Analysis and Usefulness of Microelectrode Recording during Deep Brain Stimulation Surgery in Movement Disorders (이상운동질환에 대한 뇌심부자극 수술 중에 미세전극 기록의 분석과 유용성)

  • Deep brain stimulation (DBS) is an effective surgical procedure for treating drug refractory movement disorders, and DBS involves delivering high frequency electrical stimulation to deep brain nuclei. Microelectrode recording (MER) is a complementary test that can precisely identify the location of deep brain nuclei, along with MRI correlation, during DBS surgery to improve the surgical outcome and minimize side effects. The purpose of this paper is to analyze the neuro-physiological waveforms and identify the usefulness of MER by analyzing the MER performed during DBS surgery for treating movement disorders. We retrospectively reviewed 28 patients who underwent MER during DBS surgery for movement disorders from January to December 2018. Of the 28 patients, 38 MERs for the subthalamic nucleus (STN), 10 MERs for the globuspallidusinternus (Gpi), and 4 MERs for the ventral intermediate thalamic nucleus (VIM) were performed. In all the cases, the target sites were found and micro-stimulations were used to check for side effects and to readjust the target sites. The clinical symptoms of all 28 patients improved after surgery. In conclusion, MER is a useful test that employs neuro-physiological waveforms to accurately identify the deep brain nuclei, along with MRI correlation, to improve the DBS surgical outcomes for movement disorders and to minimize side effects.

  • In order to obtain a fine picture, a camera has many convenient functions. Its representative functions are Auto Focus(AF), Auto White Balance(AWB) and Auto Exposure(AE). In this paper, we present the new algorithm of Auto Exposure control system, one of its useful functions The proposed algorithm of Auto Exposure control system is based on IIR Filter with Variable Time Constant. First, in order to establish the standards of exposure control, we compare change of the picture luminance with luminance of an object in the Zone system. Second, we make an ideal characteristic graph of luminance by using the results. Finally, we can find the value of the right exposure by comparing an ideal characteristic graph of the luminance with the value of the current expose of a scene. We can find an appropriate exposure as comparing the ideal characteristic graph of the luminance with current exposure of a scene. In order to find a suitable exposure state, we make use of IIR Filter instead of a conventional method using micro-controller. In this paper, the proposed system has therefore simple structure, we use it for compact image sensor module used in the handheld device.

A Study to Acquire Sharp Images in the Haas(Skull PA Axial Projection) (Haas 촬영법에서 선예한 영상 획득을 위한 연구)

  • The Study In order to obtain a sharpness Image from Skull PA axial projection (Haas) in a head axial X-ray Examination, this study changed the posture angle using Skull Phantom and evaluated the image subjectively to 5 radiologists who worked in the Department of Imaging at University Hospital. In the prone position, the head was lowered 4 cm from the back of the head, entered 25° toward the head, and the image evaluation score was high with 20 points, such as the back bone, dorsum sellae projected in the large hole, and posterior clinoid process. In addition, the score significance was verified, and the Cronbach Alpha value was evaluated to have good reliability of 0.789. As a result of calculating the signal-to-noise ratio (SNR) by setting the region of interest (ROI) of the image, it was the highest at 5.957 for 25° incident at the back of the head and 6.430 for 30° incident at the back of the head. As a result of the study, in order to obtain a sharp image of the back of the head bone, dorsum sellae, and posterior clinoid process when shooting in the axial direction after the head, it is filmed by tilting 25° toward the head from 4 cm below the back of the head. In order to obtain a sharp image of rock pyramid symmetry, petrous ridge, sagittal suture, and lambdoid suture, it is thought that it will be helpful for clinical use if you shoot it 8cm down from the back of the head and tilt it 30° toward the head.

Study of Computed Tomography Reconstruction Angle of Bony Stenosis of the Cervical Foramen (경추 신경공 골성 협착에서 CT 재구성 각도 연구)

  • This study is an image reconstruction method after cervical CT scan. According to the oblique coronal reconstruction angle, i would like to suggest the reconstruction angle that can best express neural foraminal bony stricture. The angle created by drawing a line passing through the center of the spinal cord in the midsagittal plane of the cervical spine and the uncovertebral joint was measured. A line passing through the center of the spinal cord was drawn from the neural foramen, and the angle formed with the midsagittal plane was measured and compared. At the C4-5 level, the average was 9.2°, at the C5-6 level, the average was 9.9°, and at the C6-7 level, the average was 8.4°, the neural foraminal angle was measured to be larger than the uncovertebral joint angle. There was a statistically significant difference in mean (p<0.01). Also, it was found that the angle increased toward the lower cervical vertebrae. The angle between the neural foramen of the lower cervical vertebrae (C5-6, C6-7) and the center of the spinal cord is between 55 and 60°. Here, if the oblique coronal image is reconstructed to be 90°, the degree of neural foramen stenosis can be observed well. Because it is an image reconstruction using a conventional CT scan image, it does not receive additional radiation exposure. It is of great significance in diagnosing cervical neural foramen bony stenosis.

  • In this study, an exact solution of governing differential equation for the bending problem of partially loaded orthotropic rectangular plates is presented and also its computer program is developed. The method requires that two opposite edges be clamped or simply supported, or one edge clamped and the other simply supported. Any combination of boundary conditions could exist along the other edges. The plate could he subjected to uniform, partially uniform, and line loads. The solution for the deflection of rectangular plate is expressed as a Levy type single Fourier series and the loads arc expressed as a corresponding series. The advantage of the solution is that it overcomes the limitations of the previous Navier's and Levy's methods (limitation of boundary condition and loading conditions of plate), it is easy to program on a computer and it becomes fast to solve the bending problem with computer program. Calculations are presented for isotropic and orthotropic plates with different loading and boundary conditions. Comparisons are made for the isotropic plate with various boundary conditions between the result of this paper and the result of Navier, Levy and Szilard. The deflections were in excellent agreement.

Low-Power 4th-Order Band-Pass Gm-C Filter for Implantable Cardiac Pacemaker (이식형 심장 박동 조절 장치용 저 전력 4차 대역통과 Gm-C 필터)

  • Low power consumption is crucial for medical implantable devices. A low-power 4th-order band-pass Gm-C filter with distributed gain stage for the sensing stage of the implantable cardiac pacemaker is proposed. For the implementation of large-time constants, a floating-gate operational transconductance amplifier with current division is employed. Experimental results for the filter have shown a SFDR of 50 dB. The power consumption is below $1.8{\mu}W$, the power supply is 1.5 V, and the core area is $2.4\;mm{\times}1.3\;mm$. The filter was fabricated in a 1-poly 4-metal $0.35-{\mu}m$ CMOS process.

  • This is the case report of huge complex odontoma treated with sagittal splitting of buccal bone plate and iliac bone graft in left mandible angle. The 22 years old patient was admitted to the department of Oral and Maxillofacial Surgery of Kyungpook National University Hospital with the chief complaint of swelling on the left mandible angle area. We used extra oral Risdon incision and splitted the buccal cortical bone after making the horizontal bone cut buccally. The tumor mass was removed with cutting into the pieces with surgical bur to prevent mandibular fracture. The dead space was grafted with autogenous iliac bone graft and the splitted buccal cortical bone was fixed with two L-type miniplate. After 12months follow up check, we noticed good process of bone healing and satisfactory aesthetic result. In this case, my operative approach provided the excellent surgical access to the hard tissue mass and minimized post operative complication comparing with the conventional surgical approaches.

  • The case of a 73-year-old Korean female with dysgeusia due to an acute left thalamic infarction is presented. The patient was treated with the herbal medicine Yanggyuksanhwa-tang, in addition to acupuncture and electronic acupuncture treatment. The filter-paper test and Oral Health Impact Profile-14, (OHIP-14) test were administered to evaluate dysgeusia. Post-treatment, the patient's dysgeusia improved. Korean medicine may be an effective treatment for dysgeusia caused by a thalamic infarction.

  • "소기도"라 일컬어지는 해부학적 부위는 말단부 막성 세기관지와 호흡성 세기관지로 구성된 직경 3 mm 이하의 기도부위이다. 방사선학적으로는 고해상 전산화단층촬영(CT)에서 흉막직하의 직경 약 1.0 cm으로 이루어진 2차 소엽내의 중심부에 위치하게 된다. 그러므로 이 부위의 질환때에는 중심소엽성 세기관지내의 가득찬 물질로 인해 나타나는 중심소엽성 결절들과 선상음영들이 보인다. 이외의 소견으로는 중심소엽성 폐기종, 모자이크 모양의 폐음영, 분절하 무기폐등이 있고, 호기시 CT 촬영에서 나타나는 공기포획이 있다. 최근에는 다검출기형식의 CT (multidetector CT)의 발전으로 인하여 이차원 재구성 (2 dimension reformat) 관상면, 시상면 CT 스캔을 매우 명확하고 빨리 얻을 수 있고, 기관지에 대한 삼차원 볼륨 영상 (3 dimentional volume rendering image) 등을 얻어서 가시적인 효과를 높이고 진단의 정확성에 보다 더 접근하게 되었다. 소기도를 침범하는 질환은 일차적인 것과 이차적인 것이 있는데, 병리조직학적으로는 원인별로 흡연으로 인한 소기도 질환, 세포성 세기관지염, 수축성 세기관지염, 증식성 세기관지염등으로 구분하며 여기에는 이와 같은 병리질환을 일으키는 다양한 원인들이 포함된다. 이외에도 드문 질환으로 미만성 범세기관지염, 광물질에 의한 소기도 질환등이 있다.

  • Maxillary-malar deficiency is the most frequently occurring midface dentofacial deformity. Clinicaly patients with maxillary-malar deficiency exhibit malar and infraorbital rim deficiency and class III malocclusion. For treatment of these deformities, modified LeFort III osteotomy have been used. Modified LeFort III osteotomy advances maxilla with orbital rims and zygomatic bone anteroposteriorly. This is a case of patient who had severe mandibular prognthism with midface deformity. We performed modified LeFort III osteotomy for maxillary-malar advancement and simultaneous bilateral sagittal split ramus osteotomy for mandibular prognathism and autogenous iliac bone graft.
