Stapling and Suturing of Blebs and Bullae without Excision in Thoracoscopic Surgery (비디오 흉강경하에서 절제없이 봉합에 의한 기포정복술)

  • Background: Prolonged air leak is a common complication after thoracoscopic bullectomy. Materials and methods: A technique is described to minimize postoperative air leak in thoracoscopic surgery for the treatment of recurrent or persistent spontaneous pneumothorax. Results: A 3.5cm utility incision is made in the anterior axillary line at the level of the third intercostal space, and blebs and bullae are stapled and sutured without excision, using standard surgical instruments and stapler. Conclusions: This technique may be useful to reduce prolonged air leak after removal of the bleb and bullous lesion, and may minimize the delayed recurrence of ipsilateral pneumothorax.

  • The effects of acute hematogenous osteomyelitis vary with a patient's age because of the differences in the blood supply and structure of the bone. In children older than 2 years of age, this process results in extensive abscess formation when both the endosteal and periosteal blood supply are destroyed. Thorough drainage of abscess cavity and removal of all dead or necrotic material are not always possible although large skin incision is made along the abscess. Authors successfully managed acute osteomyelitis of the tibia with extensive large abscess in a 11 year-old female, using minimal incisions and 4-mm endoscope.

  • A 49-year-old man visited our hospital via the emergency room. He had suffered chest trauma by falling down. His chest X-Ray showed pneumomediastinum with pneumopericardium. We checked the Chest CT, and it showed pneumopericardium without any injury to the other organs, the compressed heart and a minimal pneumothorax on the left hemithorax. Closed thoracostomy was then done under local anesthesia. We then performed open pericardiostomy under general anesthesia. We got a good result and so we report on this case.

  • Purpose: Fracture of the frontal sinus accounts for 5-15% of all facial fractures. Only anterior table fracture occupies one third of the frontal sinus fracture. Conventional coronal approach is the standard treatment, but this method is very aggressive, especially in patient with simple, depressed, anterior frontal sinus wall fracture. We introduce new, simple technique for these patients, using small incision and Dual-top screws. Methods: A 27-year-old male patient was introduced to our department under the impression of fracture of frontal sinus, involving only anterior table. Under general anesthesia, 1.5 cm-sized, small incision was made on the suprabrow area. We reduced fractured fragment with two dual-top screws and 25G wires. Fixation was not necessary Results: The operation took about forty minutes, and the patient discharged 2 days after the operation. Postoperative 2 months follow up CT shows maintenance of reduction site. Gross depression of the forehead and visible scar was not observed. Conclusion: Although the indication of this procedure is limited, less aggressive, simple and very effective to the patient with simple. depressed, anterior frontal sinus fracture.

Arthroscopic Assisted Mini-open Repair of Rotator Cuff Tear - 5∼8 Years Follow-up Results - (관절경 보조 소절개 봉합술을 이용한 회전근 개 파열의 치료 - 5∼8년 추시 결과 -)

  • Purpose: To report our experience of treatment of rotator cuff tear and evaluate the mid-term outcome. Materials and Methods: We have performed 50 cases of mini-open rotator cuff repair from March 1996 to March 1999. Male to female ratio was 34:16, the average age 46.5(23∼57) years old, mean follow-up period was 78(62∼93) months. All-arthroscopic repair and open repair cases were excluded. Mean symptomatic period was 12.5(6∼38) months, operation was indicated in cases of no improvement by 6 months of conservative management. Preoperative simple radiographs and Magnetic resonance arthrography were obtained in all cases. We also observed the inflammation, synovitis, thickness of tear and associated pathology intraoperatively. We evaluated pain, function, and range of motion by UCLA score. Results: Overall UCLA score was mean 29.5 points. Excellent 25, good 18, poor 7 cases. Mean pain score was improved 2.6 to 7.5, 44 cases(88%) were improved and 6 cases(12%) were not improved. Mean functional score was improved 3.4 to 8.5, and activity at follow up, 25 cases(50%) were same, 8 cases(14%) were above, 17 cases(34%) were below compared with preoperative level. Mean active forward flexion was preoperative 112° to postoperative 160°, forward flexion strength was improved 3.8 to 4.7. 43 cases(86%) of patients were satisfied at the result, 7 cases(14%) were not satisfied or aggravated. Results: Mini-open rotator cuff repair was effective method in treating rotator cuff tear.

Mini-Open Repair in Large and Massive Rotator Cuff Tears (소절개 봉합술을 이용한 대범위 이상 회전근 개 파열 환자의 치료 결과)

  • Purpose: Although surgical arthroscopic repair of rotator cuff has become much more common as surgeons have developed proficient techniques, it is still technically difficult. The purpose of this study was to evaluate the clinical results and the usefulness of mini-open repair in large and massive size tears. Materials & Methods: From January 2000 to December 2004, sixteen patients were treated with mini-open repair. There were 10 male and 6 female patients with the average age of 62.5 years. The size of tear was massive in 4 cases and large in 12 cases. All tears were repaired with metal anchor sutures. The mean duration of follow-up period was 23 months. Postoperative results were evaluated based on American Shoulder and Elbow Society scoring system. Results: Five patients showed excellent results, five good, and two fair in large tears while one patient showed excellent result, one good, and two fair in massive tears. Poor outcome was not seen during the follow-up period. There was no significant relationship between the patient's age and the size of tear, and postoperative results. However, the relationship between the duration of symptomatic period in preoperation and postoperative results showed significant correlation. Conclusion: Mini-open repair combined with the preservation of deltoid and early rehabilitation is clinically useful in large and massive size rotator cuff tear patients treatment.

  • Tuber yield and content of general component and diosgenin which is a main bioactive property were investigated in order to determine the growth characteristics of round typed yam (Dungeon-ma, Dioscorea opposita L.) and the potential of artificial culture at Suwon, Korea. Sprouting rates of segmented small tubers such as 3, 5, 7, 9, and 12g were respectively 60.3, 80.5, 85.3, 98.6 and 99.3% when they were investigated 30 days after planting. Segmented small tubers of 3, 5, 7, 9, and 12g enlarged respectively to 66, 87, 98, 120, and 140g in the green-house cultivation. Segmented small tubers of 3, 7, 12, 18, and 23g enlarged respectively to 35.2, 124.7, 142.7, 174.8, and 200,7g in the open-air field cultivation.

  • Low density ceramic supporter was prepared by using fly ash as a starting material for the application to the biological aerated filter system. Wheat powders were used to control the porosity of the supporter and the carbon content of the raw material. Apparent density of 1.6~1.8 g/$\textrm{cm}^3$ was obtained when the fly ash was sintered at $1200^{\circ}C$ in a weak reducing atmosphere. By maintaining the reducing atmosphere and sintering at a high heating rate, the liquid phase was formed from the reduced composition of fly ash. This resulted in the closed pore formation which enabled the low apparent density.

The Evaluation for the Usefulness of Arthroscopic Miniopen Repair which Related with Large and Massive Sized Full Thickness Rotator Cuff Tear and Clinical Results (대범위 이상의 회전근 개 파열에 대한 관절경 감시하 소절개 봉합술의 임상 결과 및 수술의 유효성)

  • Purpose: To evaluate the usefulness of arthroscopic decompression and miniopen repair that was related with large and massive sized full thickness rotator cuff tear and assess clinical result. Materials and Methods: Twenthy-nine cases of miniopen repaired full thickness tear of rotator cuffs that arthroscopically decompressed were studied. From October 1998 to December 2004 we have analysed 29 repairs of large and massive sized FTRCT, the average age 44 ($32{\sim}71$) years old, mean follow-up was 34 ($12{\sim}84$) months. We analyzed the results statistically by paired t-test. Results: Postoperative VAS of pain improved average 7.0 to 1.7, UCLA score improved 13.7 to 31.9, ADL improved 11.3 to 25.3 respectively (all, P=0.000). Twenty five cases(82.8%) of the patients showed excellent & good results at the final follow-up. The satisfied rate was 26 cases(89.7%). Conclusions: Arthroscopic decompression and miniopen repair in large and massive sized full thickness rotator cuff tears are effective surgical methods.
