IMPEDANCE AUDIOMETRY IN RELATION TO CHRONIC MAXILLARY SINUSITIS (만성 상악동염이 중이기능에 미치는 영향)

  • 만성 상악동염은 동점막의 복잡한 변화를 일으키고, 만성증으로 지속되는 비분비물 및 비점막의 염증이 이관에 파급되어 중이에 영향을 미치며 그 치유경과를 늦추게 한다. 저자는 만성 상악동염에서 동점막의 비후정도가 이관 및 중이에 미치는 영향을 알아 보고자, 1986년 1월부터 3월까지 이화여자대학교 의과대학 부속병원 이비인후과에 비폐색 및 비루를 주소로하여 내원한 환자중 만성 상악동염으로 진단된 16세 이상의 성인 환자를 대상으로 검사하였다. 동점막 비후정도에 따라 비정상군을 경도(2~3mm), 중등도(4mm이상), 고도(상악동 전체혼탁) 3군으로나누어 각각 40예를 조사군으로 하고 동점막의 두께가 0~1mm인 경우를 정상대조군으로 하여 40예를 GSI 28 AUTO TYMP를 사용하여 screening audiometry, tympanometry, acoustic reflex threshold (ipsilateral and contralateral)를 시행하여 다음과 같은 결과를 얻었다. 1) 남녀의 비는 1 : 2.6이고 21세에서 30세군이 53예(33.0%)로 가장 많았다. 2) 증상발현기간은 6개월 이내가 42.5%로 가장 많았다. 3) Tympanogram의 분포에서 비정상적 type으로 나오는 비율이 정상군과 비정상군중에서 경도군이 각각 22.5%이었고 중등도군이 30.0%, 고도군이 50.0% 순이었다. 4) 등골근반사역치는 정상군과 비정상군 사이에 차이가 없었다. 5) 등골근무반사율 역시 정상군과 비정상군 사이에 차이가 없었다.

  • Acute osteomyelitis of the maxilla with orbital cellulitis is occasionally seen secondary to a buccal infection, dental infection or necrotic process of the maxillary antrum. A case of acute osteomyelitis of the maxilla with orbital cellulitis in 5 years old boy has recently been experienced and cured by immediate incision and drainage combined with adequate antibiotics therapy. This paper was attempted to report the case with a brief review of reference.

The Effect of Acute Sinusitis on the Ultrastructure and Sialic Acid Distribution on the Sinus Mucosa Cell Surface of the Rabbit (실험토끼 상악동염이 상피세포 표면의 미세구조변화와 Sialic acid의 분포에 미치는 영향)

  • Experimatal maxillary sinusitis was induced in New Zealand white rabbits by blocking the maxillary sinus ostium. The distribution of lectin receptors was explored in the mucosa with induced maxillary sinusitis using colloidal gold label complex with lectin WGA purified from wheat germ (Triticum vulgaris). The lectin WGA gold complex, shown to recognize GlcNac (N-acetylglucosamine) and NeuNAc (N-acetylneuraminic acid) regions, was applied to detect binding sites in Lowicryl HM 20 sections and viewed under the electron microscope. An increased height of the cylindric cells, ciliary loss and hyperplasia of the secretory cells were observed. Examination of normal sinus mucosa labeled with gold-labeled lectins showed the distribution of sialoglycoconjugates to be mainly in the ciliary layer and the granules in the secretory cells. Inflamed mucosa had increased labeling intensity of gold-labeled WGA in the cilia and the secretory granules. These results indicate that lectin WGA receptors are located in the cilia and secretory granules. Specific changes in the lectin binding pattern were apparent in the inflamed mucosa in the experimentally induced acute sinusitis, in comparison with normal mucosa, conceivably as a part of host defense reactions.

  • The author had a case report of right maxillary molar tooth root apex cyst involving maxillary sinus, which was misdiagnosed maxillary sinusitis or some cancer. Right maxillary 2nd premolar, 1st molar, & 2nd molar tooth involved root apex cyst was extracted and curetted.

  • The maxillary floor sinus augmentation is considered as a safe and predictable procedure to ensure optimal implant placement. However, this procedure may have a variety of intra-surgical or post-surgical complications, also the major drawback of the procedure is deemed maxillary sinusitis. This case is a very unusual delayed occurrence of acute maxillary sinusitis after simultaneous maxillary sinus augmentation, using xenograft and implant placement. This report describes a serious complication of the maxillary sinus augmentation.
