Purpose: The prognosis for patients with a Borrmann type IV gastric cancer is extremely poor despite an aggressive surgical approach. We evaluated the clinicopathological features for Borrmann type IV cancers to find treatment strategy. Materials and Methods: The 1098 patients with advanced gastric cancer who underwent surgical resection between 1990 and 2001 were analyzed. These patients were divided into two groups: 81 patients with a Borrmann type IV carcinoma, and 1017 patients with all other types of gastric carcinomas. Results: Patients with a Borrmann type IV carcinoma were younger than those with other types, and female was prevalent (p=0.000). Of the patients with a Borrmann type IV gastric carcinoma, 68 patients (84%) were classified as stage III or IV at the initial diagnosis. The histologic type was commonly undifferentiated and serosal infiltration; nodal involvement and lymphatic invasion were more frequent in patients with a Borrmann type IV than in those with other types of cancer. Multivariate analysis confirmed that the extent of lymph node metastasis was a negative prognostic factor for Borrmann type IV gastric carcinomas. The curability for a Borrmann type IV carcinoma was only 53.1%, and peritoneal dissemination rate was 25.9%. The predominant pattern of recurrence for a Borrmann type IV gastric carcinoma was peritoneal dissemination, and it was significantly different with other types (93.1% vs 55.8%, P<0.05). The 5-year survival rate of patients with a Borrmann type IV gastric carcinoma was significantly lower than those of patients with other types of cancer, even though a curative resection had been accomplished (26% vs 63%, p<0.005). The 5-year survival rates of patients with a Borrmann type IV carcinoma following a curative resection were 44.9%, 24%, and 0% for stages II, III and IV, respectively (p<0.05). Conclusion: Because the prognosis for patients of a Borrmann type IV gastric cancer is extremely poor despite a curative resection, preoperative and/or intraperitoneal chemotherapy should be considered. And diagnostic laparoscopy and peritoneal cytology may be used to play an important role in accurate staging workup. (J Korean Gastric Cancer Assoc 2006;6:97-102)

Background : Severe community-acquired pneumonia (CAP) can develop into respiratory failure that requires mechanical ventilation (MV), which is associated with a higher rate of mortality. It was recently reported that a hydrocortisone infusion in severe CAP patients was associated with a significant reduction in the length of the hospital stay and mortality. This study evaluated efficacy of a hydrocortisone infusion for patients with severe CAP requiring MV. Methods : From February 2005 to July 2005, 13 patients (M : F = 10 : 3, mean age: $68.6{\pm}14.1years$), who were diagnosed with severe CAP and required MV, were enrolled in this study. Hydrocortisone was administered as an intravenous 200mg loading bolus, which was followed by an infusion at a rate of 10mg/hour for 7 days. The control group was comprised of patients with severe CAP requiring MV but in whom corticosteroid was not used before study period. The clinical and physiologic parameters on or by day 8 and the outcome in the hydrocortisone infusion group were compared with those in the control group. Results : 1) There was no significant difference in age, gender ratio, SAPS II, SOFA score, temperature, leukocyte count, PaO2/FiO2 (P/F) ratio, the number of patients with P/F ratio < 200, chest radiograph score, lung injury score and catecholamine-dependent septic shock between the hydrocortisone infusion group and control group at day 1. 2) At day 8, the proportion of patients with an improvement in the P/F ratio ${\geq}100$ and the chest radiograph score was significantly higher in the hydrocortisone infusion group than in the control group (61.5% vs. 15.4%, 76.9% vs. 23.1%, p< 0.05). However, there was no significant difference in the other clinical and physiologic parameters. 3). There was no significant difference in the duration of the MV, ICU stay, hospital stay and 10th and 30th day mortality between the two groups. Conclusion : Hydrocortisone infusion for patients with severe CAP requiring invasive mechanical ventilation may be effective in improving the level of oxygenation and the chest radiograph score.
The purpose of this study was to offer fundamental data to manage countertransference, and to research into countertransference management ability by aspects in therapists' experiences in countertransference and their professional characters. For the paper, a survey was conducted on 62 music therapists who provided professional music therapy after finishing graduate school of music therapy through clinical practice and internship, and the result was drawn as follows. Around 84% of participants answered that they had been in trouble by countertransference in the analysis of a questionnaire regarding experience in countertransference. 48% among them first experienced countertransference during the practice in graduate school. 27% and 14% respectively answered that they experienced it within 3 years after graduation and during internship. Also, the result showed that therapists usually had difficulty with adults with mental disease, and the second most difficult clients were children with developmental disabilities. 76% of participants who had difficulty by countertransference answered that they were able to manage it to some degree, and almost all who answered thought that research into countertransference and management were necessary. About the question as to how to manage countertransference, 54% suggested self-analysis and self-therapy. 33% answered that countertransference should be treated through supervision. Finally, 13% of participants answered that it should be handled in graduate school. In this paper, which empirically examined therapists' experience in countertransference and countertransference management ability had meaning in providing essential basic data for music therapists to apply and manage countertransference for therapists themselves, as well as for clients.

This study was carried out to survey the early growth characteristics of Ligularia fischeri, plants of half-shaded grounds, primarily used as functional wild edible greens and examine its shading treatment to transplant seedling. The shading treatment was regulated with the shading level(full sunlight, 35%, 50%, 75%, and 95% of full sunlight). According to the experiment, the height was the highest under 75% of shading (17.3 cm), and root diameter was the highest under full sunlight (2.13 mm). It was found that fresh weight (leaf, shoot, root and whole) was the highest (1.784 g, 1.330 g, 0.791 g, 3.905 g respectively ) under 50% of shading. In case of dry weight, leaf, shoot and whole dry weight were surveyed the highest under 35% of shading, and root dry weight was the highest under full sunlight. It was found that S/R ratio and moisture contents (leaf, shoot, root and whole) were the highest under 95% of shading. Leaf area was the highest under 50% of shading ($39.3cm^2$) and the higher shading level, the higher SLA and LAR. It was surveyed that the higher shading level, the thinner leaf thickness. In case of root characteristics, it was showed that the relatively high height growth under 35%~75% of shading, excepting 95% of shading. As a result of surveying the whole experiment, it is concluded that the early growth of L. fischeri is more effective in producing it under 50% of shading.

Objective : The purpose of this study was to conduct a comprehensive survey of children's play in parents of disabled and non-disabled children prior to commencing school. This study aimed to further understanding play recognition and to present a specific direction of play necessary to each parent. Methods : A questionnaire based on prior studies was sent to 700 people who had previously agreed to the take part. A total of 596 questionnaires were analyzed. Uncollected and insincere surveys, of which 106 were questionnaires for parents of disabled children, were exclused from analysis. The SPSS Window 23 program was used for data analysis and frequency analysis and the independent sample T test were performed. Results : Disabled children's parents perceived playing with their children as more important than that of non-disabled children's parents(p<0.01). There was no statistical difference between disabled children and non-disabled children's parents, but there were some differences(p=0,053). Both disabled and non-disabled children had more time to participate with their mothers than with their fathers. Mothers with disabilities had more time to play however, fathers with no disabilities had more time. Both disabled and non-disabled children's parents had the most "ordinary" frequency of buying toys and there was very littele difference between the 2 groups. Both disabled and non-disabled children's parents primarily used the internet to acquire play information, and consideration when buying fun was followed by interest inducement, development level and safety. Conclusion : Through this study, it was possible to compare the status of play recognition and participation by parents of children with or without disabilities. Based on this study, parents will be able to find out what they really need to play and will be provided as a basis for future play studies for children.
Due to change in circumstances in the 2000s such as severe birthrate decline and shortened military service period, the armed forces of the Republic of Korea is currently turning to technologies and equipments from manpower, developing it to become high-tech, high-speed, and complex, resulting in an environment in which a single mistake could cause a mass mortality crisis. It is also evident that, considering aspects such as safety training curriculums and achievements of advanced countries and private education, hands-on training is a must in preventing suicides and accidents in the military, and establishing safety training centers is crucial for systematic and effective hands-on training.Soldiers who are joining the army as of now have experienced the Internet ever since they were born and easily use both virtual and augmented reality, and the current level of science and technology has developed to the point where most of the public safety experience centers are able to be replaced by virtual and augmented reality (VR/AR). Therefore, considering the aspects such as installation space, construction costs, maintenance costs, user characteristics, and education effects, other than for those trainings where real models and objects are more effective such as first aid training, it is with a strong recommendation that establishing military safety training facilities with VR/AR (Virtual and augmented reality) is a must in the coming future. We have derived the need for hands-on training by considering the development of virtual and augmented reality (VR/AR), analysis of operation status of the public safety experience centers, characteristics of military units, installation and maintenance costs, and proposed plan to establish safety training centers where effective training performance can be achieved at a lower cost than the public safety experience center. In addition, we suggested the scale of the required safety training center and the composition of the experience rooms considering the number of trainees and the environment of each military units. Given this analysis it will contribute to the prevention of military safety and suicide by building a safety training center in the future.

The clinical assessment for preschool children who are known to have problems in selfreporting tends to be dependent on outsiders' reporting. Thus, the direct assessment of children's inner experience, thoughts and feelings is difficult. MacArthur Story-Stem Battery(MSSB) developed to learn more about preschool children's mental representation in play is used in this study to help assess clinical preschool children through developmental study of normal children's mental representation. Fifty five children(32boys and 23girls) who performed MSSB, IQ Test, Peabody Picture Vocabulary Test-Revised(PPVT) were videotaped and were analyzed. The results of this study were as follows:1) Children frequently displayed negative mental representation such as atypical negative response, reparation/guilt, punishment, personal injury and so on during emotionally laden play situation. 2) Mental representation of parent appeared positive, disciplinary, and negative in respective. 3) As a result of factor analysis of MSSB content themes, aggressive, prosocial, and oppositional content theme composites were generated. Aggressive content included atypical negative response, aggression, personal injury, and exclusion. Prosocial content included affection, affiliation, and reparation/guilt. Oppositional content included punishment and non-compliance. 4) Mental representation of parent and content themes showed significant correlation. Positive, negative, and disciplinary representation were significantly correlated for prosocial(r=0.40), aggressive (r=0.52), and oppositional(r=0.75) content theme respectively. 5) Among the correlations between parental mental representations and emotional responses, positive parental representation and anxiety showed significant negative correlation(r=-0.43). 6) Among the correlations between content themes and emotional responses, there were significant positive correlations between aggressive(r=0.28) and oppositional content themes(r=0.29) and distress, and were significant negative correlations between prosocial content theme and concern(r=-0.29) and anxiety(r=-0.43). According to the above results, preschool children frequently displayed negative mental representation in emotionally conflictual play situation. Children with more prosocial themes in their stories exhibit more positive parental mental representation. Also, children with more aggressive themes tend to display more negative parental representation and negative emotional responses.
Several parameters of hydraulic architecture relating to hydraulic conductance in xylem vessels were investigated in the current-year shoots of six species of deciduous oak trees. The above parameters were also investigated in the sprouts of Quercus mongolica and Q. variabilis, as well as in the seedlings of Q. mongolica and Q. acutissima. The values of specific conductivity, leaf specific conductivity and Hagen-Poiseuille's relative hydraulic conductivity relating to vessel diameter of Q. dentata were the highest in all of the species studied. The above values of most of the species studied were higher in May-June than in September-October because of increasing the vessel embolism by cavitation and so on through the growing season. The estimated values of relative hydraulic conductivity of vessel by Hagen-Poiseuille's empirical equation and the real values of hydraulic conductivity presented positive relationships in most of the species studied. Huber value and leaf specific conductity using leaf area or leaf weight generally exhibited similar patterns each other even if having some exceptions. The hydraulic conductances of sprouting shoots were much better than those of normal growing shoots in Q. rnongolica and Q. variabilis. The specific conductivity and leaf specific conductivity were rapidly decreased by the vessel embolism through cavitating just after cutting the shoots in Q. mongolica and Q. acutissima seedlings. Diurnal changes of the conductivities in the seedlings of Q. mongolica and Q. acutissima presented the possibility of their self-controlling of conductance by active moisture absorption under mild water stress. Specific conductivity and leaf specific conductivity, and so on of Q. acutissima seedlings subjected to periodical moisture stress or not have decreased through the growing season, but the influences of moisture stress to the conductance were not proved definitely because of influencing similarly and simultaneously to the development of xylem and leaf having inverse relation in the influences. The values of conductivities were higher generally in middle or upper parts of stems than root collar in the seedlings.
The coal-bearing siliciclastic rocks of the Lower Permian Jangseong Formation, Samcheog coalfield, represent a megacyclothem which shows cyclic repetitions of sandstone, shale, coaly shale, and coals. Petrographic, geochemical, and SEM studies for sandstone samples, and XRD analysis for clay minerals were carried out to understand diagenesis in the sandstones of the Jangseong Formation. The Jangseong sandstones are composed of 60% quartz (mainly monocrystalline quartz) and 36% clay matrix and cement with minor amounts of feldspar, lithic fragments and accessory minerals (less than 4%). Jangseong sandstones are classified mostly as quartzwackes and partly as lithic graywackes according to the scheme of Dott(1964). The textural relationships between authigenic minerals and cements in thin sections and SEM photomicrographs suggest the paragenetic sequence as follows; (1) mechanical compaction, (2) cementation by quartz overgrowth, (3) formation of authigenic clay minerals (illite, kaolinite), (4) dissolution of framework grains and development of secondary porosity, and (5) later-stage pore-filling by pyrophyllite. We propose that these diagenetic processes might be due to organic-inorganic interaction between the dominant framework grains and the formation water. The Al, Si ions and organic acid, derived from dewatering of interbedded organic-rich shale and coals, were transported into the Jangseong sandstones. This caused changes in the chemistry of the formation water of the sandstones, and resulted in overgrowth of quartz and precipitation of authigenic clay minerals of kaolinite and illite. The secondary pores, produced during dissolution of clay and framework grains by organic acid and $CO_2$ gas, were conduit for silica-rich solution into the Jangseong sandstones and the influx of silica-rich solution produced the late-stage pyrophyllite after the expanse of kaolinite. The origin of the solution that formed pyrophyllite is not likely to be the organic-rich formation water based on the observation of fracture-filling pyrophyllite in the Jangseong sandstones, but the process of pyrophyllite pore-filling was indirectly related to organic-inorganic interaction.
Metamophic rocks of Samcheog area, northeastern Yeongnam massif, was studied petrochemically. This area includes Precambrian Hosanri Formation (schists and gneisses) and granitoid (Icheon granitic gneiss, leucocratic granite and Hongjesa granite), Cambrian sedimentary rocks, and Cretaceous sedimentary and acidic volcanic rocks. Hosanri formation is composed of quartz+plagioclase+K-feldspar+biotite+muscovite+granet${\pm}$cordierite${\pm}$sillimanite. Mineral assemblage of biotite granitic gneiss, which is massive granodioritic rock with weak foliation, is similar to Hosanri formation. According to mineral assemblages, metamorphic rocks of studied area can be divided into two metamorphic zones (garnet and sillimanite zones). From Icheonri area, major, trace and rare earth element data of biotite granitic gneiss and luecocratic granite suggest that source rock is politic rocks of Hosanri formation and source magma was formed by anatexis and experienced fractionation of plagioclase. Trace element diagram show collisional environment such as syn-collisional, volcanic arc granite. Orientation of faults in study area have three maximum concentrations, $N54^{\circ}\;W/77^{\circ}\;SW,\;N49^{\circ}\;W/81^{\circ}\;NE\;and\;N10^{\circ}\;W/38^{\circ}\;NE$. Structure analysis suggests that faults in study area ware formed by uplift and compression. Faulting age is guessed after Tertiary because some shear joints is developed in dikes to intrusive Cretaceous acidic volcanic rock. Hosanri formation and Icheon granitic gneiss had experienced similar deformation history because they have maximum concentration to foliations, $N89^{\circ}\;E/55^{\circ}\;SE\;and\;N80^{\circ}\;E/45^{\circ}\;SE$, respectively.
