenogram revealed a typical pneumopericardium with pericardial air filling. Although intensive care was done. he expired 4 days later. We report a case of typical pnemopericardium caused secondary to esophageal cancer.

Treatment of Laryngeal Web (Laryngeal web의 치료)

  • Laryngeal web was first reported by Fleischmann in 1822, and it causes upper airway obstruction and abnormalities of phonation. Congenital webs result from an arrest of development of the larynx around the tenth week of fetal life. Acquired webs may result from cicatrical strictures of infectious lesion, traumatic and postoperative wounds. The most common site of webbing is glottic, followed by subglottic and supraglottic. We have experienced a case of simultaneous glottic and supraglottic laryngeal webs probably due to tuberculous lesions in 28 aged male who was treated by endolaryngeal microsurgery.
