A MODIFICATION OF THE MICHELET TECHNIQUE FOR SLIDING HORIZONTAL OSTEOTOMY;A CASE REPORT (MICHELET 테크닉의 변형을 이용한 이부성형술의 치험례)

  • 앞 턱 부위가 짧고 후방으로 위치해 있을 때 턱의 높이를 증가시키고 전방으로 돌출시키는 축조 이부성형술의 방법으로는, 매식체 삽입술, 골 절단술, 골 절제술 또는 이들을 동시에 시행하는 혼합술등이 행해져 왔다. 그러나 매식체 삽입술에서는 감염 또는 매식체의 유동성 등의 합병증의 발병이 빈발하고 매식체의 밑 부위에 골이 흡수된다든지 연조직의 변화를 예측하기 어렵다든지 하는 복잡한 문제점들이 종종 보고되고 있다. 따라서 매식체를 이용하지 않는 골 절단술이나 골 절제술만을 이용한 다양한 방법의 이부성형술이 많이 시행되고 있다. 하악골이 전후방 관계로 발육부전인 환자에게 양측 하악지 시상분할술을 시행하여 전방 이동시킨 후 앞 턱의 위치를 좀더 개선하기 위하여 $Michel{\'{e}}t$ 테크닉의 변형법을 이용한 이부성형술을 추가로 시행하였다. 이 방법의 이부성형술은 턱의 수직적, 수평적 길이를 증가시키기 위하여 하악결합면 부위에 장부와 장붓구멍 모양을 한 골 절단술을 시행하고 광범위한 연조직경을 보존하여 골편으로의 혈류 차단을 최소화하였으며 나사를 이용한 견고한 고정을 용이하게 하였다. 이 술식은 턱을 수직적으로 증가시킬 때 두 골편 사이의 공간에 골 이식을 시행할 필요가 없으며, 금속판을 이용하지 않아도 정확한 위치에 골편을 고정하기가 용이하고, 또한 고정용 나사가 향후 골형성 부위에 위치하게 된다는 장점을 경험하게 되어 이에 한 증례를 보고하는 바이다.

SKELETAL RELAPSE AFTER ORTHOGNATHIC SURGERY OF CLASS III SKELETAL OPEN-BITE (개구교합을 가진 3급 부정교합환자의 악교정수술후 재발에 관한 연구)

  • This study was intended to evaluate a post-operative relapse tendency in mandibular prognathism patients with open-bite. 18 patients with or without open-bite have undergone sagittal split ramus osteotomy and were investigated radiologically with cephalogram. The results were as following 1. The preoperative anterior facial height, mandibular plane angle and mandibular gonial angle were larger in open-bite patients than in nonopen-bite patients.(p>0.05) 2. There was significant correlation between surgical change of SNA, mandibular plane length and long-term relapse.(p<0.01). 3. There was relatively stable postoperative anterior facial height in open bite patients. 4. There was no significant correlation between the surgical change of mandibular plane angle, amount of mandibular setback and long-term relapse.(p>0.01) 5. This study showed that horizontal relapse was more significant than vertical relapse.

A STUDY ON THE VARIATIONS OF THE SOFT TISSUE PROFILE CONTOUR IN RELATION TO THE SKELETAL PATTERNS (악안면 유형에 따른 측모 연조직 양상에 대한 연구)

  • The purpose of this study was to investigate the variations of the soft tissue profile contour in relation to the skeletal patterns and to confirm the correlation of soft tissue angles to the sagittal or vertical skeletal elements. Lateral cephalograms of 79 cases which were older than 17 years in age, were traced and statistically analyzed. The obtained results were as follows : 1. Nasolabial, interlabial, lower lip, mentolabial, and symphyseal angles showed the significant differences between skeletal Class II and skeletal. Class III group, but nasofrontal, upper lip, mentolabial, and symphyseal angles showed the significant differences between high angle and low angle group. 2. ANB value showed the significant positive correlation to nasolabial, symphyseal, and interlabial angles and the significant negative relation to lower lip angle. 3. SN-GoMe value showed the significant positive correlation to mentolabial, symphyseal, nasofrontal, and upper lip angles. 4. Soft tissue profile contour in SK. Class II group showed greater accordance to SN-GoMe value than those in SK. Class III group.

  • Improvements in jaw relationship through clockwise rotation of the mandible may be desirable in some Class III patients with short low facial height. The aim of this study was to examine the treatment effect of face mask for Class III malocclusion patients according to their low facial morphology. Methods: Class III patients in their pubertal growth period were divided into two groups (Group 1, high LFH; Group 2, low LFH) according to lower facial height (LFH) by Ricketts (norm, 47). treatment changes between groups after face mask treatment was compared not only for hard tissue but also for soft tissue. Results: There were no significant differences between the two groups for the skeletal and soft tissues of the maxilla. There were no significant differences between the two groups for the skeletal posterior movement of the mandible, but posterior movement of the mandibular soft tissues in group 2 was larger than group 1. There were no significant differences between the two groups for the vertical hard tissue proportion changes of the mandible, but the vertical soft tissue proportion changes of the mandible in group 2 was larger than group 1. There was a significant correlation between the sagittal hard tissue and soft tissue changes of the maxilla and mandible, but there was no significant difference in the vertical changes. Conclusion: The clockwise rotation of the mandible occurred from use of the face mask, and posterior movement of soft tissues of the mandible was higher in Cl III patients with low LFH than with high LFH.

The Relationship between the Growth of Cranial Base and the Position of Maxilla, Mandible in Complete Unilateral Cleft Lip and Palate Patients (완전 편측성 순구개열자의 두개저 성장과 상,하악골 위치의 상관 관계에 관한 연구)

  • This study was designed to evaluate the correlations between the morphology and growth of cranial base and the position of nasomaxillary complex and mandible in complete unilateral cleft lip and palate patients. Craniofacial skeletal morphology Pattern was analyzed on the lateral cephalometric radiographs of the 100 subjects of complete unilateral cleft lip and palate group and the 100 normal grower group and each group was divided three sub-groups by age-range like spheno-ethmoidal synchodrosis growing group, spheno-occipital synchondrosis growing group and finished synchondrosis growth group. These data were statistically analyzed to examine significant difference between both groups and between each sub-groups. The results of this study were as follows: 1. In complete unilateral cleft group, the length, thickness of clivus, and the rate of increase in length of clivus showed smaller amount by adolescence. The anterior length of cranial base and the rotation pattern of clivus with age showed no significant difference between two group. 2. In complete unilateral cleft group, nasomaxillary complex were located more posteriorly. This difference between two groups is larger by adolescent group than adult. Vertical position showed no significant difference between two groups. There was significant correlation between the cranial base of cleft group and the horizontal measurements(p<0.01). PtmS showed no significant increment in cleft group. This showed the deficiency of growth in posterior part of maxilla. 3. In mandible, there was no significant difference between normal group and complete unilateral cleft group but articular angle showed significantly smaller in cleft group than in normal one. And the measurements of nasomaxillary complex position, cranial base and the position of mandible had significant correlation(p<0.01).

Cephalometric study on the profile differences in adult Class I malocclusion relative to overbite (피개교합(overbite)양상에 따른 성인 I급 부정교합자의 측모특성)

  • This study was investigated to evaluate the morphologic characteristics of openbite and deep bite in Class I malocclusion patients and to find skeletodental factors which contributed to vertical discrepancy in Class I malocclusion. The subjects were consisted of 40 control subjects (male 20, female 20) and 40 Class I openbite patients and 40 Class I deep bite patients. Lateral cephalograms in centric occlusion were taken, traced and digitized lot each subjects. The computerized statistical analysis were carried out with SPSS program. The results were as follows. 1. The pattern of vortical discrepancy in Class I malocclusion is mainly influenced by the skeletodental factors under palatal plane. 2. In openbite group, vortical discrepancy is prominent on anterior lower face and is closely related with skeletal factors such as mandibular form and inclination. 3. In deep bite group, dental factors such as ewe of Spee, vertical height of maxillary molar and skeletal factor such as articular angle were contributed to the vertical discrepancy. 4. The multiple regression analysis showed that overbite in Class I molar relationship was determined mainly by dental factors such as lower incisor to occlusal plane angle, curve of Spee, interincisal angle, and ODI.

Postoperative Stability and Occlusal Plane Alternation by Orthognathic Surgery of Skeletal Class III Malocclusion with Anterior Open Bite (전치부 개교를 동반한 골격성 III급 부정교합 환자의 악교정 수술 후 교합평면의 변화와 안정성에 관한 연구)

  • The purpose of this study is to investigate the stability of counterclockwise rotation of mandible by sagittal split ramus osteotomy to correct the skeletal Class III malocclusion with anterior open bite. Twenty five skeletal Class III open bite patients(mean age 20.6 years) who were treated by the sagittal split ramus osteotonues with rigid fixation were examined in this study. Cephalometric radiographs were taken for each Patients Preoperative(T1), ewly Postoperative(T2), and late postoperative Period(T3). Mean postoperative period was 8.0 months. Cephalometric analysis was done and data from T1, T2, and T3 were analyzed statistically by Paired t-test and Pearson correlation analysis. The following results were obtained. 1. Mandibular plane angle decreased $2.9^{\circ}$ and mandibular occlusal plane angle related to SN Plane decreased $2.7^{\circ}$ after orthognathic surgery(T2). At 6 months after orthognathic surgery(T3), mandibular plane angle increased $1.0^{\circ}$, but mandibular occlusal plane angle did not changed. 2. The amount of horizontal relapse long time after orthognathic surgery(T3) was 1.6 mm at B point and it was $22\%$ of the total posterior movements. There was no vertical relapse in the anterior facial height. 3. The related factor with horizontal relapse at late postoperative period was mandibular plane angle(p<0.01). The related factors with decreasing posterior facial height were amount of mandibular setback(p<0.01), increasing of mandibular ramus height(p<0.01), and decrease of the mandibular plane angle during operation(p<0.01). 4. There was no relationship between the amount of changes in mandibular occlusal plan angle during operation and the amount of relapse after surgery.

  • It has been known that head posture may influence directly and/or indirectly the growth and development of craniofacial morphology and can also be influenced by the funtional demand of physiologic activity. It was reported that facial morphology has close relationships with hyoid bone position and head posture. In many previous studies, Natural Head Posture(NHP) was guided, and also it was shown that NHP has high degree of reproducibility. Otherwise, There was few study about the relationship of head posture, with routine cephalometric film which is used for clinical orthodontic purpose. In this study, according to the Wits and ANB of initial cephalometric film which was taken with vertical pendulum as representative of true vertical reference line. We classified the subjects which is comprised of 60 adult female patients into Class I, II, III (Cl I, II, III)and we tried to find out the correlation of head posture and hyoid bone position according to the classification of malocclusion. As a result of our research, we found the followigs. 1. In comparison of vertical position of hyoid bone relative to the cranial base. the position of hyoid bone of Cl III was lower than that of Cl II. 2. In comparison of anteriorpostes or position of hyoid bone, relative to the cervical column. The position of hyoid bone of Cl III was more anterior than that of a II 3. in comparison of vertical position of hyoid bone relative to mandible. There was no significant correlation aumoug the groups of malocclusion. 4. ANB and Wits showed no significant correlation with hyoid bone position. 5. The relative extension of head, which was noted in Cl II, showed negative with Sum, ANB. 6. In Cl II and Cl III, Post to Ant facial height showed positive correlation with NSL/VER.

A study on treatment effects of different activator types in Angle's Class II div.1 malocclusion patients (2급 1류 부정교합 환자에서 activator유형에 따른 치료 효과에 관한 연구)

  • This research was carried out to compares the treatment effects of Horizontal and Vertical type activators in Angle's Class II div. 1 maloccusion patients with mandibular retrusion dand to find out whether different treatment effects or growth pattern were observed between sexes in each study groups. The results were as follows: 1. In Horizontal activator group, forward positioning of mandible and vertical increase in anteror face as examplified by increase of LAFH and AEM were observed when pre and post-treatment datas were evaluated. 2. Males samples in Horizontal activator group showed increase in mandiular length accmpanied by posterior positioning of maxilla, wheras female samples in Horizontal activator group showed increase in mandibular body length, labial inclination of mandibular incisors and increase in lower anterior facial height .3. In vertical activator group, increase in AFH, LAFH, PFH and LPFH were observed when pre and post treatment datas were evaluated. 4. Male samples in Vertical activator group showed increase in mandibular body length and anterior and posterior facial heights, whereas females samples of Vertical activator group showed mainly increase in anterior facial height. 5. When pre and post treatment datas of Horizontal and Vertical activator groups were compared, skeletal difference were mainly observed in pretreatment datas but dental difference were observed in post treatment datas ,indicating that two actiators differ only in their effects to dental variables. 6. Difference between sexes were noted after treatment although no difference were observed between sexs in each groups before treatmentt. This indicates that inherent growth effects in each sex exerts more influence 1km appliances used for treatment.

STUDY OF CRANIOCERVICAL POSTURE AND CRANIOFACIAL MORPHOLOGY IN KOREAN YOUNG ADULTS (한국 성인의 두경부자세와 두개안면형태에 관한 두부 X-선 계측학적 연구)

  • The purpose of this study was to estimate correlations of craniocervical posture and craniofacial morphology in Korean young adults. The sample consisted of 50 young adults (25 males and 25 females) who had good profile and Class I molar relationship. The analysis of craniocervical posture and craniofacial morphology was performed on lateral cephalograms taken in natural head position. The results were as follows, 1. The mean and the standard deviation of postural and morphologic variables were obtained. 2. Korean young adult had cervical lordosis of which degree between OPT and CVT shows $3.55{\pm}2.58^{\circ}$ 3. Craniocervical posture and each of vertical ratio, facial prognathism, mandibular rotation showed high correlation. 4. Correlation coefficients between postural variables and each of intermaxillary relation, anteroposterior ratio were low. 5. The head positioning error of natural head position was smaller than the inter-individual variability of postural variables.
