DENTAL MANAGEMENT OF A PATIENT WITH KABUKI SYNDROME : A CASE REPORT (가부키 증후군(Kabuki syndrome) 환자의 치과적 관리의 치험례)

  • Kabuki syndrome is characterized by typical facial features (elongated palpebral fissures with eversion of the lateral third of the lower eyelid; arched and broad eyebrows; short columella with depressed nasal tip; large, prominent, or cupped ears), minor skeletal anomalies, persistence of fetal fingertip pads, mild to moderate intellectual disability, and postnatal growth deficiency. A 6-year-old male with kabuki syndrome was referred from the local dental clinic for left facial swelling and dental caries on all primary molars. He was treated for acute periapical abscess with incision and drainage under physical restraint, and left maxillary second primary molar was extracted. Other caries treatment was performed under general anesthesia. As the syndrome involves many different medical problems, special cares should be considered. Dental treatment should be carried out in comprehensive consultation system.

  • Aspiration of tooth can occur not only during dental treatment, but also due to factors like trauma or physiologic exfoliation of primary tooth. If this occurs, complications such as fever, cough, dyspnea, pain, and bronchitis can be appeared. 9 years-old girl with Lennox-Gastau syndrome visited the dental clinic for regular checkups. Calcified tooth-like material was observed in the chest PA x-ray, and maxillary left primary first molar was not observed in the patient's oral cavity. She had a history of hospitalization for pneumonia two months ago. Because it was considered that the tooth was likely to be aspirated and caused pneumonia, the extraction of remaining primary teeth with mobility was performed. It is necessary to reduce the risk of aspiration in patient with disabilities by performing active treatment such as removal of primary tooth with mobility or ill-fitting restorations.

Bond Strength of Band on Zirconia Crown with Compomer and Resin Cement (지르코니아 기성관에 컴포머와 레진 시멘트로 접착한 교정 밴드의 결합력)

  • The aim of this study was to evaluate the compomer cement and resin cement as an orthodontic band cement on zirconia crown. A total of 30 specimens were prepared. Preformed stainless steel crowns and zirconia crowns of upper right second primary molar were used. Orthodontic bands were cemented on stainless steel crowns (Group I, n = 10) and zirconia crowns (Group II, n = 10) with compomer cement. The other bands were cemented on zirconia crowns with resin cement (Group III, n = 10). The tensile loads were applied to band to measure the bond strength. The mean of bond strengths of group I, II and III were 0.79 MPa, 1.09 MPa and 1.56 MPa respectively. Bond strength of group II is significantly higher than group I. There was no significant difference between group II and III. Compomer cement and resin cement containing functional monomers showed favorable bond strength of orthodontic bands.

  • Molar-incisor malformation (MIM) is a newly reported dental anomaly with molar root deformity and incisor crown defects. MIM-affected teeth may cause severe pain with no apparent tooth caries. Since the affected molars clinically appear normal, radiographs are recommended for accurate diagnosis on the first visit. Since MIM-affected patients are in mixed dentition, timely and appropriate interventions are needed to avoid unnecessary pain and complicated clinical issues. This report was written to describe two patients who had MIM in early mixed dentition and report their 2-year follow-ups.

TREATMENT OF RADICULAR CYST USING DECOMPRESSION (감압술을 이용한 치근단 낭의 처치)

  • Radicular cyst is a true epithelium lined cyst formed when epithelium at the apex of a nonvital tooth is stimulated by inflammation. It is the most common type of cyst in the oral cavity, but its frequency is low in the primary dentition. Treatment of large-sized cyst aims at conservation of adjacent structure and allowance of proper eruption of the successive permanent teeth in the primary dentition. Considering these two aspects, marsupialization or enucleation following decompression is recommended as a treatment means for large-sized radicular cyst. In this case, 8-year old boy visited the pediatric dentistry department. Yonsei University Dental Hospital, with the chief complaint of pain on the lower right area. Clinical and radiographic examinations revealed periapical radiolucent lesion on #84, which had previous pulp treatment and restorated with the stainless steel crown. Dislocation of its successive tooth, #44, was also observed. #84 was extracted and sent for biopsy. Through histologic examination, it was diagnosed as radicular cyst. Following its extraction, removable space maintainer was delivered, which was also used as a decompressor. During periodic check-up for next 22 months, favorable healing of the lesion and eruption of the successive tooth were observed.

A STUDY ON THE VOLUMES AND FORMS OF THE PALATE FOR DECIDUOUS AND PERMANENT DENTITION (유치열과 영구치열의 구개 형태에 관한 연구)

  • The purpose of this study was to clarify the palatal forms through palatal curvatures of right to left, anterior to posterior direction, and difference of right and left palatal volumes in the primary and permanent dentition. Samples were consisted of normal occlusion of both dentition(50 males and 50 females each). Their upper plaster casts were used, measuring points were decided, through 3-dimensional laser scanning(3-D Laser Scanner, DS4060, LDI, U.S.A.), and fitting standard horizontal plane were made for measuring the height and sectioned volumes of palate. The results were as follows: 1. Palatal volume and height were greater at the right side of the palate in the primary and permanent dentition of male and female, but there was no significance. 2. Palatal height was greater in male compared to female, especially, there was significant difference at intercuspid, inter-second premolar area in the permanent dentition(P<0.05). 3. To the height of A-P direction of mid-palatal area, the highest point was 20mm in the primary dentition, 30mm in the permanent dentition from interdental papilla of central incisors. 4. Palatal height of inter-cuspid and inter-second premolar became shallow and broad, high and broad each, compared to inter-deciduous canine and inter-second deciduous molar.

RETENTIVE FORCES OF CLASPS OF REMOVABLE ORTHODONTIC APPLIANCES FOR CHILDREN (어린이에게 사용되는 가철식 교정장치용 clasp의 유지력비교)

  • The purpose of this study was to compare the retentive forces of various types of clasps for removable orthodontic appliances for primary and mixed dentition. Seven metal models of a single tooth and two teeth were made, including maxillary left primary canine, first primary molar, second primary molar and first molar. Retentive forces of Adams clasp, circumferential clasp(C clasp), Jackson clasp, Duyzing clasp, arrowhead clasp, ball clasp, eyelet clasp, and triangular clasp were measured by Universal Testing Machine(Zwick Z020, Germany). The obtained results were as follows. 1. Jackson clasp and Adams clasp showed the highest retentive force among single tooth clasps. 2. C clasp showed the lowest retentive force, and there was no statistically significant difference in retentive force between mesial end C clasp and distal end C clasp. 3. Eyelet clasp showed the highest, and ball clasp showed the lowest retentive farce among clasps for interdental undercut. 4. Triangular clasp showed higher retentive force than ball clasp.

NECROSIS OF ALVEOLAR BONE BY FORMOCRESOL : CASE REPORT (Formocresol에 의한 치조골 괴사의 치험례)

  • Various chemotherapeutic agents have been recommended for pulpotomy of primary teeth, and there are formocresol, ferric sulfate, and calcium hydroxide. Of those, formocresol has fixation effect of pulp tissue and high clinical success rate, so it is most commonly used agent. But formocresol has strong cytotoxic effects, thus many articles reported displacement and loss of permanent successor, amelogenesis imperfecta, mutation by general absorption, possibility of cancer induction. Recently, it has been reported that leakage by imperfect temporary sealing when FC-soaked cotton was inserted into the root canal caused necrosis of surrounding tissues. and that necrosis of alveolar bone related to the use of excessive formocresol. In this case, 2nd primary molar of upper left jaw was treated using formocresol in local clinic, but extracted because of lasting pain. Furthermore, symptoms didn't disappear so patient was refered to us. The patient was 8-year-old male, had foul odor from oral cavity and circular alveolar bone necrosis around the permanent successor' crown. Thus sequestrectomy was operated and observed through 19 months after operation, we found normal root development of permanent successor but no complete regeneration of alveolar bone defect and attached gingiva. Lesion of periodontal tissues by formocresol is irreversible, so we have to confirm the indication in using formocresol and pay attention to complete temporary sealing.

SPONTANEOUS ERUPTION OF ECTOPIC IMPACTED TOOTH BY INTENTIONAL EXTRACTION OF DECIDUOUS TOOTH (의도적 유치발치술에 의한 이소매복 영구치의 맹출유도)

  • When many factors involved in the eruption of the teeth act as negative effects, they can cause eruption disturbance. Periodic observation, space acquirement, surgical exposure, orthodontic traction, orthodontic traction accompanied with surgical exposure, and surgical repositioning are considered as the treatment options of an impacted tooth, which is a form of eruption disturbance. In the first case, a male patient, age 9, visited Yonsei University Dental Hospital (YUDH) with a chief complaint of ectopic impaction of the upper left lateral incisor. We extracted the upper left primary lateral incisor and primary canine, and 5 months later, a window opening procedure was executed. Eight months later, the upper left lateral incisor partially erupted, and 18 months after the extraction, the axis of the tooth improved and the tooth erupted spontaneously. In the second case, a male patient, age 10, visited YUDH with a chief complaint of ectopic impaction of the upper right first premolar. We extracted the upper right first primary molar. Ten months later, the upper right first premolar erupted partially, and 19 months after the extraction, the upper right first premolar erupted spontaneously. We reported two cases in which improvement of eruption path and spontaneous eruption of an ectopic impacted tooth was achieved by extracting the deciduous tooth which interfered with the proper eruption of it.

A SURVEY ON DENTAL HEALTH OF CHILDREN OF MULTICULTURAL FAMILIES IN CHONBUK PROVINCE (전북지역 다문화가족 어린이의 구강건강 실태조사)

  • The purpose of this study was to obtain the data for dental caries experience and occlusal pattern to utilize these data for motivating oral health promotion and activating oral health care programme in preschool children of multicultural family. Caries prevalence on primary teeth was surveyed for 3-5 years old 190 children of multicultural families in Chonbuk province. The results were as follows : 1. Dental caries prevalence rate of primary teeth (dmf rate) was 61.7% at 3 years of age, 88.7% at 4 years of age, and 92.6% at 5 years of age (p>0.05). 2. Average score of dmft index was 2.22 at 3 years of age, 4.97 at 4 years of age, and 6.62 at 5 years of age (p>0.05). 3. Most of the caries-prevailed tooth was maxillary primary central incisor and showed 28.8% at 3 years of age, 56.7% at 5 years of age. 4. dmft index and dmfs index of children of Vietnamese, Cambodian, and Filipinos women were higher than those of Chinese and Japanese. 5. In sagittal primary molar relationship, flush terminal plane type was 82.6%, distal step type was 5.2% and mesial step type was 8.4%. This study is the first report for children of multicultural families in Korea. More attention for children of multicultural families will be needed in aspect of oral disease prevention and treatment.
