FIBROMA ON THE PALATE : A CASE REPORT (구개의 섬유종 : 증례보고)

  • The fibroma is the most common benign soft-tissue tumor found in the oral cavity and most commonly occurrs in sites predisposed to irritation or trauma. Treatment is conservative surgical excision. Seldom does the lesion recur. A 14-year old girl was referred to the Department of Pediatric Dentistry for evaluation and management of a mass on the palate. The patient's parents reported the slow growth of this soft tissue lesion over a 6-month period. A history of trauma or irritable habits was denied. Clinical examination revealed an asymptomatic, soft, pedunculated elevation of mucosa, measuring 3cm in diameter. Excisional biopsy of the lesion supported a diagnosis of fibroma. There have been no signs of recurrence during follow up of 4 years after surgical intervention. The clinical picture, pathology and management of the case are described, and the differential diagnosis and treatment are discussed.

  • Cemento-ossifying fibroma of the jaws is well circumscribed, generally slow-growing, benign lesions which enlarge in an expansile manner. Clinically it presents as a slowly enlarging lesion commonly in the premolar-molar area of the mandible and only occasionally in the maxilla and other locations. It occurs twice as often in females and primarily in the 20 to 30 year age group. Differential diagnosis should be peformed, preferably with other fibro-osseous lesions such as fibrous dysplasia. A faster growing and more destructive variant of cemento-ossifying fibroma sometimes occurs in patients under age 15 and is termed juvenile (aggressive) ossifying fibroma. Treatment is surgical removal with the extent depending on the size and location of the individual lesion. Recurrence is considered rare. A case involving a 12-year-old male patient with delayed eruption of right mandibular canine is discussed. Following an incisional biopsy, the histopathologic diagnosis established was cemento-ossifying fibroma. After the surgical enucleation of the lesion, no sign of recurrence was detected.

  • Samsebul, the altar portrait behind the statue of Buddha in the main building of Bonghwang temple in Andong, has been designated as Tangible Cultural Property No. 406. These alter portraits have significance as the standard of the research of Samsebul in Joseon period. In this study, fibre of the ground textile is identified using microscopic examination, solubility test, ATR-FT-IR, SEM, XRD. Two samples from Yaksabul(A, B), one sample from Seokgabul(C), and one sample from Amitabul(D), which were collected during the conservation process, were prepared for this study. In previous record, above samples were documented as hemp. Due to severe deterioration and accumulated dust layers on these samples, it was hard to recognize them with naked eyes, but through this study, we could identify that all samples except one from Yaksabul(A) are silk.

Immunoglobulin G4-Related Lung Disease Mimicking Lung Cancer: Two Case Reports (폐암으로 오인된 면역글로불린 G4 연관 폐 질환: 2예에 대한 증례 보고)

  • Immunoglobulin G4 (IgG4)-related disease is a rare systemic fibroinflammatory condition characterized by elevated serum IgG4 levels and infiltration of IgG4-positive plasma cells in various organs. IgG4-related lung disease shows varied radiologic features on chest CT. Patients usually present with a solid nodule or mass mimicking lung cancer; therefore, distinguishing between IgG4-related disease and other conditions is often challenging. Additionally, co-existing radiologic findings of IgG4-related lung disease may mimic metastasis or lymphangitic carcinomatosis of the lung. We report two cases of histopathologically confirmed IgG4-related lung disease mimicking lung cancer. Chest CT revealed a solid nodule or mass with ancillary radiologic findings, which suggested lung cancer; therefore, IgG4-related lung disease was radiologically indistinguishable from lung cancer in both cases. Measurement of serum IgG4 levels and clinical evaluation to confirm involvement of various organs may be useful to establish the differential diagnosis. However, surgical biopsy evaluation is needed for confirmation.

Imaging Findings of Breast Metastasis from Malignant Peripheral Nerve Sheath Tumor of Mediastinum: A Case Report (종격동 악성 말초 신경 초종으로부터 유방 전이의 영상 소견: 증례 보고)

  • Malignant peripheral nerve sheath tumor (MPNST), a rare soft-tissue sarcoma, is most commonly located in the trunk, extremities, and head and neck, but rare in the breast. We report a metastatic breast MPNST in a 27-year-old woman with neurofibromatosis type 1 (NF-1). Chest computed tomography revealed a well-defined, oval, mildly enhancing nodule in the right breast. US revealed a circumscribed, oval, heterogeneous echoic mass with vascularity and intermediate elasticity in the right upper outer breast. The breast mass was excised and diagnosed as MPNST on histopathology evaluation. Although rare, it should be included in the differential diagnosis of breast mass in NF-1 patient.

Role of Cytologic Scoring System in Minimizing "Gray Zone" in Breast Aspiration Cytology (진단이 애매했던 유방 세침흡인 세포검사에서 등급 점수표의 역할)

  • Fine needle aspiration(FNA) has been quite successful in identifying benign and malignant breast lesions, but a "gray zone" exists. A total of 697 FNAs of breast were performed at Korea Cancer Center Hospital for a period of one year. One hundred and eleven of the 697 FNAs were diagnosed as atypical or suspicious for malignancy. Among them, we reviewed 74 FNAs, un which histologic diagnoses were made, and applied cytologic grading system proposed by Masood et al (1990) to evaluate the usefulness of this system in minimizing the size of gray zone. Technical problem was responsible for equivocal diagnoses in 19 FNAs Of the remaining 55 FNAs, 18 were benign and 37 were malignant. Among benign conditions, fibroadenoma(5 cases) and fibrocystic disease with fibroadenomatous feature(3 cases) constituted the largest groups. The majority of malignant conditions were infiltrating ductal carcinoma(29 cases), however, those low grade carcinomas Including tubular carcinoma(3 oases), cribriform carcinoma(2 cases), and mucinous carcinoma(2 cases) occupied a relatively large proportion Cytologic grading system was quite useful in minimizing the size of gray lone. The scores of 27 out of 29 usual infiltrating ductal carcinomas belonged to the group of cytologic malignancy, how-ever, only 2 out of 7 low grade carcinomas got scores of malignancy FNA from fibroadenoma or fibrocystic disease with fibroadenomatous features showed a tendency toward high scores. Experience of the cytopathologist and familiarity with cytologic alteration in breast disease cannot be overemphasized.

  • Aneurysmal bone cyst of the rib is a rare benign tumor which resembles the giant cell tumor or fibrous dysplasia of the bone. It rarely develops in the rib although it affects the metaphysis of the long bone and facial bone. We have treated a 14 year old male patient who had the aneurysmal bone cyst in the legt 4th. rib. Wide surgical resection of the affected rib was carried out with good clinical result. He is well 16 months after the resection.

  • An 81-year-old man underwent bronchoscopy to investigate the cause of his dyspnea symptoms. A benign natured mass was observed in the bronchus and was excised. The pathology examination revealed a fibroepithelial polyp. He has been asymptomatic since the simple bronchoscopic excision. A fibroepithelial polyp is a benign tumor with a mesodermal origin. It is commonly found in the skin, urogenital area and chest, but is extremely rare in the respiratory system. We report a case of a fibroepithelial polyp in the bronchus, which was treated with a bronchoscopic excision, with a brief review of the relevant literature.

Nodular Fasciitis of the Thigh Mimicking Soft Tissue Sarcoma - Two Cases Report - (연부조직 육종으로 오인된 대퇴부의 결절성 근막염 - 2예 보고 -)

  • Nodular faciitis is generally considered to be benign proliferation of fibroblasts and myoblasts, and it measures dimension up to 3cm. The characteristics such as rapid growth, abundant cellularity, and mitotic activity occasionally cause these lesions to mimic sarcoma. The authors experienced two cases of nodular fasciitis of the thigh, which were unusually large with dimension of more than 5cm. All of these two cases mimicked sarcoma and one of two cases, which initially mimicked sarcoma clinically and histologically in our hospital, was finally diagnosed as nodular fasciitis after requesting external consultation to several experienced pathologists. All of two cases had no evidence of recurrence at 2 years postoperatively. The lesions of unusually large dimension such as in our cases must be included in the differential diagnosis of soft tissue sarcoma. So we report two cases of nodular fasciitis of the thigh with a review of the current literature.

  • Focal nodular hyperplasia is a benign hepatic tumor mainly composed of nodules of hepatocytes and Kupffer cells separated by fibrous septa. In general, it is difficult to differentiate focal nodular hyperplasia and hepatocellular carcinoma on ultrasonography, conventional CT(computerized tomography), and angiography. But IV bolus CT is of particular value in the diagnosis of focal nodular hyperplasia because it can divide enhanced CT into early and late phase and can characterize tumor vascularity and analyze any intratumoral elements. In our case, it was seen as a hypoechoic mass lesion on ultrasonography and hyperdense mass lesion on early-phase IV bolus CT and isodense mass lesion on late-phase IV bolus CT. On angiography, hypertrophy of the feeding artery and tumor staining were well visualized. The patient underwent operation and the mass was pathologically confirmed to a focal nodular hyperplasia. We report the first case of focal nodular hyperplasia on IV bolus CT in Korea.
