Clinacal investigation of child sexual abuse (소아 성폭력에 관한 임상적 고찰)

  • Purpose : Child sexual abuse is a significant and serious problem that affects public health and society. Pediatricians are responsible for preventing and detecting sexual abuse of children. The aim of this study was to examine clinical findings of child sexual abuse. Methods : The authors retrospectively studied 292 patients between the ages of 0 and 18 who were referred for evaluation of sexual abuse to the Emergency Department of the National Police Hospital from Oct 1, 2001 to May 5, 2005. Results : Sixty three (21.6 percent) of sexual abuse victims were younger than 6 years of age; 89 (30.5 percent) were 6-12 years of age; and 104 (47.9 percent) were 12-18 years of age. Attack time was the most common between noon and 6 pm. Victims were attacked in their home or nearby (51.4 percent). One hundred fifty six (53.6 percent) offenders were accquaitances, thirty nine (13.4 percent) of whom were family members. The disclosure of attack by parents with abnormal physical symptoms was more common in younger children than in adolescence. Eighty nine (30.5 percent) victims had no specific physical findings, 51 percent had injury to the hymen, 37.3% had injury to external genitalia, and 4 percent had anal injury. One case of gonorrhea and thirteen cases of chlamydia were found. Sperm was found in 19 cases and acid phosphatase was positive in 28 cases. The absence of physical findings and laboratory findings was more common in younger children than in those in adolescence. The time interval from attacks to hospital visits had a significant negative correlation with age. Conclusion : For younger victims, the time interval from attack to hospital visitis was more prolonged. Victims may have no physical evidence of sexual abuse. Therefore, a careful history should be obtained and a through physical examination should be performed. Pediatricians must play a leading role in evaluation and treatment of sexually abused children, considering children,s growth and development. Coordination with other professionals is necessary to provide consultation, medical treatment, and legal assistance for the sexually abused children and families.

  • Sexual abuse is emerging as one of the major form of child abuse. In the late 1990s, official reports of sexual abuse began to mushroom at a much more rapid rate than reports of other forms of abuse in Korea. In addition, sexual abuse can cause the most serious emotional-cognitive-behavioral sequelae to victims. Although child & adolescent psychiatrists meet many sexual abuse victims who are referred from many types of child protective services, the psychiatrists may not be likely to give them sufficient and appropriate treatment and guideline for victims and their parents. In this article, the historical concepts, pathophysiological processes, shortterm & longterm sequelae of sexual abuse are reviewed and summarized. And I summarize the intervention and outcome studies for sexual abuse victims. In the close future, the active participation of psychiatrists who help the child and adolescents will be expected in the development of nationwide preventive and therapeutic projects for victims and families of sexual abuse.

  • Objectives : Child sexual abuse is a highly prevalent societal problem that cuts across all ethnic, racial, educational, and socioeconomic groups. There has been mounting empirical evidence for the efficacy of manual-based, cognitive behavioral treatment(CBT) for sexually abused children. Therefore, the aim of this review was to address the rationale and efficacy of CBT for sexually abused children, and to introduce a broad outline of a typical CBT program for young people suffering the emotional and behavioral consequences of sexual abuse. Methods : A selective literature review was undertaken. Results : To date, trauma-focused CBT is the most effective treatment for the sexually abused child. The rationale and efficacy of CBT was reviewed and a typical CBT program was outlined session by session. Conclusion : Based on trauma-focused CBT for sexually abused children, the results demonstrate the need for future development of a standard CBT program for the Korean population.

Factors Associated with Disclosure of Sexual Abuse of Children and Adolescent Victims (아동청소년 성폭력 피해자의 피해사실 폭로에 영향을 미치는 요인)

  • Objectives : The purpose of this study was to analyze the factors related to the disclosure patterns of sexual abuse in children and adolescent victims. Methods : A sample of 153 children and adolescents who visited the Sunflower children's Center (Incheon) between January 1, 2011 and December 31, 2012 were analyzed. The medical records of the victims were reviewed retrospectively. Results : Ninety (58.8%) subjects made a disclosure of sexual abuse voluntarily. The revelation was most commonly made to a parent. The severity and duration of sexual abuse were related to the disclosure patterns. In addition, the group that displayed below average intelligence, particularly subjects in the group with borderline intellectual functioning, tended to disclose their history of abuse less voluntarily than the subjects with normal intelligence. Conclusion : The current study provides insight into the factors that affect disclosure patterns in children and adolescent sexual abuse victims. To prevent sexual abuse, we suggest that special legal assistance and social concern be required for children and adolescents with borderline intellectual functioning.

A Study of Sexual Assaults on Children and Adolescents: Based on Data from a One-Stop Service Center (소아.청소년 성폭력 피해의 실태 : 원스톱 지원센터 대상자를 중심으로)

  • Objectives: The aim of this study was to identify characteristics of children and adolescents who were victims of sexual assault in Korea. Methods: The subjects were 60 children and adolescents who visited the one-stop center as victims of sexual assault. The medical records of victims of sexual trauma were retrospectively reviewed. We studied the demographic data of the victims, their relationship to their perpetrator, the characteristics of the assault (frequency, duration, place, type), and the process from sexual assault to treatment. We also paid special attention to how the characteristics of the victims or perpetrators affected the characteristics of the assault or follow-up treatment. Results: There were several differences between sexual assaults committed by strangers and those committed by acquaintances. Sexual assaults committed by acquaintances lasted for a longer period of time than those committed by strangers. In addition, it took more time for victims of sexual assaults committed by acquaintances to seek treatment than those who were victims of sexual assaults committed by strangers. The majority (55.0%) of victims were between 10 and 15 years of age. Forty percent of the perpetrators were teenagers, and two of them were under the age of 10. Voluntary discontinuation of treatment was more frequent in adolescents than in children. Conclusion: All teenage victims of sexual assault need some sort of urgent intervention. In addition, approachable methods are needed in order to prevent sexual abuse by strangers or acquaintances.

Impact of Trauma due to Sexual Violence on Psychopathology and Quality of Life in Children and Adolescents (성폭력 외상이 소아청소년의 정신병리와 삶의 질에 미치는 영향)

  • Objectives : This study was conducted in order to investigate the influence of sexual trauma on the psychopathology and quality of life of children and adolescents in Korea. Methods : Twenty-seven children and adolescents and their caretakers who visited the Kangwon Sunflower Center participated in a cross-sectional study. Participants completed the Korean version of Kiddie-Schedule for Affective Disorders and Schizophrenia-Present and Lifetime Version, Child Behavior Checklist (CBCL), and Child Health Questionnaire-Parent Form-50 (CHQ-PF-50). Their scores were compared with those of a age and sex-matched control group of 27 healthy children and adolescents. Results : Victims of sexual violence showed higher t score in Withdrawn, Social problems, Delinquent behavior, Externalizing problems, and Total problems in CBCL, compared with control group. In CHQ-PF-50, there were lower scores on Bodily pain/discomfort, Behavior, Mental health, Time impact in parents, and Family activities subscales in victims of sexual violence. While Behavior and Family activities subscale showed negative correlations with Delinquent behavior, Externalizing problems, and Total problems in CBCL. Mental health subscale showed negative correlations with Social problems, Delinquent behavior, Externalizing problems, and Total problems. In addition, Time impact on parent subscale showed a negative association with Delinquent behavior in CBCL. Conclusion : The current study provided evidence suggesting that victims of sexual violence had a higher level of psychopathology and lower level of quality of life.

  • Objectives: The purpose of the case study is to report the effects of post traumatic stress disorder from sexual violence treated by an oriental medical treatment. Methods: The patient was treated by acupuncture at Baekhoi, Naegwan, Hapkok, Taeyang, Changmun and moxa at Baekhoi, Sin-Guel, Chungwan, Kwanwon. Results: The oriental medical treatment resulted considerable improvements for the patient. Conclusions: This study shows that the oriental medical treatment can be an effective treatment for sexual violence victims. However, the symptoms may recur, so we need to observe and manage the patients continuously.

A SURVEY OF EXTRAFAMILIAL CHILD SEXUAL ABUSE BY PHYSICIANS' REPORTS (의사들의 보고에 의한 가정외 아동성학대 연구)

  • The authors surveyed extrafamilial sexual abuse in the children under 15years old by the physician's reports. We sent the semi-structured child sexual abuse questionnaires to 7055 board certified pediatrics, obstetrics and gynecology, family medicine and emergency medicine. Total respondents were 1205. The results from these respondents were as followings. 1) The number of respondents who have had the experience of treating victims of extraf/amilial child sexual abuse were 641(53.2% of total respondents). 2) 338(52.7%) of the perpetrators were known persons and 277(43.2%) were strangers, the most common age bracket were 20s, 30s and 10s, and almost all(99.8%) of the perpetrators were male. 3) The mean age of victims was $9.7{\pm}3.5$ years old, and almost all(98.6%) of the victims were female. 4) The ways by which extrafamilial child sexual abuses were found were victim’s own reports:273(62.6%), pain complaint, 156(24.3%) and abnormal behavior 96(15.0%), other person’s report 72(11.2%), detection during examination 19(3.0%), and pregnancy 4(0.6%). 5) Time lags between extrafamilial child sexual abuses and hospital visits were within 24 hours 332 (51.8%) and from 1 day to 1 week 232(36.2%), victims were rather quickly 6) Physical complications were perineal wound 571(89.1%), hymen rupture 349(54.4%), wound of other part of body 124(19.3%), pregnancy 37(5.8%), and sexually transmitted disease 18(2.8%), and other serious complications such as vaginal-rectal lacerations 8, intastinal bleeding 7, death 2, hypotensive shock 1. These results suggest considerable numbers of physicians have had the experience of treating victims of extrafamilial child sexual abuses, and extrafamilial child sexual abuses are the major medical as well as social issue in children in Korea.

The Effectiveness of Traumatic Symptom Checklist for Children(TSCC) - Comparisons of Sexually Abused Children and Nonabused Normal Children - (아동용 외상관련 증상 평가척도의 효용성 - 성폭력 피해 아동과 일반 아동의 비교 -)

  • Objectives : The research is designed in order to know how much Traumatic Symptom Checklist fur Children(TSCC) is effective in the case of child abuses, especially in the estimation of the psychopathology of child sexual abuse. Methods : A sample includes a group of thirty children of 4th, 5th and 6th grade whose sexual abuse have confirmed and also a group of thirty four normal kids who have no abuse. Along with TSCC, Children's Depression Inventory (CDI) and Revised Children's Manifest Anxiety Scale (RCMAS) were used to children and child behavior checklist (K-CBCL), sexual behaviors (CSBI), and Kiddie-Schedule for Affective Disorder and Schizophrenia-Present and Lifetime version (K-SADS-PL) record sheets were used to kids' parents. Results : In case of the sexual abuse group, the most meaningful difference from the analyzed result is shown at sexual concerns scale which is one of the clinical scale in TSCC (p<.01). Also, the sexual abuse group has the meaningful higher score than the general group's one in the stress scale after the other anxiety, depression, anger and dissociation (p<.01). Conclusion : The scale of TSCC has relatively shown the characteristic symptom and severity which were gained from the children who have experienced trauma, especially sexual abuse.

Cultural Politics of Gendered Schadenfreude Surrounding an Idol Focusing on the debate over IU (아이돌을 둘러싼 젠더화된 샤덴프로이데(Schadenfreude)의 문화정치학 <아이유 사태>를 중심으로)

  • This study aims to reveal the content of and logic behind a recent negative public sentiment toward female idols with the example of a debate over songstress IU's fourth album that was released late last year. While previous studies on fandom have focused on the identification process towards entertainers and making community, a recent phenomenon of "anti-fandom" or "malicious comments" implies that more research is needed on negative emotions such as hostility or schadenfreude (feelings of pleasure from others' misfortunes). Schadenfreude is a social sentiment that originated in modern liberalism, which features contradictions between public equality and private ownership, and that has been intensified in neoliberalism, which features a maximization of this contradiction centering on a meritocracy. Celebrities in Korea often become the targets of schadenfreude, which is associated with the suspicion that they gain popularity not from their abilities but from "just being popular." It should also be noted that this kind of schadenfreude operates differently between male and female entertainers. Specifically, the acquisition of money and fame by modern women whose presence used to be located in the private possessions of males is considered to be due to their unjustified use of sexuality. This is also the background of the recent online misogyny culture in Korea. In this context, IU, who had been successful at building a differentiated image of "sister-like idol artist," became a valid target. Although accusing IU of utilizing pedophilia reflects a stalemate that a current politics of sexual violence faces, it rather damages the name of an individual than attracts public attention to the structural causes of childsexualabuse. This is why I see the way that pedophilia was used in the debate over IU as a schadenfreude. Consequently, the term pedophilia here contributes to an expansion of the entertainment economy that is sustained by rises and falls of the celebrities' stock prices.
