Vim Thalamotomy for Intractable Rubral Tremor Associated with Midbrain Tumor - Case Report - (중뇌종양에 의한 적핵진전에 대한 Vim 시상핵절제술 - 증례보고 -)

  • This 26-year-old man gradually developed a disabling midbrain tremor involving both distal and proximal part of left upper arm. On neurologic examination, oculomotor palsy, and ataxia of the left arm were noted. Radiologic examination revealed a mass lesion on midbrain tegmentum. He was tentatively diagnosed as brain tumor (presumably germinoma) without stereotactic biopsy. Conventional radiation therapy was given for this lesion. Although there was improvement in the radiologic imaging, his midbrain tremor worsened and became untolerable. The authors performed MR-guided stereotactic Vim-thalamotomy. With macroelectrode stimulation and radiofequency lesioning, his resting, postural and action tremors were almost completely abolished in both distal and proximal part of left upper extremity. Authors consider that Vim thalamotomy is still an effective means of controlling midbrain tremor involving proximal upper limb.

The Effect of Treatment with MSG(monosodium L-glutamate) and/or Phenylalanine on the Hypothalamus and Cerebral Cortex of the Rats (MSG와 phenylalanine 처리가 흰쥐 시상하부와 대뇌피질에 미치는 영향)

  • These experiments were performed to investigate the effect of saline, monosodium L-glutamate(MSG), MSG-phenylalanine, and/or phenylalanine on TH-immunoreactivity in arcuate nucleus of hypothalamus and hind limb area of cerebral cortex in rats using the immunohistochemical methods. The result were as follows: 1. TH-immunoreactive neurons in hind limb area of cerebral cortex and arcuate nucleus of hypothalamus were decreased in MSG treated group compared to the saline treated group and also lesions in arcuate nucleus and median eminence of hypothalamus were increased with MSG treatment 2. TH-immunoreactive neurons in median eminence and arcuate nucleus were increased in phenylalanine treated group compared to the saline treated group and also neurons were more increased in lamina V of hind limb area of cerebral cortex with phenylalanine treatment. 3. TH-immunoreactive neurons in median eminence and arcuate nucleus were decreased in MSG-phenylalanine treated group compared to the phenylalanine treated group and increased compared to the MSG treated group. In lamina V of hind limb area of cerebral cortex, TH-immunoreactive neurons were more decreased in MSG-phenylalanine treated group than that of the phenylalanine treated group, and more increased than that of MSG treated group. These experiments indicated that TH-immunoreactive neurons in hypothalamus and cerebral cortex were increased due to the activation of phenylalanine and decreased by suppressing activation of phenylalanine through MSG treatment.

  • Difficulties in riding, driving and parking are a common issue which inevitably appears in city urbanization and city modernization development all over the world. However, those cities differ in controlling the traffic. According to the specific practice in Shanghai, this article presents the various factors which affect the traffic capacity and the operation quality. This article also outlines transportation system management technique of overall coordination and comprehensive control in the following seven aspects: controlling traffic motivation; improving traffic distribution; reasonably and scientifically selecting travel mode, travel route and travel time; reinforcing traffic management and road construction. The article finally suggests that above mentioned points are cost-effective ways of handling traffic plight for metropolitan transport.

  • This is a report of 4 cases of the surgical correction of mandibular prognathism with chief complaint of mastication difficulty, facial asymmetry and protrusive chin. We performed sagittal spit ramus osteotomy for the prognathism, we used the repositioning plate for conserving the condylar segment into its original position and fixed the osteotomized bone segments rigidly with adjustable monocortical plate. Intermaxillary fixation was performed during 2 weeks. As a results, we found the following advantage. 1. Rigid fixation effect like the fixation by the tandem screw. 2. Decreased postoprative swelling. 3. It is needless to do the stab incision for the transbuccal set instrument. 4. It is more convenient to perform the rigid fixation in the monocortical plate method than screw technique.

  • We treated a 37-year-old male with traumatic hematoma in the suprapatellar bursa that had developed in the form of persistent swelling on suprapatellar area of left knee after blunt trauma. Though there were no obvious abnormal findings on plain roentgenographs, an isolated suprapatellar cystic lesion with fluid-fluid level on T2-weighted sagittal image of MRI was noted. We found the suprapatellar plica with complete septum and no synovitis in the knee joint proper by arthroscopy. We incised the plica and found leakage of blood-stained fluid from the suprapatellar bursa. There were no findings of pigmented villonodular synovitis or other tumorous lesions. At 6 months after surgery, the patient felt symptom-free and there was no recurrence.

A Study on the Development of Hospital Management Award Programme in Korea (병원경영대상 시상제도의 설계 연구)

  • The purpose of this study to develop the Hospital Management Award(HMA) programme which would be a basic information for advancing hospital industry in Korea. HMA programme in the study are designed with general census building method of researchers and with the data of professions opinion which was survey with structured questionnaire Major results of this study are as follows. First, Hospital Management Award(HMA) programme is perceived very necessary and useful for advancing hospital industry of Korea (positive response rate: 93.0%). Second, Criteria, Classification of hospitals may be the function, size and ownership of hospital. Third Evaluating area of hospital management, examining method and evaluation marking points are designed on the basis of hospital management information system. Forth, this study suggests same strategies for execution of the programme. On the basis of the study results this study suggests that Hospital Management Award(HMA) programme would rather actualized in advance by formal institute and/or association. And some in depth studies are required mainly for development of the action plan of HMA programme.

  • The authors applied anterior sagittal transrectal apporach (ASTRA) for the repair of urethrovaginal fistula which developed after total repair of persistent cloaca. The patient had been diagnosed to have persistent cloaca, double uterus and double vagina, and received PSARP, excision of right-side uterus and vagina, and left vaginal switch operation at 22 months old. After operation, the patient admitted several times due to frequent urinary tract infection and ectopic stone formation in bladder and neovagina. Urethro-neovaginal fistula was confirmed by cystoscopy and corrected with ASTRA. Postoperative voiding cystourethrogram showed no fistula tract. ASTRA showed improved surgical field, minimized ureterocystic damage, and preserved perirectal nerve due to limited incision of rectum.

Comparative Study of Skeletal Relapse According to the Fixation Method after BSSRO for Mandibular Setback. (하악 시상골 절단술 후 고정 방법에 따른 회귀 성향에 대한 비교 연구)

  • Objective : To compare two different methods of rigid fixation in postoperative stability after mandibular setback. Material and Methods : 28 patients with Class III malocclusion were treated by bilateral sagittal split ramus osteotomy(BSSRO) and mandibular setback were selected for this study. Group A(n=14) had the bone segments fixed with monocortical miniplate on the lateral side of the mandibular body and Group B(n=14) had three noncompressive bicortical screw inserted at the genial area through a transcutaneous approach. Cephalograms were taken preoperatively, postoperatively within 1 weeks and at a follow-up period (mean 8.9 months after surgery) and the amount of setback and postoperative change were measured. Results : Postoperative relapse between two groups was minimal in setback of the mandible. Statistical analysis showed no significant difference in postoperative relapse. Conclusion : This study suggests that both methods of skeletal fixation investigated give comparable postoperative stability and their use in mandibular setback appears to be a fairly stable clinical procedure .

SKELETAL RELAPSE AFTER SAGITTAL SPLIT RAMUS OSTEOTOMY AND SCREW FIXATION (하악전돌증에서 하악지 시상분할골절단 및 Screw고정후 골성회귀에 관한 연구)

  • Skeletal and dental changes were examined in 38 patients of mandibular prognathism who been treated by a bilateral sagittal split osteotomy(SSRO) and internal fixation using titanium mini-screws. All patients were followed up for over 8 months after the surgeries, and postoperative cephalometric measurements were compared at 2 months and at 8 months. Linear measurements of the "Pog-most posterior screws" and angular measurementsts of "SN-Pog'were compared to figure out the change of bony fragments. The significancy of data were tested by unpaired T-test. The results were as follows : 1. The fixation screws were changed in cephalometric position as little as $0.32{\pm}2.51mm$ in SSRO and $0.15{\pm}1.00mm$ in SSRO & Le Fort I Osteotomy.(P<0.05) 2. Mandibular set-back over 5mm resulted in less stability of the fixation screws and higher relapse tendency. 3. The internal fixation using two screws along the inferior border and one on the superior ridge is considered to be very resistant to postoperative relapse of the repositioned bony segments.

Prediction of Amount of Mandibular Set Back with 3 Plain Radiographs in Mandibular Sagittal Split Ramus Osteotomy (하악상행지 시상분할골절단술 시 하악후퇴량의 방사선학적 예측)

  • Purpose: The present study examined the reproducibility of an operation plan by comparing the jaw position of STO with the postoperative mandibular set back measurement in sagittal split ramus osteotomy. Methods: Thirty patients with class III dental and skeletal malocclusion and who were treated with BSSRO were reviewed. Three plain radiographs such as the panoramic view, the lateral cephalogram and the submentovertex view were taken before and after operation. Also, paper surgery for STO and model surgery were used to evaluate the amount of mandibular set back. Results: On the panoramic view, the amount of mandibular set back in STO was similar to the postoperative results of model surgery, but the amount of mandibular set back on the lateral cephalogram was smaller than the postoperative result of model surgery and then the amount of set back on submentovertex view was similar to the postoperative result of model surgery. Conclusion: Precise tracing and paper surgery should be performed for a combined expected STO in order to predict the exact amount of preoperative mandibular set back.
