A Case Report of Digestive Symptoms in Gastric Ulcer Treated with Insamyangwi-tang (인삼양위탕을 이용한 소화성궤양 환자에 나타난 소화기 증상 치험 1례)

  • Objectives: This study reports on the efficacy of using Korean medical treatment for suspected gastric ulcer. Methods: We treated a patient with suspected gastric ulcer using Korean medical treatments (herbal medicines, acupuncture, and herbal acupuncture therapy). We evaluated the results using a numerical rating scale (NRS) to measure abdominal pain. The degree of gastrointestinal symptoms was also evaluated using the Korean Gastrointestinal Symptom Rating Scale (KGSRS). The generic health status was evaluated using the European Quality of Life-5 Dimensions (EQ-5D) scale. Results: The NRS and EQ-5D scale scores were improved after the treatments. The change in the KGSRS scores showed that the patient's abdominal pain was relieved. Conclusions: The results of this case study suggest that Korean medicine therapies may be effective in the treatment of suspected gastric ulcer.

  • The effectiveness, the safety and the usefulness of HPS-A(An Jung San with Chyag Yag Kam Cho Tang), herbal digestive, was examined on 32 patients complained digestive symptoms. This medicine was administered 1 package per 1 time 3 times per 1 day, during 2 weeks. From the examination of the improvemnt and the safety mainly by subjective symptoms, the usefulness was gained. The results were as follows ; 1. In the improvement rate of susbjective symptoms, high rate symptoms was gastric distension, anorexia, heart burn, gastric discomport, dyspepsia, acrid eructation, indigestion, nausea. 2. The improvment over moderate effective was 85.7% after 1 week 96.4% after 2 weeks. The 4 cases breaked medicine during experimental period were all over moderate offectiveness. 3. There weren't any cases complaind particular problem about safety except for 1 case complained mild pushing up of heat. There weren't also any side effect of medicine on common blood count, blood chemistry and urinalysis. 4. In the dicision of usefulness, 19cases(59.4%) were usefulnesss, 12cases(37.5%) were moderate usefulness, 1 case was uselessness(3.1%). So the cases over moderate usefulness were 31 and rate was 96.9%. 5. The usefulness rates according to physical features were all over 95.5% in sturdiness, middle feature and infirmity. So the rates were no relations with the features. 6. The usefulness rates over moderate usefulness according to total seriousness were all over 90% in high, middle, and mild cases. So the rates were no relations with the seriousness. Totally HPS-A was a medicine that was highly effective, safe and useful to patients complained various digestive symptomes.

Five Cases of Diphyllobothrium latum Infection (광절열두조충감염 5예)

  • Five cases of Diphyllobothrium latum infection were proved by collection of worms after treatment with bithionol, niclosamide or praziquantel, during 1975~1983. The patients were 4 males and one female aged 10~64 years residing in Seoul or Wando-gun, Chollanam-do, Korea. All of them remembered the history of eating several kinds of raw marine fish and 2 persons said they ate the raw mesh of perch, Lateolabrax japcnicus. Three amcnE them experienced abdominal pain, 2 natural discharge of a chain of segments, but none revealed any sign of anemia. The average egg size in stool varied $59~67{\times}41~48{\mu\textrm{m}}$ according to cases. The eggs were characterised by ovoid to elliptical shape, terminal operculum, and distinct abopercular protuberance. One naturally discharged segment (30cm) and 4 complete strobilae (320~680cm) with scolices obtained after treatments were examined. The morphological characters of proglottids such as rosette-form uterus with 3~6 loops, vaginal pore included in the cirrus sac, separated seminal vesicle from cirrus sac, etc. were all compatible with D. latum. These are the 7th~11th cases of D. latum infection proved by worms in Korea.

Pyloric Stenosis in Premature Infants -Report of two cases- (미숙아에서 발생한 유문협착증 2례)

  • The incidence of Hypertrophic pyloric stenosis (HPS) in premature infants is rare, the presentation is not typical, and the diagnosis delayed due to uncertain diagnostic criteria in abdominal ultrasonography (US). We report two premature infants with HPS diagnosed by US and upper gastrointestinal (UGI) contrast study. Patient 1. A premature female infant (birth weight 1950 gm at 34 week's gestation) with the onset of intermittent vomiting at 9 days of age was evaluated. US was normal at 13 days of life, however, abnormal at 41 days of life (pyloric muscle length 16.5 mm). Patient 2. A premature male infant (birth weight 1470 gm at 29 week's gestation) with the onset of intermittent vomiting at 10 days of age was evaluated. US showed pylorospasm at 11 days of life, however, findings compatible with HPS at 57 days of life (pyloric muscle thickness 11 mm). UGI contrast study at 48 days of life showed similar findings in both cases. Both patients had undergone pyloromyotomy. In conclusion, the diagnosis of HPS in premature infants requires careful follow-up by US and UGI contrast study.

  • Sigmoid volvulus may cause acute or subacute colonic obstruction. Excessive length of the sigmoid colon may be a contributing factor. Typically, the patient develops bilious vomiting and marked gaseous abdominal distension. We report a case of sigmoid volvulus in a 9-year-old boy who presented with recurrent, sudden onset abdominal pain, abdominal distension, and vomiting for 1 year, which was diagnosed by simple abdominal X-ray, barium enema, computed tomography, and colonoscopic examination. Colonoscopic reduction failed and a sigmoid colectomy with primary repair was performed. The intra-operative findings showed that the sigmoid colon was noted to be dilated, and redundant with a lax mesentery. Two clear areas of compression (proximal and distal) were present. After sigmoidectomy, the symptoms resolved. After 5 years of follow-up, he had no new symptoms.

Cytomegalovirus-induced Childhood Menetrier's Disease with Peripheral Eosinophilia (호산구 증가증을 동반한 거대세포바이러스 감염 소아 Menetrier병 1례)

  • Herein, the case of a 5-year-old boy with cytomegalovirus-induced Menetrier's disease, with peripheral eosinophilia, presenting with abdominal pain and vomiting, followed by generalized edema, is reported. The initial laboratory findings, hypoalbuminemia and peripheral eosinophilia were noted, with no evidence of renal, hepatic, cardiac or allergic diseases. Gastrofiberscopy was performed under the suspicion of eosinophilic gastroenteritis with protein losing gastropathy. The gastrofiberscopy showed the characteristic features of giant hypertrophy of the gastric rugae, with large quantities of adherent gelatinous material on the gastric fundus and body. The histological findings revealed foveolar hyperplasia, compatible with Menetrier's disease, with massive eosinophilic infiltrations. The presence of cytomegalovirus infection was identified by serology and confirmed by urine PCR. His symptoms, gastrofiberscopic findings and peripheral eosinophilia resolved spontaneously, and he has remained well for 10 months.

  • Cowden syndrome is very rare clinical condition with characteristic mucocutaneous lesions associated with abnormalities of the breast, thyroid, and gastrointestinal tract. Here, the case of a 16-year-old girl with diverse gastrointestinal polyposis with breast and thyroid lesions is reported as a definite case of Cowden syndrome. During follow up for 4 years, changes in the characteristic lesions were observed; Esophageal acanthosis and oropharyngeal polyps were newly developed. Gastric, duodenal, rectal and ileal polyps were noted at the first visit when she was 12 years of age and revealed histologically hyperplastic polyps. Mucocutaneous lesions, the pathognomonic finding of Cowden syndrome, were not noted at the first visit or during the follow up period. Breast and thyroid masses were noted at the first visit. Breast aspiration showed highly cellular ductal epithelial fragments and fibroblastic stromal cell fragments, and a thyroid biopsy shows proliferation of irregular sized follicles, with variable colloid contents. Macrocephaly was also noted at the first visit. The characteristic lesions of Cowden syndrome change with the patient's age, and the long-term follow up is recommended in cases suspected with this syndrome.

A Case of Tuberculous Enteritis with Active Pulmonary Tuberculosis in a 12-Year-Old Girl (12세 여아에게서 활동성 폐결핵에 동반된 장결핵 1례)

  • Intestinal tuberculosis (TB) is presented with nonspecific and variable clinical manifestations such as abdominal pain, diarrhea, fever and weight loss. Diagnosis of tuberculous enteritis may be missed or confused with many other chronic gastrointestinal disorders such as the Crohn disease and intestinal neoplasms. The diagnosis should be based on careful clinical evaluations, such as extra-intestinal signs and colonoscopic and histologic findings. Newer techniques such as PCR tests from the specimens through colonoscopic biopsy may be helpful to confirm diagnosis of tuberculous enteritis. The treatment regimens for pulmonary tuberculosis are generally effective for tuberculous enteritis as well. If not treated early, the prognosis of intestinal tuberculosis is poor. We report a case of tuberculous enteritis diagnosed by colonoscopic biopsy and TB PCR which was presented with diarrhea, abdominal pain, intermittent fever and weight loss in a 12-year-old girl with active pulmonary tuberculosis. The patient was treated successfully with antituberculosis agents for 11 months without any complications.

Bleeding due to Vitamin K Deficiency as Presenting Symptom of Cholestasis (비타민 K 결핍에 의한 출혈로 증상 발현한 담즙정체증)

  • Clinical findings in neonates and infants with cholestasis characteristically include prolonged jaundice, acholic stool, pruritus and failure to thrive. We report two cases of cholestasis presenting with spontaneous bleeding due to vitamin K deficiency. Laboratory studies on admission revealed moderate liver dysfunction and a bleeding tendency due to vitamin K deficiency. After administration of vitamin K, the bleeding tendency disappeared. Vitamin K deficiency was resulting from a combination of cholestasis-induced fat malabsorption and low vitamin K supplementation due to breast-milk feeding. Cholestasis should be considered in neonate and infant with a bleeding tendency.

A Case of Pathologic Aerophagia with Mental Retardation Managed by Percutaneous Endoscopic Gastrostomy (경피적 내시경적 위루술을 이용한 병적 공기 연하증 치료 경험 1례)

  • We experienced a case of pathologic aerophagia in a 10-year-old girl who has mental retardation. It was observed that the abdomen was non-distended in the morning and became maximally distended in the evening. Increased passage of flatus, but normal eructation, was noted. Roentgenographic examination showed 'esophageal air sign', abnormal air shadow on proximal esophagus adjacent to the trachea, in simple chest X-ray and visible air swallowing fluoroscopically. We tried a percutaneous endoscopic gastrostomy (PEG) to decompress swallowed air in stomach. We suggest that placement of a PEG catheter in early life, especially in mentally retarded patients, that can be used as desufflator, can prevent the complications of aerophagia. 'Esophageal air sign' may be very helpful for early detection of pathologic aerophagia.
