Analysis of Healthcare Convergence on Bacterial Contamination of Radiological Equipment in Emergency Rooms of General Hospitals (종합병원 응급실내의 영상의학과 장비에서의 세균 오염에 관한 보건학적 융합 분석)

  • It is to establish preliminary data about healthcare convergencr of infection control by performing a bacterial contamination test for CT scanner as well as general radiographic systems in the emergency rooms of general hospitals. The period was December 1 ~ December 31, 2015. It is detect in the emergency rooms of 7 medical facilities including 2 in Seoul, and 1 in Gyeonggi-do Province, and 2 in Jeonla-do Province, and 2 in Chungcheong-do Province. The surface contamination strains detected from CT scanner in the radiology department emergency rooms were Micrococcus species(4,5%), Stenotrophomonas maltophilia(9%), Enterococcus faecium(4.5%), Providencia stuartii(4.5), and Gram negative bacilli(4.5%). Also, the surface contamination strains detected from general radiographic system in the radiology department emergency rooms were Providencia stuartii(11%), Klebsiella pneumonia(3.5%), Stenotrophomonas maltophilia(11%), Pantoea species(11%), Acinetobacter baumannii(3.5%), Micrococcus species(3.5%), Escherichia coli(3.5%), Enterobacter species(3.5%), and Gram negative bacilli(11%). Considering that the regions of most detection were all the places closely related to patients, radiologists would have to thoroughly clean with alcohol before and after a test.

Hazard Analysis and Determination of CCPs for Powdered Raw Grains and Vegetables, Saengshik (생식의 위해요인 분석 및 중요관리점 설정)

  • Biological, physical, and chemical hazards in raw manufacturing processes of Saengshik, powdered raw grains and vegetables, were analyzed to identify critical control points (CCPs). In raw materials, total plate and coliform counts ranged 2.82-8.23 and $1.40-6.57\;{\log}_{10}\;CFU/g$, respectively. In final products, total plate counts, except for Lactobacillus spp., were $1.51-7.40\;{\log}_{10}\;CFU/g$. During processing steps, both total plate and coliform counts decreased after washing, whereas no changes were observed after freeze-drying. Physical hazards, such as contents of metal and other contaminants, and chemical hazards, such as moisture content, were assessed. Suggested CCPs for Saengshik were: washing process for controlling microbial contamination, freeze-drying process for controlling moisture content to prevent deterioration and growth of microorganisms, and pulverization process for controlling contamination of foreign substances such as metals. These results will provide guideline to apply HACCP system standards to this product.

우리나라에서 보고된 집단 식중독의 발생 특징(1981-1989)

  • 1. 환자발생 규모는 10명 이하가 $38.0\%$로 가장 많았고 50명 이상의 대규모 발생도 $13.0\%$이었다. 대규모 환자발생은 세균성 식중독이 원인이었다. 2. 전체 식중독에 $67.3\%$가 7월부터 9월에 집중되어 계절의 변화에 영향받고 있음을 알 수 있다. Vibrio에 의한 발생은 특히 하절기에 높았고 Salmonella, Staphyloccus는 상대적으로 연중 계속해서 발생 가능성을 보였다. 독성물질에 의한 식중독은 계절과 무관하였으나 식물성 식중독은 7, 8월에 집중되고 있다. 3. 세균성 식중독이 $56.2\%$, 독성물질에 의한 식중독이 $17.8\%$이었다. 세균성 식중독의 원인균은 Vibrio $35.4\%$, Salmonella $27.2\%$, Escheri-chia coli $17.7\%$, Staphylococcus $17.7\%$ 순이었다. 독성물질에 의한 식중독은 식물성 식중독이 $64\%$로 가장 많았다. 4. Vibrio 식중독의 주원인식품은 해산물이지만 cross-contamination에 의해 오염된 다른 식품으로도 발생한 것으로 나타났다. Salmone-lla 식중독은 관혼상제시 거의 빠지지 않고 제공되는 돼지고기 편육이 주원인식품이었다. 5. 전체 식중독의 $76.8\%$가 가정과 식품접객업소(구내식당 포함)에서 발생되었으며, 특히 도시지역은 $66.4\%$가 식품접객업소에서 농촌지역은 $58.9\%$가 가정에서 발생되어 지역간의 차이를 보였다. 6. 독버섯은 독성물질에 의한 식중독의 주원인식품으로 농촌가정에서 일상식을 통한 섭취로 많이 발생되고 있으며, 사망자의 발생이 가장 많은 치명적인 원인식품이었다. 7. 식품접객업소에서 발생된 식중독의 주원인식품은 해산물이며, 덜익혀지거나 생선회 같이 날것을 섭취하여 발생하였다. 8. 생일잔치 및 회갑연, 결혼 피로연, 문상, 제사 등의 관혼상제시 발생되는 식중독은 농촌지역의 가정에서 주로 발생되고 있다. 구내 식당 및 관광지 음식점의 식중독 발생이 많은 것은 이들 시설에 대한 위생관리가 미비하여 대규모 식중독 발생의 위험이 항시 존재하고 있다고 할 수 있다.

Validation of Korean Water Quality Standards to Hot Springs for Agreement with Legionella-Incidence Risk (레지오넬라균 출현위해도에 대한 현행 온천수 수질기준의 적합성 분석)

  • Observed trends in climate change, globalization and an aging population have an effect on public health conditions in Korea, prompting a reevaluation of current environmental regulations. In this study, we evaluated the performance of the total coliform (TC) standard, which is the only microbiological standard in the current regulation regime for hot spring water, by estimating correlation with the presence/absence of Legionella, a non-fecal opportunistic pathogen with heat-tolerance. Microbiological data in 7 studies that surveyed Legionella in hot spring waters were subjected to meta-analyses with the odds ratio (OR) as the effect size. The presence/absence of Legionella was significantly correlated to TC levels [OR = 3.1(1.5–6.4, 95% CI), p = 0.002]. Due to there being no direct explanation as to the reason for the occurrence of TC, mesophilic fecal bacteria, being coupled with Legionella presence, the mechanism of the correlation between the two kinds of bacteria was further investigated. Legionella presence was more prevalent with a high heterotrophic plate count [HPC; 4.0(2.2–7.2); p < 0.001] and water temperature [4.3(1.4–13.6), p = 0.011] when the temperature range was <40℃. However, it was reverse-correlated with water temperature when the temperature was >50℃ [0.2(0.1–0.4), p < 0.001]. Therefore, bacterial standing crops in hot spring waters appear to be determined by water temperature in general, and this forces TC and Legionella levels be correlated. In accordance with this relationship, HPC rather than TC reflect the levels of non-fecal contamination better. Therefore, employing HPC as the sole microbiological standard, or adding HPC into the current standard of hot spring water assessment, is suggested as a proactive measure to prevent health issues arising from contamination.

A Study Regarding Bacterial Contamination of Surfaces in Dental Offices (치과진료실 내 표면 세균 오염에 관한 연구)

  • This study studied samples taken off surfaces at three sites (Unit chairs, light handles, cuspidors) of 19 dental hospitals and 28 clinics located in Gyeonggi-do and Incheon, South Korea. The bacterial contamination levels of surfaces were $44.82{\times}10^3CFU/mL$ in cuspidors, higher than in unit chairs ($5.47{\times}10^3CFU/mL$) and light handles ($16.28{\times}10^3CFU/mL$). The values were statistically higher at dental hospitals than at dental clinics, the greater number of patients being associated with the higher bacterial cell count in the cuspidors. The results of identifying the strains isolated purely from surfaces at dental clinic showed Gram positive 47.3% and Gram negative 52.7%. Among Gram positive, the most numberous bacteria were Micrococcus luteus (10.9%), Bacillus pumilus (3.6%), and Staphylococcus aureus (3.6%). Among Gram negative, the most numberous bacteria were Acinetobacter ursingii (5.5%), Brevundimonas diminuta (4.5%), Chryseobacterium (Flavo.) indologenes (CDC IIb) (4.5%), and Methylobacterium sp. (4.5%). This study measures the level of bacterial contamination and identifies the strains isolated in dental clinics. It recognizes the importance of infection control, and the results of the study may be considered as the basis for establishing specific plans for prevention of infection.

Analysis of Microbial Contamination of Sprouts and Fresh-cut Salads in a Market (유통중인 즉석섭취 새싹채소와 샐러드의 세균오염 분석)

  • Although bacterial outbreaks from ready-to-eat foods have increased, little information is available on microbial quality of sprouts in markets. Fifty sprouts and 30 salads were collected from wholesale markets. Total aerobic count (TAC), coliform, Escherichia coli, and some pathogens were detected. TAC for sprouts was 7.95 log CFU/g and 6.70 for salads, indicating that sprouts were more contaminated by 1 log CFU/g than that of salads. The numbers of coliform were 6.69 log CFU/g for sprouts and 5.42 for salads. E. coli was detected in 16 of 50 sprout samples at 2.38 log CFU/g and eight of 30 salads at 2.21 log CFU/g. Bacillus cereus was detected in 29 of 50 sprout samples and 16 of 30 salads, and the counts were mostly <3 log CFU/g. Salmonella, Staphylococcus aureus, Listeria monocytogenes, Campylobacter jejuni, and Clostridium perfringens were not detected. Therefore, although pathogens may not be a high risk for these foods, the high TAC and E. coli contamination require improved production and distribution methods, particularly for sprouts.

Distribution of Microorganisms in Perilla Leaf and Cultivation Area (들깻잎과 생산환경의 미생물 분포)

  • The prevalence and distribution of hazardous microorganisms were investigated from the major perilla cultivation area at Milyang, Gyeongnam province, Korea. Aerobic plate count (APC) and coliform count of perilla leaves were 4.82 log CFU/g and 3.85 log CFU/g, respectively. E. coli, S. aureus and B. cereus were detected in 3.0% (4/114), 7.9% (9/114) and 46.5% (53/114) of examined perilla leaves. However, E. coli O157:H7, Salmonella spp, and L. monocytogenes were not detected. The distribution of hazardous microorganisms in perilla leaf cultivation environment were compared and the concentration of APC and coliform counts were more than 3.0 log CFU/(mL, g, $100cm^2$, hand) from most of the samples. S. aureus were detected from irrigation water, packing table, packing vinyl, hand, and clothes. Also, B. cereus was frequently detected from the examined samples. Especially, packing table and collection container were contaminated with maximum 5.5 log $CFU/100cm^2$ of B. cereus. Good Agricultural Practice (GAP) system should be introduced to farms to enhance the safety of perilla leaves.

A Study on Hygiene and Safety of Sanitary Wet Towel (물수건의 위생실태 및 안전성 연구)

  • The risks of sanitary indicative bacteria, heavy metals and chlorinated derivatives in 94 cases of sanitary wet towels used in food services (39 from sanitary wet towel treatment business, 55 from food services) were assessed in the present study. Lead was detected in the range of N.D.~0.41 mg/kg (75 cases were not detected), N.D.~0.25 mg/kg of arsenic (93 cases were not detected), N.D.~0.01 mg/kg of cadmium (7 cases were lower than limit of quantitation; 87 cases were not detected), 0.003 mg/kg ~ 0.09 mg/kg of mercury. And chromium (VI) was not detected from all samples. The level of lead was the highest among the tested heavy metals, and the highest concentration of lead was 0.41 mg/kg. However, it was only 2.1% of legal limit (less than 20 mg/kg). The average moisture content of the samples was 61.9% (50.0% ~ 77.0%) and it showed no relevance to the detection of bacterial counts. Escherichia coli was not detected. Bacterial counts were detected 43 cases and among them, 24 cases were exceeded the legal limit. It was verified that the packaging conditions of sanitary wet towel (whether it is packed by a piece or not and sealed or not) are critical factors to cause the germ contamination and cross contamination in the wet towels. The chlorinated derivatives (chlorites and chlorates) were detected in 17 (19.3%) out of 88 cases. The results would be used as preliminary information to establish the programs of "Safety education for manufacturers and public policy of safety".

Microbial Analysis of Processing and Evaluation of Shelf life of Fried Bean Curd (유부의 가공공정중 미생물 분석과 저장 수명 평가)

  • This study was undertaken to find out distribution and contamination sources of microbes on the processing steps and to estimate quality index and shelf life of fried bean curd. It was necessary that the sanitation for water, processing environment and instruments at digestion, formation, cutting and processes after frying must be controlled and microbial growth at digestion and formation must be inhibited, to process efficiently and to improve shelf life of fried bean curd. It was evaluated that quality indexes as to sensory evaluation, especially texture, mold generation and total viable cell counts will be useful to estimate shelf life of fried bean curd and that shelf life of fried bean curd was 6 days.

Environmental Microbial Assessment of Food Services at Elementary Schools in Western Gyeongnam Pyovince (서부 경남 지역 초등학교 급식소에서의 환경미생물 평가)

  • Two aims of the present study were the evaluation of hygienic, microbial safety on food services of elementary schools in Western Gyeongnam province, and then, the construction of database for the SSOP (Sanitation Standard Operation Procedures) practice. A total of 98 samples were collected from drinking water, waterworks, kitchen utensils, kitchen equipments, employees and cooked foods. Total bacteria was counted and pathogenic bacteria such E. coli, salmonella, yersinia, vibrio parahaemolyticus and staphylococcus were identified based on the biochemical analysis. Following are the results: the number of bacteria showed from 1.0x10$^2$CFU/ mL to 1.0x10$^{6}$ CFU/mL in most samples, which the level exceeds normal range. Over 1.0x10$^{5}$ CFU/ml bacteria were observed from the kimchi in 4 places (B, C, D, E), because cooked food such as kimchi had not been heated. As a rule, the bacterium level in place B was higher than that in any of the other places. E-coli were isolated from kitchen knives (C, E) and Kimchi (E): staphylococci were isolated from drinking water (A), hands (D), refrigerator (E) and apron (E). But, salmonella, vibrio and yersinia were not detected in anywhere. In conclusion, the presence of bacteria and pathogenic agents in school food service was closely related to hygienic practice. For that reason, it is necessary to have more systematic and efficient management in order to enhance the food safety.
