Isolated Activation of the Upper Trapezius in Three Manual Muscle Testing Position : Convergence Study (세 가지 도수근력 검사 자세에서 상부 승모근의 독립적 수축 : 융합적 연구)

  • The purpose of this study was to investigate the optimal position among three manual muscle tested positions for upper trapezius in which to obtain an isolated upper trapezius EMG signal for the normalization of upper trapezius muscle EMG activity. A total of 28 healthy adult men participated in the experiment. The UT (upper trapezius) and LS (levator scapulae) muscle activities were measured using the TeleMyo 2400T and analyzed using MyoResearch software. The muscle activity of the US and LS was measured by performing three manual muscle test positions for the upper trapezius. The UT/LS ratio during the S-MVIC was 80.25 and was significantly higher than that during the T-MVIC (76.50; p = 0.011) and the C-MVIC (60.95; p < 0.001). And, the UT/LS ratio during the T-MVIC and was significantly higher than that during the C-MVIC (p < 0.001). Based on the results of present study, we suggest a switch from T-MVIC to S-MVIC for the independent normalization reliability of upper trapezius EMG activity. The UT muscle strength or normalization test using S-MVIC will be able to measure muscle strength or activity of UT compared to previous measurement methods.

The Effect of Stretching to Muscle Stiffness in Hospital Office Employees

  • This study is to investigate the effects of stretching of hospital office employees on muscle stiffness. A total of 40 healthy young women voluntarily participated in the study. The participants were randomized to the stretching groups and control groups. Before stretching, the participants measured muscle stiffness of sternocleidomastoid muscle, upper trapezius muscle suboccipital muscles and pectoralis minor. And then applied stretching to stretching group. Muscle stiffness remeasured five minutes later in each group. In all muscles, muscle stiffness of stretching group were significant decreased(p<.05), but control group were no significant difference between pre and post. therefore appropriate stretching is expected to prevent and reduce musculoskeletal diseases caused by increased muscle stiffness in hospital office employees.

The effect of taping applied to the lower trapezius muscle on the upper trapezius muscle tone, pain, and cervical range of motion in chronic upper trapezius pain patients (만성 상부승모근 통증 환자의 하부승모근에 적용한 테이핑이 상부승모근 긴장도 통증 목뼈 가동성에 미치는 영향)

  • Background: The purpose of this study was to investigate the effect of taping applied to the lower trapezius on the upper trapezius muscle tone, pain intensity, cervical rotation range of motion in chronic upper trapezius pain patients. Design: Case-control study. Methods: Twenty subjects with chronic upper trapezius pain were classified into an experimental group and a control group. The experimental group applied lower trapezius facilitation taping and the control group applied sham taping. Taping Before and after the application of taping, muscle tone, pain intensity, and cervical rotation range of motion of the upper trapezius were measured. Results: In the experimental group, there were significant differences in the pressure pain threshold and muscle tone before and after taping. In the comparison between groups, there was a significant difference in muscle tone between the experimental group and the control group. Conclusion: The application of the lower trapezius facilitation taping was found to be effective in reducing the pressure threshold and muscle tone of the upper trapezius. Therefore, it is expected that more effective treatment can be provided by adding lower trapezius facilitation taping to the treatment protocol for patients with chronic shoulder pain.

The Effects for Brain stress by SUKI Alternative Therapy (SUKI 대체의학에 의한 뇌스트레스 감소 효과 연구)

  • The purpose of this study is to know the change of brain wave with stress by SUKI pressure alternative therapy. The experiment group was 12 students(male 6/female 6) with complained upper trapezius pain. Calculated the brain wave before and after stimulate the upper trapezius trigger point by SUKI and the stimulate time was 3min each persons(FP2, F3, F4, FP1, T3, T4, P3, P4). the experimet periods was 5times in a weeks with EEG(QEEG-S). The Date collecting used by Telescan(LXE5209). All the data was analyzed with SPSS 22.0 for window program. To compare the differences before and after the point pressure method, a corresponding sample of t-test was performed, and the statistical significance level was p<.05. The results was followed. The points of Fp2, F3, F4(*p<0.049, *p<0.042, *p<0.019) of EEG was showed a significant differences but Fp1, T3, T4, P3, P4 points did not showed. The SUKI alternative medicine techniques had a reduced effects for the some kind of brain stress. It is need to continuous research in the future.

Acupuncture Stimulation on LR3 Reduced Shoulder Pain Caused by Upper Trapezius Rigidity. A Case Report (상부 승모근 경직으로 유발된 견비통에 대한 태충혈 자침의 효과 1례)

  • 이 임상례는 근육 경직으로 인한 견비통에 태충혈이 유효함을 제시하고 있다. 환자의 주관적인 만족도를 평가하기 위해 점수식 평점 척도(Numerical rating scale;NRs)를 측정하였고 어깨 경직의 객관적인 호전 정도를 평가하기 위하여 양측 견정혈의 통증 통각 역치(Pressure pain threshold;PPT)와 어깨 외전력, 어깨관절의 외전시 가동범위(Range of motion;ROM)을 측정하였다. 양측 태충혈에 15분간 유침 후 NRS는 5에서 2로 감소하였다. 치료 전 왼쪽 견정혈의 PPT는 19N이었고 오른쪽은 22N이었다. 치료 후 왼쪽 견정혈의 PPT는 22N이었고 오른쪽은 27N이었다. 치료 전 견관절의 외전력은 왼쪽에서 29N, 오른쪽에서 22N으로 측정되었다. 치료후 견관절의 외전력은 왼쪽에서 33N, 오른쪽에서 22N으로 측정되었다. 견관절 ROM은 치료 전과 후 동일하게 왼쪽 관절 170도, 오른쪽 관절 165도로 나타났다. 결론적으로, 태충혈 자극은 어깨의 경직을 해소하고 어깨 근력을 강화하는 것으로 나타났다. 또한 그것은 상부 승모근의 압통을 감소시킬 수 있다. 이러한 측면에서, 태충혈의 효과에 대해서 보다 추가적인 연구가 필요할 것으로 보인다.

Botulinum Toxin type A injection Versus Lidocaine Injection for Myofascial Pain Involving upper Trapezius (승모근 근막동통에 대한 보툴리눔 독소와 리도카인 주사의 치료효과 비교)

  • The purpose of this double-blind study was to evaluate clinical effects of botulinum toxin type A (BTX-A) injection on myofascial pain syndrome (MPS) involving upper trapezius and compare with those of lidocaine injection. 21 patients presenting with active TrP1 and/or TrP2 in the upper trapezius over 6 months were selected for this study. The subjects were randomly divided into two groups; one group injected with BTX-A (15 unit of $Botox^{(R)}$ / 0.3 ml per trigger point (TrP)) and the other group injected with 0.5% lidocaine (0.3 ml /TrP). The clinical effects were evaluated by VAS and PPT at baseline, 2, 4, 6 and 8 weeks after treatment. BTX-A group showed persistent decrease of VAS values and increase of PPT values following treatment. While there was no significant difference in VAS values between BTX-A and lidocaine groups (p=0.347), there was significant difference in PPT values after treatment between two groups (p=0.000). The subjects received BTX-A showed noticeable improvement in PPT values after treatment, suggesting more reliable effect of BTX-A injection compared with lidocaine injection. The results of this study support that the direct injection of BTX-A to a TrP is an effective and safe treatment for MPS involving upper trapezius.

The Effects of Sling and Vibrator Application of Knee Push-Up Plus Motion on Trunk Muscle Activities in Healthy Subjects (무릎 푸시업 플러스 동작의 슬링과 진동기 적용이 정상인의 체간 근육 근활성도 변화에 미치는 영향)

  • The purpose of this study was to investigate the effects of sling and vibrator application of knee push-up plus motion on trunk muscle activities. Ten healthy adult males(age, 23.00±0.45 years; height, 176.60±1.64 cm; body mass, 67.50±1.22 kg; and BMI, 21.65±0.34 kg/㎡) were participated in this study as subjects. Three types' knee push-up plus motions were performed(basic knee push-up plus motion, BKPP; knee push-up plus motion with sling, KPPS; knee push-up plus motion with vibration, KPPV). We measured the right side's trunk muscle activities of the upper trapezius(UT), pectoralis major(PM), serratus anterior(SA), rectus abdominis(RA), and external oblique(EO). The research findings were as follows. UT, PM, SA, RA, and EO muscle activities were greatest during KPPV(p<.001). These results are expected to serve as reference materials for knee push-up plus motion applications in training programs for trunk muscle strengthening.

The Effect of Low Level Laser Therapy at the Trigger Points in Masseter and Trapezius Muscles (저출력레이저를 이용한 교근 및 승모근 발통점의 치료에 관한 연구)

  • To investigate the effect of low level laser therapy, the author selected 37 dental students with tender points in both masseters and trapeziuses, also measured maximum comportable opening(MCO), Numerical analog scale(NAS) and pressure pain threshold (PPT). 20 subjects were assigned randomly and were treated with GaAlAs diode laser after ultrasound. The other 17 subjects were treated with ultrasound and laser without irradiation. All the subjects were treated after 2 and 4 day respectively and were examined again after 6 days. And the obtained results were as follows : 1. The MCO of irradiated group increased more significantly after treatment than non-irradiated group. 2. The NAS of irradiated group decreased more significantly after treatment than non-irradiated group. 3. The PPT or irradiated group increased more significantly after treatment than non-irradiated group.

Changes in Physiological Characteristics of Trapezius, Multifidus and Hamstring Muscles According to Tilt Angle of Exercise Bed in Elderly (운동침대 틸트 각도 변화에 따른 노인의 승모근, 다열근 및 슬괵근 근육의 생리학적 특성 변화 연속 증례 연구)

  • Purpose: This study investigated the effects of changing the tilt angle of the exercise bed on physiological characteristics of trapezius, multifidus, and hamstring muscle function. Methods: Twenty elderly who complains of back and lumbar pain or discomfort participated in this study (average age: $64.70{\pm}4.55$ years). The physiological characteristics, including muscle tone (F), dynamic stiffness (S), and elasticity (D) of trapezius, multifidus, and hamstring muscles were measured by a Myoton device at three tilt angles (0, 15, and 30 degrees) during exercise. Results: The muscle tone, dynamic stiffness, and elasticity of the right trapezius and left hamstring muscle showed significant differences (p<0.05) according to exercise bed tilt angle, while the other muscles showed no significant differences. Conclusions: Our results demonstrate that physiological characteristics of the upper and lower extremity muscles are affected ipsilaterally, according to the tilt angles of the exercise bed.
