A Case of Continuous Positive Airway Pressure Therapy in a Patient with Central Sleep Apnea and Heart Failure (중추성 수면 무호흡이 동반된 심부전 환자에서 지속적 상기도 양압술 적용 1례)

  • Central sleep apnea (CSA) is a highly prevalent comorbidity in patients with heart failure and may present in 25 to 40 percent of heart failure patients. Continuous positive airway pressure (CPAP) is the primary therapeutic option and effective in treatment of obstructive sleep apnea (OSA). In heart failure patients with CSA, several trials of CPAP showed a number of positive effects in heart failure treatment. A 58-year-old male visited the hospital because of dyspnea and he was diagnosed as heart failure with ischemic heart disease. He underwent coronary angiography and received percutaneous coronary intervention due to stenosis at the middle of left anterior descending coronary artery. However, dyspnea was not completely improved after treatment with percutaneous coronary intervention. The patient also experienced snoring and sleep apnea which worsened with symptom of dyspnea in the recent year. We suspected CSA and the patient underwent polysomnography to confirm whether sleep apnea was present. During the polysomnography, CSA with Cheyne-Stokes respiration (CSR) was observed and apnea-hypopnea index was 45.9/hr. The patient was treated with CPAP. After CPAP treatment, hypoxemia and CSA were resolved and dyspnea was improved with reducing NYHA class. We report a case successfully treated with clinical improvement by presuming CSA in a patient with heart failure.

  • This study was performed to evaluate sleep efficiencies and conditions for comfortable sleep based on the analysis of sleep efficiency and MST under four thermals conditions ($22^{\circ}C,\;24^{\circ}C,\;26^{\circ}C,\;30^{\circ}C$). Five female subjects who have similar life cycle and sleep patterns were participated for the sleep experiment. Their age was from 20 to 22 years old. They were healthy, and had regular sleep with consistent bed and wakeup time. It was checked whether they had a good sleep before the night of experiment. Experiments were performed in an environmental chamber using thermo-hygrostat. The physiological signal (EEG) for sleep stage were obtained from C3-A2 and C4-Al electrode sites. Sleep stages were classified, then SWS latency and SWS/TST were calculated for the evaluation for sleep efficiencies on thermal conditions. As results, mean skin temperature for comfort sleeping was $34.5{\sim}35.4^{\circ}C$. Considering sleep efficiency and mean skin temperature, indoor room temperature of upper limit was $28.1^{\circ}C$.

Development of Screening Test for Prediction of Sleep Apnea Syndrome (수면무호흡증 예측을 위한 선별검사 개발)

  • Objective : Patients with sleep apnea should be diagnosed with polysomnography(PSG). However, it is not easy to recommend PSG for all patients suspected with sleep apnea in practice. Therefore, we tried to develop the screening test for referral of PSG. Method : 140 patients with snoring and sleep apnea syndrome were studied by the PSG. Sleep apnea questionnaire. Zung's scale for depression. Stanford Sleepiness Scale(SSS), insomnia scale and neuropsychological test were administered. Also, blood pressure, height, weight and neck circumference were measured and some histories were taken. Correlations between respiratory disturbance index(RDI) and various parameters mentioned above and discriminant coefficients of the parameters to RDI were computed. And, we investigated sensitivities of screening tests for selection of the patients with RDI above 20. Results : Using six parameters(neck circumference, systolic blood pressure before sleep, degree of alcohol drinking, frequency of breath-holding during sleep, degree of dry mouth during sleep, sleep apnea score), the patients with RDI above 20 could be discriminated in 92.8% sensitivity. In case of more than two among six parameters(neck circumference of above 40cm, systolic blood pressure of above 125mmHg, frequent alcohol drinking, frequent breath-holding during sleep, frequent dry mouth during sleep, sleep apnea score of above 35), same patients could be discriminated in 87.6% sensitivity. And, in case of more than one among four parameters(neck circumference of above 40cm. systolic blood pressure of above 125mmHg, frequent alcohol drinking, body weight of above 80kg), discrimination sensitivity was 83.5%. Conclusions : Patients with RDI above 20 could be discriminated by above parameters with high sensitivity. Therefore, the screening test using above parameters can be applied in selection of the patients with sleep apnea for PSG in practice.

Effects of Sleep on Balance Control and Reaction Time to Visual Stimuli (수면이 균형조절과 시각적 자극 반응시간에 미치는 영향)

  • Objectives: To find evidence that sleep is necessary for normal brain function, thus indicating that declines in both sleep quality and quantity are related to worse performance of many daily tasks and deteriorated physical functions. The present study investigates the relationships of balance control and reaction time with sleep quality. Methods: 58 healthy (male 20, female 38) volunteers with informed consent participated in this study. The Self-reported Pittsburgh Sleep Quality Index (PSQI) was used to evaluate sleep quality and relevant factors, and the subjects were divided into groups A (PSQI < 5) and B ($PSQI{\geq}5$) based on this index. Static balance control and reaction time to visual stimuli were conducted to assess their relationship with sleep quality. Results: Group B exhibited excessive daytime sleepiness significantly more often compared to group A. Static balance control did not markedly change relative to sleep quality, but reaction time and error to visual stimuli were significantly increased in group B compared to group A. Conclusion: These findings indicate that a decline in sleep quality can result in delayed reactions, as well as decreased accuracy of these reactions. They also suggest that low sleep quality may be associated with changes in physical functions, including balance control through reduced selective attention.

Relationship between Depressive Symptoms and Sleep Parameters in Patients with Obstructive Sleep Apnea Syndrome (폐쇄성 수면무호흡증 환자의 우울증상 정도에 따른 수면 양상)

  • Objectives: Obstructive sleep apnea syndrome (OSAS) is the most common form of sleep-disordered breathing and often presents with comorbid depressive symptoms. In this study, we evaluated the relationship between depressive symptoms and sleep parameters as measured by nocturnal polysomnography (NPSG) and simultaneous wrist actigraphy. Methods: Two hundred sixty-four subjects with clinically suspected cases of OSAS underwent one-night polysomnography, while simultaneously wearing a wrist actigraphy device. They also completed two questionnaires;the Epworth Sleepiness Scale-Korean version (ESS-K) and the Beck Depression Inventory (BDI). Of the cases studied, 105 subjects were proven by NSPG to have OSAS without other sleep disorders. NPSG and wrist actigraphy data from the subjects were analyzed. Pearson correlation and paired t-test were used in order to evaluate the relationship between depressive symptoms and sleep-parameters. Results: Mean age of the subjects was $46.1{\pm}13.1$ years. Means of the ESS-K score and BDI scores were $10.9{\pm}4.7$ and $12.8{\pm}8.1$, respectively. NPSG sleep parameters significantly differed from those of wrist actigraphy. There was no correlation found between subjects' respiratory disturbance index (RDI) and BDI scores. When directly comparing sleep parameters between subjects who were more depressed versus subjects who were less depressed, both total sleep time and sleep efficiency were decreased in the more depressed. A correlation between RDI and ESS-K scores was also found in the more depressed group. Conclusions: Although our findings suggest that there is no relationship between RDI and depressive symptoms, there are other significant differences in the sleep parameters between subjects who are more depressed versus those without depression. We recommend that patients with depression should also be evaluated for clinical symptoms of OSAS.

Effects of Minor Stressful Events on Sleep in College Students (대학생에서 스트레스가 수면에 미치는 영향)

  • Objective: Stress is known to be a common cause of short-term insomnia and insomniacs often complain that stress induces sleep problems. However, previous studies on the correlation between stress and sleep do not show consistent results. We aimed to investigate the effects of minor stressful events on sleep among college students. Method: Physically and mentally healthy college student volunteers filled out a self-assessment questionnaire to evaluate their stress and sleep. To find out the status of average stress and sleep, the volunteers filled out K-DSI and daily sleep assessments on three consecutive days. In addition, we surveyed the amount of caffeine beverage intake and assessed the degree of depression and anxiety. Results: The total number of students participating in this study was 202, 101 men and 101 women. Minor stress turned out to significantly affect non-restorative sleep and secondary symptoms of insomnia (awakening difficulty, displeasure, feeling of dissatisfaction with sleep, physical uneasiness or pain at awakening, daytime sleepiness, depressive moods, tiredness and concentration difficulty). However, global PSQI score, self-reported sleeping hours, sleep latency, awakening frequency, frequency and duration of napping, were not explained by stress scores. Conclusion: In this study, minor stresses seemed to affect sleep, especially secondary symptoms caused by non-restorative sleep. We can thus infer that minor stresses impair the restorative effects of sleep by inducing arousal, and the direct relationship the two can be confirmed by polysomnogram.

  • 2개의 simulated된 잠수 실험 즉, 심도 19 ATA와 24 ATA의 고압 Heliox 환경하에서, 4명의 다이 버를 대상으로 하여연 188일간 매일 23:00부터 익일7:00시 사이에 각 다이버에대해서 표준적인 polysomnography를 기록했다. 2개의 잠수조건 하에서 양 실험 모두 해저에서 체재하는 동안 의 밤과 감압하는 동안의 밤 동안에는 총수면시간 (Total Sleep Time)의 단축, 수면효율(Sleep Efficiency)의 감소, 입면잠시(Sleep Latency)의 연장, 중도각성회수 (Number of Awakeningss) 와 감소를 보였지만, 어느쪽도 뚜려한 변화가 있었다라고는 할 수 없었다. 24 ATA 잠수조건에서 의 해저에서 체재하는 동안의 밤과 감압하는 동안의 밤 동안에서는 19 ATA 잠수조건에 비해서 4 단계수면(stage 4 sleep )만 유의한 감소를 보였다. 2개의잠수조건 모두 잠수기간 중에 있어서 는 각성하기 쉬운 상황이었으며, 기본적인 수면패턴에 있어서는 큰 변화는 보이지 않았다.

  • This study analysis that how related to media use time before shuteye time. Excessive SNS and smatphone use were related to sleepquality and pattern. A positive correlation has been found between the time spent using smartphone before bedtime and sleep deprivation and disability phenomenon. The time spent is found to also affect the language of youths and behavioral violence.

  • 최근 하천에서 발생하는 침수피해는 대규모 하천보다는 중소규모의 하천이 전체의 약 50%이상 발생하고 있으며, 기후변화에 따른 강우량의 증가와 도시화에 따른 불투수층의증가로 인하여 도시유역을 통과하는 중소규모 하천에서의 침수피해는 증가하고 있다. 특히, 도시를 통과하는 중소하천의 경우 강우발생시 짧은 도달시간으로 갑작스런 침수피해가 발생하며, 최근 급격한 인구의 증가, 산업화 및 도시화로 인한 불투수면적의 증가로 도시유역의 유출특성이 변화되고 있다. 기존의 선행된 하천 연구는 주로 유역단위의 연구가 진행되었지만 하천이 통과하는 도시유역의 경우 대부분 합류지점에 도시지역이 밀집되어 있어 하천유역을 기반으로 하는 도시지역의 구분은 어려움이 있다. 따라서 본 연구에서는 기존 하천분류인 국가하천, 지방하천, 소하천에서 침수피해가 발생하는 지방하천과 소하천사이의 중소하천에 대한 구분 기준을 제시하고 중소하천의 유역의 불투수면적뿐만 아니라 하천 선을 중심으로 하천폭에 대한 불투수면적에 대하여 비교 분석하고자 한다.

  • 현대 의료 진단 분야 중 하나인 수면다원 검사에서 수면 단계 분류는 평가에 많은 시간이 소요되고 평가자 간 일관성 문제가 대두되고 있다. 이러한 평가 문제를 해결하기 위하여 최근 급격하게 발전하고 있는 딥러닝 기술을 이용하여 자동화하려는 연구가 활발히 진행되고 있다. 본 논문에서는 오토 인코더 (autoencoder)와 대조 학습 (contrastive learning)을 통해 수면 시 측정된 생체 신호에서 보다 중요한 특징을 추출하는 방법을 제안하고 제안된 방법의 딥러닝 모델을 구성 및 평가한다.
