Effects of Pain Control Education on Pain Control Barrier, Postoperative Pain and Pain Control Satisfaction in Gynecological Patients (통증조절 개별교육이 부인과 수술환자의 통증조절 방해, 수술 후 통증 및 통증조절 만족도에 미치는 효과)

  • Purpose: The purpose of this study was to evaluate the effects of preoperative pain control education on the pain control barrier, postoperative pain and pain control satisfaction in gynecological patients. Method: The study was a quasi-experimental research design. There were 58 subjects who were admitted for gynecological surgery to D University Hospital in B city. Pain control education was provided individually to the experimental group one day before their operation day for 20 minutes with the 'Pain Control Guide Book' in the patient's admission room. The education book was made by researchers based on pain management references and patient interviews. For assessing the pain control barrier, a simplified version of Barriers Questionnaire was used, postoperative pain was assessed on a numeric scale(0-10) and satisfaction of pain control was assessed by one question. Results: The pain control barrier(F=15.828, p<.001) and the post pain score of the experimental group was lower than that of the control group. In addition, pain control satisfaction of the experimental group(t=3.612, p<.001) was higher than the control group's. Conclusion: With the above results, preoperative pain control education could be an effective nursing intervention for pain control of surgical patients.

  • Although variable surgical methods of sympathetic nerve for palmar hyperhidrosis are curative and safe therapeutic options, they have some limitations such as compensatory sweating and anhidrosis of hand in long term satisfaction rate. Material and Method: Therefore, we tried to decrease severity of compensatory sweating and prevent excessive dryness of hand through selective division of rami communicantes of thoracic sympathetic ganglia distributed to the hands(ramicotomy). Result: In postoperative results, about half of the patients maintained humidity of hands and most of them showed no more than mild degree of compensatory sweating. Conclusion: Therefore, ramicotomy of thoracic sympathetic ganglia can be recommended as selective and physiologic surgical method for palmar hyperhidrosis.

  • Empyema with bronchopleural fistula is a serious complication after resectional surgery of the lung. Several methods in treating empyema with bronchopleural fistula have been developed, but the results were not always satisfactory. Recently, successful results have been reported with the surgical treatments using tissue flap (the omentum and f or the chest wall muscles) to close the bronchopleural fistula in patients with thoracic empyema. We have performed surgical procedures to treat bronchopleural fistula using omentum in 1 patient an chest wall muscles in 3 patients. Their postoperative courses were uneventful.

  • Lumbar decompressive surgery is a standard surgery for lumbar spinal stenosis. Many surgical techniques have been introduced, ranging from open surgery to percutaneous procedures. Minimally invasive techniques are preferred because of the less postoperative pain and shorter hospital stay. Uniportal percutaneous endoscopic decompression has technical difficulties due to the narrow field. Biportal percutaneous endoscopic decompression is a satisfactory technique that can compensate for the shortcomings and provide sufficient decompression.

  • Purpose: We wanted to evaluate the results of surgical treatment for fracture of the scapular body. Materials and methods: The subjects of this study were seven patients with fracture of the scapular body and all these patients were managed surgically. We considered the operative indications as being 100% translation of the lateral border, or $25^{\circ}$ of angular deformity of the glenoid, or 1cm medialization or the variant of the double disruption of the superior scapular suspensory complex. There were five males and two females, and their ages ranged between 40 and 58 years (mean age: 49) with average follow up of 11 months (6~24 months). Two patients sustained multiple fractures of the ribs and spines. The surgical results were evaluated according to the subjective satisfaction, the UCLA score and the Korean shoulder score. Results: The mean UCLA score was 29 (17~33) and the mean Korean shoulder score was 86 (63~94). The self assessment for subjective satisfaction was 7.7 (4~9). There were two complications; one case of screw pull-out without the loss of the fracture fixation and the other case with screw penetration of the glenoid cavity. Conclusion: Surgical treatment may be effective for the management of severely displaced fracture of the scapular body in order to prevent the impairment of the shoulder function caused by the altered glenohumeral and scapulothoracic kinematics.

  • Total knee arthroplasty is performed widely in degenerative arthritis of the knee joint, and the frequency of use is increasing. Efforts should be made to achieve functional recovery, such as returning to daily life, and the recovery of strength and range of motion after surgery. The procedure should be approached from a range of perspectives, such as patient factors, surgical technique and rehabilitation. The patient's age, degree of obesity, sex, and strength of the quadriceps muscle can affect the functional recovery after surgery. In addition, the patient's mental state, such as expectation value and satisfaction, can also have an effect. For functional recovery, patient education, pain control, and strengthening of the quadriceps muscle can be performed prior to surgery. Postoperative physiotherapy, such as icing and compression, transcutaneous electrical nerve stimulation therapy, neuromuscular electrical stimulation therapy, low frequency low intensity magnetic field therapy, strengthening of quadriceps muscle, and range of motion exercise can also be applied. In recent years, hydrotherapy, which restores the strength and balance of the leg, is being performed increasingly. These treatments are not only performed shortly after surgery, but are also performed continuously. In addition, the surgeon should apply it appropriately considering the patient's condition, compliance, and social and psychological conditions.

A Study on the Medical Services Satisfaction of Cancer Patients in Busan Area (부산지역 암환자의 의료서비스 만족도에 관한 연구)

  • Hospital out-shopping by local patients has become more popular. In particular, many cancer patients have visited hospitals in the capital area to get better medical services regardless of how far the hospitals are from their home. However this kind of hospital out-shopping may discontinue medical treatment and cause a waste of medical resources including manpower. In addition, it creates additional economic and social expenses such as caretakers' transportation and lodging expenses. It is necessary to study patients who would take the medical treatment in Busan go to Seoul for better medical services are more satisfied based on objective data. For this, a questionnaire survey has been performed against 223 cancer patients who had a surgery in Busan and 187 patents who had an operation in Seoul. According to the survey, the patients who had an operation in the capital area(Group A) were more satisfied than the patients who had a surgery in Busan(Group B) in terms of convenience facilities, doctors' competence. However the Group A was very dissatisfied in terms of accessibility and satisfaction on overall medical services.

Development of Postoperative Self Care Mobile App for Kidney Transplantation Patients (신장이식 환자의 수술 후 자가간호 어플리케이션 개발)

  • The purpose of this research was to develop a self-care application for kidney transplantation patients based on a review of previous literatures and the results of a survey that evaluated the needs of patients. The research proceeded in ADDIE order of analysis, design, development, implementation, and evaluation stages. In the analysis phase, interviews were conducted on over 5 kidney transplantation patients. Moreover, related applications and literatures were reviewed to develop application contents. App-based postoperative self-care program composed of the following: Health teaching, measurement, checklist, views, alarms, and App information. For the evaluation phase, a survey was conducted on 9 experts and 5 patients, using a smartphone application. SPSS/WIN 21.0 program was used for data analysis. Descriptive statistics were used to analyze the validity and suitability of data obtained from experts and users. Content was validated using CVI. Expert assessment of application for the self-care after kidney transplant showed 3.5 out of 4. Patient assessment showed 3.7 out of 4. We determined that a self-care application for patients that underwent kidney transplant is helpful. Moreover, a future study is necessary to test and verify the effects of using this application on self-care and self-care knowledge.

The Short Term Clinical Results of Hemiarthroplasty to Treat Humeral Head Osteonecrosis (상완골 두 비 외상성 골괴사에 시행한 견관절 반 치환술의 단기 추시 결과)

  • Introduction: While uncommon, humeral head osteonecrosis is an indication for arthroplasty when the humeral head collapse is advanced. The current authors report the short-term clinical results of 7 hemiarthroplasties to treat humeral head osteonecrosis. Materials and Methods: This study focused on 7 reconstructed shoulders of 5 patients whose humeral head osteonecrosis was treated with hemiarthroplasty. The postulated causes were alcohol-induced (4 cases) and steroidinduced (3 cases). The minimum follow-up was 12 months. This study compared the preoperative and postoperative shoulder pain, range of motion, and ASES scores. The postoperative patient satisfaction was assessed. Results: The level of pain during exercise was reduced from a preoperative average of 7.6 to a postoperative average of 1.9. The range of motion, in terms of forward flexion, abduction, and external rotation, improved from preoperative averages of $105.7^{\circ}$, $80^{\circ}$, and $22.1^{\circ}$ to postoperative averages of $146.6^{\circ}$, $139.3^{\circ}$, and $44.3^{\circ}$, respectively. The ASES scores increased from a preoperative mean of 39.0 to a postoperative mean of 84.1. The patients' ratings of the outcomes were excellent (5 cases) and good (2 cases). Conclusions: These short-term results indicate that hemiarthroplasty is a reliable treatment method for humeral head osteonecrosis improving shoulder pain, range of motion, and patient satisfaction.

A Study on the Brassiere Wearing Evaluation for Augmentation of Mammaplasty Patients (시판 유방 확대 수술 환자용 브래지어의 착의 평가)

  • The frequency of breast augmentation surgery continues to increase annually; however, the method of follow-up care varies from hospital to hospital. In particular, many different types of post-operative bras are available in the market. This study evaluated the wearing comfort of various commercial bras that were worn immediately after breast enlargement surgery prior to the manufacture of the bra. According to interviews of medical professionals and market research, five types of brassiere were selected and evaluated by wearing satisfaction, functional performance, and an external appearance test for 6 subjects with breast augmentation surgery. The evaluation questionnaires were based on a 5 point Likert scale with data analyzed using SPSS 20.0. The study results revealed that the bra with the highest degree of satisfaction was CNB (without bra cup) type. However, the use of CNB type showed dissatisfaction in functional evaluation questions regarding breast shaking and material & tactile sensation. In the future, it is necessary to develop a new post-operative brassiere based on a CNB type bra that showed the best evaluation. However, it is also necessary to identify the merits of the other four experimental bras and reflect these advantages.
