Characteristics of Patients Who Need Hypnotics on the Night before Elective Surgery (수면전일 수면제를 필요로 하는 환자들의 특성)

  • Objects : This study was carried out to investigate characteristics of patients who need hypnotics on the night before elective surgery as well as contributing variables for the necessity of hypnotics. Methods : After reviewing the clinical charts of patients who were scheduled to receive surgery by general anesthesia the following day, researchers had semi structural interviews with patients. In addition, Spielberger's State-Trait Anxiety Inventory(SSTAI), Beck Depression Inventory(BDI), Zung's Self-Rating Pain and Distress Scale(ZPDS), and Presleep and Postsleep Questionnaires were administered to patients. A total of 167 patients, who gave reliable information, were divided into two groups based on subjective judgement regarding the necessity for hypnotics on the night before surgery; 29 eligibles for hypnotics and 138 non-eligibles for hypnotics. Demographic and clinical characteristics of patients, some possible factors affecting sleep, psychological characteristics of patients and daytime status and nighttime sleep before surgery were compared between the two groups. In addition, discriminant function analysis was done to find the variables which would best discriminate among patients who differ in terms of necessity for hypnotics on the night before surgery. Results : There was no difference in demographic and clinical characteristics between the two groups; however, the satisfaction level with ward environment was significantly lower in the eligible group for hypnotics than the non-eligible group. Psychologically, the eligible group for hypnotics, compared to the non-eligible group, showed significantly more severe depression, pain, and distress; whereas anxiety level was not different between the two groups. For nighttime sleep before surgery, the eligible group for hypnotics, compared to the non-eligible group, expected poorer sleep before retiring and in fact, reported poorer sleep the following morning. In discriminant function analysis, 'expectation for sleep' and 'pain and distress' were the most potent contributors to discriminate the necessity of hypnotics. Conclusion : For the improvement of the patient's sleep on the night before elective surgery, giving hypnotics and/or analgesics should be determined by patient's opinion about the necessity of the drugs rather than by the therapist's own judgement or any other objective indices.

  • 기온이 높고 습한 여름이면 불면증이나 수면장애를 호소하는 사람이 많다. 또한, 스트레스, 불안감 등으로 인한 심리적인 이유로 밤에 잠 못 이루는 사람들도 많다. 수면을 위해 수면제를 먹는 것은 일시적인 도움은 될 수 있으나 수면제 장가복용으로 이어질 수 있다. 음주는 오히려 수면을 방해할 뿐이고 샤워를 하는 것도 수면에 방해될 뿐이다. 그렇다면 수면을 돕는 색채와 명화의 도움을 받아보는 것은 어떨까.

Drug Treatment of Insomnia : Sedative-Hypnotics (불면증의 약물 치료:진정수면제)

  • A growing number of people are concerned about their sleep. There are many people with chronic sleep disorders. Sedativehypnotics including benzodiazepine and non-benzodiazepine have been widely used in chronic insomniacs. It is widely accepted that current hypnotics are efficient in alleviating subjective symptoms of insomnia. Non-benzodiazepine hypnotics include zolpidem, zopiclone, and melatonin. These novel non-benzodiazepine hypnotics that have efficacy comparable to benzodiazepines were developed with more understanding of benzodiazepine receptor pharmacology. Their unique pharmacologic profiles may offer few significant advantages in terms of adverse effects of benzodiazepines. However, most of hypnotics including non-benzodiazepine have some of dependence, tolerance, impaired daytime function and rebound insomnia. Currently, it is accepted that combination therapy with pharmacologic and behavioral intervention is the most effective for chronic insomniacs.

Sedative Hypnotics Induced Parasomnias (복진정제 및 수면제 유발 사건수면)

  • Parasomnias induced by hypnosedatives are rare but serious side effect. Such parasomnias have not been reported with all hypnosedatives. However, frequent use of hypnosedatives, particularly nonbenzodiazepine receptor agonists is associated with parasomnias. Associated symptoms are sleep eating, sleepwalking with object manipulation, sleep conversations, sleep driving, sleep sex and sleep shopping etc. Mechanisms include high affinity for $GABA_A$ receptor, interruption of the consolidation phase of memory formation by drug, pharmacokinetic or pharmacodynamic drug-drug interaction and concomitant administration with alcohol. Managements for parasomnias induced by hypnosedatives involve stopping medication, switch to other medications or nonpharmacological treatment, lowest effective dose of NBRAs (Non-Benzodiazepine Receptor Agonists), taking into consideration drug-drug interactions, identification and treatment of underlying disease states.

중추신경계 작용약물의 자발운동 양상에 미치는 영향

  • 동물의 일반활동은 중추신경계에 작용하는 약물에 관한 많은 정보를 제공한다. 따라서 약물의 중추신경계에 대한 독성평가는 물론이고 중추신경계 작용약물의 검색에도 널리 이용된다. 본 연구에서는 여러 가지 기전으로 중추신경계에 작용하는 약물들의 일반활동을 분석하여 중추신경계 작용 약물들의 작용기전에 따르는 일반활동의 특성을 비교 분석하고자 하였다. 실험장치는 UGO BASILE제 활동측정 장치를 이용하였으며, 실험동물은 서울대학교 의과대학 실험동물 사육장에서 사육한 ICR mouse를 사용하였다. 동물의 활동측정은 환경요인을 최소한으로 배제하기 위하여 매일 9:00-11:00 까지 2시간 동안의 행동을 10분 간격으로 측정 기록토록 하였다. 중추신경 흥분제 amphetamine, 진정수면제 phenobarbital, 항불안제 diazepam, 항정신병약 chlorpromazine, 항우울제 haloperidol는 모두 용량 의존적으로 동물의 자발운동에 영향을 미쳤다. 특기할 것은 항불안제 diazepam 은 용량의존적으로 운동 억제를 나타내었으나 진정수면제 phenobarbital의 경우는 용량 의존전 운동억제를 나타내기 이전에 초기약물 유발성 aggressive behavior를 나타내었다. 본 실험결과는 동물의 자발행동의 분석으로 중추신경계 작용약물의 작용기전 규명 및 이에 따르는 약물의 분류 방법으로 유용하게 이용될 수 있을 것으로 생각된다.

A Study on the Systematic Management of Zolpidem (졸피뎀의 체계적 관리 방안 연구)

  • The Zolpidem, commonly used as a sleeping pill, binds to GABA receptors to increase neuro-inhibiting transporters. There are many cases where people take Zolpidem for more than four weeks in a row. The Drug Utilization Review, currently in effect in South Korea, can identify the use patterns of Zolpidem. It is necessary to prevent Zolpidem's misuse. The study was conducted from May 10, 2019 to May 15, 2019. Primary and secondary screening was performed out of 125,197 total retrieved documents. The final 254 literature cases directly related to Zolpidem's misuse were selected. Through systematic literature research through meta-analysis, we tried to find a way to prevent Zolpidem's misuse through the Drug Utilization Review. The distortion was minimized by applying it in the screening process. Zolpidem's misuse was severe and continued to increase its use. There has also been an increase in crime using Zolpidem. It has been analyzed that Zolpidem can be managed through Narcotics Integrated Management System and Drug Utilization Review. In order to reduce the use of Zolpidem, the supply through the Narcotics Integrated Management System and the demand through long-term user education should be suppressed.

중추신경계 독성평가 방법개발 및 기작에 관한 연구

  • 중추신경계 작용약물은 동물행동에 대단히 민감하게 작용한다. 본 연구에서는 동물일반 활동에 대한 중추신경계 약물의 효과와 단독 사육동물의 공격성 행동에 대한 중추신경계 약물의 효과를 연구한 결과 생쥐의 일반활동 pentobarbital, chloral hydrate, paraldehyde 등 수면제 투여 후 마취에 이르기 전 초기활동이 대조군에 비하여 현저하게 증가하였으나 diazepam, clomazepam등 항불안제 투여 동물에서는 초기활동 증가현상을 나타내지 않았다. 또한 생쥐를 단독 사육후 침입자에 대한 행동은 항불안제 약물투여시 공격성 행동이 현저하게 감소하였다. 이상의 결과는 중추신경계 작용약물중수면, 마취 효과를 나타내는 약물과 항불안제의 효과를 감별하는 방법으로 대단히 유용할 것으로 기대된다.

Survey of Knowledge on Insomnia for Sleep Clinic Clients (수면클리닉을 방문한 환자들의 불면증에 대한 인식조사)

  • Objectives: Insomnia is not only the most common sleep-related disorder, but also is one of the most important. Knowledge of the comorbidities of insomnia is essential for proper treatment including pharmacological and non-pharmacological methods to prevent disease chronification. This study aimed to determine sleep clinic patients' knowledge of insomnia. Methods: This study recruited 44 patients (24 males and 20 females; mean age $54.11{\pm}16.30years$) from the sleep clinic at National Center for Mental Health. All subjects were asked to complete a self-report questionnaire about their reasons for visiting a sleep clinic and about their knowledge of treatment and comorbidities of insomnia. Results: The reasons for visiting the sleep clinic were insomnia symptoms of daytime sleepiness, irregular sleeping time, nightmares, snoring, and sleep apnea, in that order. Of the responders, 72.7% had a comorbidity of insomnia, and 22.7% showed high-risk alcohol use. In addition, 70.5% of responders chose pharmacological treatment of insomnia as the first option and reported collection of information about treatment of insomnia mainly from the internet and medical staff. More than half (52.3%) of the respondents reported that they had never heard about non-pharmacological treatments of insomnia such as cognitive behavioral treatment (CBT-I) or light therapy. The response rate about comorbidities of varied, with 75% of responders reporting knowledge of the relation between insomnia and depression, but only 38.6% stating awareness of the relation between insomnia and alcohol use disorder. Of the total responders, 68.2% were worried about hypnotics for insomnia treatment, and 70% were concerned about drug dependence. Conclusion: This study showed that patients at a sleep clinic had limited knowledge about insomnia. It is necessary to develop standardized insomnia treatment guidelines and educational handbooks for those suffering from insomnia. In addition, evaluation of alcohol use disorders is essential in the initial assessment of sleep disorders.
