  • Recurrent motor branch entrapment syndrome is a compressive mononeuropathy of recurrent motor branch of median nerve. It is a rare condition as a cause of thenar muscle wasting and may have different pathogenesis. If such an anatomical variation is the cause, there is a possibility that thenar muscle atrophy remains if only the transcarpal ligament release is performed. We report a 25-year-old male patient with carpal tunnel syndrome with thenar muscle wasting 1 month ago.

Effect of Virtual Reality Training Focus on ADL on Upper Extremity Function and Activities of Daily Living in Stroke Patients (일상생활에 중점을 둔 가상현실 훈련이 뇌졸중 환자의 상지기능과 일상생활활동에 미치는 효과)

  • This study aimed to investigate the effect of virtual reality training focus on ADL on upper extremity function and activities of daily living in stroke patients. 20 patients with chronic stroke were included in this study. The participants were divided into the experimental (n=10) or control (n=10) groups. Both groups received conventional rehabilitation therapy for 30 min a day, 5 times per week for 4 weeks. The experimental group also performed virtual reality training focus on ADL for 30 min a day, and the control group performed general virtual reality training for 30 min a day. The Fugl-Meyer Assessment (FMA) and Functional Independence Measure (FIM) scores were evaluated before and after the intervention, and both groups showed significant improvement (p<.05). Compared to the control group, the experimental group showed significant improvement in the FMA wrist score and the FIM total, self-care, and sphincter control scores (p<.05). These findings suggest that virtual reality training focus on ADL may have a positive effect on wrist function and overall ADLs compared to that using general virtual reality training in stroke patients.

Clinical Effectiveness of Upper Extremity Performances on Mirror Therapy for Adult with Post Stroke Hemiplegia: A Systematic Review (뇌졸중으로 인한 편마비 환자의 상지기능 향상을 위한 거울치료의 임상적 효과에 대한 체계적 고찰)

  • Objective: To illustrate effects and application potential of Mirror Therapy (MT) for patients with post-stroke hemiplegia. Method: With reference to 9 journals (published Jan.2005-Jan.2016) on Pubmed, selected based on in/exclusion standards. Result: Simple wrist/hand movements and task-based MT were used as intervention methods to examine the effects. Tools used to assess intervention effects included upper limb functioning, Activities of Daily Living (ADL), physical condition and quality of life. Upper limb functioning turned out to have significance for ADL with higher effectiveness at the distal than the proximal region. Yet the quality of life disparity between the experiment group and the control was not found to be significant. Conclusion: We believe that research can aid clinical therapists in applying MT accordingly to individual patient characteristics. Despite prolonged difficulty in confirming efficient application due to varied protocols, development of systemized treatment protocols for maximization of MT's effectiveness remains necessary.

The EFFECT of HAND-SPLINT for 2 CASES (두 사례에 의한 수부 스프린트 효과)

  • 본 연구의 목적은 손 기능에 문제가 있는 환자들을 중심으로 splint의 효과를 알아보기 위한 기초조사로써 2명의 환자를 제주 한마음병원(Case1, 2)에서 2001년 6월부터 11월까지 조사하였다. 2명의 환자는 다음과 같다. i) 사례 1 : 뇌성마비 아동(4세, 남, 오른손)으로 한쪽 손목과 엄지손가락에 강직으로 인하여 잡기 기능에 제한이 있는 편마비 아동이다. ii) 사례 2 : 전기화상(56세, 남, 왼손)으로 4-5번째 손가락의 M.P joint와 I.P joint에 관절운동 제한으로 완전하게 주먹을 질 수가 없다. 연구를 위하여 손 기능의 평가는 표준화가 되어있는 Jebsen-hand function test, Total passive Motion(TPM)을 사용하였으며, 환자들의 만족도를 알아보기 위해서는 Canadian Occupational Performance Measure(COPM)을 사용하였다. i) 사례 1 : 두 가지 splint를 적용하였다. 즉 낮 동안에는 splint 1, 밤 동안에는 splint 2를 6시간 이상 착용하도록 하였으며 작업치료는 시행하지 않았다. ii) 사례 2 : 이 환자에게는 작업치료가 끝나는 동시에 splint. 3을 6시간 이상 착용하도록 하였으며, M.P joint와 I.P joint의 관절 변화를 조절하도록 특별히 고안된 splint를 적용하였다. 그 결과 사례 1의 Jebsen-hand function test시 초기에는 측정을 할 수가 없었으나, splint착용 후 크고 가벼운 물건 옮기기(44.15), 크고 무거운 물건 옮기기(42.66), 적목쌓기(44.63), 먹기 흉내내기(54.47) 등에서 처음 동작만 도와주면 측정이 가능할 수 있도록 진전을 보였다(그래프1), 사진(착용 전과 착용 후). 사례 2의 경우 Jebsen-hand function test의 적목쌓기와 먹기 흉내기가 가장 높았으며, 글씨 쓰기가 가능했다. 손의 관절가동 변화에서도 splint 착용전의 측정치와 착용 후 관절 가동 범위의 변화 폭이 약 $30_{\circ}$이를 보여주었다(그래프 2), (그래프3). 결론적으로 손 기능 회복에 직접적인 재활 치료뿐만 아니라, 치료 후에도 splint를 착용하여 가정에서도 계속적인 치료가 될 수 있도록 하는 것이 손의 기능회복에 도움이 됨을 알 수 있었다. 그러므로 각 환자의 문제점을 파악하여 적절한 splint를 제작해 줄 필요가 있다고 사료된다. 이 기초조사를 시작으로 하여 더 많은 환자들을 대상으로 splint가 손 기능 회복에 어떠한 영향을 미치는지 그 효과에 대하여 계속적인 연구가 필요할 것이다.

Systematic Review on the Type and Method of Convergence Study of Inertial Measurement Unit (관성 측정 장치의 융합연구 형태와 방법에 관한 체계적 고찰)

  • The purpose of this study is to identify trends in the type and method of Inertial Measurement Unit (IMU) by investigating studies on the type and method of convergence study of the IMU by systematic review. The study was conducted using PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines. 23 studies that meet the selection criteria were selected from 630 studies identified by three databases. As a result of this study, showed that various research using IMU was being conducted around the world, and the type of IMU was strap, full body suit, belt, wrist watch, shoes and glove. Among them, the number of strap-type IMUs was the largest at 11. The IMU's strengths were simplicity, real-time data collection and ease of application, which were used as measurement methods such as task, walking, and range of joint. The result of this study is expected to be used as basic data for experts in the medical and rehabilitation fields that conduct IMU research.

  • The utilization of virtual reality is increasing not only in games but also in medical care such as rehabilitation. Due to the convenience, the motion of the upper limb is detected using a non-contact method using video or a handheld type mouse, etc. In this paper, we implemented a glove which can measure finger movements and upper limb movements by using flex sensors whose resistance value changes according to the degree of folding and inertial sensors which can obtain direction information in space. We showed the upper arm movements including finger movements with signals obtained from the implemented glove on the open software platform, Processing. The sensitivity of each finger movement was 0.5deg, and the sensitivity of the upper limb motion was 0.6deg.

  • This study is to compare and analyze the evaluation of cleaning cleanliness and usability according to the muscle activity and cleaning method in the wrist rotation motion when using a bottle washer for women with childbirth experience. To this end, when using an automatic bottle washer and a manual bottle washer, the muscle activity and cleaning cleanliness of the forearm muscles were quantitatively evaluated using equipment, and the effects of variables were analyzed and the following conclusions were obtained. First, it was found that the muscle activity was lower than that of the automatic baby bottle cleaner when using a manual bottle cleaner. Second, the cleanliness was judged to be the best in automatic washing (forward + reverse direction), followed by automatic washing (forward direction) and manual washing. Third, the usability evaluation of the use of the baby bottle washing machine showed higher satisfaction than that of the automatic baby bottle washing machine. Through these conclusions, this study can be used as basic data for product advancement in the future by proving the effectiveness of using an automatic bottle washer.

  • In this paper, we present the score classification accuracy of BBS(Berg Balance Scale) which is the most commonly used balance evaluation tool using machine learning. Data acquisition was performed using the Noraxon system and an inertial sensor of Noraxon system was attached to the body in 8 locations (left and right ankle, left and right upper buttocks, left and right wrists, back, forehead). Based on the 3-axis accelerometer of the inertial sensor, the feature vector STFT(Short Time Fourier Transform) and SAM(Signal Area Magnitude) were extracted. Then, the items of the BBS were divided into static movement and dynamic movement depending on the operation characteristics, and the feature vectors were selected according to the sensor attachment positions which affect the score for each item of the BBS. Feature vectors selected for each item of BBS were classified using GMM(Gaussian Mixture Model). As a result of the accuracy calculation for 40 subjects, 55.5%, 72.2%, 87.5%, 50%, 35.1%, 62.5%, 43.3%, 58.6%, 60.7%, 33.3%, 44.8%, 89.2%, 51.8%, 85.1%, respectively.

A Needs Assessment of People with Hearing Impairment for Hearing Augmentation Technology Development: Focusing on Risk Context Awareness Communication (청각증강 기술 개발을 위한 청각장애인의 욕구조사: 위험상황 인식 및 의사소통 분야를 중심으로)

  • The purpose of this study is to find the application point of hearing augmentation technology development through examining the risk context experience of people with hearing impairment and the use of assistive device used as an alternative technology. Data of 355 people with hearing impairment with official disability grading was analyzed. The results of this study are first, research participants had no experience of recognizing any sound or vibration in situations highest in the order of means of transportation, material, and nature. Especially the ratio of being unable to recognize the sound and vibration of means of transportation was high, which implies the high possibility of people with hearing impairment experiencing risk. Secondly, the risk context that people with hearing impairment will most likely to experience are highest in the order of traffic accident, pedestrian accident, and daily life at home. Thirdly, the recognition of 2G phone/smart phone, vibrating digital alarm clock, light bar, vibrating wrist watch as assistive device for risk context awareness and notification was high and the satisfaction level of 2G phone/smart phone was the highest. Fourthly, the research participants had high recognition of assistive device for communication in the order of hearing aid, smart phone, videophone, cochlear implant and 2G phone and it was found that the satisfaction level and communication improvement level was the highest using the smart phone. Lastly, for the development of hearing augmentation technology the research participants recognized the importance of portable/wear convenience, price, and motion accuracy and for notification delivery means they preferred the method of using sight(text and light). Based on the results of this study policy and practical plans for hearing augmentation technology development for people with hearing impairment in risk context are proposed.

An Analysis of Swimming Injuries and Their Rehabilitation (근육 골격계의 질환 및 재활분석(수영선수를 중심으로))

  • While swimming is a very popular competitive sports activity, swimming injuries are unique due to the repetitive nature of the swimming stroke and demanding training programs that can result in upper limb overuse. Therefore, the primary objective of this review was to analyze swimmers' injury areas, injury types by stroke type, and swimming rehabilitation, as well as to discuss safety management for improving swimming performance. In this study, the injuries incurred in swimming events were discussed in the order of upper limb injuries (neck, shoulder, arm, and wrist), lower limb injuries (knee and ankle), and waist injuries. An analysis by stroke type found that shoulder injuries occurred most often with freestyle, backstroke, and butterfly strokes, followed by rotator cuff injury, impingement syndrome, and SLAP (superior labral tear from anterior to posterior) lesions. Knee injuries were associated with the breaststroke, whereas spinal cord injuries occurred with the breaststroke and butterfly stroke. Finally, back injuries were associated with the butterfly stroke. During the freestyle stroke, the shoulder undergoes repetitive overhead movement; hence, shoulder and musculoskeletal pain are the most common and well-documented complaints of swimmers. For safety management, coaches and instructors must ensure that athletes do sufficient warm-up and cool-down exercises to avoid injuries. In case of an injury, they should be familiar with first aid measures so that secondary damage can be prevented with its quick application. In addition, coaches and instructors need to be trained in injury prevention and treatment so that they can provide appropriate rehabilitation treatment for athletes. Although swimming-related injuries cannot be completely eliminated, to reduce them to a minimum, leaders need the knowledge to apply scientific and systematic training principles and methods individualized for each athlete.
