A STUDY ON THE EFFECTS OF CHEWING PATTERNS TO OCCLUSAL WEAR (저작형태가 교합면 마모에 미치는 영향에 관한 연구)

  • 저작은 치아, 악골, 저작근 뿐만 아니라 근 신경계, 고위 중추까지 복합적으로 관여되는 기능적 행위이다. 저작 형태는 다양한 모양을 가지나 두 가지 전형적인 군, 즉, 전방에서 관찰 시 그 양상이 수직적이며 절단 (chopping) 운동을 하는 군과 주로 측방으로 이루어지며 연마 (grinding)를 하는 군으로 나눌 수 있다. 본 연구의 목적은 저작 형태의 차이가 교합면 마모에 미치는 영향을 조사하는 것이다. 두개 하악 관절과 저작 습관에 이상이 없으며 교합면에 수복물이 없는 치과 대학생으로 하악 운동 궤적 기록기를 이용하여 상기의 전형적인 2가지 저작 형태를 보이는 각 15명씩을 피검자로 선택하였다. 각 피검자에 대한 임상 검사를 통해 ordinal scale로 교합면 마모의 등급을 조사하여, 평균 치아 마모도와 부위에 따른 치아 마모도를 비교 조사하였다. 각 피검자에 대한 인상 채득 후 모형을 제작하고 arbitrary scale로 교합면 마모의 등급을 조사하여, 평균 치아 마모도와 부위에 따른 치아 마모도를 비교 조사하고 저작측과 비저작측 마모를 비교 조사하였으며 수평 마모면과 수직 마모면을 비교 조사하였다. 1. 평균 치아 마모도는 ordinal scale로 측정하였을 때, 절단형과 연마형간에 유의할 만한 차이가 없었다.(p >0.05) 2. 부위에 따른 치아의 마모도는 ordinal scale로 측정하였을 때, 절단형과 연마형간에 유의할 만한 차이가 없었다.(p >0.05) 3. 평균 치아 마모도는 arbitrary scale로 측정하였을 때, 절단형에 비교하여 연마형에서 높은 마모도를 보였다.(p<0.05). 4. 절단형에 비교하여 연마형은 arbitrary scale로 측정하였을 때, 구치부에서는 높은 마모도를 보였으며 전치부에서는 유의할 만한 차이가 없었다(p<0.05) 5. 구치 평균 치아 마모도와 부위에 따른 치아의 마모도는 균형측에서 절단형과 연마형간에 유의할 만한 차이를 보이지 않았다(p>0.05). 6. 전치 평균 치아 마모도와 부위에 따른 치아의 마모도는 수평 마모면과 수직 마모면 비교시, 절단형과 연마형간에 유의할 만한 차이를 보이지 않았다.(p>0.05)

  • 반복 응력 상태 아래서 알루미나 세라믹의 피로 표면손상 누적현상이 분석되었다. 연속 미끄럼 접촉 시에 발생하는 응력 상태를 재현하기 위해서 동시에 작용하는 수직-수평 반복 압축하중 기법이 사용되었다. 알루미나 구와 평판의 접촉면에서 알루미나 미세 결정의 피로 파손에 의한 마모 입자 형성 기구가 관찰되었고, 반복하중의 횟수와 수직-수평 하중비가 커질수록 마모량은 증가하였다. 반복 접촉하중에 의한 표면손상 누적이 접촉 수직 변위 측정으로 정량화 되었다. 두 접촉 구조물의 강성 (하중-변위 선도의 기울기) 변화가 두 재질의 탄성계수의 변화로 표현되었다.

Full mouth rehabilitation of the patient with worn dentition using full-contour monolithic zirconia prostheses at an increased vertical dimension of occlusion: a case report (치아 마모 환자에서 수직고경 증가를 동반하여 단일구조 지르코니아 보철물로 완전구강회복을 시행한 증례)

  • Excessive occlusal wear results in unacceptable damage to the occluding surfaces, and can cause pulpal injury, occlusal disharmony, impaired function, and aesthetic deformity. Patients with severely worn dentition need to be classified as several categories relative to the vertical dimension of occlusion (VDO) and the interocclusal distance for the prosthetic space. When treating patients classified as those who have excessive wear without loss of occlusal vertical dimension but with limited space, we could consider treating them at an increased vertical dimension of occlusion. Treatments at an increased vertical dimension of occlusion may provide stability, given that the amount of its increase was minimal, and a perfected occlusion was achieved after enough stabilization period. A 50-year-old male visited the department of Prosthodontics in Yonsei University Dental Hospital with the chief complaint of worn teeth on lower anterior area. Reconstruction at an increased vertical dimension of occlusion was planned. After the compatibility of the new vertical dimension of occlusion had been confirmed under interim fixed restorations, definitive fixed restorations composed of full-contour monolithic zirconia prostheses were placed. This case presents that a satisfactory clinical result was achieved by restoring the worn dentition at an increased vertical dimension of occlusion with an improvement in esthetics and function.

  • The progressive attrition of teeth is a normal process by aging. However, excessive tooth wear with decreased vertical dimension of occlusion and collapse of occlusal plane may cause pathologic pulpal condition, occlusal disharmony and functional disorders. In this case, a patient with severely worn dentition was treated. Diagnostic wax-up was performed at the increased vertical dimension. After evaluation of provisional restorations for 12 weeks, final restorations were fabricated with zirconia crown and routine clinical assessments were made. Esthetically and functionally satisfactory results were obtained.

  • Pathologic attrition leads to pathologic damage on occlusal plane, functional disorders, occlusal disharmony, esthetic problems, pulpal lesion, temporomandibular joint (TMJ) disorder. In this case, treatment plan should be considered for possibility of vertical dimension loss, occlusal pattern, esthetics, phonetics, amount of vertical dimension increase. This case report was a 71-year-old man who had severely worn dentition. Full mouth rehabilitation was carried out with vertical dimension increase due to limited space for prosthesis. After evaluation of provisional restorations for patient's compliance, final restorations were fabricated and routine clinical assessments were made. This case presents that a satisfactory clinical result was achieved by restoring the worn dentition.

  • Physiological wear of occlusal surface is inevitable throughout the lifetime, and considered to be normal. Pathologic wear, however, may cause fatal damage to the occlusal surface and destroy esthetics and proper anterior guidance. In such cases, rehabilitation of occlusal wear should be followed by thorough evaluation and diagnosis. In this present case, severe occlusal wear was found in the labial surfaces of mandibular anterior teeth and palatal surfaces of maxillary anterior teeth. Occlusal vertical dimension was evaluated and decided to be regained due to insufficient amount of restorative space. After determining new occlusal vertical dimension, diagnostic wax-up is usually performed to continue prosthodontic treatment. In this case with collapsed occlusion and loss of occlusal vertical dimension, conventional diagnostic wax-up was replaced with various digital devices. Diagnostic wax-up was digitally performed in the software and provisional prosthesis was fabricated with digital tools. With verification with provisional prosthesis, the entire dentition was rehabilitated with newly set occlusal vertical dimension and proper anterior guidance.

  • Multiple tooth loss and excessive occlusal wear can result in damage to occlusal disharmony, functional disorders and esthetic problems, requiring comprehensive prosthetic treatments. Changing vertical dimension harmonized with surrounding muscle tissue is important. In this case, the patient with loss of vertical dimension caused by severe tooth loss and tooth wear was treated with the analysis of vertical dimension, such as diagnostic model, radiography and various clinical exams. the patient was satisfied with favorable functions and esthetics for 1 years of follow-up.

  • Excessive tooth wear can cause irreversible damage to the occlusal surface and can alter the anterior occlusal relationship by destroying the structure of the anterior teeth needed for esthetics and proper anterior guidance. The anterior deep bite is not a morbid occlusion by itself, but it may cause problems such as soft tissue trauma, opposing tooth eruption, tooth wear, and occlusal trauma if there are no stable occlusal contacts between the lower incisal edge against its upper lingual surface. The most important goal of treatment is to form stable occlusal contact in centric relation. In this case report, patients with decrease in vertical dimension and anterior deep bite due to maxillary posterior tooth loss and excessive tooth wear were treated full mouth rehabilitation with increased vertical dimension to regain the space for restoration and improve anterior occlusal relationship and esthetics. The functional and aesthetic problems of the patient could be solved by the equal intensity contact of all the teeth in centic relation (CR), anterior guidance in harmony with the functional movement, and restoration of the wear surface beyond the enamel range.

Evaluation of Dry Tribological Characteristics of Hybrid Metal Matrix Composites with Temperature Rising (온도 상승에 따른 혼합금속복합재료의 건식 마찰특성 평가)

  • $Al_2O_3$ fiber and SiC particle hybrid metal matrix composites (MMCs) were manufactured by squeeze casting method investigated for their tribological properties. The pin specimens had different ratios of fiber to particle content but their total weight fraction was constant at 20 wt. %. Tribological tests were performed with a pin-on-disk friction and wear tester. The investigation of the dry tribological characteristics of hybrid MMCs were carried out at room temperature and elevated temperature of$100^{\circ}C$ and$150^{\circ}C$. The morphologies of worn surfaces were examined by scanning electron microscope (SEM) to observe tribological characteristics and investigate wear behavior. The results revealed that the wear resistance improved with the content of SiCp increased of the planar random (PR) MMCs at room temperature. At the elevated temperature, it revealed that the wear resistance of normal (N) MMCs was superior to that of the PR-MMCs due to PR-fibers were easily pulled out holistically from the worn surface. Meanwhile, the coefficient of friction decreased with the temperature increasing.

Case report: Full mouth rehabilitation for patient with heavy occlusal force and excessive abrasion (강한 교합력과 과도한 마모를 보이는 환자의 전악수복 증례)

  • Many of the patients with extensive abrasion need comprehensive restorative treatment. The abrasion is usually caused by attrition, besides of it, there are many reasons for it. The plan of treatment should be started on assessment of the type of attrition and the etiologic analysis. Patient with well-developed masticatory muscle, alveolar process, and high occlusal force and also with little muscle length difference between the stable and the contracted state should be carefully assessed for the vertical dimensional loss and the restoration should be carefully designed. Decrease of tooth length can be compensated by the growth of the alveolar bone height; therefore, consistency of the occlusal vertical dimension is maintained. Accordingly, a careless increase of the vertical dimension can produce muscle fatigue, depressed tooth and pain, and fracture of the restoration. In this case, the patient with multiple tooth abrasion and clenching habit, the edentulous maxillary area is restored with amalgam inserted RPD, and the dentulous area of the maxilla and mandible are treated with fixed restoration accompanying with the increase of vertical dimension. Consequently, we are going to report about the satisfying result in both functional and esthetic aspects.
